Abstract:
Traumatic cervical hematoma is a relatively rare disease, but due to the rapid progress of airway narrowing, the airway must be quickly secured. We report a case of traumatic cervical hematoma accompanying airway narrowing. A 68–year–old man was taking bayaspirin and clopidogrel for angina pectoris. He fell down while riding a motorcycle and was transported to our hospital. There was no cervical swelling during the examination, but a head crush wound was noted. CT examination revealed hematoma compressing the trachea. Emergency tracheal intubation was performed because of upper airway narrowing and respiratory distress. On the contrast CT examination, bleeding from the vertebral artery of the lower cervical vertebra, a branch of the right subclavian artery, was suspected and emergency transcatheteric arterial embolization was performed. Thereafter, evaluation of the soft tissue width on the front of the vertebral body with cervical spine X–ray indicated reduction in the hematoma. As the airway narrowing had improved on the 8th day according to cervical spine CT, he was extubated after checking the cuff leakage. After extubation, his respiratory condition was stable, and he left the ICU on the 9th day.
