    Abstract: Abstract. Internally consistent, quality-controlled (QC) data products play an important role in promoting regional-to-global research efforts to understand societal vulnerabilities to ocean acidification (OA). However, there are currently no such data products for the coastal ocean, where most of the OA-susceptible commercial and recreational fisheries and aquaculture industries are located. In this collaborative effort, we compiled, quality-controlled, and synthesized 2 decades of discrete measurements of inorganic carbon system parameters, oxygen, and nutrient chemistry data from the North American continental shelves to generate a data product called the Coastal Ocean Data Analysis Product in North America (CODAP-NA). There are few deep-water (〉 1500 m) sampling locations in the current data product. As a result, crossover analyses, which rely on comparisons between measurements on different cruises in the stable deep ocean, could not form the basis for cruise-to-cruise adjustments. For this reason, care was taken in the selection of data sets to include in this initial release of CODAP-NA, and only data sets from laboratories with known quality assurance practices were included. New consistency checks and outlier detections were used to QC the data. Future releases of this CODAP-NA product will use this core data product as the basis for cruise-to-cruise comparisons. We worked closely with the investigators who collected and measured these data during the QC process. This version (v2021) of the CODAP-NA is comprised of 3391 oceanographic profiles from 61 research cruises covering all continental shelves of North America, from Alaska to Mexico in the west and from Canada to the Caribbean in the east. Data for 14 variables (temperature; salinity; dissolved oxygen content; dissolved inorganic carbon content; total alkalinity; pH on total scale; carbonate ion content; fugacity of carbon dioxide; and substance contents of silicate, phosphate, nitrate, nitrite, nitrate plus nitrite, and ammonium) have been subjected to extensive QC.     Abstract: Prolonging survival in good health is a fundamental societal goal. However, the leading determinants of disability-free survival in healthy older people have not been well established. Data from ASPREE, a bi-national placebo-controlled trial of aspirin with 4.7 years median follow-up, was analysed. At enrolment, participants were healthy and without prior cardiovascular events, dementia or persistent physical disability. Disability-free survival outcome was defined as absence of dementia, persistent disability or death. Selection of potential predictors from amongst 25 biomedical, psychosocial and lifestyle variables including recognized geriatric risk factors, utilizing a machine-learning approach. Separate models were developed for men and women. The selected predictors were evaluated in a multivariable Cox proportional hazards model and validated internally by bootstrapping. We included 19,114 Australian and US participants aged ≥65 years (median 74 years, IQR 71.6–77.7). Common predictors of a worse prognosis in both sexes included higher age, lower Modified Mini-Mental State Examination score, lower gait speed, lower grip strength and abnormal (low or elevated) body mass index. Additional risk factors for men included current smoking, and abnormal eGFR. In women, diabetes and depression were additional predictors. The biased-corrected areas under the receiver operating characteristic curves for the final prognostic models at 5 years were 0.72 for men and 0.75 for women. Final models showed good calibration between the observed and predicted risks. We developed a prediction model in which age, cognitive function and gait speed were the strongest predictors of disability-free survival in healthy older people.     Abstract: Netarts Bay is a shallow, temperate, tidal lagoon located on the northern coast of Oregon and the site of the Whiskey Creek Shellfish Hatchery (WCSH). Data collected with an autonomous continuous flow-through system installed at WCSH capable of high-resolution (1 Hz) partial pressure of aqueous CO 2 (pCO 2 ) and hourly total dissolved inorganic carbon (TCO 2 ) measurements, with combined measurement uncertainties of & lt; 2.0% and 0.5%, respectively, is analyzed over the 2014–2019 interval. Summer upwelling, wintertime downwelling, and in situ bay biogeochemistry represent significant modes of the observed variability in carbonate system dynamics. Summer upwelling is associated with large amplitude diel pCO 2 variability, elevated TCO 2 and alkalinity, but weak variability in salinity. Wintertime downwelling is associated with bay freshening by both local and remote sources, a strong tidal signature in salinity, TCO 2 , and alkalinity, with diel pCO 2 variability much less amplified when compared to summer. Further, analysis of alkalinity-salinity relationships suggests multiple water masses inhabiting the bay during 1 year: mixing of end-members associated with direct precipitation, coastal rivers, southward displacement of the Columbia River plume, California Current surface and deep upwelled waters. The importance of in-bay processes such as net community metabolism during intervals of high productivity are apparent. These direct measurements of pCO 2 and TCO 2 have been useful to local hatchery owners who have monitored intake waters following historic seed-production failures related to high-CO 2 conditions exacerbated by ocean acidification.

    Abstract: Accurately measuring the incidence of major postoperative complications is essential for funding and reimbursement of healthcare providers, for internal and external benchmarking of hospital performance and for valid and reliable public reporting of outcomes. Actual or surrogate outcomes data are typically obtained by one of three methods: clinical quality registries, clinical audit, or administrative data. In 2017 a perioperative registry was developed at the Alfred Hospital and mapped to administrative and clinical data. This study investigated the statistical agreement between administrative data (International Statistical Classification of Diseases and Related Health Problems (10th edition) Australian Modification codes) and clinical audit by anaesthetists in identifying major postoperative complications. The study population included 482 high-risk surgical patients referred to the Alfred Hospital anaesthesia postoperative service over two years. Clinical audit was conducted to determine the presence of major complications and these data were compared to administrative data. The main outcome was statistical agreement between the two methods, as defined by Cohen’s kappa statistic. Substantial agreement was observed for five major complications, moderate agreement for three, fair agreement for six and poor agreement for two. Sensitivity and positive predictive value ranged from 0 to 100%. Specificity was above 90% for all complications. There was important variation in inter-rater agreement. For four of the five complications with substantial agreement between administrative data and clinical audit, sensitivity was only moderate (61.5%–75%). Using International Statistical Classification of Diseases and Related Health Problems (10th edition) Australian Modification codes to identify postoperative complications at our hospital has high specificity but is likely to underestimate the incidence compared to clinical audit. Further, retrospective clinical audit itself is not a highly reliable method of identifying complications. We believe a perioperative clinical quality registry is necessary to validly and reliably measure major postoperative complications in Australia for benchmarking of hospital performance and before public reporting of outcomes should be considered.
