    Kurzfassung: Introduction. The study results on the single interaction under the centrifugal rotary part machining in the abrasive discrete medium are presented. Simultaneously with the numerical simulation, experiments were carried out on a centrifugal-rotary unit, and the maximum depth of penetration into the surface of the part, the single-track sizes, metal removal in one blow of an abrasive granule, were investigated. The removal of metal from workpieces was investigateddepending on the processing modes, characteristics of the abrasive particle and the processed material. Materials and Methods. The dependences for determining the metal removal from workpieces (steels 45, copper Cu-OF, and aluminum alloy D16T) are taken into account depending on the grain size (N3) of abrasive particles. The process of a single interaction of an abrasive particle and the workpiece surface is considered within the framework of the dynamic contact problem of the elasticity theory. The authors have carried out finite element modeling of the considered structures in CAE ANSYS package. Results. The results of theoretical and experimental studies on the metal removal from workpieces depending on the grain size of abrasive particles are presented. The technique of their implementation, the tool and equipment used are described. The results of theoretical and experimental studies are compared. Their fine precision is established. Abrasive tools and processing modes are selected. Discussion and Conclusions. The dependences constructed in the work provide determining the rational values of the technological parameters of the centrifugal rotary machining (CRM) process. They can be used under designing the CRM processes. Therefore, time and financial resources can be saved to achieve the desired surface quality.

    Kurzfassung: Due to the need for producing intermediate chemicals such as p-chloroaniline (p-CA) using more environmentally friendly and efficient processes, nanostructured nickel and nickel sulfide-based catalysts were synthesized and studied. The selective pchloronitrobenzene (p-CNB) gas-phase hydrogenation to produce p-CA was tested. The catalysts were characterized by N2 physisorption (SBET), temperature-programmed reduction (TPR), X-ray diffraction (XRD), scanning electron microscopy (SEM), energy dispersive X-ray spectroscopy (EDS), transmission electron microscopy (TEM), and X-ray photoelectron spectroscopy (XPS). The catalytic activity was evaluated in terms of conversion and selectivity towards p-CNB and p-CA, respectively. Based on the experimental results, the addition of sulfur promoted the selectivity towards p-CA in a range of temperature from 120 to 220 ºC. The selectivity towards p-CA was close to 100 %, avoiding the formation of undesirable side products. Therefore, it is suggested that sulfured-Ni based catalysts increase the surface acidity, which allowed improved hydrogenolysis of the N-O bond.

    Kurzfassung: Introduction. The study results of the abrasive processing of parts made of polymer-composite materials are presented. The features of processing polymer composites and the technology of preforming through waterjet cutting are described. The stages of preparation of a part made of polymer-composite material for the “glueing” operation are investigated. Materials and Methods. Dependences for determining the surface roughness under waterjet cutting of polymercomposite material are considered. Research is carried out to achieve the required surface roughness under adhesive bonding of workpieces. The dependence is given that describes the roughness that is required for a reliable adhesive bond. Results. The theoretical and experimental studies of the waterjet cutting process are resulted. Their implementation technique, the tool and equipment used are described. The results of theoretical and experimental studies are compared. Their high convergence is established. The results of experimental studies on the preparation of parts made of polymercomposite materials for glueing are shown. The abrasive tools and processing modes are selected. Discussions and Conclusions. The process design procedure of abrasive treatment of workpieces from polymercomposite materials is proposed.

    Kurzfassung: The development of a new direction in anticancer medical therapy – the use of immune checkpoint inhibitors targeting PD-1/ PD-L1 and CTLA-4 – has significantly changed the approach to tumor treatment in the last few years. The PD1 blocker nivolumab in major registered clinical trials improved overall survival, including in metastatic melanoma, with a favorable toxicity profile. However, its efficacy in patients with brain metastases from melanoma was poorly studied, since the inclusion criteria  for  most clinical trials do not envisage recruiting such patients. The  immune-mediated toxicity of  immune checkpoint inhibitors is currently well enough studied. However, cases of cutaneous toxicity are quite rare and present certain difficulties for differential diagnosis and treatment. This article presents two cases of effective nivolumab treatment in patients with generalized BRAFwt and BRAFmut cutaneous melanoma. The  first case is of  interest due to the  presence of  brain  metastases in the patient. Nivolumab therapy helped achieving complete regression of intracranial metastases with the long-term effect. The second case, in addition to effective treatment, demonstrates a rare manifestation of skin toxicity – vitiligo on the face and upper extremities.

    Kurzfassung: Peninsular Malaysia is being distributed into three parallel belts (Western, Central and Eastern). Kelantan is one of the states in Peninsular Malaysia and consider as a unique territory to have all three belts (Western, Central and Eastern). Each belts divided into several formations which are Western Belt (Main Range Granite), Central Belt (Jeli Granite, Kemahang Granite, Noring Granite, Kenerong Leucogranite, Berangkat Tonalite and Senting Granite) and Eastern Belt (Boundary Range Granite). Rare Earth Elements (REEs) are usually concentrated related to the alkaline – peralkaline, carbonalite igneous rocks, as well as sedimentary rocks. Granitoid and some intrusive volcanic rocks are widely exposed in Malaysia, as well as Kelantan state. REEs are relatively abundant in the Earth crust, however these elements are rarely concentrated in the mineable ore deposit. There are a lot of research about granitoids, but very limited studies about the distribution of the REE. The objective of this study is to investigate the distribution of REEs in different type of granitoid rocks in Kelantan. For this purpose, 15 samples were selected and analysed using Inductive Coupled Plasma Mass Spectrometry (ICP-MS). Result shows that, distribution of light REEs in all samples up to 78% and heavy REE up to 22% with total value 5350.69 ppm and 1491.27 ppm respectively. Surprisingly, Jeli Granite formation (LT15) is high potential of REE among the samples tested with total REE 3164.93 ppm and Kemahang Granite (JD18) is least potential with total REE 31.95 ppm. The granitoid distribution can be found widely in Kelantan with special characteristic and detailed study about mineral composition will help identifying the REE potential as well as generate more relatable study about genesis and nature of rocks in Malaysia.

    Kurzfassung: Idiopathic acute pancreatitis (IAP) remains a dilemma for physicians as it is uncertain whether patients with IAP may actually have an occult aetiology. It is unclear to what extent additional diagnostic modalities such as endoscopic ultrasonography (EUS) are warranted after a first episode of IAP in order to uncover this aetiology. Failure to timely determine treatable aetiologies delays appropriate treatment and might subsequently cause recurrence of acute pancreatitis. Therefore, the aim of the Pancreatitis of Idiopathic origin: Clinical added value of endoscopic UltraSonography (PICUS) Study is to determine the value of routine EUS in determining the aetiology of pancreatitis in patients with a first episode of IAP. Methods and analysis PICUS is designed as a multicentre prospective cohort study of 106 patients with a first episode of IAP after complete standard diagnostic work-up, in whom a diagnostic EUS will be performed. Standard diagnostic work-up will include a complete personal and family history, laboratory tests including serum alanine aminotransferase, calcium and triglyceride levels and imaging by transabdominal ultrasound, magnetic resonance imaging or magnetic resonance cholangiopancreaticography after clinical recovery from the acute pancreatitis episode. The primary outcome measure is detection of aetiology by EUS. Secondary outcome measures include pancreatitis recurrence rate, severity of recurrent pancreatitis, readmission, additional interventions, complications, length of hospital stay, quality of life, mortality and costs, during a follow-up period of 12 months. Ethics and dissemination PICUS is conducted according to the Declaration of Helsinki and Guideline for Good Clinical Practice. Five medical ethics review committees assessed PICUS (Medical Ethics Review Committee of Academic Medical Center, University Medical Center Utrecht, Radboud University Medical Center, Erasmus Medical Center and Maastricht University Medical Center). The results will be submitted for publication in an international peer-reviewed journal. Trial registration number Netherlands Trial Registry (NL7066). Prospectively registered.

    Kurzfassung: Laparoscopic gastrectomy is rapidly being adopted worldwide as an alternative to open gastrectomy to treat gastric cancer. However, laparoscopic gastrectomy might be more expensive as a result of longer operating times and more expensive surgical materials. To date, the cost-effectiveness of both procedures has not been prospectively evaluated in a randomized clinical trial. Objective To evaluate the cost-effectiveness of laparoscopic compared with open gastrectomy. Design, Setting, and Participants In this multicenter randomized clinical trial of patients undergoing total or distal gastrectomy in 10 Dutch tertiary referral centers, cost-effectiveness data were collected alongside a multicenter randomized clinical trial on laparoscopic vs open gastrectomy for resectable gastric adenocarcinoma (cT1-4aN0-3bM0). A modified societal perspective and 1-year time horizon were used. Costs were calculated on the individual patient level by using hospital registry data and medical consumption and productivity loss questionnaires. The unit costs of laparoscopic and open gastrectomy were calculated bottom-up. Quality-adjusted life-years (QALYs) were calculated with the EuroQol 5-dimension questionnaire, in which a value of 0 indicates death and 1 indicates perfect health. Missing questionnaire data were imputed with multiple imputation. Bootstrapping was performed to estimate the uncertainty surrounding the cost-effectiveness. The study was conducted from March 17, 2015, to August 20, 2018. Data analyses were performed between September 1, 2020, and November 17, 2021. Interventions Laparoscopic vs open gastrectomy. Main Outcomes and Measures Evaluations in this cost-effectiveness analysis included total costs and QALYs. Results Between 2015 and 2018, 227 patients were included. Mean (SD) age was 67.5 (11.7) years, and 140 were male (61.7%). Unit costs for initial surgery were calculated to be €8124 (US $8087) for laparoscopic total gastrectomy, €7353 (US $7320) for laparoscopic distal gastrectomy, €6584 (US $6554) for open total gastrectomy, and €5893 (US $5866) for open distal gastrectomy. Mean total costs after 1-year follow-up were €26 084 (US $25 965) in the laparoscopic group and €25 332 (US $25 216) in the open group (difference, €752 [US $749; 3.0%]). Mean (SD) QALY contributions during 1 year were 0.665 (0.298) in the laparoscopic group and 0.686 (0.288) in the open group (difference, −0.021). Bootstrapping showed that these differences between treatment groups were relatively small compared with the uncertainty of the analysis. Conclusions and Relevance Although the laparoscopic gastrectomy itself was more expensive, after 1-year follow-up, results suggest that differences in both total costs and effectiveness were limited between laparoscopic and open gastrectomy. These results support centers’ choosing, based on their own preference, whether to (de)implement laparoscopic gastrectomy as an alternative to open gastrectomy.

    Kurzfassung: The oil palm tree, or scientifically called as Elaeis guineensis is native to West Africa, where it grows in the wild, transformed into a crop that later was introduced to Malaysian industry. The cultivation of oil palm improved rapidly under the agricultural sector causes degradation, particularly when the oil palm plantation goes uncontrolled. Tree plantation identification is very important for plantation management, environmental management, biodiversity monitoring and many other applications. Accurate inventories and monitoring oil palm estates can be a challenge and critical towards the plantation management and plant area expansion. Managing oil palm estate manually can almost be impossible, so do the tree counting. Manual field-based tree counting is time-consuming and high cost. Conventional method for tree counting can be carried out by manually marked on images or carry out field surveying using GPS to collect the positions of oil palm trees and display their position on image. Developing easier, simpler and cheaper method for tree counting is needed. The aim of this study is to analyse oil palm trees using drone-based remote sensing images. The algorithms used in this research study including Gray-Level Co-occurrence Matrix (GLCM), wavelet transform and template matching. The database of oil palm tree been developed with a total of 131 oil palm trees and 161 of non-oil palm trees have been collected. The window size of oil palm tree been analysed where 250 x 250 pixels which GLCM showed the best overall accuracy of 73.10% for both oil palm and non-oil palm. In this specific window, the oil palm crown can be covered and the result given is more accurate compared to other window sizes. The resulting analysis shows that wavelet transform algorithm gives the highest overall accuracy value which is 82.07%. The other eight statistic parameters can also used to modify the GLCM in order to observe the accuracy and identify which give the best classification accuracy. The availability and ubiquity of drone technologies with high resolution images and regular basis monitoring, new techniques in image and pattern recognition using drone-based remote sensing images let the idea of high accuracy oil palm tree detection become a reality.
