    Abstract: Lymphopenia is a marker of immunosuppression after severe coronavirus disease-2019 (COVID-19) which is characterized by acute respiratory distress syndrome (ARDS). This study aimed to evaluate the relationships between persistent lymphopenia and ARDS. Methods A retrospective cohort study of 125 patients with COVID-19 admitted to government-designated treatment center between 14 January 2020, and 20 March 2020 was conducted. We recorded all complete blood cell counts during the day 0th, 3rd, and 7th following the diagnosis of COVID-19. Patients were grouped based on the depression of the lymphocyte cell count, their return, or their failure to normal. The primary outcome was the occurrence of ARDS, and secondary outcomes included developing vital organ dysfunction and hospital lengths of stay. Results 17.6% (22/125) patients developed ARDS. The lymphocyte counts with ARDS and non-ARDS were 0.94 × 109/L, 1.20 × 10 9 /L at admission, respectively ( p = 0.02). On the 3rd and 7th day, the median of lymphocyte count in ARDS was significantly lower than that of non-ARDS. Multivariable logistic regression, which was adjusting for potentially confounding factors (including age, comorbidities, and APACHE II score), showed that persistent lymphopenia within the 7th day was independently associated with ARDS (OR, 3.94 [95% CI, 1.26–12.33, p = 0.018). Further, patients with persistent lymphopenia had longer hospital lengths of stay ( p 〈 0.001). Conclusion The results showed persistent lymphopenia predicted ARDS after COVID-19. Further studies are needed to investigate whether immunostimulation of lymphocytes within 1 week can reduce ARDS occurrence in patients with COVID-19.

    Abstract: The data in regard of the clinical characteristics and diagnosis of somatic cough syndrome in adults were limited. The aim of this study was to fill that gap. Methods: This was a retrospective analysis of patients with somatic cough syndrome. We described clinical characteristics of adult patients with somatic cough syndrome. Results: Twenty-three somatic cough syndrome patients were identified in 543 adult patients with chronic cough. Psychiatric disorder of these patients was identified as anxiety ( n = 8), obsessive-compulsive ( n = 7), somatoform ( n = 6), depression ( n = 3), and cognitive bias ( n = 1). Twelve patients showed abnormal results of investigations related with common causes of chronic cough, including gastroesophageal reflux, sputum eosinophilia, bronchial hyper-responsiveness, or signs of sinusitis but did not respond to the treatments directed to those conditions. All these patients were ever misdiagnosed as other causes of chronic cough. Compared to 520 non-somatic cough syndrome patients, patients with somatic cough syndrome were younger (32 (29.0–43.0) vs 42.0 (32.0–55.0) years, p = 0.013), longer disease duration (48.0 (19.5–102.0) vs 24.0 (9.0–72.0) months, p = 0.037), more common in dry cough (100% vs 57.6%, p  〈  0.001), and lower proportion of nocturnal cough (13.0% vs 40.2%, p = 0.009). Common cold (60.9%) was the most common initial trigger of cough and itchy throat (60.9%) was the most common accompanying symptom in patients with somatic cough syndrome. Notably, there were similar distribution in cough triggers and accompanying symptoms between two groups. Conclusion: In spite of much higher proportion of dry cough and smaller proportion of nocturnal cough, adult patients with somatic cough syndrome show similar clinical characteristics with other chronic cough patients, in regard of cough triggers, accompanying symptoms as well as abnormal results of investigations, which should be an important reason for misdiagnosis of somatic cough syndrome. Psychiatric disorder should be addressed in clinical management of chronic cough.

    Abstract: Cerebral infarction has a poor prognosis and causes a serious burden on families and society. Recombinant tissue plasminogen activator (rt-PA) and urokinase (UK) are commonly used thrombolytic agents in the clinic. However, direct and powerful clinical trial evidence to determine the therapeutic effect of rt-PA and UK on intravenous thrombolysis is lacking. Methods In this study, 180 patients with acute cerebral infarction were treated with rt-PA or UK. The National Institutes of Health Stroke Scale (NIHSS) scores, Barthel index, bleeding complications, and biomarkers were evaluated. Results No significant differences in NIHSS or Barthel scores were found between the groups. However, UK increased the risk of intracranial haemorrhage compared with rt-PA. rt-PA had increased activity in reducing serum levels of MMP-9 than UK. Conclusion Intravenous thrombolysis with rt-PA and UK in the time window of acute cerebral infarction can achieve similar therapeutic effects, but rt-PA can further reduce the risk of cerebral haemorrhage and is relatively safer than UK.

    Abstract: Many irregular shaped high-rise buildings have been built for graceful demand of architecture or better amenity and ventilation in the world. In this study, a benchmark rectangular high-rise building model and six high-rise building models with various irregular shapes, including two T-shaped, two U-shaped, H-shaped and X-shaped, are tested in a boundary wind tunnel to investigate their wind loads characteristics. The coefficients, power spectral density and cross-correlation of base moments as well as local wind forces are analyzed and discussed in details. Based on the wind tunnel test results, it is founded that the effect of U-shape and H-shape on wind loads is negligible. The along-wind mean base moment coefficient of X-shaped model is increased by 10.0% in 90° wind direction. The wind force characteristics of T-shaped models are quite different from the benchmark model in many cases. The mean base moment coefficient of T-shaped 2 model is reduced by 18.6% in 0° wind direction. Larger mean lift forces and torques of T-shaped models are appeared and there is a certain correlation between along-wind and across-wind local wind forces in 90° wind direction.

    Abstract: Cloth diapers also known as eco-diapers, traditional sandwich-structured eco-diapers are composed of top and bottom layers that are made of cotton or polyester nonwoven fabrics. On account of the hydrophilic bottom layer, urine permeates when the water absorption reaches saturation. In this study, polypropylene is melt-blown into hydrophobic polypropylene nonwoven fabrics to be used as the top and bottom layer. Polypropylene is hydrophobic but after being fabricated into nonwoven fabrics, the porous structure enables the urine to leak to the absorbent interlayer of eco-diapers. Hence, the top layer of diaper does not contain urine, which makes smart diapers more comfortable than cloth diapers that are made of cotton or other moisture-absorbent materials. Moreover, the sensing mechanism via Bluetooth module can detect the water content of the interlayer with a view to improving the demerit of urine leakage. The interlayer is the sensing layer that has antibacterial function. Two types of antibacterial yarns are treated by zinc oxide and silver ions. The yarns are fabricated into antibacterial woven fabrics, after which the antibacterial properties of fabrics are investigated with quantitative and qualitative tests. Next, two parallel metallic wires are assembled in order to trigger short circuit when sensing moisture, thereby obtaining different electric resistance based on different moisture levels. Furthermore, the miniature senor can signify the cellular phones or buzzers when the two metallic wires generate electrical resistance due to the presence of urine. The metallic wires are silver-plated copper yarns, stainless steel fibers, and copper fibers, which possess different electric resistance for the corresponding miniature sensors. This study proposes an efficient manufacture of smart diapers that require only a combination of woven fabrics and two metallic wires to sense moisture, the design of which can be encompassed in diverse fields.

    Abstract: The mechanism of early blood–brain barrier (BBB) disruption after stroke has been intensively studied but still not fully understood. Here, we report that microRNA-30a (miR-30a) could mediate BBB damage using both cellular and animal models of ischemic stroke. In the experiments in vitro, inhibition of miR-30a decreased BBB permeability, prevented the degradation of tight junction proteins, and reduced intracellular free zinc in endothelial cells. We found that the zinc transporter ZnT4 was a direct target of negative regulation by miR-30a, and ZnT4/zinc signaling pathway contributed significantly to miR-30a-mediated BBB damage. Consistent with these in vitro findings, treatment with miR-30a inhibitor reduced zinc accumulation, increased the expression of ZnT4, and prevented the loss of tight junction proteins in microvessels of ischemic animals. Furthermore, inhibition of miR-30a, even at 90 min post onset of middle cerebral artery occlusion, prevented BBB damage, reduced infarct volume, and ameliorated neurological deficits. Together, our findings provide novel insights into the mechanisms of cerebral ischemia-induced BBB disruption and indicate miR-30a as a regulator of BBB function that can be an effective therapeutic target for ischemic stroke.

    Abstract: Finding appropriate drugs to improve cerebral autoregulation (CA) in patients with acute ischemic stroke (AIS) is necessary to improve prognosis. We aimed to investigate the effect of butylphthalide on CA in patients with AIS. In this randomized controlled trial, 99 patients were 2:1 randomized to butylphthalide or placebo group. The butylphthalide group received intravenous infusion with a preconfigured butylphthalide-sodium chloride solution for 14 days and an oral butylphthalide capsule for additional 76 days. The placebo group synchronously received an intravenous infusion of 100 mL 0.9% saline and an oral butylphthalide simulation capsule. The transfer function parameter, phase difference (PD), and gain were used to quantify CA. The primary outcomes were CA levels on the affected side on day 14 and day 90. Eighty patients completed the follow-up (52 in the butylphthalide group and 28 in the placebo group). The PD of the affected side on 14 days or discharge and on 90 days was higher in the butylphthalide group than in the placebo group. The differences in safety outcomes were not significant. Therefore, butylphthalide treatment for 90 days can significantly improve CA in patients with AIS. Trial registration: ClinicalTrial.gov: NCT03413202

    Abstract: Experimental investigations of the condensation and evaporation of R410A on the outside of two double-side enhanced heat transfer tubes were conducted, and the tube performance was compared with that of a smooth copper tube. The research object consisted of a smooth tube with a length of 2000 mm and an outer diameter of 25.4 mm and two double-side enhanced tubes of the same size. The evaporation experiments were run under the following conditions: the evaporation saturation temperature was 7°C, water inlet temperature was 8°C–18°C, and water flow rate was 0.6–1.6 m 3 /h. The condensation experiments were run under the following conditions: the saturation temperature was 35°C, water inlet temperature was 21°C–30°C, and water flow rate was 0.4–1.6 m 3 /h. During the data processing, the heat transfer coefficient of the water side was obtained using the Gnielinski-Wilson method, and that of the refrigerant side was obtained using the thermal resistance separation method. The effect of the surface structure on the heat transfer performance was analyzed, and a strengthening measure was proposed. The experimental results indicate that the evaporation heat transfer coefficients outside the smooth tube increased with the raise of the inlet water temperature, while the double-side enhanced tubes decreased; both the condensation heat transfer coefficients outside the smooth tube and the enhanced tubes increased with the increase of the water flow rate. Due to the different type of enhanced tube ribs, tube #3 surface structure is more conducive to the formation of vaporization core, which is about 30% higher than the outer heat transfer coefficient of tube #2. It is more in line with the requirements of strengthening horizontal single tube external pool boiling heat transfer. In the condensation experiment, the condensation heat transfer coefficients outside the smooth tube increased with the raise of the inlet water temperature, while the double enhanced tubes decreased; both the condensation heat transfer coefficients outside the smooth tube and the enhanced tubes increased with the increase of the water flow rate. With the analysis of the rib structure, the outer surface structure of tube #2 has stronger drainage capacity, and the external average condensation heat transfer coefficient is about 7% higher than that of tube #3, which is more in line with the requirements of condensing heat transfer outside the single tube.

    Abstract: Dexmedetomidine combined with opioids has been extensively used to blunt cardiovascular responses to endotracheal intubation. To determine their interaction, we aimed to develop a response surface model between dexmedetomidine and sufentanil. Methods One hundred and twenty patients undergoing scheduled gynaecological surgery were recruited. According to a simulation of slice design, patients received different dose pairs of dexmedetomidine (0 to 1.1 μg/kg) and sufentanil (.1 to .5 μg/kg). The mean arterial blood pressure and heart rate of patients were recorded just before endotracheal intubation, immediately after intubation, and during the first 3 min after intubation. The primary outcomes were haemodynamic changes. The full dose–response relationship between dexmedetomidine and sufentanil was analysed using a logit model. Results This response surface model revealed that the interaction between dexmedetomidine and sufentanil was additive. The dose pairs that could effectively attenuate the haemodynamic response to endotracheal intubation primarily ranged from .3 to .4 μg/kg and .5 to 1.1 μg/kg for sufentanil and dexmedetomidine, respectively. Conclusion When used propofol as the main hypnotic drug during anaesthesia induction, dexmedetomidine could effectively reduce the requirement of sufentanil in an additive manner. However, it is not an effective drug for ablating the cardiovascular response to endotracheal intubation when used alone. The clinical trial registry.     Abstract: Nasopharyngeal small cell carcinoma (SmCC) is a rare histological type of nasopharyngeal cancer, and its prognosis remains poor. This study aimed to determine the clinical characteristics and survival prognostic factors of nasopharyngeal SmCC. Methods Detailed clinicopathologic and therapeutic characteristics of a patient diagnosed with nasopharyngeal SmCC were determined. Nasopharyngeal SmCC cases reported previously were reviewed and summarized. Furthermore, a retrospective analysis was performed on data from the Surveillance, Epidemiology, and End Results (SEER) Program database. Kaplan–Meier analysis was conducted to compare survival within groups. Univariate and multivariate analyses were performed to investigate prognostic factors. Results A nasopharyngeal SmCC patient treated with chemoradiotherapy who achieved 46 months long-term survival was reported. In reviewing 16 reported cases with epidemiologic and therapeutic details, we found most of nasopharyngeal SmCC patients were diagnosed with advanced grades and received chemoradiotherapy. In total, 13,993 cases of nasopharyngeal cancer were extracted from the SEER database, from which 57 nasopharyngeal SmCC cases were eventually screened out. The mean age of the patients was 55.70 years, and 64.9% of these cases were either grade III or IV; the median overall survival (OS) was 18 months. Statistically significant differences were observed in the OS values of groups categorized by age (P = .025) or radiotherapy (P = .037). Age ( 〈 70 years) and radiotherapy were identified as independent survival and prognostic factors. Conclusion Patients with nasopharyngeal SmCC are usually diagnosed with advanced grades and have poor prognoses; nevertheless, they can benefit from radiotherapy with prolonged overall survival.
