    Abstract: Balance problems are common in patients with stroke, and the Mini-Balance Evaluation Systems Test (Mini-BESTest) is a reliable and valid assessment tool for measuring balance function. Determining the minimal clinically important difference (MCID) is crucial for assessing treatment effectiveness. This study aimed to determine the MCID of the Mini-BESTest in patients with early subacute stroke. Methods In this prospective multicenter study, 53 patients with early subacute stroke undergoing rehabilitation in inpatient units were included. The mean age of the patients was 72.6 (SD = 12.2) years. The Mini-BESTest, which consists of 14 items assessing various aspects of balance function, including anticipatory postural adjustments, postural responses, sensory orientation, and dynamic gait, was used as the assessment tool. The global rating of change (GRC) scales completed by the participants and physical therapists were used as external anchors to calculate the MCID. The GRC scale measured subjective improvement in balance function, ranging from −3 (very significantly worse) to +3 (very significantly better), with a GRC score of ≥ + 2 considered as meaningful improvement. Four methods were used to calculate the MCID: mean of participants with GRC of 2, receiver operating characteristic–based method, predictive modeling method, and adjustment of the predictive modeling method based on the rate of improvement. From the MCID values obtained using these methods, a single pooled MCID value was calculated. Results The MCID values for the Mini-BESTest obtained through the 4 methods ranged from 3.2 to 4.5 points when using the physical therapist’s GRC score as the anchor but could not be calculated using the participant’s GRC score. The pooled MCID value for the Mini-BESTest was 3.8 (95% CI = 2.9–5.0). Conclusions The Mini-BESTest MCID obtained in this study is valuable for identifying improvements in balance function among patients with early subacute stroke. Impact Determination of the MCID is valuable for evaluating treatment effectiveness. The study findings provide clinicians with practical values that can assist in interpreting Mini-BESTest results and assessing treatment effectiveness.

    Abstract: CIZA J1358.9−4750 is a nearby galaxy cluster in the early phase of a major merger. The two-dimensional temperature map using XMM-Newton EPIC-PN observation confirms the existence of a high-temperature region, which we call the “hot region,” in the “bridge region” connecting the two clusters. The ∼500 kpc wide region between the south-east and north-west boundaries also has higher pseudo-pressure compared to the unshocked regions, suggesting the existence of two shocks. The southern shock front is clearly visible in the X-ray surface brightness image and has already been reported by Kato et al. (2015, PASJ, 67, 71). The northern one, on the other hand, is newly discovered. To evaluate their Mach number, we constructed a three-dimensional toy merger model with overlapping shocked and unshocked components in the line of sight. The unshocked and pre-shock intracluster medium (ICM) conditions are estimated based on those outside the interacting bridge region, assuming point symmetry. The hot-region spectra are modeled with two-temperature thermal components, assuming that the shocked condition follows the Rankin–Hugoniot relation with the pre-shock condition. As a result, the shocked region is estimated to have a line-of-sight depth of ∼1 Mpc with a Mach number of ∼1.3 in the south-east shock and ∼1.7 in the north-west shock. The age of the shock waves is estimated to be ∼260 Myr. This three-dimensional merger model is consistent with the Sunyaev–Zel’dovich signal obtained using the Planck observation within the cosmic microwave background fluctuations. The total flow of the kinetic energy of the ICM through the south-east shock was estimated to be ∼2.2 × 1042 erg s−1. Assuming that $10\%$ of this energy is converted into ICM turbulence, the line–of–sight velocity dispersion is calculated to be ∼200 km s−1, which is basically resolvable via upcoming high spectral resolution observations.

    Abstract: To examine associations between perceived neighborhood social environment (PNSE) and sleep, mediated by physical activity (PA) and psychosocial stressors. Methods A sample (n = 4705) of African Americans (mean age 55.0 years; 63.4% female) enrolled in the 2000–2004 Jackson Heart Study (JHS). Four self-reported sleep outcomes were analyzed: sleep duration (minutes/night), sleep quality (high/low), short sleep (short ≤ 6 h vs recommended = 7–8 h), and long sleep (long ≥ 9 h vs recommended). PNSE factors included violence (e.g. robbery), problems (trash/litter), and social cohesion (trusting neighbors). PA and psychosocial stressors (lifetime and everyday discrimination, perceived stress, and depressive symptoms) were tested as mediators. With bootstrap-generated 95% bias-corrected confidence intervals (BC CIs), linear regression was used to test for mediation adjusting for covariates. Results Neighborhood violence and problems were associated with sleep duration, mediated through PA (B = −1.97, 95%BC CI = −3.76, −0.60; B = −1.23, 95%BC CI = −2.55, −0.27, respectively), lifetime discrimination (B = 2.61, 95%BC CI = 0.93, 4.80; B = 2.25, 95%BC CI = 0.93, 3.94), perceived stress (B = −3.08, 95%BC CI = −6.20, −0.41; B = −2.17, 95%BC CI = −4.33, −0.28), and depressive symptoms (B = −2.22, 95%BC CI = −5.09, −0.25; B = −1.94, 95%BC CI = −4.10, −0.35). Social cohesion was positively associated with sleep duration, mediated through PA, lifetime discrimination, and perceived stress. Similar patterns were demonstrated for binary outcomes. Yet, effect sizes were relatively small. PNSE was neither directly nor indirectly associated with sleep outcomes by everyday discrimination. Conclusions Each PNSE factor was associated with sleep outcomes, mediated by PA and psychosocial stressors. Further research should emphasize effective community efforts to decrease adverse neighborhood conditions and psychosocial factors and increase PA; thereby reducing CVD events for African Americans.

    Abstract: Finerenone, a non-steroidal mineralocorticoid receptor antagonist, improved kidney, and cardiovascular outcomes in patients with CKD and T2D in two Phase 3 outcome trials. The FIND-CKD study investigates the effect of finerenone in adults with CKD without diabetes. Methods FIND-CKD (NCT05047263 and EU CT 2023–506897-11–00) is a randomized, double-blind, placebo-controlled Phase 3 trial in patients with CKD of non-diabetic aetiology. Adults with a urinary albumin-creatinine ratio (UACR) of ≥ 200 to ≤3500 mg/g and eGFR ≥ 25 to & lt;90 mL/min/1.73 m2 receiving a maximum tolerated dose of a renin-angiotensin-system (RAS) inhibitor were randomized 1:1 to once daily placebo or finerenone 10 or 20 mg depending on eGFR above or below 60 mL/min/1.73 m2. The primary efficacy outcome is total eGFR slope, defined as the mean annual rate of change in eGFR from baseline to Month 32. Secondary efficacy outcomes include a combined cardiorenal composite outcome comprising time to kidney failure, sustained ≥57% decrease in eGFR, hospitalization for heart failure, or cardiovascular death, as well as separate kidney and cardiovascular composite outcomes. Adverse events are recorded to assess tolerability and safety. Results Across 24 countries, 3231 patients were screened and 1584 were randomized to study treatment. The most common causes of CKD were chronic glomerulonephritis (57.0%) and hypertensive/ischaemic nephropathy (29.0%). Immunoglobulin A nephropathy was the most common glomerulonephritis (26.3% of the total population). At baseline, mean eGFR and median UACR were 46.7 mL/min/1.73 m2 and 818.9 mg/g, respectively. Diuretics were used by 282 participants (17.8%), statins by 851 (53.7%), and calcium channel blockers by 794 (50.1%). SGLT2 inhibitors were used in 16.9% of patients; these individuals had a similar mean eGFR (45.6 vs 46.8 mL/min/1.73 m2) and slightly higher median UACR (871.9 vs 808.3 mg/g) compared to those not using SGLT2 inhibitors at baseline. Conclusions FIND-CKD is the first Phase 3 trial of finerenone in patients with CKD of non-diabetic aetiology.

    Abstract: Most vertebrate sex-determining genes (SDGs) emerge as neofunctionalized genes through duplication and/or mutation of ancestral genes that are involved with sexual differentiation. We previously demonstrated dm-W to be the SDG in the African clawed frog Xenopus laevis and found that a portion of this gene emerged from the masculinization gene dmrt1 after allotetraploidization by interspecific hybridization between two ancestral species around 17–18 Ma. dm-W has four exons consisting of a noncoding exon 1, dmrt1-derived exons 2 and 3, and an orphan exon 4 (Ex4) of unknown origin that includes coding sequence (CDS). In this study, we searched for the origin of Ex4 and investigated the function of the CDS of this exon. We found that the Ex4-CDS is derived from a noncoding portion of the hAT-10 family of DNA transposon. Evolutionary analysis of transposons and determination of the Ex4 sequences from three other species indicated that Ex4 was generated before the diversification of most or all extant allotetraploid species in subgenus Xenopus, during which time we hypothesize that transposase activity of this hAT superfamily was active. Using DNA–protein binding and transfection assays, we further demonstrate that the Ex4-encoded amino acid sequence increases the DNA-binding ability and transrepression activity of DM-W. These findings suggest that the conversion of the noncoding transposon sequence to the CDS of dm-W contributed to neofunctionalization of a new chimeric SDG in the ancestor of the allotetraploid Xenopus species, offering new insights into de novo origin and functional evolution of chimerical genes.

    Abstract: To externally validate the utility of the albumin, C-reactive protein and lactate dehydrogenase model to predict the overall survival of previously treated metastatic renal cell carcinoma patients. Patients and methods The ability of the albumin, C-reactive protein and lactate dehydrogenase model to predict overall survival was validated and compared with those of other prognostication models using data from 421 metastatic renal cell carcinoma patients receiving second-line axitinib therapy at 36 hospitals belonging to the Japan Urologic Oncology Group. Results The following factors in this cohort were independently associated with poor overall survival in a multivariate analysis: a low Karnofsky performance status, & lt;1 year from diagnosis to targeted therapy, a high neutrophil count, and low albumin, elevated C-reactive protein, and elevated lactate dehydrogenase, and the Japan Urologic Oncology Group model was newly developed based on the presence/absence of these independent factors. In this cohort, 151 (35.9%), 125 (27.7%) and 145 (34.4%) patients were classified into the favorable, intermediate and poor risk groups, respectively, according to the albumin, C-reactive protein and lactate dehydrogenase model; however, the proportions of patients in the intermediate risk group stratified by the Japan Urologic Oncology Group, Memorial Sloan Kettering Cancer Center and International Metastatic Renal Cell Carcinoma Database Consortium models were  & gt;50%. The superiority of the albumin, C-reactive protein and lactate dehydrogenase model to the Memorial Sloan Kettering Cancer Center and International Metastatic Renal Cell Carcinoma Database Consortium models, but not the Japan Urologic Oncology Group model, was demonstrated by multiple statistical analyses. Conclusions The utility of the albumin, C-reactive protein and lactate dehydrogenase model as a simple and objective prognostication tool was successfully validated using data from 421 metastatic renal cell carcinoma patients receiving second-line axitinib.

    Abstract: We present an electrodeposition method for fabricating thiolated polymer-based hydrogels through oxidation of hydroquinone (HQ). HQ is oxidized to benzoquinone (BQ) at an electrode, and the generated BQ is utilized for disulfide formation through an electrocatalytic reaction. As a simple demonstration of bioapplications, an electrodeposited hydrogel containing glucose oxidase was applied for glucose sensing. To the best of our knowledge, this is the first report on the electrodeposition of thiolated polymer-based hydrogels via disulfide formation using the indirect approach.
