    Abstract: We have explored the potential of auricular intradermal acupuncture (AIA) in standard rehabilitation and acupuncture treatment for motor recovery in poststroke patients. This was a randomized, controlled preliminary clinical study in which the patients were randomly assigned to the CT group (conventional treatment, standard rehabilitation, and routine acupuncture) or AIA group (AIA combined with conventional treatment) and underwent 6 sessions in 1 week (6 days). Standard procedures and previously reported acupuncture points were used. Clinical outcomes were measured by the Fugl-Meyer motor assessment (FMA) of flexor and extensor synergy movement (FSM and ESM) of the upper and lower extremities (UE and LE) at days 0, 3, and 6. The assessment was performed by blinded assessors. The AIA group showed a significant increase in FMA-UE/FMA-LE scores on day 3 ( P = 0.012 and 0.001, respectively) and day 6 ( P = 0.041 and P 〈 0.001 , respectively), but this was not observed in the CT group. Furthermore, unlike the CT group, the AIA group exhibited a significant increase in the FMA-LE score on day 3 ( P = 0.004 ) and the FMA-UE scores on day 6 ( P = 0.048 ). Finally, the correlation between ESM and FMA-UE/FMA-LE was higher than that between FSM and FMA-UE/FMA-LE after treatment: for ESM and UE, r  = 0.759, P = 0.007 ; for ESM and LE, r  = 0.697, P = 0.003 ; for FSM and UE, r  = 0.604, P = 0.049 ; for FSM and LE, r  = 0.347, P = 0.188 . AIA is useful for motor rehabilitation in poststroke patients, particularly in terms of improving extensor synergy. This trial is registered with CHiCTR1800020150 .

    Abstract: Objective. To better understand the status of medical treatment for human epidermal growth factor receptor 2 (HER2)-positive breast cancer and the differences between the Chinese and the international clinical practice. Methods. This was a retrospective, nationwide, multicenter, epidemiological study of advanced breast cancer patients from China. Between January 01, 2012, and December 31, 2014, a total of 3649 patients, covering 7 geographic regions and 21 institutions, participated in this series of studies. HER2-positive breast cancer was selected among the group and adopted into this study. In comparison, we summarized the demographics and clinical characteristics of HER2-positive breast cancer from the Surveillance, Epidemiology, and End Results (SEER) database. Results. A total of 918 patients diagnosed as HER2-positive breast cancer patients were included. The median age at diagnosis was 46 years (ranging, 23 to 78) with a single-peak incidence. The proportions of stages II–IV at diagnosis and distance metastasis in viscera were more than half of the participants. In comparison, the prevalence of estrogen or progesterone receptor-positive expression and luminalB subtype was relatively lower than that of the United States. The receipt of chemotherapy was fairly higher, while the usage of targeted therapy was seriously insufficient. Tumor size was in significantly positive associations with the duration of targeted therapy (Kendall’s correlation coefficient = 0.3, P 〈 0.0001 ), while no prohibitive variables among clinical characteristics were detected. Conclusion. Our study suggested that HER2-positive breast cancer patients were characterized as a younger trend, a lower prevalence of hormonal receptor (HR)-positive expression, and less accessible to anti-HER2 targeted therapy with insufficient duration over the past few years in China. Concerted efforts should be exerted for promising survival benefits in the future. The trial registration number is https://clinicaltrials.gov/ct2/show/NCT03047889.

    Abstract: Objectives. This study aimed to analyze the clinical features, antibiotic susceptibility profiles, and outcomes of patients with invasive pneumococcal disease (IPD) at a hospital in Ningxia Hui Autonomous Region, to provide the basis for improving the clinical treatment effect. Methods. Patients with IPD were retrospectively collected from 2013 to 2021. Clinical manifestations, laboratory tests, antimicrobial susceptibility, antibiotic treatment, and outcomes of the disease were analyzed. Results. In this study, we identified 127 IPD cases, of whom 49 (38.6%) had meningitis and 78 (61.4%) had bacteremia. The median ages of pediatric cases and adult cases were 2 years (IQR: 0–5) and 52.5 years (IQR: 35–62), respectively. There were 27 and 45 males in the pediatric and adult groups, and no significant gender difference in the different age groups ( p = 0.584 ) was found. Of 75 cases with underlying diseases, pneumonia (11%), malignancy (11%), hypertension (9.4%), and hepatic cirrhosis (7.9%) were the most common. The incidence of underlying diseases was even higher in the adult group (67.1%) than in the pediatric group (47.1%) ( p = 0.028 ). The frequency of fever, cough, and seizures was significantly higher in the pediatric group than in the adult group, with p -values of 0.004, 0.004, and 0.001, respectively. The percentage of neutrophils in the blood was significantly higher in the adult cases than in the pediatric cases ( p   〈   0.001 ). Furthermore, there was a significantly higher WBC count ( p   〈   0.001 ), percentage of neutrophils ( p = 0.012 ), and protein level ( p = 0.019 ) in the CSF samples in the adult patients compared to pediatric patients. The susceptibility rates of S. pneumoniae isolates to vancomycin, linezolid, and levofloxacin were 100%. The susceptibility rates of penicillin were 98.7% and 34.1% in bacteremia and meningitis patients, respectively. Most isolates were resistant to erythromycin, clindamycin, tetracycline, and azithromycin. The most common antibiotic treatment was β-lactams. Seven (5.5%) patients died during hospitalization, and 38 (29.9%) patients' health deteriorated. Conclusion. These results may provide a reference basis for the diagnosis and empiric treatment of IPD in the region.

    Abstract: Acupuncture has been practiced to treat neuropsychiatric disorders for a thousand years in China. Prevention of disease by acupuncture and moxibustion treatment, guided by the theory of Chinese acupuncture, gradually draws growing attention nowadays and has been investigated in the role of the prevention and treatment of mental disorders such as AD. Despite its well-documented efficacy, its biological action remains greatly invalidated. Here, we sought to observe whether preventive electroacupuncture during the aging process could alleviate learning and memory deficits in D-galactose-induced aged rats. We found that preventive electroacupuncture at GV20-BL23 acupoints during aging attenuated the hippocampal loss of dendritic spines, ameliorated neuronal microtubule injuries, and increased the expressions of postsynaptic PSD95 and presynaptic SYN, two important synapse-associated proteins involved in synaptic plasticity. Furthermore, we observed an inhibition of GSK3 β /mTOR pathway activity accompanied by a decrease in tau phosphorylation level and prompted autophagy activity induced by preventive electroacupuncture. Our results suggested that preventive electroacupuncture can prevent and alleviate memory deficits and ameliorate synapse and neuronal microtubule damage in aging rats, which was probably via the inhibition of GSK3 β /mTOR signaling pathway. It may provide new insights for the identification of prevention strategies of AD.

    Abstract: Background . Hyperuricemia has long been associated with increased cardiovascular risk, and arterial stiffness is proposed as a mediator. The present study is aimed at examining the associations of uric acid (UA) in blood and urine with arterial stiffness in a Chinese cohort. Methods . A total of 2296 participants (mean age: 43.0 years) from our previously established cohort of Hanzhong Adolescent Hypertension Study were included. The participants were classified as subjects with or without arterial stiffness, which was defined as brachial-ankle pulse wave velocity baPWV ≥ 1400   cm / s and/or carotid intima-media thickness CIMT ≥ 0.9   mm . Multivariate regression analyses were used to examine the relationship between serum and urinary UA and the risk of arterial stiffness after adjusting for age, gender, systolic blood pressure, fasting glucose, BMI, heart rate, total cholesterol, and triglycerides. Results . baPWV was positively correlated with urinary uric acid/creatinine ratio (uUA/Cre) ( β = 0.061 , P 〈 0.001 ), while CIMT was correlated with uUA/Cre ( β = 0.085 , P 〈 0.001 ) and fractional excretion of uric acid (FEUA) ( β = 0.044 , P = 0.033 ) in all subjects. In addition, uUA/Cre was significantly associated with the risk of high baPWV [1.032 (1.019-1.045)] and arterial stiffness [1.028 (1.016-1.040)] . Conclusion . Our study showed that urinary UA excretion was significantly associated with the risk of arterial stiffness in Chinese adults. These findings suggest that UA, especially urinary UA, may be used as a simple, noninvasive marker for early detection of arterial stiffness in otherwise healthy subjects.

    Abstract: Chronic hyperlipidemia leads to pancreatic β-cell apoptosis and dysfunction through inducing oxidative stress. Chikusetsu saponin IVa (CHS) showed antioxidant and antidiabetic properties in our previous studies; however, its protective effects against lipotoxicity-induced β-cell oxidative stress and dysfunction are not clear. This study was designed to investigate the effects of CHS against lipotoxicity-induced β-cell injuries and its possible mechanism involved. High-fat (HF) diet and a low dose of streptozotocin- (STZ-) induced type 2 diabetes mellitus (T2DM) model in vivo and βTC3 cells subjected to 0.5 mM palmitate (PA) to imitate the lipotoxic model in vitro were performed. Pancreatic functions, ROS, and antioxidant protein measurements were performed to evaluate the effects of CHS on cell injuries. Protein expression levels were measured by Western blotting. Furthermore, siRNA-targeted Nrf2, PI3K/Akt inhibitor (LY294002), or GSK-3β inhibitor (LiCl) was used to investigate the crosstalk relationships between proteins. As the results showed, CHS treatment inhibited apoptosis, promoted insulin release, and reduced oxidative stress. CHS treatment significantly increased the expression of Nrf2 in the cytoplasm and nuclear protein. The antioxidative and benefit effects of CHS were inhibited by siNrf2. The phosphorylation of IRS-2, PI3K, Akt, and GSK-3β was markedly increased by CHS which were inhibited by PA. In addition, inhibition of PI3K/Akt or GSK-3β with specific inhibitors dramatically abrogated the protective effects of CHS, revealing that the IRS-2/Akt/GSK-3β signaling axis was involved in the protective effects of CHS. These results demonstrate that CHS protected βTC3 cells against PA-induced oxidative stress and cell dysfunction through Nrf2 by the IRS-2/Akt/GSK-3β-mediated pathway.

    Abstract: Patients with type 2 diabetes have twice as much of the risk of acute ischemic stroke (AIS) occurrence as healthy individuals, and the AIS patients with type 2 diabetes have a higher risk of death and a poorer prognosis. This study was to investigate the interrelationship between hyperglycemia and AIS and provided a reference for blood glucose management of AIS patients. The blood glucose level of AIS patients of the present study was controlled by insulin below 180 mg/dL (standard group) and between 80 and 130 mg/dL (management group). And the fasting venous blood samples were collected for determination of blood glucose level, homeostasis model assessment of insulin resistance (HOMA-IR), peptide C, and basal insulin level. Furthermore, lipids of the blood samples were detected using metabolomics, so as to clarify the similarities and differences in metabolic patterns in AIS patients with diabetes after the intervention of different glycemic strategies. The results revealed that compared to the standard group, the blood glucose level and HOMA-IR in the management group were significantly decreased, and levels of peptide C and basal insulin level were greatly increased. Through lipidomics detection, 83, 50, and 44 types of significantly upregulated differential lipids were detected in the standard vs. normal groups, the standard vs. management groups, and the management vs. normal groups, respectively, with triacylglycerol dominated. This study preliminarily revealed metabolic differences among AIS patients with hyperglycemia after different blood glucose intervention methods, hoping to provide a theoretical basis for clinical prevention and treatment of this disease.

    Abstract: Background. Non-small-cell lung cancer (NSCLC) is a major health problem that endangers human health. The prognosis of radiotherapy or chemotherapy is still unsatisfactory. This study is aimed at investigating the predictive value of glycolysis-related genes (GRGs) on the prognosis of NSCLC patients with radiotherapy or chemotherapy. Methods. Download the clinical information and RNA data of NSCLC patients receiving radiotherapy or chemotherapy from TCGA and geo databases and obtain GRGs from MsigDB. The two clusters were identified by consistent cluster analysis, the potential mechanism was explored by KEGG and GO enrichment analyses, and the immune status was evaluated by estimate, TIMER, and quanTIseq algorithms. Lasso algorithm is used to build the corresponding prognostic risk model. Results. Two clusters with different GRG expression were identified. The high-expression subgroup had poor overall survival. The results of KEGG and GO enrichment analyses suggest that the differential genes of the two clusters are mainly reflected in metabolic and immune-related pathways. The risk model constructed with GRGs can effectively predict the prognosis. The nomogram combined with the model and clinical characteristics has good clinical application potential. Conclusion. In this study, we found that GRGs are associated with tumor immune status and can assess the prognosis of NSCLC patients receiving radiotherapy or chemotherapy.

    Abstract: Carbonate reservoirs typically have complex pore structures, so the production wells typically have high production in the early production stage, but they decline rapidly. It is highly challenging to achieve accurate interpretation results. In this paper, a new and practical methodology for production data analysis of fractured and fractured-vuggy carbonate reservoirs is proposed. Firstly, analytical solutions to characterize the different multipore media and simulate transient production behavior of fractured and fractured-vuggy carbonate reservoirs during the transient flow regime are presented. Then, a new function f q D and f ′ q D that related to the dimensionless production rate is introduced, and a series of new decline type curves are drawn to make a clear observation of different flow regimes. In addition, the effects of the storativity ratio, interporosity flow coefficient, skin factor, and dimensionless radial distance of external boundary on production performance are also analyzed. Finally, two example wells from the fractured and fractured-vuggy carbonate reservoirs are used to perform rate decline analysis with both the Blasingame type curves and the new type curves. The validation of the new method is demonstrated in comparison to the results of well test interpretation. The results show that the curves of 1 / f ′ q D vs. t D are ∧ -shaped for dual-porosity reservoirs and M -shaped for triple porosity reservoirs and also indicate that the interpreted parameters such as permeability, skin factor, storativity ratio, and interporosity flow coefficient using new decline type curves are aligned well test interpretation. In correlation with other traditional well test analysis, this approach effectively reduces the multisolution probability of interpretation.
