    Description / Table of Contents: Summary To investigate the role of the renin angiotensin system in the pathogenesis of hypertension in Cushing's syndrome two patients with hypercorticism were infused with 20 mg saralasin (1-sar-8-alaangiotensin II) over a period of 30 minutes under constant blood pressure control. In addition, one patient with primary aldosteronism, an established form of mineralocorticoid hypertension, served as control. Neither in the two patients with Cushing's syndrome nor in the patient with primary aldosteronism could a blood pressure lowering effect of saralasin be observed. In the two patients with hypercorticism both renin activity and plasma aldosterone increased during saralasin infusion. The patient with primary aldosteronism only showed a weak increase in plasma aldosterone concentration. These results seem to exclude an important role of the renin angiotensin system in the pathogenesis of hypertension in Cushing's syndrome. The unresponsiveness of elevated blood pressure to saralasin in the two patients with hypercorticism and in the patient with primary aldosteronism indirectly supports the assumption that in patients with Cushing's syndrome increased mineralocorticoid activity may be the main factor in the pathogenesis of hypertension.
    Notes: Zusammenfassung Um die Bedeutung des Renin-Angiotensin Systems in der Pathogenese der Hypertonie bei Cushing-Syndrom zu untersuchen, wurden bei 2 Patienten mit Hyperkortizismus 20 mg Saralasin (1-Sar-8-Ala-Angiotensin II) über einen Zeitraum von 30 min unter ständiger Blutdruckkontrolle infundiert. Zusätzlich diente ein Patient mit primärem Aldosteronismus, einer etablierten Form von Mineralokortikoidhochdruck, als Kontrolle. Weder bei den 2 Patienten mit Cushing-Syndrom noch bei dem Patienten mit primärem Aldosteronismus ließ sich ein blutdrucksenkender Effekt des Saralasins nachweisen. Die beiden Patienten mit Hyperkortizismus zeigten unter Saralasin sowohl einen Anstieg der Renin-Aktivität als auch des Plasmaaldosterons. Bei dem Patienten mit primärem Aldosteronismus ließ sich nur ein geringgradiger Anstieg der Plasmaaldosteronkonzentration nachweisen. Diese Ergebnisse sprechen gegen eine wichtige Rolle des Renin-Angiotensin Systems in der Pathogenese der Hypertonie bei Cushing-Syndrom. Die Unbeeinflußbarkeit des Hochdrucks durch Saralasin bei den beiden Patienten mit Hyperkortizismus und dem Patienten mit primärem Aldosteronismus stützen indirekt die Annahme, daß bei Patienten mit Cushing-Syndrom eine erhöhte Mineralokortikoidaktivität der Hauptfaktor in der Pathogenese der Hypertonie ist.

    Notes: Zusammenfassung Eine 34jährige Patientin stellte sich mit anfallsartigen Kopfschmerzen und Hypertonie vor. Der klinische Verdacht eines Phäochromozytoms ließ sich bestätigen. Danben bestand bei Zunahme des Gesichtsumfanges, verändertem Hautturgor, Hypokaliämie und Hyperglykämie der Verdacht eines Hyperkortisolismus, der laborchemisch gesichert wurde. Als Ursache des Hyperkortisolismus konnte eine paraneoplastische CRH-Produktion diagnostiziert werden. Postoperativ kam es zu einer raschen Normalisierung des Kortisolstoffwechsels. Bei regelmäßigen Nachkontrollen ergab sich bisher kein Hinweis für ein Rezidiv.

    Notes: Abstract The National Institute of Standards and Technology (NIST) has developed several Standard Reference Materials (SRMs) based on human serum. NIST SRM 909b, Human Serum, is a lyophilized human serum material with concentrations for seven organic and six inorganic analytes at two levels certified solely by definitive methods (DMs). This material provides the vehicle by which high precision, high accuracy measurements made with DMs at NIST can be transferred through the measurement hierarchy to other laboratories. Isotope dilution gas chromatographic-mass spectrometric (GC-IDMS) methods were applied to measure cholesterol, creatinine, glucose, urea, uric acid, triglycerides, and total glycerides. Thermal ionization isotope dilution mass spectrometry (TI-IDMS) was used for determination of lithium, magnesium, potassium, calcium, and chloride. In addition, chloride was determined by coulometry, providing a comparison between two DMs. Sodium, which lacks a stable isotope that would permit isotope dilution mass spectrometric (IDMS) measurement, was determined by gravimetry. SRM 909b includes certified values for total glycerides and triglycerides, which were not certified in the previous lot of this material (SRM 909a). Improvement in uniformity of vial fill weight in the production of SRM 909b resulted in smaller certified uncertainties over previous freeze-dried serum SRMs. Uncertainties at the 99% level of confidence for relative expanded uncertainty (%) for certification of the organic analytes on a mmol/L/g basis ranged from 0.44% for urea (level II) to 5.04% for glucose (level II). (In-house studies have shown glucose to be a relatively unstable analyte in similar lyophilized serum materials, degrading at about 1% per year.) Relative expanded uncertainties (99% C.I.) for certification of inorganic analytes on a mmol/L/g basis ranged from 0.25% for chloride (level I) to 0.49% for magnesium (level II).

    Notes: Abstract. Chloroplasts isolated from spinach (Spinacia oleracea L.) leaves and green sweet-pepper (Capsicum annuum L. var. grossum (L.) Sendt.) fruits contain NADP-dependent malate dehydrogenase (MDH; EC 1.1.1.82) and the bispecific NAD(P)-glyceraldehyde 3-phosphate dehydrogenase (GAPDH; EC 1.2.1.13). The NADP-dependent MDH and GAPDH are activated in the light, and inactive in the dark. We found that chloroplasts possess additional NAD-dependent MDH activity which is, like the NAD-dependent GAPDH activity, not influenced by light. In heterotrophic chromoplasts from red sweet-pepper fruits, the NADP-dependent MDH and the NAD(P)-GAPDH isoenzymes disappear during the developmental transition and only NAD-specific isoforms are found. Spinach chloroplasts contain both NAD/H and NADP/H at significant concentrations. Measurements of the pyridine dinucleotide redox states, performed under dark and various light conditions, indicate that NAD(H) is not involved in electron flow in the light. To analyze the contribution of NAD(H)-dependent reactions during dark metabolism, plastids from spinach leaves or green and red sweet-pepper fruits were incubated with dihydroxyacetone phosphate (DHAP). Exogenously added DHAP was oxidized into 3-phosphoglycerate by all types of plastids only in the presence of oxaloacetate, but not with nitrite or in the absence of added electron acceptors. We conclude that the NAD-dependent activity of GAPDH is essential in the dark to produce the ATP required for starch metabolism; excess electrons produced during triose-phosphate oxidation can selectively be used by NAD-MDH to form malate. Thus NADPH produced independently in the oxidative pentose-phosphate pathway will remain available for reductive processes inside the plastids.

    Notes: Abstract Purpose: To examine the efficacy of the low-molecular-weight heparin, reviparin, for prevention of femoropopliteal stent restenosis. Methods: Forty-two patients who had implantation of flexible tantalum stents for the treatment of stenosis (n= 24) or occlusion (n= 18) of the femoral (n= 27) or popliteal (n= 15) arteries were included in this study protocol. An intraarterial bolus of 5000 IU heparin was given before percutaneous transluminal angioplasty (PTA), and in the case of stent implantation due to unsuccessful PTA, an additional dose of reviparin (3500 anti-factor Xa IU) was given. Postprocedurally, 10,500 anti-factor Xa IU of reviparin were administered intravenously over 24 hr, followed by 3500 anti-factor Xa IU subcutaneously twice a day for 23 days. Oral aspirin (100 mg/day) was prescribed for the long term. Follow-up criteria (maximum follow-up 37 months) were clinical symptoms, Doppler ankle arm indices, color and duplex sonography, and angiography for suspicion of restenosis. Results: Early stent thromboses were not observed. Overall primary patency rate (PPR) was 88% ± 6.0% (1 year) and 74% ± 10.1% (2 years). Major hemorrhagic complications have not occurred. Conclusion: Reviparin administered in a high dose over a period of 24 days is a safe medication regimen and provides excellent patency rates after stent implantation.

    Notes: Abstract Total sulfur (STOT), elemental sulfur (S°) and sulfur-isotope compositions (δ34S) of marine animals were analyzed to determine whether these chemical characteristics could help distinguish animals with a sulfur-based, thiotrophic nutrition from animals whose nutrition is based on methanotrophy or on more normal consumption of phytoplankton-derived organic matter. The presence of S° was almost entirely confined to the symbiont-containing tissues of thiotrophs, but was sometimes undetectable in thiotrophic species where sulfide availability was probably low. When S° contents were subtracted, the remaining tissue-sulfur concentrations were similar for all nutritional groups. δ34S values were typically lower for thiotrophs than for other groups, although there was overlap in methanotroph and thiotroph values at some sites. Field evidence supported the existence of small to moderate (1 to 10‰)34S fractionations in the uptake of sulfides and metabolism of thiosulfate. In general, a total sulfur content of 〉3% dry weight, the presence of elemental sulfur, and δ34S values less than +5‰ can be used to infer a thiotrophic mode of nutrition.

    Description / Table of Contents: Summary The effect of acute (intravenous) β-adrenergic blockade with propranolol or pindolol on arterial pressure (BP), plasma renin activity (PRA), and plasma concentration of aldosterone (PA) was evaluated in 20 essential hypertensive men. BP, PRA and PA were determined during continuous recumbency overnight (8 p.m. to 6 a.m.) every 30 min. Two groups of patients were observed. Patients of group I exhibited a characteristic day-night rhythm of PRA with low values before midnight and large increases early in the morning. Conversely, no rhythm and very low PRA values were observed in patients of group II. BP was higher in group II than in group I. In group I following intravenous propranolol or pindolol, BP fell within minutes and levels as well as rhythms of PRA were converted to those of group II without treatment. In group II day-night profiles of PRA and BP remained unchanged. Rhythm and concentration of PA in the two groups were not influenced by either drug. In 4 patients of group I infusion of angiotensin II inhibitor did not lower BP. The observations suggest that in the two groups dissimilarities in rhythms of PRA as well as in BP responses to β-blockade may reflect differences in neuro-adrenergic tone.
    Notes: Zusammenfassung Der Einfluß einer akuten (intravenösen) β-Rezeptorenblockade mit Propranolol oder Pindololauf den Blutdruck (RR), die Plasma-Reninaktivität (PRA) und die Plasma-Aldosteronkonzentration (PA) wurde bei 20 Männern mit essentieller Hypertension untersucht. RR, PRA und PA wurden am liegenden Patienten nachts (20.00–6.00 Uhr) alle 30 min bestimmt. Zwei Gruppen von Patienten konnten unterschieden werden: Patienten der Gruppe I wiesen einen charakteristischen Tag-Nacht-Rhythmus in der Plasma-Reninaktivität auf, mit niedrigen Werten vor Mitternacht und hohen Werten am frühen Morgen. Im Gegensatz hierzu hatten die Patienten der Gruppe II sehr niedrige PRA-Werte; ein Rhythmus für PRA ließ sich nicht nachweisen. RR war höher in der Gruppe II als in Gruppe I. Nach Infusion von Propranolol oder Pindolol kam es in der Gruppe I zu einem schnellen RR-Abfall. Das Verhalten des Tag-Nacht-Profils für PRA war nach der β-Blockade vergleichbar mit demjenigen der Gruppe II vor der β-Blockade. In der Gruppe II blieben RR und Tag-Nacht-Profil von PRA unter β-Blockade unverändert. Rhythmus und Konzentration von PA wurden in beiden Gruppen nicht beeinflußt. Bei 4 Patienten der Gruppe I führte Angiotensin II-Blockade zu keiner RR-Senkung. Die Ergebnisse sind mit der Annahme vereinbar, daß die Unterschiede im Renin-Rhythmus und im Blutdruckverhalten nach akuter β-Rezeptorenblockade durch eine unterschiedliche neuroadrenerge Aktivität der beiden Gruppen bedingt sind.

    Notes: Abstract We investigated the association between cerebral venous thrombosis and hereditary resistance to activated protein C (APC) in 12 consecutive German patients with non-fatal cerebral venous thrombosis and in 187 controls without a history of thrombotic disorder. Three patients (25%) had a mutation in the factor V Leiden gene against only one subject in the control group. This difference was significant (P〈0.05), with an odds ratio of 11.7 (1.5–87 ; 95% confidence interval). Two patients carrying the mutation had additional common risk factors for thrombosis, and 2 had a positive family history of thromboembolism. We conclude that inherited APC resistance by a mutation in factor V Leiden is an important risk factor in non-fatal cerebral venous thrombosis. We recommend testing for APC resistance and, if abnormal for factor V Leiden mutation in patients with cerebral venous thrombosis.

    Notes: Abstract This study investigates sarcoplasmic reticulum (SR) calcium-(Ca2+) transport ATPase (SERCA2a) and phospholamban (PLB) in cultured spontaneously contracting neonatal rat cardiomyocytes (CM) to ascertain the function of both SR proteins under various culture conditions. The two major SR proteins were readily detectable in cultured CM by immunofluorescent microscopy using specific anti-SERCA2 and anti-PLB antibodies. Double labeling technique revealed that PLB-positive CM also labeled with anti-SERCA2. Coexpression of SERCA2 and PLB in CM was supported by measurement of cell homogenate oxalate-supported Ca2+ uptake which was completely inhibited by thapsigargin and stimulated by protein kinase A-catalyzed phosphorylation. Under serum-free conditions, incubation of CM with the SERCA2a expression modulator 3,3′,5-triiodo-L-thyronine (100 nM, 72 h) resulted in elevated Ca2+ uptake of +33%. Specific Ca2+ uptake activity was not altered if insulin was omitted from the serum-free culture medium but total SR Ca2+ transport activity was reduced under this culture condition. The results indicate that primary culture of spontaneously contracting neonatal rat CM can be employed as a useful model system for investigating both short- and long-term mechanisms determining the Ca2+ re-uptake function of the SR under defined culture conditions.
