    Abstract: The genomic variant features (mutations, deletions, structural variants, etc.) within gastric cancer impact its evolution and immunogenicity. The tumor has developed several coping strategies to respond to these changes by DNA repair and replication (DRR). However, the intrinsic relationship between the associated DRR-related genes and gastric cancer progression remained unknown. This study selected DRR-related genes with tumor mutation burden based on the TCGA (The Cancer Genome Atlas) database of gastric cancer transcriptome and mutation data. The prognosis model of seven genes (LAMA2, CREB3L3, SELP, ABCC9, CYP1B1, CDH2, and GAMT) was constructed by a univariate and LASSO regression analysis and divided into high-risk and low-risk groups with the median risk score. Survival analysis showed that overall survival (OS) was lower in the high-risk group than that in the low-risk group. Moreover, patients with gastric cancer in the high-risk group have worse survival in different subgroups, including age, gender, histological grade, and TNM stage. The nomogram that included risk scores for DRR-related genes could accurately foresee OS of patients with gastric cancer. Interestingly, the tumor mutation burden score was higher in the low-risk group than that in the high-risk group, and the risk score for DRR-related genes was negatively correlated with tumor mutation burden in gastric cancer. Next, we further combined the risk score and tumor mutation burden to evaluate the prognosis of gastric cancer patients. The low-risk cohort had a better prognosis than the high-risk cohort in the high tumor mutation burden subgroup. The number of mutation types in the high-risk group was lower than that in the low-risk group. In the immune microenvironment of gastric cancer, more naïve B cells, memory resting CD4+ T cells, Treg cells, monocytes cells, and resting mast cells were infiltrated in the high-risk group. At last, PD-L1 and IAP expressions were negatively correlated with the risk scores; patients with gastric cancer in the low-risk group showed better immunotherapy outcomes than those in the high-risk group. Overall, the DRR-related gene signature based on tumor mutation burden is a novel biomarker for prognostic and immunotherapy response in patients with gastric cancer.

    Abstract: In this work, some novel optical solutions for the ( 1 + 1 )-dimensional generalized M -fractional coupled nonlinear Schrödinger system (GMFCNLS) arising in ocean engineering, plasma waves, and nonlinear optics have been investigated. After utilizing a modified ( G ′ / G , 1 / G )-expansion method and the G ′ / b G ′ + G + a -expansion method, many types of novel optical solutions including the bell-shape soliton solutions, the blow-up solutions, the periodic wave solutions, and the mixed solitary wave solutions are obtained; if we select different values of wave velocity, coefficients, and orders, the dynamic properties and physical structures of these optical solutions are simulated and discussed, which can help us to further understand the inner structure of the system.

    Abstract: Packet loss-resilient and security are two major challenges faced by real-time audio transmission over IP networks. Due to the capability of recovering the signal from a small set of samplings and the randomness in the acquisition process, compressive sensing (CS) has a vast prospect in dealing with these problems. In this paper, we propose a secure and packet loss-resistant real-time audio transmission framework (CS-SPT) based on the principle of CS. Inspired by the interleaving technique, an ultralow complexity scrambling matrix was adopted in the proposed CS-SPT to improve its packet loss-resilient capability by increasing the information redundancy. Moreover, the energy of ciphertext is homogenized using a diffusion operation. Experimental results show that compared with existing methods, the proposed CS-SPT not only improves the packet loss-resilient ability significantly but also can resist several major attacks, such as COAs and KPAs.

    Abstract: Objective . To investigate the clinical features and potential risk factors of aneurysmal lesions in Behcet’s disease (BD). Methods . We retrospectively reviewed the clinical data of BD patients with aneurysmal lesions in our institute from 1997 to 2017 and compared them with 207 BD patients without aneurysmal lesions. The treatment and outcome of these patients were also analyzed. Results . Sixty-nine patients were included with 117 aneurysmal lesions. The average period between BD onset and diagnosis of aneurysmal lesion was 5.4 ± 5.5 years. Thirty-three patients (47.8%) had multiple aneurysmal lesions. Ten patients developed 20 pulmonary artery aneurysms alone. For the other 97 aortic and/or peripheral artery aneurysms in 59 patients, the most commonly affected vessels were abdominal aorta (27/97, 27.8%), coronary artery (10/97, 10.3%), and superficial femoral artery (8/97, 8.2%). Multivariate analysis revealed pathergy reaction ( OR = 3.78 (1.70-8.41)), arterial stenosis or occlusion ( OR = 44.12 (11.56-168.35)), and arterial thrombosis ( OR = 9.27 (2.33-36.93)) as independent predictors of aneurysmal lesions in BD. With a mean follow-up of 5.5 ± 4.0 years, 40 patients (58.0%) achieved clinical improvements, 15 patients (21.7%) relapsed, and 10 patients (14.5%) died. The respective estimated cumulative 1- and 5-year relapse-free rates were 91.3% and 76.3%, and the respective estimated 1- and 5-year survival rates were 95.0% and 87.2%. Conclusion . Aneurysmal lesions are severe complications in BD. Pathergy reaction, arterial stenosis or occlusion, and arterial thrombosis are the risk factors of aneurysmal lesions in BD. Achieving BD remission and performing surgical or interventional procedures are both important in the treatment of these patients.

    Abstract: To investigate the clinical features of parenchymal neuro-Behcet’s disease (p-NBD), we retrospectively reviewed the medical records of 1009 BD patients admitted to Peking Union Medical College Hospital from 2000 to 2016. Forty-two patients (25 males and 17 females) with p-NBD and eighty-four age- and sex-matched BD patients without neurological involvement who were served as controls were enrolled. Neurological onset was concomitant with the onset of BD in six cases (14.3%). Pyramidal signs (50.0%) and headache (33.3%) were the most common manifestations. On MRI, the lesions were mainly in the midline structures and hyperintense in the T2-weighted image. The most common lesion was the brainstem (54.8%). Spinal cord involvement was observed in five cases, four of which with cervical cord involvement. Multifocal lesions were observed in 13 patients. Ocular involvement was more prevalent in p-NBD (35.7%) ( P = 0.041 , OR = 2.36 , 95% CI = 1.03 -5.44) compared with controls. All patients received corticosteroids and immunosuppressants, mainly cyclophosphamide (39/42). Six patients with severe/refractory condition received biological agents and achieved response measured by decreased Rankin score ( P = 0.002 ). With a median follow-up of 28 months, 22 patients (61.1%) achieved clinical improvements, while 10 (27.8%) relapsed and 4 died (mortality rate 11.1%). p-NBD is a rare yet disabling and life-threatening complication of BD. Ocular involvement is a risk factor for p-NBD. Promptly aggressive treatment is essential for improving prognosis, and biological agents might be a promising approach for severe/refractory p-NBD.

    Abstract: The runoff components were identified in the headwater area of Shule River Basin, using isotopic and chemical tracing with particular focus on the temporal variations of catchment sources. A total of 95 samples, including precipitation, groundwater, and glacial meltwater, were collected and analyzed for stable water isotopes ( 18 O and 2 H) and major chemical ion parameters (potassium, sodium, calcium, magnesium, sulfate, chloride, and bicarbonate). Based on the isotope and water chemistry data, we applied end member mixing analysis (EMMA) to identify and quantify the major runoff generating sources and their contributions. The contributions of groundwater, precipitation, and glacial meltwater were 66.7%, 19.9%, and 13.4%, respectively. The study indicated that groundwater dominated runoff in the headwater area of Shule River Basin. The roles of glacier meltwater should be remarkable in water resource management in this basin. The uncertainties of the EMMA method were summarized and estimated via a classical Gaussian error propagation technique. Analyses suggested that the uncertainty in the measurement method was less important than that in the temporal and spatial variations of tracer concentrations. The uncertainty was sensitive when the difference between mixing components was small. Therefore, the variation of tracers and the difference of mixing components should be considered when hydrograph separation was applied in the basin.
