    Abstract: Short segment myelitis (SSM, 〈 3 vertebral segments) is an under-recognized initial manifestation of neuromyelitis optica spectrum disorders (NMOSD). Though infrequent, failure to recognize SSM in patients with NMOSD would lead to incorrect diagnosis and treatment. Therefore, delineation of features of NMOSD-associated SSM is of paramount importance. Objective: Our study aimed to determine the demographic, clinical and radiological features of NMOSD-associated SSM, and compare those with NMOSD-associated longitudinally extensive transverse myelitis (LETM) and multiple sclerosis (MS)-associated SSM, respectively. Methods: Chinese patients presenting initially only with acute myelitis and diagnosed with NMOSD ( n = 46) and MS ( n = 11) were included. Clinical, serological, imaging and disability data were collected. Mann–Whitney U test or two-tailed Fisher’s exact tests were used to analyse the data. Results: Of the 46 enrolled NMOSD patients, 34 (74%) collectively had 38 LETM lesions, while 12 (26%) had 14 SSM lesions. When compared with LETM, NMOSD presenting with SSM were more likely to have a delayed diagnosis and a lower level of disability at nadir during the first attack. T1-weighted imaging hypointensity was more prominent in NMOSD-associated LETM lesions than NMOSD-associated SSM lesions. When compared with MS-associated SSM, NMOSD-associated SSM lesions were more likely to be centrally located, grey matter involving and transversally extensive on axial imaging and spanned no less than 2 vertebral segments on sagittal imaging. Conclusion: These findings suggest that SSM does not preclude the possibility of a NMOSD diagnosis. Testing for serum aquaporin-4 immunoglobulin G (AQP4-IgG) and careful study of lesions on spinal cord magnetic resonance imaging could aid in an earlier and correct diagnosis.

    Abstract: Diagnostic criteria for multiple sclerosis have evolved over time, with the most recent being the 2017 McDonald criteria. Evidence is lacking regarding the validity of the 2017 McDonald criteria among the Asian population. Therefore, this study aims to evaluate the diagnostic performance of the 2017 McDonald criteria in Chinese patients with clinically isolated syndrome (CIS). Methods: A total of 93 patients with initial findings suggestive of CIS in a tertiary hospital in China from 2012 to 2017 were included in this retrospective study. Baseline and follow-up data were reviewed. Diagnostic performance (sensitivity, specificity, accuracy), was assessed and survival analysis was performed for the 2017 and 2010 McDonald criteria respectively. Results: Among the 93 Chinese patients with CIS, 57 were female (61.3%) and the median (interquartile range) age of onset was 37 (31.3–41.8) years. The 2017 McDonald criteria displayed a higher sensitivity (75.0% versus 14.6%, p  〈  0.0001), lower specificity (47.1% versus 100.0%, p  〈  0.05) but an overall higher accuracy (67.7% versus 36.9%, p  〈  0.0001) when compared with the 2010 iteration. The novel criteria allow for a better detection of MS at baseline (40.8% versus 9.9%, p  〈  0.0001). Conclusion: The 2017 McDonald criteria had a higher sensitivity but lower specificity than the 2010 iteration. Overall it facilitated an earlier and more accurate diagnosis of multiple sclerosis in Chinese patients with CIS.

    Abstract: To validate the recently proposed imaging criteria in distinguishing aquaporin-4 antibody (AQP4-ab)-seropositive neuromyelitis optica spectrum disorder (NMOSD) from multiple sclerosis (MS) and myelin oligodendrocyte glycoprotein antibody-associated disorder (MOG-AD) at disease onset in a Chinese population. Methods: We enrolled 241 patients in this retrospective study, including 143 AQP4-ab-seropositive NMOSD, 73 MS, and 25 MOG-AD. Cacciaguerra’s criteria were described as fulfillment of at least 2/5 conditions including the absence of the combined juxtacortical/cortical lesions, the presence of longitudinal extensive transverse myelitis (LETM) lesions, the presence of periependymal-lateral ventricles lesions, the absence of Dawson’s fingers lesions, and the absence of periventricular lesions. Results: Fulfillment of at least 3/5 conditions was able to differentiate NMOSD from MS with a good diagnostic performance (accuracy = 0.92, sensitivity = 0.91, specificity = 0.93), yet failed to differentiate NMOSD from MOG-AD. LETM lesions showed the highest accuracy (0.78), sensitivity (0.70), and specificity (0.97) for NMSOD. Conclusion: Our research suggested the utility of Cacciaguerra’s criteria in a Chinese population at disease onset. A better diagnostic performance in NMOSD could be attained with at least 3/5 conditions fulfilled. Yet their utility in distinguishing NMOSD from MOG-AD was limited.

    Abstract: In order to explore the characteristics of pressure pulsation signals and energy distribution of water flow at the guide vane considering impeller–guide vane interaction. The numerical simulation of the vertical axial flow pump device's steady and unsteady three-dimensional flow fields was carried out. The Hilbert–Huang method was used to conduct empirical mode decomposition decomposition and Hilbert spectrum analysis of pressure pulsation signal at each monitoring point in the inlet and outlet regions of the guide vane. The results show: Under the condition of 0.3 Q bep , the internal pressure of the guide vane is obviously affected by the impeller, and there are large block-shaped vortex structures in the guide vane. Under the operating conditions of 1.0 Q bep and 1.2 Q bep , the size of the pressure area in the guide vane is basically not affected by the impeller, and the vortex structures in the guide vane are concentrated near the outlet of the guide vanes, and there are long strip-shaped vortex structures at the edge of the guide vane. The size and number of vortex structures decrease with the increase in flow rate. The pressure pulsation signal at the inlet of the guide vane is affected by the rotation of the impeller and exhibits good periodicity, with the main frequency centered around 146 Hz, and the energy ratio of the main frequency is up to 97.7%. There are low-frequency signals below 100 Hz and high-frequency signals fluctuating around 146 Hz in all three flow conditions. When the flow rate increases, the fluctuation amplitude of the high-frequency signal increases. The flow rate has a significant impact on the water flow at the outlet of the guide vane. At 0.3 Q bep , its frequency is distributed in the range of 0–500 Hz, mainly concentrated in the area below 400 Hz. At 1.0 Q bep , the frequency of pressure pulsation is distributed below 250 Hz after the guiding function of the guide vane. At 1.2 Q bep , the water flow is mainly controlled by the rotation of the impeller, and after the energy recovery of the guide vane, its main frequency is still concentrated around 150 Hz, which is 337.2% and 268.5% of 0.3 Q bep and 1.0 Q bep . Under the working condition of 0.3 Q bep , the proportion of intrinsic mode function energy corresponding to the dominant frequency at the center of the guide vane inlet is as high as 95.9%, and the proportion of intrinsic mode function energy corresponding to the dominant frequency at the shroud side and hub side of the guide vane is rather low. If the flow rate rises from 0.3 Q bep to 1.2 Q bep , the proportion of intrinsic mode function energy increases by more than 42%. Under the working conditions of 0.3 Q bep and 1.0 Q bep , the main frequency of pressure pulsation signal of water flow at the guide vane outlet is less affected by the impeller and the corresponding energy proportion is low. Under the working condition of 1.2 Q bep , the main frequency of pressure pulsation signal is 4 times the rotational frequency and the corresponding energy proportion is higher than 60%.

    Abstract: In this current work, two fresh transition metal coordination polymers (CPs) have been created with the reaction between terphenyl-3,3 ″ ,5,5 ″ -tetracarboxylic acid (H 4 L), the symmetrical rigid carboxylic acid ligand and the relevant metal salts under the reaction conditions of solvothermal, and the chemical compositions of the compounds respectively are {[Cu 2 (TPTC) (DMF) 2 (H 2 O)]·2DMF·2H 2 O} (2) and {[Zn (TPTC) (1,2-bimb) 2 ]·4(H 2 O)} n (1, bimb = 1,2-bis((1H-imidazol-1-yl)methyl)benzene). Moreover, the prevention and nursing values of the compound on postoperative infection was explored and their corresponding mechanism also was investigated. Firstly, enzyme linked immunosorbent assay kit (ELISA) was employed to detect the release of inflammatory cytokines into plasma. Additionally, the expression of the bacterial survival genes was determined via exploiting the real time reverse transcription-polymerase chain reaction (RT-PCR) after treated with the compound.

    Abstract: We aimed to investigate whether sex differences influence the clinical outcomes of patients who undergo thoracic endovascular aortic repair (TEVAR) for type B aortic dissection (TBAD). We retrospectively analyzed a prospectively maintained single-center cohort of patients with TBAD who underwent TEVAR between January 2010 and June 2017. We evaluated the in-hospital and long-term mortality and composite end point. Of the 913 patients, 793 (86.8%) were male and 120 (13.1%) were female. Compared to male patients, the female patients were older, more likely to have diabetes mellitus, but less likely to smoke or have hypertension. The proximal landing zone in 0 and 1 was higher in male patients ( P = .023), who were more likely to require an aortic arch bypass. Endoleak, delirium, and ICU stay after stent-graft implantation were also more frequent in men. Sex factor was not associated with in-hospital or long-term mortality or the composite end point in the multivariable regression analyses and Cox regression model. The mean estimated survival time was similar between males and females (2462.9 ± 141.2 vs 2804.1 ± 117.4 days, P = .167) in the propensity score–matched cohort. Despite distinct characteristics between sex, there was no sex-related difference in long-term clinical outcomes after TEVAR for TBAD.

    Abstract: The scroll compressor is a kind of positive-displacement compressor applied in refrigeration systems, heat pump systems, and air conditioner systems. Using the spring smoothing method and considering the heat transfer, the numerical simulation model of the scroll compressor is established under standard working conditions. The results show that the CFD model is well validated by the experiment result. The compressor performance and the working process are analyzed. The pre-compression and the over-compression phenomenon are also studied. Moreover, the temperature distribution and velocity distribution of gas flow in the compressor are illustrated. The result shows that the pre-compression can improve the performance of the scroll compressor and the optimization of the suction channel geometry can provide a smoother suction process. Under the combined effect of the pre-compression and heat transfer, the refrigerating capacity is increased by approximately 0.54 kW and the volume efficiency is increased by approximately 2.2%. The additional power loss due to over-compression is 47.6615 W, and the setting of over-compression discharges can reduce the pressure and power loss. This study is helpful to the performance prediction and the structural optimization of the scroll compressor.
