Abstract: On April 18, 2023, the 1152nd meeting of the Moscow Society of Dermatologists and Cosmetologists named after A.I. Pospelov took place. The meeting was held in a face-to-face format. There were 115 participants in total. Ac-cepted as a member of the MSDC 1 person. Two reports are presented in the clinical part of the meeting: clinical variations of the course of skin sarcoidosis: the mini-lecture describes 8 clinical cases of cutaneous sarcoidosis; Kaposi's sarcoma in a 24-year-old patient with positive antibodies to cytomegaly and EpsteinBarr viruses. Three reports were presented in the scientific part of the meeting: The possibilities of im-proving the effectiveness of onychomycosis therapy (speaker I.S. Maximov, Sechenov University); The use of botulinum toxin type A for the correction of erythematous-telangiectatic subtype of rosacea (speaker A.O. Yakovleva, State Scientific Center of Dermatovenerology and Cosmetology); A post-graduate student of the Department of Dermatovenereology of the Peoples' Friendship University of Russia Aun Rami Yusefovi made a report on targeted therapy of children with moderate and severe forms of atopic dermatitis (co-speakers ― Prof. E.A. Batkaev, Prof. V.Yu. Ujuhu).

Abstract: The 1147th meeting was held in a face-to-face format. In total there were 95 participants. Four people were accepted as members of the MSDC. Among the clinical observations presented at the meeting were patients with psoriatic erythroderma (two cases) and erythroderma in Deverjis disease. Erythroderma is a severe, often life threatening condition characterized by a universal inflammatory lesion of the skin, itching, generalized lymphadenopathy and fever. A case of cryoglobulinemic vasculitis, characterized by polymorphism of the course and a tendency to necrosis, is also presented in a 65-year-old patient who developed drug-induced ItsenkoCushing syndrome during treatment. A case of multiform exudative erythema was also considered in a 40-year-old patient with abundant rashes on the skin of the trunk and extremities caused by subacute SARS-CoV-2-associated myocarditis of high immunological activity, moderate course. The topics of scientific reports were devoted to auto-inflammatory syndrome (SAPHO; PEPA; PUSH; PUSH) and acne. It has been reported that biological anticytokine therapy ― IL-1 antagonists ― is the most acceptable for the treatment of all syndromes with the main dermatological manifestation in the form of acne. For severe forms of acne, an algorithm for patient management was proposed.

Abstract: On December 21, 2021, the 1145th meeting of the Moscow Society of Dermatovenereologists and Cosmetologists named after A.I. Pospelov took place. The meeting was held in a face-to-face format traditionally in the countries of the V.A. Rakhmanov Skin Clinic. There were 110 participants. Accepted as a member of the MSDV 4 people. Clinical observations of linear IgA-dependent dermatosis, systemic lupus erythematosus, annular granuloma were presented. Two scientific reports were heard ― about foot mycetoma; about skin reactions to tattoos. Linear IgA-dependent bullous dermatosis is a rare immune ― mediated vesicular skin disease characterized by the presence of homogeneous linear fixation of IgA along the basement membrane. Trigger factors: medications (vancomycin, cephalosporins, diclofenac, captopril, etc.); infectious diseases; vaccination against influenza; exposure to ultraviolet rays; paraneoplastic process (5% of cases) in lymphoproliferative diseases; inflammatory bowel diseases (ulcerative colitis and Crohns disease). The differential diagnosis in adults is Durings herpetiform dermatitis, Levers bullous pemphigoid, an atypical form of multiform exudative erythema; in children ― bullous impetigo, genetic epidermolysis bullosa. Systemic lupus erythematosus is a multisystem connective tissue disease with lesions of the skin and mucous membranes, joints, kidneys, heart, lungs and central nervous system. Systemic glucocorticoids, hydroxychloroquine, azathioprine, cyclophosphamide, belimumab, rituximab are used in the treatment. Annular granuloma is a benign inflammatory dermatosis of unknown etiology, which is characterized by the development of flesh colored or red papules on the skin, often arranged in the form of rings. Fractional laser action is used in the treatment. Mycetoma is a one sided, slightly painful dense tumor-like infiltrate limited on the foot or hand, edema with the formation of granulomas, microabcesses, the formation of fistulas and the release of actinomycetes or fungal mycelium from them. Differential diagnosis: chronic arthritis, arthrosis, sprain and rupture of ligaments, synovitis, chronic and postoperative osteomyelitis, bacterial mycetoma, botryomycosis, tumors, cold abscess, phlegmon, post-injection infiltrate, etc. Tattooing is a subspecies of avant ― garde art and a way of changing appearance, in which the integrity of the skin is violated with the help of various piercing tools and a persistent indelible pattern is applied by introducing a coloring agent. The pigments trapped in the tattoo partially exit through the lymph and are deposited in the regional lymph nodes. The nodes are imperceptibly colored along with the skin. A certain amount of pigment nanoparticles can enter the bloodstream and theoretically cause harm somewhere in the body.

Abstract: On November 17, 2020, the 1140th meeting was held in on-line format. In total there were 92 participants. Five people were accepted as members of the MDCS. The clinical agenda of the meeting considered cases of subacute (disseminated) cutaneous lupus erythematosus and reactions to tattooing in the form of pseudolymphoma of the skin. The interest of observing disseminated lupus erythematosus was in the difficulties of diagnosis due to its combination with Sjogrens syndrome, a long history of the disease, the severity of the skin process and the peculiarities of the selection of adequate drug therapy. Analysis of the case of pseudolymphoma development as a reaction to a tattoo in a patient with atopic dermatitis helped to approach a more accurate understanding of the clinical and pathogenetic aspects of this process: probably, the pathophysiological reaction is based on a reaction to a foreign body, which makes this disease related to sarcoidosis. The scientific agenda of the meeting reported on the advantages of using photodynamic therapy for skin T-cell lymphomas, such as good cosmetic results; non-invasive nature of the procedure; selectivity; low risk of toxicity; low photosensitivity; low carcinogenic potential. In another report ― on the use of signaling pathway blockers in the treatment of psoriasis ― the results of an open uncontrolled prospective clinical study of the effectiveness of apremilast in the treatment of moderate and severe psoriasis were presented.

Abstract: On October 18, 2022, the 1149th meeting of the A.I. Pospelov Moscow Society of Dermatovenerologists and Cosmetologists (MSDC) took place. The meeting was held in a face-to-face format. There were 128 participants in total. Accepted as a member of the MSDC 53 people. Two clinical cases are presented for discussion: trichoadenoma ― a rare benign follicular tumor consisting of strands of epithelial cells and keratinizing cysts embedded in the sclerotic stroma, and Dariers follicular dyskeratosis ― hereditary dermatosis characterized by the formation of follicular hyperkeratotic vegetative papules on seborrheic areas of the skin caused by a violation of the keratinization process by the type of dyskeratosis. Scientific reports were devoted to the analysis of differential diagnosis and therapeutic tactics in BrookeSpiegler syndrome (multiple cylindromes and trichoepitheliomas), the mechanisms of action and effectiveness of zinc pyrithione as a means of controlling colonization of the hairy skin by fungi-commensals of the genus Malassezia, as well as the most urgent problems of modern medicine ― the features of the course of chronic-severe dermatoses after SARS-CoV-2 infection (COVID-19) and an increase in the incidence of mucormycosis, which is a marker of immune breakdown that occurs immediately after COVID-19 infections.

Abstract: On December 20, 2022, the 1150th meeting of the Moscow Society of Dermatovenerologists and Cosmetologists (MSDC) named after A.I. Pospelov took place. The meeting was held in a face-to-face format. There were 110 participants in total. Accepted as a member of the MSDC 3 people. Two clinical cases are presented in the clinical part of the meeting: widespread cutaneous leishmaniasis in an HIV-infected patient. The peculiarity of this clinical case was the extreme prevalence of rashes. No systemic pathology was detected; panniculitis in a patient after a COVID-19 infection. The process proceeded according to the type of non-stagnant Weber-Christian panniculitis: after surgical autopsy, a mucus-like substance was released. Three reports were presented in the scientific part of the meeting. Corresponding member Prof. O.Y. Olisova (Sechenov University) made a report on the benefits of treating severe and moderate psoriasis with netakimab; researcher of State Scientific Center of Dermatovenereology and Cosmetology Central Dermatology and Venereology Institute A.A. Vorontsova reported on the results of an original study of the generality of clinical and pathomorphological manifestations of psoriasis and T-cell lymphoma; with pathogenetic justification of the development of cutaneous toxicity as a result of targeted therapy of oncological diseases was performed by E.V. Orlova (Sechenov University): it is based on the suppression and dysfunction of EGFR-controlled processes in the skin.

Abstract: Superficial fungal infections of the skin are common on all continents. Errors in the diagnosis and treatment of these patients are often observed. The lack of symmetry of the process, erythematous-squamous foci with a raised roller and more pronounced peeling along the periphery, polycyclic and annular outlines, as well as the presence of papules and pustules in the lesion can give a hint in making a correct diagnosis. Laboratory tests (microscopy and seeding) are mandatory for accurate diagnosis. This article presents photos with anamnesis of the disease and treatment options for common superficial fungal infections of the skin.

Abstract: The 1148th meeting was held in a face-to-face format. In total there were 135 participants. Six people were accepted as members of the MSDVC. A number of clinical observations were presented at the meeting. The case of severe gangrenous pyoderma with mucosal lesions is of interest due to the complexity of differential diagnosis with Wegeners granulomatosis, while the main method confirming the diagnosis of gangrenous pyoderma is histological examination of a biopsy. The agenda also discussed the validity of the allocation of the clinical concept of red scrotum syndrome. Patients with red scrotum syndrome, as a rule, seek advice from a dermatologist late, which leads to a decrease in their quality of life. In the presented observation, the patient was treated as with rosacea. The clinical experience of the use of IL-17 inhibitors in the treatment of generalized pustular psoriasis of Tsumbush is also analyzed. The scientific part of the meeting was entirely devoted to the issues of psoriasis, a disease that is a significant psychosocial problem and has a great impact on the quality of life of patients. Not only the differences between psoriasis and seborrheic dermatitis were considered, but also the similarity of the clinical picture, including the pathophysiological features of the stratum corneum, namely the generality of keratinization processes, while the leading therapeutic approach for both diseases is the use of external agents with anti-keratinizing and anti-inflammatory effects. Liposomal cream with silymarin is used for external therapy of psoriasis. Dermatocosmetic products containing silybin demonstrate a positive effect in the treatment of psoriasis, which is confirmed by the results of the study. Nevertheless, the development of effective and safe local remedies for psoriasis therapy remains an urgent problem of modern dermatology.
