    Kurzfassung: Amaç: Son dönem böbrek hastalığı (SDBH) morbidite ve mortalitesi yüksek bir durumdur. Böbrek nakli bu durumun hasta açısından en çok tercih edilen ve en çok fayda sağlayan tedavi yöntemidir. Yaş, transplantasyonun başarısını etkileyebilecek önemli bir faktördür. Bu çalışmada donör yaşının böbrek nakli sonuçları üzerindeki etkisini araştırdık. Araçlar ve Yöntem: Bu çalışmaya canlı veya kadavradan böbrek nakli yapılmış 101 SDBH olan hasta retrospektif olarak incelendi. Vericilerin yaş, cinsiyet ve vücut kitle indeksi (VKİ) ve alıcıların yaş, cinsiyet, VKİ, hastanede yatış süreleri, tahmini glomerüler filtrasyon hızları (eGFR) ve serum kreatinin ölçümleri kaydedildi. Alıcı serum kreatinin seviyeleri, nakilden sonraki 18 aya kadar aralıklarla kaydedildi. Bulgular: Donörler yaşlarına göre (50 yaş (n:34)) olmak üzere 3 gruba ayrıldı. Vericinin cinsiyeti, VKİ, canlı/kadavra oranı ve alıcının cinsiyeti, yaşı ve VKİ açısından istatistiksel olarak anlamlı fark saptanmadı (p 〉 0,05). Verici

    BACK TO WORK TIME AND LOSS OF WORKING DAYS AFTER VARICOSELECTOMY OPERATION

    Kurzfassung: Aim: Varicocelectomy operations are mostly performed between the ages of 20-35. This age group is also very important for an effective workforce. There is no study in the literature on the loss of working days in varicocelectomy. With this study, we aimed to raise awareness on the effect of varicocelectomy surgery on loss of working days and to contribute to the literature. Materials and Methods: 77 patients who underwent varicocelectomy at Aksaray Private Inova Hospital between 07.03.2019 and 14.10.2022 were included in the study. Varicocelectomy was performed by a single surgeon using the subinguinal method with a Carl-Zeiss operating microscope (Carl Zeiss, Thornwood, NY) under 10 magnification. The early complications of the patients and the loss of working days were questioned at the postoperative 10th day and 1st month follow-ups. Statistical Package for Social Sciences (SPSS), version 22.0 (SPSS Inc. Chicago, USA) computer package program was used for statistical analysis of research data. p 0.05). A statistically significant difference was found between work type, marital status, types of early complications and lost work days (p:0.014, p:0.03 and p:0.02, respectively). Conclusion: After varicocelectomy, blue-collar employees return to work a few days longer than white-collar employees. Employees can return to work after approximately 15 days after varicocelectomy. We believe that prospective, larger case series of advanced centers are needed in this regard. Keywords: lost working days; varicocelectomy; microsurgery

    Türkiye'de Yeni Hastane Modelinde Böbrek Nakli: Ankara Şehir Hastanesi'nde 3 Yıllık Deneyim

    Kurzfassung: Öz Amaç: Şehir hastaneleri, yeni bir model hastane olarak Türkiye'de yaklaşık 20 farklı ilde hizmet vermeye başlamıştır. Bu çalışmada, Ankara Şehir Hastanesi böbrek nakli ünitesinin 100 vakayı aşan 3 yıllık deneyimini sunmayı amaçladık. Materyal ve Yöntem: Ankara Şehir Hastanesi Üroloji Kliniğinde canlı veya kadavradan böbrek nakli yapılan 101 son dönem böbrek (SDBH) hastasının verileri retrospektif olarak toplandı. Alıcı ve vericinin demografik verileri ameliyat sonrası komplikasyonlar, hasta sağkalımı ve greft fonksiyonları kaydedildi. Bulgular: Hasta ve greft sağkalım oranları sırasıyla %96 ve %98 idi. Post-op ilk gün, 1 ay, 6 ay, 12 ay, 18 ay, 24 ay ve 36 ay sonraki ortanca serum kreatinin düzeyi 1,3 (aralık, 0,7-5,7) mg/dl, 1,3 (aralık, Sırasıyla 0,8-1,7) mg/dl, 1,3 (aralık, 0,8-2,5) mg/dl, 1,3 (aralık, 0,7-2) mg/dl, 1,2 (aralık, 0,8-1,9) mg/dl, 1,4 (aralık, 0,8-2,4) mg/dl ve 1,4 (aralık, 1-2,4) mg/dl olarak bulundu. Ürinomu olan 6 (%5,9) hastalar üriner kateterizasyon ve perkütan drenaj ile konservatif olarak tedavi edildi. Renal arter stenozu 2 (%2) hastada gözlendi. Sonuçlar: Böbrek nakli, dünyada olduğu gibi Türkiye'de de SDBH hastaları için halen en önemli tedavi seçeneğidir. Merkezimizde 36 aylık takipte greft fonksiyon sonuçlarının kabul edilebilir derecede iyi olduğunu gözlemledik. Anahtar Kelimeler: böbrek nakli, greft fonksiyonu, şehir hastanesi

    Kurzfassung: Bu çalışmanın konusu karı-koca ilişkisi bağlamında bir eş olarak kadın algısının Câhiliye Arap şiirine yansımalarıdır. Problemi ise günümüze ulaşan Câhiliye Arap şiirinde yer alan örnek beyitler çerçevesinde dönemin Arap toplumlarındaki karı-koca ilişkilerinin kadın özelinde sorgulanmasıdır. Çalışma konusu beyitler Câhiliye Arap toplumlarında bir eş olarak kadın algısını, karı-koca ilişkilerini ve davranış biçimlerini yakından tanıma bakımından önem arz etmektedir. Her ne kadar Câhiliye Arap toplumları, Câhiliye şiiri ve kadını ile ilgili çalışmalar yapılmış olsa bile söz konusu çalışmalarda konuların tabiatı gereği bir eş olarak dönemin kadın algısı hususunda detaylara inilmemiş ve satır aralarında bazı özelliklere değinmekle yetinilmiştir. Çalışmamızı ayrıcalıklı kılan husus, Câhiliye Arap şiirlerinde yer alan örneklerden hareketle karı-koca ilişkisi, sadakatsizlik ve kıskançlık, eşten yakınma ve ölen kocanın ardından ağıt yakılması gibi ince detaylardır. Karı-koca arasında sevgi ve saygı esasına dayalı ilişki, kocadan uzun süre haber alınamaması durumunda kadının yeniden evlenmeye zorlanılması, kadına kaba davranılması ve zor kullanılması, kadının ikna yoluyla sorun çözmesi, sorunlu kadını boşama, sadakatsiz kadın tarafından kocaya komplo kurulması, sadakatsiz kadının koca tarafından öldürülmesi, iktidarsız kocaya sadakatsiz davranması, kocanın hayatında başka kadın olabileceği şüphesiyle kadının kıskançlık krizine girmesi, kadının kocayı hicvetmesi ve ölen kocanın ardından mersiye okuması gibi detaylar örneklendirilmiştir. Çalışmada nitel yöntem tekniklerinden doküman incelemesi kullanılmıştır. Öncelikle konunun daha iyi anlaşılması amacıyla literatür değerlendirmesi yapılmış ve alanla ilgili yapılan çalışmalara değinilmiştir. Ardından arka plan mahiyetinde Câhiliye Arap toplumunda kadının yeri hakkında özet bilgiler verilmiştir. Daha sonra tespit edilen örnek beyitler yorumlanmış ve farklılıklara dikkat çekilmiştir. Varılan kanaatlere sonuç bölümünde yer verilmiştir.

    Kurzfassung: The financial sector, along with the development of the global economy, is experiencing a substantial development and transformation process. Over time, it also creates new needs and paves the way for new financial market instruments. On the other hand, in many Islamic countries, despite existence of ancient Islamic economy, increasing influence of the conventional Neo-liberal financial markets is a fact. Islamic finance, shows a rapid development in Turkey, in parallel with the increasing popularity in the world. This study primarily focuses on development of Islamic finance in the Turkish laws, rules and regulations, and attempts to explain new regulations related to Islamic finance. The aim of this study is to determine the issues that are missing in modern Islamic finance applications; to assist the process of new regulations, to identify the problems posed by existing regulations and to contribute to the process of amendments in whole or in part.

    Kurzfassung: The aim of this study is to synthesize novel piperazine-containing dihydrofuran compounds (3a-n)from radical additions and cyclizations of diacyl and alkyl-acyl piperazine derivatives (1a-h) with 1,3-dicarbonyl compounds (2a-c) mediated by Mn(OAc)3 for the first time. From the reactions of 1a-c with dimedone (2a);1a, 1c, and 1d with acetylacetone (2b); and 1a with ethylacetoacetate(2c) ,the dihydrofuran-piperazine compounds 3a-c, 3d-f, and 3g were obtained in medium to high yields (31%–81%), respectively. In addition, dihydrofuran-piperazine compounds 3h-j and 3k-n were prepared at low to medium yields (20%–40%) from the reactions of 1e-g with 2a and 1e-h with 2c, respectively.
