    Kurzfassung: In recent years, microRNAs (miRNAs) derived from exosomes have been attracting attention as novel clinical biomarkers in a variety of cancers. In this study, plasma samples from 60 gastric cancer (GC) patients and 63 healthy individuals were collected, and the exosomal microRNAs (ex-miRNAs) were isolated. We determined the specific ex-miRNAs through miRNA microarray and a database of differentially expressed miRNAs called dbDEMC. Then, the expression levels of exosomal miR-31, miR-192, and miR-375 were analyzed by quantitative polymerase chain reaction (qRT-PCR). Compared to the matched controls, exosomal miR-31, miR-375, and miR-192 were significantly upregulated in GC patients. Also, they were found to be associated with gender, with miR-192 being significantly upregulated in male GC patients. Kaplan–Meier analysis indicated that high expressions of exosomal miR-31, miR-375, and miR-192 were positively correlated with poor clinical outcomes of GC patients. Cox univariate and multivariate analysis found that ex-miR-375 expression and TNM stage were independent prognostic factors of overall survival (OS). Our findings revealed that exosomal miR-31, miR-192, and miR-375 might serve as noninvasive, sensitive, and specific biomarkers for the diagnosis and prognosis of GC patients.

    Kurzfassung: The Kiewitt spherical reticulated shell of triangular pyramid system is taken as the object of this study; a macroprogram of parametric modeling is developed by using the ANSYS Parametric Design Language. The minimum structural total weight is taken as the objective function, and a shape optimization program is proposed and compiled by adopting the sequence two-stage algorithm in FORTRAN environment. Then, the eigenvalue buckling analysis for Kiewitt spherical reticulated shell of triangular pyramid system is carried out with the span of 90 m and rise-span ratio of 1/7~1/3. On this basis, the whole nonlinear buckling process of the structure is researched by considering initial geometrical imperfection. The load-displacement curves are drawn, and the nonlinear behaviors of special nodes are analyzed. The structural nonlinear behaviors affected by rise-span ratio are discussed. Finally, the stability of reticulated shell before and after optimization is compared. The research results show that ( 1 ) users can easily get the required models only by inputting five parameters, i.e., the shell span ( S ), rise ( F ), latitudinal portions ( Kn ), radial loops ( Nx ), and thickness ( T) . ( 2 ) Under the conditions of different span and rise-span ratio, the optimal grid number and bar section for the Kiewitt spherical reticulated shell of triangular pyramid system existed after optimization; i.e., the structural total weight is the lightest. ( 3 ) The whole rigidity and stability of the Kiewitt spherical reticulated shell of triangular pyramid system are very nice, and the reticulated shell after optimization can still meet the stability requirement. ( 4 ) When conducting the reticulated shell design, the structural stability and carrying capacity can be improved by increasing the rise-span ratio or the rise. ( 5 ) From the perspective of stability, the rise-span ratio of the Kiewitt spherical reticulated shell of triangular pyramid system should not choose 1/7.

    Kurzfassung: In order to explore the quality management efficiency of applying big data and artificial intelligence in nursing quality index, a method of building a nursing management platform integrating nursing indicators and nursing events is proposed. Based on the investigation of the application demand of nursing information system, the method achieves timely data sharing and transmission through WLAN technology and realizes nursing management monitoring, nursing quality index enquiry, and automatic statistical analysis under the vertical management mode of nursing. The results showed that 77 people (73%) thought the time decreased, 19 people (18%) thought the time was the same, and 9 people (7%) thought the time increased. In terms of intelligent application and big data of nursing information management system, there is a significant difference in nursing management efficiency before and after using nursing management information system (P 〈 0.001). The nursing management control platform is designed and applied, and the nursing quality control method and actual management process are improved, which is very good for strengthening nursing quality management. The overall optimization of the quality control process is realized, which helps to mobilize the initiative and enthusiasm of nursing staff and continuously improve the effectiveness of nursing management and nursing efficiency.

    Kurzfassung: The increasing concern of biogas slurry disposal and nitrogen loss in soils has brought back the interest in using biochar as an adsorbent of biogas slurry in soils. Three types of biochars, commercial activated carbon, pyrolysis productions derived from rice husk, and nut shell, were added as adsorbents in two types of soils (sandy and loamy) at solid weight ratio of 5%, 10%, 15%, and 20% to investigate the effects of biochars on soils adsorbing ammonium nitrogen ( N H 4 + -N) in biogas slurry using oscillation method. There was no difference of N H 4 + -N adsorbability for sand soil and loamy soil. The N H 4 + -N adsorption capacity of soils increased as the additive biochars increased from 0 to 20%. The smaller particle size of biochars led to the shorter stable adsorption time and the better N H 4 + -N adsorption capacity. Commercial activated carbon showed the best N H 4 + -N adsorption capacity in biogas slurry, followed by the nut shell carbon. The rice husk carbon was the worst. The results in this study provide a feasible and cost-effective assessment method of biochars for increasing the N H 4 + -N adsorption capacity of soils in biogas slurry, as well as good insight into effects of different biochars on improving N H 4 + -N adsorption capacity of soils.

    Kurzfassung: This retrospective study aims to examine the association of plasma Epstein-Barr virus- (EBV-) DNA levels with the tumor volume and prognosis in patients with locally advanced nasopharyngeal carcinoma (NPC). A total of 165 patients with newly diagnosed locally advanced NPC were identified from September 2011 to July 2012. EBV-DNA was detected using fluorescence quantitative polymerase chain reaction (PCR) amplification. The tumor volume was calculated by the systematic summation method of computer software. The median copy number of plasma EBV-DNA before treatment was 3790 copies/mL. The median gross tumor volume of the primary nasopharyngeal tumor (GTVnx), the lymph node lesions (GTVnd), and the total GTV before treatment were 72.46, 23.26, and 106.25 cm 3 , respectively; the EBV-DNA levels were significantly correlated with the GTVnd and the total GTV ( P 〈 0.01 ). The 2-year overall survival (OS) rates in patients with positive and negative pretreatment plasma EBV-DNA were 100% and 98.4% ( P = 1.000 ), and the disease-free survival (DFS) rates were 94.4% and 80.8% ( P = 0.044 ), respectively. These results indicate that high pretreatment plasma EBV-DNA levels in patients with locally advanced NPC are associated with the degree of lymph node metastasis, tumor burden, and poor prognosis.

    Kurzfassung: PGC-1 α is an inducible transcriptional coactivator that regulates mitochondrial biogenesis and cellular energy metabolism in skeletal muscle. Recent studies have identified two additional PGC-1 α transcripts that are derived from an alternative exon 1 (exon 1b) and induced by exercise. Given that the PGC-1 α gene also produces NT-PGC-1 α transcript by alternative 3′ splicing between exon 6 and exon 7, we have investigated isoform-specific expression of NT-PGC-1 α mRNA in mouse skeletal muscle during physical exercise with different intensities. We report here that NT-PGC-1 α -a mRNA expression derived from a canonical exon 1 (exon 1a) is increased by high-intensity exercise and AMPK activator AICAR in mouse skeletal muscle but not altered by low- and medium-intensity exercise and β 2 -adrenergic receptor agonist clenbuterol. In contrast, the alternative exon 1b-driven NT-PGC-1 α -b (PGC-1 α 4) and NT-PGC-1 α -c are highly induced by low-, medium-, and high-intensity exercise, AICAR, and clenbuterol. Ectopic expression of NT-PGC-1 α -a in C 2 C 12 myotube cells upregulates myosin heavy chain (MHC I, MHC II a) and Glut4, which represent oxidative fibers, and promotes the expression of mitochondrial genes (Cyc1, COX5B, and ATP5B). In line with gene expression data, citrate synthase activity was significantly increased by NT-PGC-1 α -a in C 2 C 12 myotube cells. Our results indicate the regulatory role for NT-PGC-1 α -a in mitochondrial biogenesis and adaptation of skeletal muscle to endurance exercise.

    Kurzfassung: Background . Nocturnal enuresis (NE) is recognized as a widespread health problem in young children and adolescents. Clinical researches about acupuncture therapy for nocturnal enuresis are increasing, while systematic reviews assessing the efficacy of acupuncture therapy are still lacking. Objective . This study aims to assess the effectiveness of acupuncture therapy for nocturnal enuresis. Materials and Methods . A comprehensive literature search of 8 databases was performed up to June 2014; randomized controlled trials which compared acupuncture therapy and placebo treatment or pharmacological therapy were identified. A meta-analysis was conducted. Results . This review included 21 RCTs and a total of 1590 subjects. The overall methodological qualities were low. The results of meta-analysis showed that acupuncture therapy was more effective for clinical efficacy when compared with placebo or pharmacological treatment. Adverse events associated with acupuncture therapy were not documented. Conclusion . Based on the findings of this study, we cautiously suggest that acupuncture therapy could improve the clinical efficacy. However, the beneficial effect of acupuncture might be overstated due to low methodological qualities. Rigorous high quality RCTs are urgently needed.

    Kurzfassung: Objective. Alveolar echinococcosis (AE) is a zoonosis caused by the larval stage of the metacestode Echinococcosis multilocularis with a tumor-like behavior in the targeted organ, especially in the liver. Surgery with albendazole is first-line modality for AE. Drug discontinuation is usually based upon the parasitic viability shown by the positron emission tomography (PET) scan. However, as a demanding and expensive method, it is not widely practiced in majority of the endemic regions. Further understanding on the cytokine and chemokine response profiles in AE patients may provide an interesting insight for potential markers in viability assessment. Methods. Mice were inoculated with Echinococcus multilocularis intrahepatically to develop the hepatic AE murine model. Oral albendazole administration was then applied for three months after the first inoculation, and peripheral and regional immune cells including type 1 T helper cells (Th), Th2, Th17, regulatory T (Treg) cells, related cytokines, and chemokines were examined. Results. The hepatic AE lesion was confirmed by ultrasound examination resulting in a successful rate of 70%. Among the 17 cytokines and chemokines detected, plasma levels of IL-23 were significantly higher in E. multilocularis-infected mice when compared to the control group; furthermore, more obvious increasing levels were found after albendazole treatment ( p 〈 0.05 ). All chemokine levels other than eotaxin and MCP-3 were slightly higher in E. multilocularis-infected mice compared to the control group ( p 〉 0.05 ). Eotaxin levels were significantly decreased in mice with E. multilocularis infection followed by albendazole treatment ( p 〈 0.05 ). Both IL-17A and IL-23 expressions in hepatic AE lesions were significantly higher and related with disease activity. Conclusion. Albendazole administration influenced the balance of immune response and promotes the secretion of proinflammatory factors which is beneficial to parasite clearance. IL-23 seems to be associated with the successful albendazole treatment in mice with E. multilocularis infection; such a change could be translated into clinical application in the near future.
