    Kurzfassung: Circulating microRNAs (miRNAs) are potential biomarkers for cardiovascular diseases. Our study aimed to determine whether miR-22-5p, miR-132-5p, and miR-150-3p represent novel biomarkers for acute myocardial infarction (AMI). Plasma samples were isolated from 35 AMI patients and 55 matched controls. Total RNA was extracted, and quantitative real-time PCR and ELISA were performed to investigate the expressions of miRNAs and cardiac troponin I (cTnI), respectively. We found that plasma levels of miR-22-5p and miR-150-3p were significantly higher during the early stage of AMI and their expression levels peaked earlier than cTnI. Conversely, circulating miR-132-5p was sustained at a low level during the early phase of AMI. All three circulating miRNAs were correlated with plasma cTnI levels. A receiver operating characteristic (ROC) analysis suggested that each single miRNA had considerable diagnostic efficacy for AMI. Moreover, combining the three miRNAs improved their diagnostic efficacy. Furthermore, neither heparin nor medications for coronary heart disease (CHD) affected plasma levels of miR-22-5p and miR-132-5p, but circulating miR-150-3p was downregulated by medications for CHD. We concluded that plasma miR-22-5p, miR-132-5p, and miR-150-3p may serve as candidate diagnostic biomarkers for early diagnosis of AMI. Moreover, a panel consisting of these three miRNAs may achieve a higher diagnostic value.

    Kurzfassung: This study was aimed to explore the value of the twin neural network model in the classification and recognition of cardiac ultrasound images of patients with atrial fibrillation. 80 patients with cardiac atrial fibrillation were selected and randomly divided into experimental group (40 cases) and control group (40 cases). The twin neural network (TNN) model was combined with traditional ultrasound, Doppler spectrum, tissue velocity, and strain imaging technology to obtain the patient’s cardiac structure parameters and analyze and compare related indicators. The results showed that the total atrial emptying fraction (TA-EF value) of the experimental group was 53.08%, which was significantly lower than that of the control group ( P 〈 0.05 ). There were no significant differences in left atrial diameter (LAD), left ventricular end-diastolic diameter (LVEDD), left atrial maximum volume (LAVmax), and left ventricular ejection fraction (LVEF) between the two groups. In the experimental group, the average peak velocity of mitral valve annulus (Em) was 8.49 cm/s, the peak velocity of lateral wall systole (Vs) was 6.82 cm/s, and the propagation velocity of left ventricular blood flow (Vp) was 51.2 cm/s, which were significantly reduced ( P 〈 0.05 ). The average values of peak strains in the middle and upper left atrium of the experimental group were significantly lower than those of the control group ( P 〈 0.05 ). It can be concluded that the combined use of the TNN model can more accurately and quickly classify and recognize ultrasound images.

    Kurzfassung: Objective. To explore the predictive value of ABC bleeding score and SAMe-TT2R2 score on the risk of bleeding in patients with nonvalvular atrial fibrillation (NVAF) complicated with coronary heart disease (CHD) after anticoagulation. Methods. 149 patients with NVAF complicated with CHD were followed up in our hospital for one year. The bleeding events during the follow-up period were observed, the ABC bleeding score and SAMe-TT2R2 score were calculated, the predictive value of the two scoring methods for the main bleeding risk was analyzed by the ROC curve, and the AUC area under the ROC curve of the two scoring methods was compared by the Delong test. Results. There were 32 bleeding events during the follow-up period. The AUC of ABC bleeding score and SAMe-TT2R2 score were 0.775 ( P 〈 0.01 ) and 0.624 ( P 〈 0.05 ), respectively. The Delong test showed that the AUC of ABC bleeding score was significantly higher than that of SAMe-TT2R2 score (d = 2.177, P 〈 0.05 ). Conclusion. Both the ABC bleeding score and SAMe-TT2R2 score can predict the risk of bleeding after anticoagulation in patients with NVAF and CHD. The critical value of the SAMe-TT2R2 score for predicting bleeding events in patients with NVAF and CHD may need to be increased to 4 or 5, and the prediction ability of ABC bleeding score is significantly better than that of the SAMe-TT2R2 score.

    Kurzfassung: Objective. Pedicle screw loosening is one of the main complications after pedicle screw fixation. However, there are few reliable measures for prediction of screw loosening. The current study was carried out to find an effective method to use preoperative CT scanning as a predictor of screw loosening in the elderly patients and provide guidance for preoperative surgical planning. Methods. Patients who were treated with lumbar pedicle screw fixation procedure in our department for degenerative lumbar disorders between January 2015 and January 2021 were retrospectively included in the current study. CT scan attenuation of each vertebra was measured with Hounsfield units (HU). Screw loosening was determined in postoperatively X-ray tests. One-way analysis of variance (ANOVA) and receiver operating characteristic (ROC) curve analysis were carried out with IBMSPSS 24.00 software. Results. Screw loosening was observed in 44 of 215 patients (124 male, 91 female, average age 58.4 ± 7.6 years) during a mean follow-up time of 19.0 ± 11.2 months (range 12-32 months). No significant differences were found among the patients concerning patient gender, BMI, habit of smoking, and whether or not the patient had diabetes or suffered from spondylolisthesis ( P 〉 0.05 ). The average HU value of lumbar vertebra was 122.4 ± 32.8  HU in the screw loosening group and 142.4 ± 38.2  HU in the control group, and the difference was significant ( P 〈 0.01 ). ROC curve analysis revealed that the average HU value of L1-L5 has a relatively larger area under the curve (AUC) of 0.689 (95% CI: 0.605-0.773). With the sensitivity of 68% and specificity of 57%, a HU cut-off value of ≤124 HU is a plausible cut-off point to predict screw loosening. Conclusions. A prospective CT scan HU value-based prediction can be used to decide whether or not to use screw augmentation methods. A cut-off L1-L5 average HU value of 124 HU can be used as an independent risk factor for screw loosening in instrumented lumbar vertebra. More predictive indexes should be involved to achieve higher sensitivity and specificity in future clinical practice.

    Kurzfassung: The composition of hydrothermal plumes reflects the physical and chemical characteristics of seafloor hydrothermal fluids, which in turn reflects the host rock and subseafloor reaction conditions as well as the water column processes that act to alter the plumes as they disperse and age. Here, we show that the turbidity, current, pH value, dissolved Fe (dFe), and dissolved Mn (dMn) compositions of hydrothermal plumes can be used to understand the spatial distribution and source of hydrothermal systems in the submarine geological environment. Data were obtained from 18 hydrocast stations, among which the water column samples were collected at 8 stations during the MANUS cruise of R/V KEXUE in 2015. The results showed that the Satanic Mills plume and Fenway plume rose approximately 140 m and 220 m above the seafloor, respectively. In the Satanic Mills plume, dFe remained longer than dMn during lateral plume dispersal. There was a clear intersection of the Satanic Mills plume and Fenway plume between 1625 m and 1550 m in the PACMANUS hydrothermal field, and the varied dispersion trends of the mixed plumes were affected by current velocities at different depths. The physical and chemical properties of the seawater columns in the Manus Basin were affected by the input of high-Mn, high-Fe, and low-Mg vent fluids. The turbidity and dFe, dMn, and dissolved Mg concentrations in the sections of the plumes proximal to the Satanic Mills, Fenway, and Desmos vent sites were generally higher (turbidity, Mn, and Fe) and lower (Mg) than those in the sections of the plumes that were more distal from the vent sites. This implied that the plumes proximal to their vent fluid sources, which were interpreted to have relatively young ages, dispersed chemically over time, and their concentrations became more similar to those of the plumes that were more distal from their vent fluid sources.

    Kurzfassung: In recent years, the incidence of esophagogastric junction cancer has increased year by year. It is a special type of gastric cancer, with 80% of patients being clinically in the middle and late stages. The traditional treatment methods are extremely ineffective, and the accuracy of preoperative staging is not good enough. At present, the medical treatment for esophagogastric junction cancer mainly adopts surgery and postoperative adjuvant therapy. The current mainstream clinical diagnostic methods of esophagogastric junction cancer before concurrent neoadjuvant chemoradiotherapy are X-ray, CT examination, and gastroscopic diagnosis. However, these clinical diagnostic methods have many limitations. Endoscopic ultrasonography (EUS) can accurately locate malignant tumors in the digestive tract, surrounding microstructures. It can diagnose lymphatic metastasis so as to provide a clear imaging basis for neoadjuvant chemoradiotherapy. This method can also effectively improve the prognosis of the esophagus and stomach according to the characteristics of the patient. In this experiment, we conducted a controlled trial on patients with stage III esophagogastric junction cancer, divided into an experimental group (neoadjuvant chemotherapy + surgery) and a control group (conventional surgery). The preoperative EUS staging in the control group, the preoperative EUS staging in the neoadjuvant chemoradiotherapy group, and the postoperative pathological staging were compared. The experiment showed that in the control group, the preoperative and postoperative accuracy of EUS was 89.2%, while the preoperative and postoperative accuracy of CT examination was only 62.5%. In the experimental group, the preoperative and postoperative accuracies of EUS and CT were 79.6% and 56.7%, respectively. EUS has both specificity and accuracy due to CT examination. Through studying EUS technology in the staging and diagnosis of esophagogastric junction cancer, the therapeutic effect of esophagogastric junction cancer can be improved. The prognosis of esophagogastric junction cancer can also be improved.

    Kurzfassung: The aim was to analyze the infection, influencing factors, and clinical manifestations of Helicobacter pylori infection, coronary heart disease, and cytotoxin-associated protein A infection, so as to provide reference for the improvement of clinical diagnosis and treatment level of in-depth treatment. This paper presents a clinical observation method based on Helicobacter pylori infection, risk factors, and cytotoxin-associated protein A in patients with coronary heart disease. Methods. 237 patients with CHD diagnosed and tested by 14C breath test were selected from inpatients of cardiovascular diseases in a hospital for retrospective analysis. The clinical data, serum deepening indicators, Hcy, and other factors were analyzed through general condition investigation, previous history investigation, and physical examination. The patients were observed by the SPSS22.0 statistical data processing method. The results showed that among the respondents, 175 cases were HP-positive, the infection rate was 73.8%, 77 patients with stable angina pectoris were 64.9%, and 160 patients with acute coronary heart disease were 78.1%. The difference between the groups was statistically significant P 〈 0.05 . Conclusion. Helicobacter pylori cytotoxic-associated protein A can increase the risk of gastric cancer, and Helicobacter pylori eradication treatment is more conducive to reduce the incidence of gastric cancer and ensure the safety of patients.
