    Abstract: The uses of intraoperative spinal sonography are reviewed, including localization of lesions and monitoring of surgical correction. The sonographic appearance of intramedullary and extramedullary masses and cysts are reviewed and illustrated. The use of sonography in the surgical treatment of spinal fractures, herniated disks, and spinal stenosis is also discussed.

    Abstract: Holoprosencephaly is a complex brain anomaly in which the primitive forebrain (prosencephaly) fails to properly divide into cerebral hemispheres. The frequency of holoprosencephaly has been estimated to be 1 in every 5,200 to 16,000 live births. The complex cerebral and facial anomalies that are observed in infants and fetuses with holoprosencephaly will challenge the sonographer who encounters them. Prenatal detection of the milder forms is particularly difficult; the infant with mild holoprosencephaly may live for years in a vegetative state. Knowledge of the range of malformations encountered in holoprosencephaly will help the sonographer accurately identify the findings.

    Abstract: Twin embolization syndrome (TES) is a rare complication of twin-twin transfusion after intrauterine demise of one of a pair of twins sharing the same placenta. In such cases, infarcts in the surviving co-twin can cause structural and functional changes, presumably due to embolization of debris, thrombus, thromboplastic material, and toxins from the dying twin's circulation into the circulation of the other twin. Sonographic, clinical, and pathologic findings in 3 new cases are reported, and 38 previously reported cases of TES are reviewed. Of the 41 cases, 34 (83%) had evidence of a destructive structural brain defect. In the 34 cases with brain defects, 23 (68%) had intracranial cystic changes. Microcephaly had developed in the surviving twin by the time of delivery in 7 (41%) of the 17 cases with brain defects and evidence of twin fetal demise before 22 weeks. Intestinal atresia was noted in 4 of 41 cases (10%), and renal dysplasia or renal cortical necrosis was observed in 11 cases (27%). In cases of intrauterine demise of a monochorionic twin, careful follow-up evaluation of the surviving twin should be performed.

    Abstract: A case of an allantoic cyst of the umbilical cord with urinary tract involvement was detected in a fetus by sonographic examination. The pregnancy was followed to term, and a healthy infant was born. Embryologic development of the umbilical cord, clinical significance, and differential diagnosis of cystic lesions of the cord are discussed.

    Abstract: Ultrasound is a rapid, noninvasive, cost-effective method of evaluating the spine in infants younger than six months of age. This report reviews sonographic scan procedure and normal sonographic spinal anatomy, and illustrates the sonographic appearance of occult spinal anomalies, such as meningocele, meningomyelocele, tethered cord, diastematomyelia, and sacrococcygeal teratoma.

    Abstract: Beckwith-Wiedemann syndrome (BWS) was first described by Beckwith in 1963 and Wiedemann in 1964 as a syndrome of macroglossia, visceromegaly,. omphalocele, gigantism, and neonatal hypoglycemia. Additonal features of the syndrome, which occurs in approximately 1 in every 14,000 births, have been subsequently described. The authors report 2 cases of fetal BWS observed sonographically, and review 16 previously reported cases. Pohydramnios and growth acceleration may occur before 20 weeks, but macrlossia has not been reported before 28.5 weeks. Searching for ear creases in a fetus with suspected BWS may be merited, but visualization of normal ear lobes does not exclude the possibility of BWS.

    Abstract: Burkitt's lymphoma is one of the fastest growing tumors, and it primarily affects children and young adults. Since sonography is often the first imaging procedure performed in these patients, knowledge of the wide range of sonographic appearances of this type of tumor can aid the sonographer in recognizing Burkitt's lymphoma. In this report, we review and illustrate the sonographic appearance of Burkitt's lymphoma involving abdominal organs, including the liver, gallbladder, pancreas, kidneys, bowel, and mesentery.

    Abstract: The term arthrogryposis is applied to a large group of conditions that result in joint contractures, abnormal positioning of hands or feet, and other neuromuscular defects. Abnormalities of the brain, spinal cord, peripheral nerves, muscles, or connective tissue lead to similar joint stiffness and contractures. Careful sonographic examination of an affected fetus can provide valuable information that will help in the assessment of diagnosis, prognosis, and risk of recurrence.

    Abstract: Alloimmune thrombocytopenia is a rare condition in which antibodies from the mother's circulation cross the placenta and destroy the platelets in the fetus. Approximately 14% of affected fetuses or neonates develop intracranial hemorrhage, leading to death or long-term central nervous system disabilities. After treating two such cases, 26 previously reported cases of alloimmune thrombocytopenia with intracranial hemorrhage were examined to discover if these hemorrhages exhibit a typical sonographic appearance, course, and time of hemorrhage. When intracranial hemorrhage occurs in cases of alloimmune thrombocytopenia, it has a typical appearance in 76% of cases: a large hematoma in the center of the cerebral hemisphere. There is no difference between genders in the occurrence of alloimmune thrombocytopenia unless hemorrhage occurs: 72% of neonates with alloimmune thrombocytopenia and intracranial hemorrhage are male. Intracranial hemorrhage occurs before labor and delivery in 76% of cases. Sonographers cannot depend on a history of the condition in previous pregnancies as an indication of alloimmune thrombocytopenia: only 35% of neonates with alloimmune thrombocytopenia and hemorrhage have previously affected siblings.
