Abstract:
In emergency departments, patients with nontraumatic associated critical illnesses are admitted nationwide. Little is known about the structure, organization and equipment for nontraumatic resuscitation room management. A survey was therefore used to analyze the status quo. Methods The “Resuscitation Room” working group of the German Society for Interdisciplinary Emergency and Acute Medicine (DGINA) sent an online survey by email to the 420 medical directors of the DGINA member register. A reminder was sent out two weeks after the initial letter. The results were extracted and evaluated in an anonymized database. Results A total of 131 completed questionnaires resulted in a response rate of 31%. The survey covered basic (24%), extended (39%) and comprehensive emergency care (37%) hospitals. As the level of care increased, so did the annual patient contacts (21,000 vs. 31,000 vs. 39,000), the monitor stations in the emergency department (9 ± 4 vs. 13 ± 6 vs. 18 ± 10), the beds in the associated decision unit (4 ± 5 vs. 10 ± 17 vs. 13 ± 12), the available resuscitation room beds (1 ± 1 vs. 2 ± 1 vs. 3 ± 1) and their size (m 2 : 31 ± 16 vs. 35 ± 9 vs. 38 ± 14). With regard to various equipment features (e.g. X‑ray gun carriage: 58% vs. 65% vs. 78%; computed tomography in the resuscitation room: 6 vs. 12 vs. 27%), there were significant differences depending on the level of care. While cooling systems were available in all levels of care in 30% of the cases, other equipment features (e.g., video laryngoscopy: 65 vs. 80 vs. 86%; bronchoscopy 29 vs. 22 vs. 45%) and special emergency procedures (e.g. REBOA [resuscitative endovascular balloon occlusion of the aorta]: 3 vs. 5 vs. 12%; ACCD [automated chest compression device] : 26 vs. 57 vs. 61%) were found more frequently in higher levels of care. Conclusion The present results show for the first time the current status of nontraumatic resuscitation room management in different levels of care in Germany. Recommendations on equipment contents for nontraumatic resuscitation room management must be formulated in the future.
