    Abstract: The coronavirus disease (COVID) pandemic has resulted in a major disruption in healthcare that has affected several medical and surgical specialties. European and American Vascular Societies have proposed deferring the creation of an elective vascular access (VA) [autologous or prosthetic arteriovenous fistula (AVF) or arteriovenous graft (AVG)] in incident patients on haemodialysis (HD) in the era of the COVID pandemic. The aim of this study is to examine the impact of the COVID pandemic on VA creation and the central venous catheter (CVC)-related hospitalizations and complications in HD patients dialyzed in 16 Spanish HD units of three different regions. Methods We compared retrospectively two periods of time: the pre-COVID (1 January 2019–11 March 2020) and the COVID era (12 March 2020–30 June 2021) in all HD patients (prevalent and incident) dialyzed in our 16 HD centres. The variables analysed were type of VA (CVC, AVF and AVG) created, percentage of CVC in incident and prevalent HD patients, CVC-related hospitalizations and complications (infection, extrusion, disfunction, catheter removal) and percentage of CVC HD sessions that did not reach the goal of Kt ( & gt;45) as a marker of HD adequacy. Results A total of 1791 VAs for HD were created and 905 patients started HD during the study period. Patients who underwent vascular access surgery during the COVID period compared with pre-COVID period were significantly younger, with a significant decrease in surgical activity to create AVFs and AVGs in older HD patients ( & gt;75 and & gt;85 years of age). There was a significant increase in CVC placement (from 59.7% to 69.5%; P & lt; 0.001) from the pre-COVID to the COVID period. During the COVID pandemic, a significantly higher number of patients started HD through a CVC (80.3% versus 69.1%; P & lt; 0.001). The percentage of CVC in prevalent HD patients has not decreased in the 19 months since the start of the pandemic [414 CVC/1058 prevalent patients (39.4%)]. No significant changes were detected in CVC-related hospitalizations between the pre-COVID and COVID periods. In the COVID period, a significant increase in catheter replacement and the percentage of HD session that did not reach the HD dose objective (Kt & gt; 45) was observed. Conclusions COVID has presented a public health system crisis that has influenced VA for HD, with an increase in CVCs relative to AVFs. A decrease in HD sessions that did not reach the HD dose objective was observed in the COVID period compared with a pre-COVID period.

    Abstract: To develop and validate a prediction rule to identify well-appearing febrile infants aged ≤90 days with an abnormal urine dipstick at low risk of invasive bacterial infections (IBIs, bacteraemia or bacterial meningitis). Design Ambispective, multicentre study. Setting The derivation set in a single paediatric emergency department (ED) between 2003 and 2017. The validation set in 21 European EDs between December 2017 and November 2019. Patients Two sets of well-appearing febrile infants aged ≤90 days with an abnormal urine dipstick (either leucocyte esterase and/or nitrite positive test). Main outcome Prevalence of IBI in low-risk infants according to the RISeuP score. Results We included 662 infants in the derivation set (IBI rate:5.2%). After logistic regression, we developed a score (RISeuP score) including age (≤15 days old), serum procalcitonin (≥0.6 ng/mL) and C reactive protein (≥20 mg/L) as risk factors. The absence of any risk factor had a sensitivity of 96.0% (95% CI 80.5% to 99.3%), a negative predictive value of 99.4% (95% CI 96.4% to 99.9%) and a specificity of 32.9% (95% CI 28.8% to 37.3%) for ruling out an IBI. Applying it in the 449 infants of the validation set (IBI rate 4.9%), sensitivity, negative predictive value and specificity were 100% (95% CI 87.1% to 100%), 100% (95% CI 97.3% to 100%) and 29.7% (95% CI 25.8% to 33.8%), respectively. Conclusion This prediction rule accurately identified well-appearing febrile infants aged ≤90 days with an abnormal urine dipstick at low risk of IBI. This score can be used to guide initial clinical decision-making in these patients, selecting infants suitable for an outpatient management.

    Abstract: Introduction Graft versus leukemia (GvL) effect after hemotopoietic stem cell transplantation (HSCT) is mediated by donor immune cells recovery. Natural Killer (NK) cell alloreactivity is controlled by the interaction of activatory receptors and inhibitory killer-immunoglobulin-like receptors (KIRs) with major histocompatibility locus class I antigens on the leukemia cells. Haploidentical setting is a plattform for NK cell alloreactivity however HLA identical setting remains unclear. Methods and Patients We performed KIR-genotyping of HLA-identical sibling donors in 35 pediatric CD34 selection peripheral blood stem cell transplantations to identify genetic factors affecting leukemia relapse and overall survival. Univariate analysis of leukemia relapse and KIR genotyping was performed in order to identify independent variables predictive of outocome for pediatric acute lymphoblastic leukemia (ALL), (n=20) and acute myeloblastic leukemia (AML), (n=15). Results Donor B haplotype was observed in 21 cases (60%). Statistical analysis shown that donor B haplotype was associated with significantly more relapse in leukemia pediatric patients (38% vs. 0%) and worse overall survival (40% vs. 7%). Further analysis revealed that 2DL5a, 2DS3 and 2DS5 were associated with an increased rate of leukemia relapse (47% vs. 6%, 54% vs. 10% and 58% vs. 13%, respectively) and 2DL5a, 2DS1 and 3DS1 were associated with a worse overall survival (48% vs. 6%, 64% vs. 10% and 58% vs. 13%, respectively). No difference was observed in patients KIR haplotype or donor-recipient KIR haplotype mismatch. Conclusion In our study, which included only reduced intensity conditioning without antithymocyte globulin followed by related peripheral blood stem cell transplantation, we found a significant worse impact of donor KIR B haplotype in stem cell transplantation outcome. Our results suggest that donor KIR B haplotype (2DL5a, 2DS3 and 2DS5) increases leukemia relapse and also donor KIR B haplotype (2DL5a, 2DS1 and 3DS1) confer significant survival damage to HLA-identical sibling HSCT.     Abstract: With an increase in human population over the past 30 years, regional land use in south Texas has shifted from grassland and shrubland to a peri-urban matrix. Despite this shift from natural areas to more anthropogenically modified habitats, native red harvester ants (Pogonomyrmex barbatus) have maintained nest sites within parts of these matrices. To determine which habitat characteristics in a peri-urban landscape may play a role in red harvester ant nest site selection, we mapped the location of nests in 2020 and 2021. We then evaluated nest presence and absence relative to elevation, percentage of surrounding impervious surfaces, distance to roadways, and tree canopy cover (using NDVI). For a sub-sample of the study site, we also measured soil moisture and estimated the potential foraging area per colony with Voronoi tessellation. We found that nests were clustered together near high human-use areas such as athletic fields, lawns, sidewalks, and railroad tracks. Nests were more likely to be found in areas with higher elevation and lower tree canopy cover, with no impact from surrounding impervious surfaces or soil moisture. In fact, many nests were observed immediately adjacent to roadways and in paved parking lots. Red harvester ants are highly adept at nesting in disturbed, urbanized matrices, but still appear to be constrained by certain environmental factors like shading, potential flood risk (elevation), and access to food resources (foraging area).

    Abstract: Supplementary cementitious materials (SCMs), based on pozzolanic materials, improve durability against corrosion and mechanical properties of concrete structures by decreasing their permeability. Even though the influence of SCMs on the chloride combination with the cement phases has been widely studied, its effects on electrokinetic remediation processes such as electrochemical chloride extraction (ECE) have not been clarified. For this reason, the influences of two SCMs, fly ash (FA) and blast-furnace slag, on the extraction of chloride through the concrete net pore have been studied in this paper to determine the viability of the application of electrochemical chloride treatments in these structural materials. Alternative electrochemical indicators to the ones included in the standards are also proposed to better determine the final point of the treatment. A cement replacement of 8% on both SCM (FA and slag) has been tested, and in addition to charge density, chloride content, and corrosion measurement at the end of the treatment (included in the standards), different electrical and electrochemical indicators such as electrical resistivity, EIS, or depolarized potentials are used to monitor the ECE. The influence of the treatment on disconnected steel has been also studied. In the case of slag mortar, no steel passivation was reached, while in the case of FA, the passivation of the steels connected to treatment was reached in the same way as in plain CEM I specimens. A degree of protection is also detected in the nonconnected steel, which means that substitution of 8% in FA does not affect treatment efficiency and can also partially protect the metallic elements embedded in the same electrolyte but not connected to the treatment.

    Abstract: The present study analyzes the impact of parenting styles on adolescents’ self-esteem and internalization of social values in three countries, Spain, Portugal and Brazil. The sample of the study was comprised of 2091 adolescents from Spain (n = 793), Portugal (n = 675), and Brazil (n = 623) from 12–18 years old (52.1% females). The four types of parenting styles, authoritative, indulgent, authoritarian and neglectful, were measured through the warmth and strictness dimensions of the Scale of Parental Socialization ESPA29. The two criteria variables were captured with the five dimensions of the AF5, Five-Factor Self-Concept Questionnaire, and with self-transcendence and conservation Schwartz values. Results confirm emergent research in parenting socialization: the use of parental warmth is evidenced as key for adolescent self-esteem and internalization of social values in the three countries analyzed. Indulgent and authoritative parenting (both characterized by parental warmth) are associated with the highest value internalization in the three countries. Furthermore, indulgent parenting (use of warmth) is associated with the highest adolescent self-esteem, overcoming authoritative parenting (use of warmth and strictness). The influence of parenting over adolescent self-esteem and values internalization is maintained independent of the differences in self-esteem and value priorities observed in the cultural context, the sex and age of the participants.
