    Abstract: Purpose. This research mainly discussed the impacts of epidural anesthesia (EA) and general anesthesia (GA) on perioperative cognitive function (CF) and deep vein thrombosis (DVT) in patients undergoing total knee arthroplasty (TKA). Methods. One hundred and twenty-four patients undergoing TKA in our hospital between July 2015 and October 2021 were selected, of which 74 patients received EA (research group) and the other 50 patients received GA (control group). Perioperative CF, DVT, stress response indicators (norepinephrine, NE; cortisol, Cor), and heart rate (HR) levels were observed and compared. Risk factors affecting DVT of TKA patients were analyzed by logistic regression. Results. The research group had statistically better CF than the control group, with notably lower NE, Cor, and HR levels and incidence of DVT. Logistic regression analysis showed that the type of anesthesia and MoCA were risk factors for DVT in TKA patients. Conclusion. EA is more feasible for patients undergoing TKA, which is conducive to improving their CF, relieving stress responses, and reducing the incidence of DVT, with a certain sedative effect.

    Abstract: Purpose. The study is aimed at assessing the role of preoperative computerised tomography (CT) examination in the quality of reduction and outcomes in elderly patients with intertrochanteric fracture. Methods. The elderly patients with an intertrochanteric fracture who were treated with proximal femoral nail antirotation were included. The patients were divided into the CT group and the no-CT group according to the presence of preoperative CT examination. Patients’ baseline characteristics, quality of reduction, and function were recorded at follow-up. Functional outcomes were evaluated using the Harris hip scores (HHS). Results. Totally, the study included 182 patients with intertrochanteric fractures, with 85 in the CT group and 97 in the no-CT group, admitted between January 2018 and June 2019. There was no difference in the quality of reduction, HHS, the fracture healing, or postoperative complications between the CT group and the no-CT group. However, the CT group experienced the shorter mean operative time and blood transfusion, compared to the no-CT group. Conclusions. The preoperative CT examination seems to be excessive for elderly patients with an intertrochanteric fracture.

    Abstract: This study focused on the key factors affecting the tensile strength of cold recycled asphalt mixture with cement and emulsified asphalt. The specific surface areas and strength of RAP were analyzed. The interaction between the emulsified asphalt and cement was observed. Comprehensive laboratory testing was conducted to evaluate the influences of RAP, emulsified asphalt, and cement on the tensile strength of cold recycled asphalt mixture. It is found that although RAP is used as aggregates, its inner structure and strength are much different from real aggregates. The strength of RAP has decisive effect on the strength of cold recycled asphalt mixture. New aggregates and fine gradation design can help improve the bonding between RAP and binder. For emulsified asphalt, slow setting of asphalt can give sufficient time for cement to hydrate which is helpful for strength formation in the cold recycled asphalt mixture. The high viscosity of asphalt can improve the early strength of cold recycled asphalt mixture that is important for traffic opening in the field. Cement is an efficient additive to improve the strength of cold recycled asphalt mixtures by promoting demulsification of emulsified asphalt and producing cement hydrates. However, the cement content is limited by RAP.

    Abstract: In order to improve the thermal stability of wide embankment for the Qinghai-Tibet Expressway (QTE) to be constructed in permafrost zones on Qinghai-Tibet plateau, a new kind of reflective-resistant-ventilated coupling structure (RRVCS) was proposed and designed. A numerical model was then developed through ABAQUS to investigate the influences of RRVCS on the thermal regime of wide embankment. The temperature field and thawing depth of normal wide embankment without protective measures, wide embankment with crushed stone layer, and wide embankment with RRVCS were compared to verify the cooling effectiveness of RRVCS based on numerical analysis. Vermiculite powder has great influences on the thermal property of asphalt mixture, and it can be reduced by 22.8%, 29.5%, 37.3%, and 50.6% after the addition of 4%, 6%, 8%, and 10% vermiculite powder, respectively. Under the same condition, the temperature of RRVCS embankment is the lowest and its thermal stability is the best. Setting crushed stone layer can improve the thermal stability of embankment. However, the improvement is limited for wide embankment with width exceeding 26 m. RRVCS has the best protective effects on the thermal stability of wide embankment and is recommended as a protective measure for wide embankment of expressway in permafrost regions of Qinghai-Tibet Plateau.

    Abstract: Background. The number of articles of clinical and basic research for osteonecrosis of the femoral head (ONFH) is increasing, yet, to our knowledge, there is still a lack of bibliometric analysis on ONFH articles. The purpose of this study was to identify the top 100 cited (T100) articles related to ONFH research and to analyze the characteristics and qualities of these articles. Methods. The T100 articles on ONFH were retrieved from the Web of Science database. The information about each article including citations, titles, authors, journals, countries, institutions, and keywords was recorded for bibliometric analysis. Results. The T100 articles related to ONFH were mainly published from 1991 to 2010 ( n = 70 ) and were originated from 24 countries. The USA, China, and Japan were the most productive countries in this regard. The most prolific institution was the University of Pennsylvania from the USA with 6 publications and 742 citations. The most cited article was published in 1995 by Professor Steinberg ME. The five most frequently occurring keywords were “femoral head,” “osteonecrosis,” “core decompression,” “total hip arthroplasty,” and “follow up.” The keywords like “bone tissue engineering” and “extracorporeal shock wave” have emerged in recent years. Conclusions. The USA, China, and Japan contributed greatly in terms of the T100 articles. The outcomes of core decompression and total hip arthroplasty gathered the most research interests. In recent years, bone tissue engineering and extracorporeal shock wave have become new trends. However, the mechanism of ONFH is still unclear.

    Abstract: Sciatica has been widely studied, but the association of sciatica with immune infiltration has not been studied. We aimed to screen key genes and to further investigate the impact of immune infiltration in patients with sciatica. The bioinformatics analyzes were performed based on the GSE150408 dataset. Subsequently, we used CIBERSORT to study the immune infiltration in the disease group. Results showed that 13 genes were with differentially expressions in the sciatica group compared to healthy participants, including 8 up-regulated and 5 down-regulated genes. Through the LASSO model and SVM-RFE analysis, a total of 6 genes have intersections, namely SLED1, CHRNB3, BEGAIN, SPTBN2, HRASLS2, and OSR2. The ROC curve area also confirmed the reliability of this method. CIBERPORT analysis showed that T cell gamma delta infiltration decreased and neutrophil infiltration increased in the disease group. Then the association of these six key genes with immune infiltration was further verified. We found six overlapping genes and found that they were closely associated with the total immune infiltration in the sciatic nerve disease group. These findings may provide new ideas for the diagnosis and therapeutics of patients with sciatica.

    Abstract: This study characterized the impacts of air voids on the low-temperature cracking behavior of dense-graded asphalt concrete. Virtual low-temperature bending beam test for dense-graded asphalt concrete was built and executed by discrete element method and PFC3D (particle flow code in three dimensions). Virtual tests were applied to analyze the impacts by content, distribution, and size of air voids on the low-temperature properties of dense-graded asphalt concrete. The results revealed that higher air void content results in worse low-temperature property of dense-graded asphalt concrete, especially when the air void content exceeds the designed air content; even with the same designed air void content, different distributing condition of air voids within asphalt concrete leads to different low-temperature properties of asphalt concrete, especially when the air void content in the central-lower part of testing sample varies. Bigger size of single air void which tends to form interconnected air voids within asphalt concrete has more harmful impacts on the low-temperature properties of asphalt concrete. Thus, to achieve satisfied low-temperature properties of dense-graded asphalt concrete, it is critical to ensure the designed air void content, improve the distribution of air voids, and reduce the interconnected air voids for dense-graded asphalt concrete.
