    Abstract: BACKGROUND: Vaccination is currently considered the most successful strategy against the SARS-CoV-2 virus. However, cases of infection despite vaccination, so-called breakthrough infections, have been reported worldwide. AIM: To evaluate the impact of vaccination against the new coronavirus infection COVID-19 on the morbidity of university students. MATERIALS AND METHODS: The incidence of new coronavirus infection (COVID-19) among the students of North-Western State Medical University named after I.I. Mechnikov (further University) from September 1 to December 15, 2020 and 2021 was analyzed. There were 4876 and 4681 students under observation. Data on vaccination, probable site of transmission infection were collected by interviewing the ill people. Statistical processing of data was performed using EpiInfo software. RESULTS: For the analyzed period 191 cases of COVID-19 among students were detected, the incidence of COVID-19 was 4.08 per 100 students, for the same period of the academic year 2020 it was 5.50, despite the fact that the incidence among St. Petersburg residents in 2021 was 1.75 times higher than in 2020. Re-infection was detected in 35 (18.3%) cases, 18 of whom were also vaccinated against COVID-19. A probable place of transmission infection was established in 36.1% of the cases, the most frequent being contact with a patient at their place of work in a health-care facility. By December 15, 2021, a total of 62.8% of students had been vaccinated against COVID-19. The incidence among vaccinated students was 2.72 per 100 students and 4.94 per 100 among unvaccinated students. A risk factor for breakthrough infections after vaccination was close contact with the source of infection: vaccinated persons had close contact in 50% of cases, compared with 28.9% of unvaccinated persons. The most important were contact with a patient in a health care setting and having multiple sources of infection, 31.1% and 5.6%, respectively. CONCLUSIONS: Vaccination against COVID-19 was an effective preventive intervention. A risk factor for disease after vaccination is close contact with the source of infection. Establishment of collective immunity after vaccination is decisive for the vaccination-to-disease ratio, which starts to develop with 70-80% of vaccinated individuals. The use of a mask in public places and social distancing remain important preventive measures.

    Abstract: INTRODUCTION: The sudden appearance of a new coronavirus infection in the world has posed difficult challenges for science to determine the main clinical manifestations of this disease, as well as to approve standards for diagnosis, treatment and prevention. Research of the symptoms of mild COVID-19 is continuing. The data obtained in such studies are particularly relevant for primary care physicians. THE AIM: To evaluate the features of the clinical course of COVID-19 among students of the North-Western State Medical University named after I.I. Mechnikov, placed in the observatory, during the first and second waves of rising morbidity. MATERIALS AND METHODS: The study was conducted on the basis of the observatory of the NWSMU named after I.I. Mechnikov in two stages: in the period from 21.04.2020 to 31.08.2020 (the first wave) and from 16.09.2020 to 31.12.2020 (the second wave). Demographic, epidemiological, and clinical-anamnestic data were recorded in all participants of the study. RESULTS: The study involved 309 people: 39.8% men (n = 123) and 58.9% women (n = 182). The average age of the respondents was 21.5 2.6 years. The number of participants in the first and second waves was 113 and 196 people, respectively. The average age of participants with COVID-19 in the first wave was 1.5 years less and was 21.2 2.5 years (р 0.001). There was a significant increase in the frequency of the following symptoms: dry cough, loss of smell, headache, general weakness, increased body temperature to 38.0 С, nasal congestion (р 0.05) in students who were monitored in the second wave of increased incidence of COVID-19 compared to the first. The frequency of symptoms such as chest congestion, lower back pain, and fever above 38.0 С significantly decreased (р 0.05). There was a significant decrease in the incidence of viral pneumonia in patients who were observed during the second wave, compared with the data obtained during the first wave (р 0.001).

    Abstract: The article is devoted to the peculiarities of clinical manifestations and the severity of a new coronary virus infection in a university student transferred to an observatory organized in a hotel-type hostel to prevent the spread of COVID-19 among students living in hostels. The data on the epidemiological history of the patient, the results of clinical observation and examination are provided. The tactics of managing a patient with a suspected of COVID-19 on an outpatient basis, symptoms that determine the indications for hospitalization, the results of examination and treatment in a hospital, and further observation at the observatory are considered. A new coronavirus infection can lead to a rapid deterioration in the condition of young patients, which does not correlate with indicators indicating lung damage on the 5th day of the disease. Persistent hyperthermia and severe weakness with anorexia can be indicators of the complicated course of the disease, including the development of hyperactive inflammatory syndrome. Hypochromic anemia can be another disease, which is an unfavorable background for the development of COVID-19. The course of the new coronavirus infection in young patients requires careful attention and further study.

    Abstract: The article is devoted to the peculiarities of providing medical care for coronavirus infection in a university student who has been moved to an observatory organized in a hotel-type hostel of the university to prevent the spread of COVID-19 among students living in hostels. The article provides data on the positive experience of using dexamethasone in the treatment regimen for moderately severe disease, follow-up and examination data. The article considers the tactics of managing a patient with suspected COVID-19 in the presence of data for pneumonia according to computed tomography and clinical manifestations characteristic of a new coronavirus infection, which was later confirmed in the study of antibodies to SARS-CoV-2. The importance of round-the-clock medical monitoring of persons placed in the observatory, finding out the details of the epidemiological history from them, as well as determining the tactics of their treatment with limited hospitalization opportunities in a growing number of patients with severe manifestations of COVID-19 requiring inpatient treatment is emphasized. A new coronavirus infection can lead to a rapid deterioration of the condition in young patients, which is not always manifested by severe lung damage according to computed tomography data, but is characterized by persistent fever that cannot be stopped by antipyretics, as well as severe muscle pain, severe asthenia, which can be considered as clinical indicators of the development of hyperactive inflammatory syndrome. The use in the treatment regimen of intravenous dexamethasone at a dose of 0.1 mg / kg of body weight twice led to a rapid normalization of temperature, relief of all clinical symptoms and was not accompanied by any complications both in the early period of convalescence and 2 months after discharge from the observatory.

    Abstract: The choice of lung cancer treatment method directly depends on the diagnostisc and verification of the histological type of tumor. Important diagnostic criteria are DNA accumulation index and proliferation index of Ki-67. Materials and methods. DNA accumulation index was assessed in 200 cases of lung cancer using a microspectrophotometric method. Ki-67 proliferative index was calculated using immunohistochemical study in histological material with antibodies to Ki-67, Cytokeratin 5/6, TTF-1, Chromogranin A, Synaptophysin, Keratin.Pan, CD56. Results. As the degree of tumor differentiation decreases, DNA accumulation index and Ki-67 proliferative index increase, serving as criteria for the aggressive behavior of the tumor. Conclusions. Correlation analysis revealed a statistically significant relationship between DNA accumulation index and Ki-67 proliferative index (r = 0.987, p 0.05). The obtained data may be useful for diagnosis and prognosis in patients with lung cancer.

    Abstract: Improving the quality of CPR is an important task for future doctors, which is why it is necessary to understand the degree of loss of skill during the training. The study was conducted with the purpose of revealing the degree of decrease in the effectiveness of performing resuscitation measures for graduate students in the specialty “General Medicine”. Training and control skills were carried out at the Department of Family Medicine and at the Attestation and Training Simulation Center of North-West State Medical University. I.I. Mechnikov. Statistical analysis was carried out on a random sample from the general population, the results obtained (n = 366). The parameters were evaluated: the depth of compression, the position of the hands during compression, the complete release of the hands between the compressions, the frequency of compression, the volume of inspiration with artificial ventilation (IVL), the inspiration rate of the ventilator. The obtained results revealed a tendency to decrease the efficiency of cardiopulmonary resuscitation. Considering the results, it should be noted that most of the estimates for the initial control are in the range 79-85% (mean 78.51%), and at a repeated evaluation in a month in the range of 70-73% (mean value 73.23%). Based on the results obtained, it follows that the “survivability” of the skill for one month is maintained at a consistently high level, but it should be noted a slight decrease in the effectiveness of the indicators: the position of the hands during compression, the complete release of the hands between the compressions, the inspiration rate of the ventilator.

    Abstract: The article is devoted to the study problems associated with the education of graduates of medical universities. The aim of the study is to explore possible ways to optimize the training of students for the practical exam in the simulation center, taking into account their educational needs A questionnaire was developed to assess students ‘attitude to choosing the optimal method of training, including questions about the socio-demographic characteristics of the respondents, the most difficult practical exam stations, the choice of the optimal method of training and the level of students’ involvement into the process of self education for the exam during the 6th year. Statistical data processing was performed using the SPSS 20.0 software package (SPSS Inc., Chicago, IL, USA). The study included 100 students randomly selected from among those enrolled in the 6th year of the medical faculty of the NWSMU. I.I. Mechnikov. Data analysis showed that the most difficult stations are “Providing emergency medical care” and “Emergency states”. The “training with a teacher in a simulation center” had the highest rating from all proposed education methods. Neither the socio-demographic characteristics nor the form of funding for education had an effect on this choice. Despite of fears related to the complexity of these stations, the number of people trying to master them was 40%. Conclusions. 1. The most difficult for students are the skills of providing emergency medical care. This choice was not influenced by gender, age or experience in the health care system. 2. It is necessary to make changes in the curricula of medical students in accordance with the requirements of the professional standard of the district physician, whose work functions include the provision of emergency medical care. 3. The involvement of teachers into the process of preparing students for emergency and urgent care should begin at the 5th year and be systematic. 4. It is necessary to motivate students to systematic self-study of materials on the methodology for providing emergency and urgent care presented on the website of the Methodological Center for Accreditation.

    Abstract: The article is devoted to the study of the effectiveness of training graduates of medical universities when a trained student is included in the educational process, able to fulfill the role of a mentor. All over the world, the number of students is increasing in medical universities every year, but there is no equivalent increase in the teaching staff. One of the potential methods of overcoming the above problems is the use of trained mentors from among peer students. This method was used when students practiced practical skills in a simulation center in anticipation of primary accreditation. The purpose of the study is to optimize the preparation of students for a practical exam in a simulation environment with the help of a student mentor. Of the 468 students of the medical faculty of the North-Western State Medical University named after I.I. Mechnikov, studying in the VI year, was chosen by their colleague, who was trained at the center of simulation technology. A sample of 100 graduates was randomly generated, which were randomized into two groups of 50 people each (the main group in which the training was conducted with the participation of a mentor student and a control group trained under the guidance of teachers from one of the university departments). The most difficult for students was the station simulating the provision of emergency medical care (EMF). The evaluation checklist for checking the quality of the tasks at this station includes 233 points that must be implemented when providing EMF in 13 different situations requiring this type of medical care. To improve the quality of mastering practical skills, a training scheme was developed that allows you to learn the algorithm of actions when providing EMF. A basic assessment of students knowledge and skills revealed a low level of knowledge of diagnostic techniques and emergency assistance. Using the developed training scheme with the help of a student mentor has significantly increased the level of knowledge and practical skills in the provision of EMF. The quality of training in the main group did not significantly differ in most of the assessed parameters from the control group in which classes were held with the teacher. Statistical analysis of the data was performed using the SPSS 20.0 program (SPSS Inc., Chicago, IL, USA).

    Abstract: The article presents the results of research of theoretical knowledge and practical skills of general practitioners with more than 5 years of experience. The aim of the study is to assess the level of proficiency in otoscopy by General practitioners using a simulator, depending on various socio-demographic characteristics. Materials and methods. The study was conducted at the Department of family medicine of North-Western State Medical University named after I.I. Mechnikov. Doctors who agreed to participate in the project filled out a socio-demographic questionnaire, tests to test theoretical knowledge, for the level of proficiency in otoscopy, was evaluated using a checklist. Statistical analysis of the data was performed using the SPSS 22.0 program (SPSS Inc., Chicago, Illinois, USA). The study involved 24 General practitioners with more than 5 years of experience. Results. Analysis of the results showed that 66.7% of General practitioners rated their skill in otoscopy as satisfactory or higher, but only 54.1% were able to correctly perform 70% of the actions according to the checklist. Conclusions. 1. The level of theoretical knowledge in otoscopy among General practitioners with experience who took part in the study was quite low: 62.5% of respondents did not achieve satisfactory results. A low level of proficiency in otoscopy among General practitioners was also revealed when evaluating the performance of manipulation on the simulator only 20.8% of doctors correctly performed 80% of the actions according to the checklist. 2. Independent assessment of the level of proficiency in otoscopy in General practitioners is not a predictor of the success of its demonstration on the simulator in accordance with the checklist. 3. The level of proficiency in otoscopy in the study group of General practitioners did not have a statistically significant relationship with the basic level of theoretical knowledge, work experience and regular otoscopy in everyday practice.

    Abstract: BACKGROUND: The pandemic of a new coronavirus infection, which began in the Chinese city of Wuhan in December 2019, has spread to more than 200 countries and territories. There are a number of studies, the results of which indicate that deaths are mainly registered among the middle-aged and elderly population with chronic non-communicable diseases. Most of these studies are based on the study of cases of COVID-19 with severe course or in hospitalized patients. At the same time, a greater number of patients, including those with chronic non-communicable diseases, carry COVID-19 to a mild degree and remain under the supervision of primary care physicians. AIM: To identify risk factors for the development of severe COVID-19 in patients with chronic non-communicable diseases who sought outpatient care during various periods of increased incidence of new coronavirus infection from March 2020 to February 2022. MATERIALS AND METHODS: The study was carried out on the basis of the Family Medicine Center of North-Western State Medical University named after I.I. Mechnikov, which provides primary health care to the population attached under compulsory health insurance. The design of the study was published earlier. A retrospective cohort study was conducted based on the analysis of outpatient records of patients who sought medical care at the Family Medicine Center during various periods of increased incidence of COVID-19: from 19.03.2020 to 30.06.2020 (group 1), from 1.10.2021 to 30.11.2021 (group 2) and from 18.01.2022 to 28.02.2022 (group 3). RESULTS: 343 patients were included in the study: 137 men (39.9%) and 206 women (60.1%). The number of patients with at least one chronic non-communicable disease was 232 (67.9%). It was found that the severity of COVID-19 is not affected by the presence of one or more chronic non-communicable diseases of mild severity in the patient. At the same time, grade II obesity and grade III hypertension are independent factors that significantly increase the risk of developing severe COVID-19 by 13.4 and 5.4 times, respectively (p 0.05). It should be noted that the combination of these diseases significantly increased the likelihood of developing severe COVID-19 by 11.9 times (p 0.05). CONCLUSIONS: Regardless of the period of morbidity, the clinical manifestations of COVID-19 in patients with chronic non-communicable diseases have their own characteristics: weakness, sweating, dry and wet cough are significantly more common (p 0.05). With each subsequent wave of increase in the incidence of COVID-19 in patients with chronic non-communicable diseases, there is a significant tendency to decrease the frequency of severe and very severe course of the disease, pneumonia and hospitalization, as in patients without chronic non-communicable diseases. It was found that the severity of the COVID-19 course is not affected by the presence of mild chronic non-communicable diseases in the patient. At the same time, grade II and higher obesity or grade III hypertension are independent factors that significantly increase the risk of developing severe COVID-19 by 11.9 and 5.4 times, respectively (p 0.05). The combination of these diseases significantly increased the probability of developing severe COVID-19 by 13.4 times (p 0.05).
