    Abstract: The Omnipod Horizon™ System is a hybrid closed-loop system (HCL) consisting of a tubeless insulin pump and Dexcom G6 sensor which provides automated insulin delivery with glucose targets from 110-150mg/dL, adjustable by time of day to allow therapy personalization. This study is the first outpatient safety and effectiveness evaluation of the system. Participants aged 14-70y with T1D & gt;6mo and A1C & lt;10.0% used the HCL system at home for 14 days (first 2 days hotel, n=8) over winter holidays with unrestricted eating and exercise. Participants set protocol-determined higher targets of 130-150mg/dL for 9 days, then could freely choose their targets from 110-150mg/dL for the last 5 days. Primary outcomes were safety measures and percent time 70-180mg/dL for the 5 days of HCL use with free choice of target. Participants thus far (n=12) had a mean±SD age of 34±13y, T1D duration 15±12y, and A1C 6.9±0.8%. Participants primarily chose the 110 (59% of study time) and 120mg/dL (41%) targets. For 53 patient-days of HCL use, percent time from 70-180mg/dL was 74.3±7.6% (Table). Percent time & lt;70mg/dL was low: 0.9±1.3% overall and 0.9±2.2% overnight. There were no serious adverse events. The HCL system was safe and performed well in adults with T1D when used at home for 5 days with free choice of target glucose.     Abstract: The Omnipod 5 AID System is a tubeless hybrid closed-loop system with on-body operation and customizable glucose targets. Safe and effective system use was demonstrated in children aged 2-5.9y with type 1 diabetes (T1D) during a 3-mo pivotal study. To evaluate durability of glycemic benefit, we analyzed results from 9mo of an ongoing extension study, totaling 12mo of system use. In the pivotal study, participants used the system for 3mo at home, after 14d of their standard therapy (ST, pump or multiple daily injections) . They were then invited to participate in the extension study. Safety endpoints were occurrence of severe hypoglycemia (SH) and diabetic ketoacidosis (DKA) . Glycemic outcomes were A1C and percent time in ranges (TIR 70-180 mg/dL, TBR & lt;70 mg/dL, TAR & gt;180 mg/dL) during 3-mo AID intervals compared with ST. All pivotal trial participants (N=80) , aged (mean±SD) 4.7±1.0y with T1D duration 2.3±1.1y and total daily insulin of 14±4U (range: 5.3-27U) at baseline, enrolled in the extension. Improved outcomes were observed for up to 12mo, including lower A1C and greater TIR during each AID interval compared with ST (all p & lt;0.05, Table) . There were no episodes of DKA or SH in the 12-mo study. The safety and improved glycemic outcomes from the initial 3mo pivotal study persisted for an additional 9mo, indicating the potential long-term benefit of the Omnipod 5 System in very young children with T1D. Percent time & lt;54 and & lt;70mg/dL decreased with increasing target, without a corresponding increase in time ≥250 mg/dL. There were no serious adverse events. The HCL system was safe and performed well in adults with T1D when used at home with higher glucose targets.     Abstract: Hypoglycemia remains a major concern for persons with T1D using injections or “open loop” insulin pump delivery. Risk avoidance strategies are required, but often at the expense of increased hyperglycemia and elevated A1C levels. AID systems may reduce the need for human intervention by minimizing out-of-range glucose levels. We evaluated glycemic outcomes of people with T1D using the Omnipod 5 AID System, stratified by each user's time below range (TBR, % & lt;70 mg/dL) with their prior standard therapy (ST) . Participants of 3 age groups: 2-5.9, 6-13.9, and 14-70 years, with A1C & lt;10% used AID for 3 months at home after a 14-day ST phase. TBR and time in range (TIR, % 70-180 mg/dL) were evaluated across 3 ST TBR groups: ≤1%, & gt;1% to & lt;4%, and ≥4%. Trends were similar across all age groups. Overall, those with high ST TBR (≥4%) had a higher ST TIR (64.9%) ; AID substantially decreased TBR (-3.4%, -49 min/d) and increased TIR (73.9%) (Table) . Those with low ST TBR (≤1%) had lower ST TIR (50.8%) ; AID increased TIR substantially (to 67.9%) , while TBR was low (0.7%) . AID was associated with significant reductions in time below range in cohorts with high hypoglycemia risk (≥4% TBR) at baseline. In the lowest hypoglycemia risk at baseline cohort (≤1% TBR) , the median TBR remained ≤1%, although TBR was modestly higher, but this group had the largest increase in TIR.     Abstract: The Omnipod 5 System is a novel hybrid closed-loop (HCL) system with full on-body operation. A tubeless insulin pump containing a personalized MPC algorithm communicates directly with a Dexcom G6 CGM to automate insulin delivery. Following demonstration of positive safety and efficacy in older children (≥6y) and adults, we evaluated the system in very young children (2-5.9y) with T1D. Preschoolers with T1D often have erratic eating behaviors and may dislike being tethered to devices, and parental concern may lead to permissive hyperglycemia. This HCL system may be ideal for young children, given its customizable glucose targets from 110-150mg/dL, wireless system interaction through a mobile application, and bolus calculator that incorporates CGM trend information. Also, the system has no minimum weight or total daily dose (TDD) requirement. Participants with T1D and A1C & lt;10% used their respective standard therapies (ST) for 14d, and then transitioned to use of the HCL system for 3mo. Primary safety outcomes are the incidence of severe hypoglycemia (SH) and diabetic ketoacidosis (DKA). Primary effectiveness outcomes are change in A1C and percent of time in target range (TIR, 70-180 mg/dL) with the HCL system compared with ST. Baseline characteristics of participants (N=80) were (mean±SD) age 4±1y (range 2-5y), T1D duration 2.2±1.1y, and A1C 7.4±0.9%, with mean (range) TDD 13.7 (5.3-27.1) U/day and weight 20 (12-53) kg. There have been no serious adverse events, including SH and DKA, reported in over 5,254 person-days of HCL use. During the ST phase, TIR was 57.2±15.1%, time & lt;54mg/dL was 0.8±1.7%, & lt;70mg/dL was 3.4±3.9%, & gt;180mg/dL was 39.4±16.5%, and ≥250mg/dL was 14.7±12.0%. The same efficacy analysis is planned for the 3-month HCL phase upon its completion in January 2021. This multi-center pivotal study has, to date, demonstrated safety of the Omnipod 5 System in very young children with T1D. At completion, this will be the largest study of HCL in this age group reported to date. Disclosure J.     Abstract: Safe and effective use of the Omnipod 5 Automated Insulin Delivery (AID) System was demonstrated in adults and children with type 1 diabetes (T1D) during a 3-mo pivotal study. Longer studies evaluating durability of glycemic benefit of AID systems for & gt;1 year are limited. We present results from a 12-mo extension study for a total of 15mo of system use. In the pivotal study, participants aged 6-70y with T1D≥6mo and A1C & lt;10% used the AID system for 3mo, after 14d of their standard therapy (ST, pump therapy or multiple daily injections) . They were then invited to continue system use in an extension study, with outcomes measured every 3mo. Safety and efficacy endpoints included severe hypoglycemia (SH) and diabetic ketoacidosis (DKA) , and A1C and time in range (TIR, % 70-180 mg/dL) , respectively, during each 3-mo AID interval compared with the ST phase. Most participants (95%) continued into the extension study. At baseline, children (N=110, ages 6-13.9y) and adults (N=114, ages 14-70y) were aged (mean±SD) 10±2y and 37±14y with T1D duration 5±3y and 17±11y, respectively. There were 2 SH and 1 DKA episodes during the 12-mo extension. For both age groups, TIR and A1C improvements were sustained through 15mo of system use compared with ST. The safety and improved glycemic outcomes from the 3-mo pivotal study persisted for an additional 12mo, indicating the potential long-term benefit of the Omnipod 5 System.     Abstract: The Omnipod Horizon™ System is a hybrid closed-loop system (HCL) consisting of a tubeless insulin pump and Dexcom G6 sensor which provides automated insulin delivery with glucose targets from 110-150mg/dL, adjustable by time of day to allow therapy personalization. This study is the first outpatient safety and effectiveness evaluation of the system. Participants aged 6-13.9y with T1D & gt;6mo and A1C & lt;10.0% used the HCL system at home for 14 days (first 2 days hotel, n=8) over winter holidays with unrestricted eating and exercise. Participants set protocol-determined higher targets of 130-150mg/dL for 9 days, then could freely choose their targets from 110-150mg/dL for the last 5 days. Primary outcomes were safety measures and percent time 70-180mg/dL for the 5 days of HCL use with free choice of target. Children thus far (n=15) had a mean±SD age of 11±2y, T1D duration 5±3y, and A1C 7.7±0.9%. Participants primarily chose the 110 (69% of study time), 120 (10%), and 130mg/dL (21%) targets. For 72 patient-days of HCL use, percent time from 70-180mg/dL was 64.1±10.0% (Table). Percent time & lt;70mg/dL was low: 0.9±1.2% overall and 0.5±0.5% overnight. There were no serious adverse events. The HCL system was safe and performed well in children with T1D when used at home for 5 days with free choice of target glucose.
