    Abstract: Antipsychotic treatment is associated with metabolic disturbance. However, the relative degree to which metabolic alterations occur in treatment with different antipsychotics remains unclear. Furthermore, predictors of metabolic dysregulation are poorly understood, and association between metabolic-change and change in psychopathology is uncertain. Methods We searched Medline, EMBASE and PsychINFO from inception until June 30, 2019. We included blinded randomised controlled trials (RCTs) comparing 18 antipsychotics and placebo in acute-treatment of schizophrenia. We performed frequentist random-effects network meta-analyses (NMAs) to investigate treatment-induced changes in body weight, BMI, total/LDL/HDL-cholesterol, triglycerides, and glucose. We performed meta-regressions to examine relationships between metabolic change and age/gender/ethnicity/baseline-weight/baseline-metabolic parameter level. We examined the association between metabolic change and psychopathology change by estimating the correlation between symptom severity change and metabolic parameter change. Results Of 6532 citations, 100 RCTs met inclusion criteria, including 25,952 patients. Median treatment-duration was 6-weeks. According to our NMAs, mean differences for weight-gain compared to placebo ranged from -0.23 (95% CI: -0.83, 0.36) for best (haloperidol) to +3.01kg (1.78, 4.24) for worst (clozapine); for BMI from -0.25 (-0.68, 0.17) for best (haloperidol) to +1.07kg/m2 (0.90, 1.25) for worst (olanzapine); for total-cholesterol from -0.09 (-0.24, 0.07) for best (cariprazine) to +0.56mmol/L (0.26, 0.86) for worst (clozapine); for LDL-cholesterol from -0.13 (-0.21, -0.05) for best (cariprazine) to +0.20mmol/L (0.14, 0.26) for worst (olanzapine); for HDL-cholesterol from +0.05 (0.00, 0.10) for best (brexpiprazole) to -0.10mmol/L (-0.33, 0.14) for worst (amisulpride); for triglycerides from -0.01 (-0.10, 0.08) for best (brexpiprazole) to +0.98mmol/L (0.48, 1.49) for worst (clozapine); for glucose from -0.29 (-0.55, -0.03) for best (lurasidone) to 1.05mmol/L (0.41, 1.70) for worst (clozapine). Greater increases in glucose were predicted by higher baseline-weight (p=0.001) and male-gender (p=0.008). Non-Caucasian ethnicity was associated with greater increases in total-cholesterol (p=0.04). Improvements in symptom severity were associated with increases in weight (rho=0.36, p=0.002), BMI (rho=0.84, p & lt;0.0001), total-cholesterol (rho=0.31, p & lt;0.05), and LDL-cholesterol (rho=0.42, p=0.01), and decreases in HDL-cholesterol (rho= -0.35, p=0.04). Discussion There are marked differences between antipsychotics in terms of metabolic side-effects, with olanzapine and clozapine exhibiting the worst profiles. By contrast, compared with placebo, lurasidone and cariprazine respectively reduce fasting glucose and LDL-cholesterol, while aripiprazole and brexpiprazole increase HDL-cholesterol. Baseline weight, male gender, and non-Caucasian ethnicity predict vulnerability to antipsychotic-induced metabolic change. Considering the increased prevalence of metabolic syndrome, cardiovascular disease, and cardiovascular mortality in schizophrenia, these data may be used to inform antipsychotic-prescribing, especially in those at-risk groups we have identified. However, clinical decisions to preferentially use an antipsychotic with fewer metabolic side effects should consider that clinical improvement appears to be associated with development of these side effects.

    Abstract: Structural MRI is the most frequently used method to investigate brain volume alterations in neuropsychiatric disease. Previous meta-analyses have typically focused on a single diagnosis, thereby precluding transdiagnostic comparisons. Methods and analysis We will include all structural MRI studies of adults that report brain volumes for participants from at least two of the following diagnostic groups: healthy controls, schizophrenia, schizoaffective disorder, delusional disorder, psychotic depression, clinical high risk for psychosis, schizotypal personality disorder, psychosis unspecified, bipolar disorder, autism spectrum disorder, major depressive disorder, attention deficit hyperactivity disorder, obsessive compulsive disorder, post-traumatic stress disorder, emotionally unstable personality disorder, 22q11 deletion syndrome, generalised anxiety disorder, social anxiety disorder, panic disorder, mixed anxiety and depression. Network meta-analysis will be used to synthesise eligible studies. The primary analysis will examine standardised mean difference in average volume, a secondary analysis will examine differences in variability of volumes. Discussion This network meta-analysis will provide a transdiagnostic integration of structural neuroimaging studies, providing researchers with a valuable summary of a large literature.     Abstract: Network meta‐analysis (NMA) can be used to compare multiple competing treatments for the same disease. In practice, usually a range of outcomes is of interest. As the number of outcomes increases, summarizing results from multiple NMAs becomes a nontrivial task, especially for larger networks. Moreover, NMAs provide results in terms of relative effect measures that can be difficult to interpret and apply in every‐day clinical practice, such as the odds ratios. In this article, we aim to facilitate the clinical decision‐making process by proposing a new graphical tool, the Kilim plot, for presenting results from NMA on multiple outcomes. Our plot compactly summarizes results on all treatments and all outcomes; it provides information regarding the strength of the statistical evidence of treatment effects, while it illustrates absolute, rather than relative, effects of interventions. Moreover, it can be easily modified to include considerations regarding clinically important effects. To showcase our method, we use data from a network of studies in antidepressants. All analyses are performed in R and we provide the source code needed to produce the Kilim plot, as well as an interactive web application.

    Abstract: It is common experience for practising psychiatrists that individuals with schizophrenia vary markedly in their symptomatic response to antipsychotic medication. What is not clear, however, is whether this variation reflects variability of medication‐specific effects (also called “treatment effect heterogeneity”), as opposed to variability of non‐specific effects such as natural symptom fluctuation or placebo response. Previous meta‐analyses found no evidence of treatment effect heterogeneity, suggesting that a “one size fits all” approach may be appropriate and that efforts at developing personalized treatment strategies for schizophrenia are unlikely to succeed. Recent advances indicate, however, that earlier approaches may have been unable to accurately quantify treatment effect heterogeneity due to their neglect of a key parameter: the correlation between placebo response and medication‐specific effects. In the present paper, we address this shortcoming by using individual patient data and study‐level data to estimate that correlation and quantitatively characterize antipsychotic treatment effect heterogeneity in schizophrenia. Individual patient data (on 384 individuals who were administered antipsychotic treatment and 88 who received placebo) were obtained from the Yale University Open Data Access (YODA) database. Study‐level data were obtained from a meta‐analysis of 66 clinical trials including 17,202 patients. Both individual patient and study‐level analyses yielded a negative correlation between placebo response and treatment effect for the total score on the Positive and Negative Syndrome Scale (PANSS) (ρ=–0.32, p=0.002 and ρ=–0.39, p 〈 0.001, respectively). Using the most conservative of these estimates, a meta‐analysis of treatment effect heterogeneity provided evidence of a marked variability in antipsychotic‐specific effects between individuals with schizophrenia, with the top quartile of patients experiencing beneficial treatment effects of 17.7 points or more on the PANSS total score, while the bottom quartile presented a detrimental effect of treatment relative to placebo. This evidence of clinically meaningful treatment effect heterogeneity suggests that efforts to personalize antipsychotic treatment of schizophrenia have potential for success.
