    Abstract: The genetic basis of myelodysplastic syndromes (MDS) is heterogeneous, and various combinations of somatic mutations are associated with different clinical phenotypes and outcomes. Whether the genetic basis of MDS influences the outcome of allogeneic hematopoietic stem-cell transplantation (HSCT) is unclear. Patients and Methods We studied 401 patients with MDS or acute myeloid leukemia (AML) evolving from MDS (MDS/AML). We used massively parallel sequencing to examine tumor samples collected before HSCT for somatic mutations in 34 recurrently mutated genes in myeloid neoplasms. We then analyzed the impact of mutations on the outcome of HSCT. Results Overall, 87% of patients carried one or more oncogenic mutations. Somatic mutations of ASXL1, RUNX1, and TP53 were independent predictors of relapse and overall survival after HSCT in both patients with MDS and patients with MDS/AML (P values ranging from .003 to .035). In patients with MDS/AML, gene ontology (ie, secondary-type AML carrying mutations in genes of RNA splicing machinery, TP53-mutated AML, or de novo AML) was an independent predictor of posttransplantation outcome (P = .013). The impact of ASXL1, RUNX1, and TP53 mutations on posttransplantation survival was independent of the revised International Prognostic Scoring System (IPSS-R). Combining somatic mutations and IPSS-R risk improved the ability to stratify patients by capturing more prognostic information at an individual level. Accounting for various combinations of IPSS-R risk and somatic mutations, the 5-year probability of survival after HSCT ranged from 0% to 73%. Conclusion Somatic mutation in ASXL1, RUNX1, or TP53 is independently associated with unfavorable outcomes and shorter survival after allogeneic HSCT for patients with MDS and MDS/AML. Accounting for these genetic lesions may improve the prognostication precision in clinical practice and in designing clinical trials.

    Abstract: Introduction Allogeneic stem cell transplantation (allo-HCT) survivors are at a defined relevant risk of developing long-term complications: the prevalence of chronic health conditions approaches 75% among HCT survivors. The endocrine system is one of the most frequent targets of complications, providing justification for a long-term and continuous follow-up (LTFU) to assure a timely and appropriate treatment. The aim of our study is to evaluate the incidence of endocrinopathies in survivors in respect of sex, age, donor type, conditioning regimen and GvHD occurrence. Methods A standardized LTFU is applied at our center. We here analyze data consecutively collected in an Institutional database, starting from 2006, including 402 adult patients (pts) who underwent an allo-HCT between 1992 and 2016 at our Institution. A written consent was given by pts allowing the use of medical records for research in accordance with the Declaration of Helsinki. We considered for the analysis pts with an overall survival 〉 /=1y. We reviewed pts chapters with a focus on occurrence, management and treatment of diabetes, thyroid disfunctions, dyslipidemia and osteopenia / osteoporosis: diagnosis and follow-up were performed according to guidelines for long-term HCT survivors. Results With a median follow-up of 7y (r 2-25y) 328 pts were evaluable; donor was a match unrelated donor in 107 cases, HLA identical sibling in 88, haploidentical relative in 129 and cord blood in 4. The 5y-incidence of diabetes type 2 was 3%, with a median time after allo-HCT of 1138 days (r 5-4181 days); 13/22 developed diabetes after diagnosis of GvHD (median time 884 days, r 30-3753 days). All pts received indication for diet modification, 11 pts were treated with insulin and 8 pts with metformin. Thyroid disfunction was documented in 38 pts (5y-incidence 8,5%): 2 pts were diagnosed with hyperthyroidism and treated with methimazole or radioiodine treatment. Hypothyroidism was documented in 36 pts (median time after allo-HSCT 799 days, r 65-5021 days). Thirteen pts developed hypothyroidism following the diagnosis of GvHD (median time 1236 days, r 166-3540 days). Only 2 pts did not receive a specific treatment, while all the others received substitutive therapy with levothyroxine. Furthermore 1 pt was diagnosed with a papillary thyroid cancer. The 5y-incidende of dyslipidemia was 30% with a median time after allo-HSCT of 1433 days (r 366-7629), 47 pts developed dyslipidemia after the diagnosis of GVHD (median time 1425 days, r 134-7403 days). Diet-therapy was recommended to all the pts, 29 pts received a statin-based pharmacological treatment, 20 pts a polyenoic-fatty-acids based treatment, while a nutraceutical compound was given in 13 pts. Osteopenia was documented in 120 pts (median time after allo-HSCT 994 days, range 31-6605 days) with a 5y-incidence of 36%. Seventy-nine pts presented osteopenia after diagnosis of GvHD (median time after GvHD diagnosis 707 days, r 13-6379 days). Eight pts did not receive a specific treatment. Two pts received treatment with biphosphonates plus oral vitamin D and calcium supplementation, the 110 remaining pts received oral vitamin D +/- calcium supplementation only. Sixty-four pts developed osteoporosis (median time after allo-HSCT 1000 days, r 60-8836 days), the 5y-incidence was 26%. Forty-four pts developed osteoporosis following the diagnosis of GvHD (median time 724 days, r 28-8280 days). Only 4 pts did not receive any specific therapy; 31 pts received therapy with bisphosphonates, 2 pts denosumab and 27 pts oral vitamin D and calcium supplementation. In univariate analysis no relationship between host sex, age at transplant, TBI exposure, donor or history of GvHD and development of diabetes, thyroid disfunction and dyslipidemia was outlined. Otherwise, osteopenia development was strongly associated with GvHD occurrence and osteoporosis was strongly associated with age, sex and GvHD occurrence (table 1). Conclusions Allo-HCT survivors are at relevant risk of endocrinopathies after transplantation, providing justification for specific monitoring to individualize treatment and follow-up. Of note, classical transplant-related variables are not enough to justify the occurrence of endocrinological disfunction: a further deeper evaluation of a misdiagnosed donor-mediated autoimmune predisposition will be essential.     Abstract: The genetic basis of myelodysplastic syndromes (MDS) is heterogeneous, and various somatic mutations are associated with different phenotypes and clinical outcomes. The only curative treatment for MDS patients is allogeneic hematopoietic stem cell transplantation (HSCT) which is considered as a therapeutic option until the age of 65-70 in eligible patients. Whether the genetic basis influences the outcome of HSCT is currently unclear. Recently, we observed that mutations on ASXL1, RUNX1 and TP53 genes are independent predictors of relapse and overall survival in MDS patients after HSCT, and that the integration of these mutations into currently available predictive models increases the capability to capture prognostic information at individual patient level (Della Porta MG et al. J Clin Oncol, 2016 in press). In this study, we explored the possibility of developing a clinical/molecular predictive model to specifically estimate the outcome after HSCT in patients with MDS or acute myeloid leukemia evolving from MDS (MDS/AML). We studied 401 patients undergoing allogeneic HSCT for primary MDS or MDS/AML between 1997 and 2013 and reported to the GITMO registry. We used massively parallel sequencing to examine tumor samples collected before HSCT for somatic mutations in 34 recurrently mutated genes in myeloid neoplasms. In multivariable analysis with probability of relapse as endpoint the following factors showed independent prognostic value: percentage of marrow blasts ( 〉 10% vs. ≤10%, HR 1.43, P=0.04), cytogenetic risk according to IPSSR (poor/very poor vs. very low/low/intermediate risk, HR 1.85, P=.002), disease status at transplant (refractoriness to induction chemotherapy vs. complete remission, HR 2.4, P 〈 .001), type of conditioning (reduced intensity vs. standard conditioning, HR 1.35, P=.034) and mutational status of ASLX1/RUNX1/TP53 genes (present vs absent, HR 1.49, P=.021). Recipient age ( 〉 40 vs. ≤40 years, HR 1.68, P=.001), comorbidity risk according to HCT-CI (high vs. low/intermediate risk 2.10, P 〈 .001) type of conditioning (reduced intensity vs. standard conditioning, HR 0.53, P=.033) and HLA matching (≤7/8 vs. 8/8 match, HR 1.97, P=.001) were significant risk factors for transplant-related mortality (TRM). Based on regression coefficients, we developed a clinical/molecular model predictive for the risk of relapse after transplantation in MDS and MDS/AML. Accordingly, a score value of 1 was assigned for each of the following risk factors: marrow blasts 〉 10%, poor/very poor cytogenetic risk according to IPSSR, refractoriness to induction chemotherapy, and driver mutations in ASLX1/RUNX1/TP53 genes. A relapse risk index was calculated as the sum of these weighted scores, and was then categorized into 4 risk groups: low (score=0), intermediate (score=1-2), high (score=3), and very high (score=4). The cumulative incidence of relapse was estimated by a competing risks approach with TRM. In patients receiving standard conditioning, 5-year probability of survival after allogeneic HSCT was 61%, 43%, 39% and 19% for low, intermediate, high and very high risk (P 〈 .001), while cumulative incidence of relapse were 9%, 19%, 24% and 35%, respectively (P=.001). In patients receiving reduced intensity conditioning, 5-year probability of survival was 55%, 42%, 33% and 15% for low, intermediate, high and very high risk (P=.003), while cumulative incidence of relapse were 12%, 25%, 39% and 58%, respectively (P 〈 .001). This model serves as a proof of concept that the integration of somatic mutations significantly increase the capability to capture prognostic information in MDS and MDS/AML patients receiving allogeneic HSCT, and may provide a basis for improving clinical decision-making. Possible interventions in patients with high risk of disease relapse after HSCT according to genotype may include the anticipation of the transplant procedure in early disease phase, the use of innovative conditioning regimens to increase the probability to eradicate MDS clone, and prophylaxis of disease recurrence after transplantation by donor leukocyte infusions and targeted/novel therapies Disclosures Ciceri: MolMed SpA: Consultancy.
