    Abstract: Abstract: In 2000 the Government of Alberta passed legislation to regulate the activities of private health clinics in the provision of insured health services through contracts with regional health authorities. In seven years the interaction of ideas, interests, institutions, and external events forced the Government of Alberta to move from a non‐regulatory policy stance to a policy of stringent regulation. The legislation struck a balance among competing values relating to public and private sector roles, increased access and choice, regulation of the growth of public expenditures, and quality assurance. Sommaire: En l'an 2000, le gouvernement de l'Alberta a adopté une loi visant à réglementer les activités des cliniques de sante privées en matière de prestation de services de santé assurés par l'intermédiaire de contrats avec les offices régionaux de la santé. En sept ans, l'interaction d'idées, d'intérêts, d'institutions et d'événements externes a forcé le gouvernement de l'Alberta à passer d'une position de politique non exécutoire à une politique de réglementation rigoureuse. La loi établit un équilibre entre les valeurs opposées que représentent le rôle important du secteur privé, un accès et un choix accrus, la réglementation de la croissance des dépenses publiques et le contrôle de la qualité.

    Abstract: The coastal zone changed considerably during the twentieth century due to growing populations and increasing urbanization. A recent study indicated that in 1990, 23% of the world's population (1.2 billion people) were living within both a 100‐kilometer distance and a 100‐meter elevation of the coast at densities 3 times higher than the global average. Society is becoming increasingly vulnerable to sea level extremes, as Hurricane Katrina demonstrated. Rising levels will result in more flooding, even if storm intensities do not increase. Improved understanding of the reasons for sea level rise and variability is required to reduce the uncertainties in sea level rise projections, and this improved understanding could contribute to more effective coastal planning and management.

    Abstract: William J. Barrow was a unique and significant individual in the history of archives preservation, the development of permanent/durable book paper, and the application of investigative techniques in these fields. The author, who worked with Barrow in his restoration shop and research laboratory during the 1950s when much of his major work was in progress, offers in this article an appreciation of his contributions. The article corrects some recent misapprehensions concerning Barrow and provides an account of his effective personal and problem-solving attributes.

    Abstract: The Ca 2+ -dependent slow afterhyperpolarization (AHP) is an important determinant of neuronal excitability. Although it is established that modest changes in extracellular pH (pH o ) modulate the slow AHP, the relative contributions of changes in the priming Ca 2+ signal and intracellular pH (pH i ) to this effect remain poorly defined. To gain a better understanding of the modulation of the slow AHP by changes in pH o , we performed simultaneous recordings of intracellular free calcium concentration ([Ca 2+ ] i ), pH i , and the slow AHP in cultured rat hippocampal neurons coloaded with the Ca 2+ - and pH-sensitive fluorophores fura-2 and SNARF-5F, respectively, and whole cell patch-clamped using the perforated patch technique. Decreasing pH o from 7.2 to 6.5 lowered pH i , reduced the magnitude of depolarization-evoked [Ca 2+ ] i transients, and inhibited the subsequent slow AHP; opposite effects were observed when pH o was increased from 7.2 to 7.5. Although decreases and increases in pH i (at a constant pH o ) reduced and augmented, respectively, the slow AHP in the absence of marked changes in preceding [Ca 2+ ] i transients, the inhibition of the slow AHP by decreases in pH o was correlated with low pH o -dependent reductions in [Ca 2+ ] i transients rather than the decreases in pH i that accompanied the decreases in pH o . In contrast, high pH o -induced increases in the slow AHP were correlated with the accompanying increases in pH i rather than high pH o -dependent increases in [Ca 2+ ] i transients. The results indicate that changes in pH o modulate the slow AHP in a manner that depends on the direction of the pH o change and substantiate a role for changes in pH i in modulating the slow AHP during changes in pH o .

    Abstract: Over the past decade, provincial governments have embarked on ambitious plans to better integrate their healthcare systems, through the introduction of regional governance and management structures. The objective of this study was to examine physicians' perceptions of the current level and facilitators/barriers to integration in three Western Canada Health Regions. Three approaches to integration were investigated: functional, clinical services, and physician system integration. Physicians perceived that functional integration within each region was questionable. Clinical services were the least integrated approach. Physician system integration was rated highest of the approaches, particularly adherence to clinical practice guidelines usage. Physicians' perspectives of integrated health delivery systems do not appear to be influenced by regional size, maturity, urbanicity or facilities. Facilitators of integration were communication among health professionals and service providers, and using a multi-disciplinary team approach in delivery of healthcare in both regions. Barriers to integration were organizational culture, access to specialists and clinical services, and health information records. On a scale of 1–5, all three regions are at the beginning of an integrated health delivery system. Three global suggestions were provided to further integration of health delivery services: physicians should be involved in decision-making process at the Board level, clinical services should be patient-centred, and physicians endorsed the use of multi-disciplinary teams.
