    Abstract: Background . Metagenomic next-generation sequencing (mNGS) has made a revolution in the mode of pathogen identification. We decided to explore the diagnostic value of blood and bronchoalveolar lavage fluid (BALF) as mNGS samples in pneumonia. Methods . We retrospectively reviewed 467 mNGS results and assessed the diagnostic performance of paired blood and BALF mNGS in 39 patients with pneumonia. Results . For bacteria and fungi, 16 patients had culture-confirmed pathogen diagnosis, while 13 patients were culture-negative. BALF mNGS was more sensitive than blood mNGS (81.3% vs. 25.0%, p = 0.003 ), and the specificity in BALF and blood mNGS was not statistically significant different (76.9% vs. 84.6%, p = 0.317 ). For 10 patients without culture test, treatments were changed in 2 patients. For viruses, Epstein-Barr virus was positive in blood mNGS in 9 patients. Human adenovirus was detected in both BALF and blood mNGS in 3 patients. Conclusion . Our study suggests that BALF mNGS is more sensitive than blood mNGS in detecting bacteria and fungi, but blood also has advantages to identify the pathogens of pneumonia, especially for some viruses.

    Abstract: Rhein, belonging to anthraquinone compounds, is one of the main active components of rhubarb and Polygonum multiflorum. Rhein has a variety of pharmacological effects, such as cardiocerebral protective effect, hepatoprotective effect, nephroprotective effect, anti-inflammation effect, antitumor effect, antidiabetic effect, and others. The mechanism is interrelated and complex, referring to NF-κB, PI3K/Akt/MAPK, p53, mitochondrial-mediated signaling pathway, oxidative stress signaling pathway, and so on. However, to some extent, its clinical application is limited by its poor water solubility and low bioavailability. Even more, rhein has potential liver and kidney toxicity. Therefore, in this paper, the pharmacological effects of rhein and its mechanism, pharmacokinetics, and safety studies were reviewed, in order to provide reference for the development and application of rhein.

    Abstract: Background. It is generally considered that traditional Chinese medicine (TCM) therapy postpones the progression of some chronic kidney diseases (CKDs). Chinese medicine herbs are widely applied in TCM therapy. We aimed to evaluate clinical efficacy and safety of Chinese herbal formula granules in patients with CKD stage 3 through a prospective randomized controlled study. Methods. A total of 343 participants with CKD stage 3 were recruited from 9 hospitals in Jiangsu Province between April 2014 and October 2016. Participants were randomly assigned to a treatment or control group. Patients in the treatment group orally took Chinese herbal formula granules twice a day, while controls received placebo granules. The duration of intervention was 24 weeks. Primary outcomes were 24-hour proteinuria, serum creatinine, and eGFR, which were measured every 4 weeks. Results. There was no statistical difference in 24-hour proteinuria between the two groups (0.97 ± 1.14 g/d vs. 0.97 ± 1.25 g/d). Patients in the treatment group had significantly lower serum creatinine level (130.78 ± 32.55 μmol/L versus 149.12 ± 41.27 μmol/L) and significantly higher eGFR level (55.74 ± 50.82 ml/min/1.73·m2 versus 44.46 ± 12.60 ml/min/1.73·m2) than those in the control group ( P   〈  0.05). There was no significant difference between two groups in the incidence of adverse events. Conclusion. The treatment adopting Chinese herbal formula granules for 24 weeks improved kidney function of patients with CKD stage 3.

    Abstract: Vascular smooth muscle cell (VSMC) phenotypic modulation plays an important role in the occurrence and development of in-stent restenosis (ISR), the underlying mechanism of which remains a key issue needing to be urgently addressed. This study is designed to investigate the role of plasma small extracellular vesicles (sEV) in VSMC phenotypic modulation. sEV were isolated from the plasma of patients with ISR (ISR-sEV) or not (Ctl-sEV) 1 year after coronary stent implantation using differential ultracentrifugation. Plasma sEV in ISR patients are elevated markedly and decrease the expression of VSMC contractile markers α-SMA and calponin and increase VSMC proliferation. miRNA sequencing and qRT-PCR validation identified that miRNA-501-5p was the highest expressed miRNA in the plasma ISR-sEV compared with Ctl-sEV. Then, we found that sEV-carried miRNA-501-5p level was significantly higher in ISR patients, and the level of plasma sEV-carried miRNA-501-5p linearly correlated with the degree of restenosis ( R 2 = 0.62 ). Moreover, miRNA-501-5p inhibition significantly increased the expression of VSMC contractile markers α-SMA and calponin and suppressed VSMC proliferation and migration; in vivo inhibition of miRNA-501-5p could also blunt carotid artery balloon injury induced VSMC phenotypic modulation in rats. Mechanically, miRNA-501-5p promoted plasma sEV-induced VSMC proliferation by targeting Smad3. Notably, endothelial cells might be the major origins of miRNA-501-5p. Collectively, these findings showed that plasma sEV-carried miRNA-501-5p promotes VSMC phenotypic modulation-mediated ISR through targeting Smad3.

    Abstract: Aim. Patients in the hemodialysis stage are prone to psychological pressure of depression and anxiety and have resistance, which affects the clinical treatment effect. Effective psychological intervention plays a very important role in improving patients’ psychological pressure and patients’ compliance. The aim of this study is to explore the nursing effect of psychological intervention on uremic hemodialysis patients. Methods. There were 126 uremic hemodialysis patients admitted to the hospital from August 2020 to December 2021. The patients were randomly divided into the routine nursing care group ( n = 63 ) and psychological intervention group ( n = 63 ). The routine nursing care group received routine nursing care for uremia hemodialysis patients. The psychological intervention group implemented psychological intervention on uremia hemodialysis patients. The methods of psychological intervention mainly include establishing a good nurse-patient relationship, popularizing hemodialysis knowledge, timely psychological counseling for patients, and organizing patient communication meetings. The treatment compliance, Self-rating Anxiety Scale (SAS) and Self-rating Depression Scale (SDS) of the two groups were compared before and after nursing. SF-36 scale was used to evaluate the quality of life of patients. The incidence of complications and nursing satisfaction were compared between the two groups. Results. The treatment compliance rate and nursing satisfaction of hemodialysis uremic patients in the psychological intervention group were significantly higher than the routine nursing care group. The SAS and SDS of hemodialysis uremia patients in the psychological intervention group were significantly lower than the routine nursing care group after psychological intervention, and SF-36 scale was significantly higher than the routine nursing group. The main complications of uremic hemodialysis patients are hypotension, hyperkalemia, internal fistula occlusion, and infection. Compared with the routine nursing care group, the incidence of complications in the psychological intervention group was significantly reduced. Conclusion. The implementation of psychological nursing intervention for uremic hemodialysis patients have a very significant effect on reducing the incidence of complications and improving anxiety, depression, treatment compliance, and the quality of life and the nursing satisfaction.

    Abstract: Objective. The diagnostic value of CT window width technique in primary omentum infarction was evaluated by this study. Methods. The abdominal CT data of 32 patients with clinically diagnosed abdominal omentum infarction were retrospectively selected and analyzed. The fixed window position was 50 HU, and the window width was 135 HU, 250 HU (abdomen), 350 HU (mediastinum), and 500 HU, respectively. The detection rate of lesions was analyzed and compared. Results. Window widths of 135 HU, 250 HU (abdomen), 350 HU (mediastinum), and 500 HU have a detection rate of 12.5% (4 cases), 62.5% (20 cases), 100% (32 cases), 100% (32 cases) for abdominal omental lesions, respectively. However, 500 HU showed worse abdominal bowel and parenchymal organs than 350 HU. Conclusion. According to the comprehensive image quality, the ideal window width for diagnosis of primary omentum infarction is 350HU (mediastinal) window width.

    Abstract: Objective. This study was aimed at exploring the prognostic and clinicopathological roles of aspartate aminotransferase-to-lymphocyte ratio index (ALRI) in patients with hepatocellular carcinoma via a meta-analysis. Methods. The PubMed, Embase, Cochrane Library, Web of Science, China National Knowledge Infrastructure (CNKI), Wanfang, and VIP databases were comprehensively searched from inception to November 20, 2021. Pooled hazard ratio (HR) and corresponding 95% confidence interval (CI) were used to evaluate the relationship between ALRI and overall survival (OS) as well as progression-free survival (PFS) in patients with hepatocellular carcinoma. Odds ratio (OR) and the corresponding 95% CI were also used to investigate correlations between clinical factors and ALRI in patients with hepatocellular carcinoma. Results. A total of 3914 patients with hepatocellular carcinoma from eleven retrospective cohorts were included in this meta-analysis. The combined results revealed that patients with hepatocellular carcinoma with elevated ALRI tended to have unfavorable OS (HR 1.53 [95% CI 1.25–1.82]; P 〈 0.001 ). Pooled HRs revealed that high ALRI was an independent risk factor for inferior PFS in patients with hepatocellular carcinoma (HR 1.36 [95% CI 1.10–1.63]; P 〈 0.001 ). In addition, high ALRI was strongly associated with male sex (OR 1.32 [95% CI 1.02–1.70]; P = 0.035 ), presence of cirrhosis (OR 1.68 [95% CI 1.01–2.81]; P = 0.046 ), larger tumor size (OR 2.25 [95% CI 1.31–3.88]; P 〈 0.001 ), presence of portal vein tumor thrombus (OR 2.50 [95% CI 1.52–4.11]; P 〈 0.001 ), and distant metastasis (OR 1.72 [95% CI 1.05-2.82]; P = 0.031 ). Conclusion. Elevated ALRI in patients with hepatocellular carcinoma predicted inferior survival outcomes and was strongly associated with some important features of hepatocellular carcinoma.

    Abstract: The energy hole is a severe problem for underwater acoustic distributed networks in that it affects the normal operations of the network and shortens the network’s life span. To deal with this problem, a loop-free routing control technique is proposed in this paper. The classical shortest-path routing control method is used to generate multiple disjointed routing tables. The residual energy of the nodes and the changing information of the uplink/downlink matrix are added to the data frames for distribution. The source node specifies the routing path to transmit the data frames based on the changing information, and the relay nodes route the data frames strictly according to the routing path designated by the source node. Besides, the energy consumption of the relay node is saved by replying to the pseudo-ACK frame. Simulation experiments are implemented in four typical scenarios, and the results reflect that the proposed technique could extend the network’s life span by approximately 10% when compared to other mature techniques. Besides, it has no other negative effects on the normal operations of the network.

    Abstract: Rock-like materials often exhibit irregular failure deformation under long-term service conditions, and the deformation and failure of asphalt and concrete materials is a serious problem that leads to subgrade failure. In this study, two different viscous/brittle rock-like materials were prepared by the in situ loading and optical speckle synchronous monitoring test method, and the evolution characteristics of the deformation field were studied during compression. The formation process of the compression deformation localization of rock-like materials and their relationship with stability were analyzed. A quantitative description of the compression deformation stage and localization characteristics of the viscous/brittle rock-like materials is presented. The results can be summarized as follows. At the initial stage of compression, the deformation localization zone of viscous/brittle rock-like materials begins to expand from the middle area to the surrounding area. Preliminary results of the deformation localization of the linear elastic deformation stage were obtained. The failure cloud image is completely formed at the peak, which is consistent with the failure physical map. The deformation process of compression can be quantitatively described using the deformation localization characteristics of rock-like materials.
