    Abstract: Background: A patient’s satisfaction level is a crucial aspect of quality management in clinical laboratories. Satisfaction is the client’s perception of care received compared with the care expected. Accreditation from an organization such as NABL or NABH is necessary. This study aimed to evaluate patient satisfaction with laboratory services in a tertiary care hospital in Jammu, UT of Jammu and Kashmir, India. Methods: The study conducted a prospective cross-sectional patient satisfaction survey over six months w.e.f. April 2022 to September 2022 at 05 patients/day with monthly data (05×30=150). A total of 900 patients were selected randomly at 150 patients/month (150×6=900). Data was collected using structured questionnaire, entered in Epi Info and analysed with SPSS software. A p value of less than 0.05 was taken as statistically significant. Results: The study found that 76.55% of patients were satisfied, while 11% were mildly dissatisfied and 17.94% were dissatisfied with laboratory services. Patients were dissatisfied with cleanliness of latrines (47%), long waiting times (30%), clear and understandable advisory service during specimen collection (26%), adequacy of waiting area (25%), easy accessibility of laboratory (19%), latrine location (20%), availability of requested service (18%), unfair payment of service (17%), and missing results (12%). Conclusions: The study concluded that despite most patients being satisfied, there were gaps. The study recommends that the concerned departments to improve the identified gaps and meet patients’ needs. Educational status and distance were found to be significantly associated with the overall satisfaction level of clients.

    Abstract: Background: In patients with lymphoma, bone marrow involvement is definite evidence of disseminated disease and hence assessment of bone marrow status in these patients provides important information for decisions regarding treatment. Aim: To determine frequency of bone marrow involvement in cases of lymphoma. Results: Out of 60 histologically confirmed lymphoma patients, 51(85%) patients were of Non Hodgkin’s lymphoma and 9 (15%) patients were of Hodgkin’s lymphoma. International working formulation was followed to classify Non Hodgkin Lymphoma into low, intermediate and high grade. The low grade Non Hodgkin lymphoma cases comprised of 41.18% (21/51), high grade 39.21% (20/51) and intermediate grade 19.61% (10/51) cases. Out of 9 Hodgkin lymphoma (HL) cases, 8 (88.9%) were of classical type and there was a single case (11.1%) of lymphocytic predominant Hodgkin’s lymphoma. 25 (41.7%) cases showed bone marrow infiltration by the atypical lymphomatous cells. Bone marrow involvement was seen in 47.05% (24/51) cases of NHL. Among Non Hodgkin lymphoma cases, maximum involvement was seen in low grade NHL 57.14% (12/21) followed by intermediate grade NHL 50% (5/10) & minimum was seen in high grade NHL 35% (7/20). Conclusion: Thorough examination of bone marrow in lymphoma patients can increase the diagnostic accuracy as it may be the single most important finding in a patient with an otherwise localized disease thereby contributing to the prognosis and appropriate treatment modalities.

    Abstract: Background: Lymphadenopathy may be the first sign of malignancy and use of FNA in the diagnosis of metastatic disease is well established. In patients with enlarged lymph nodes and previously documented malignancy, FNAC can obviate further surgery performed merely to confirm the presence of metastasis. Aim: The objective was to study the relative frequency of various metastatic lymph node lesions in hospital based population in Jammu region. Material and Methods: In this study, we analysed the clinicopathological features of metastatic lymphadenopathy, diagnosed on FNAC in the Jammu region of Jammu and Kashmir Union Territory of India over a period of three years. Interpretation and Results: A total of 190 lymph nodes were labelled as metastatic on FNAC. The age of the patients ranged from 12 to 95 years, with maximum number of cases in 51-60 years age group. Male predominance was seen with a male: female ratio of 2.3:1.The cervical lymph nodes were involved most commonly (48.42%). The most common metastatic malignancy reported was squamous cell carcinoma (115 cases, 60.50%) with oral cavity being the most common primary site of malignancy. Conclusion: In the present study FNAC proved to be a useful diagnostic modality in the diagnosis of metastatic malignancies due to its rapidity of diagnosis, ease of performance and minimal complications.     Abstract: Sickle cell disease (SCD) is accompanied by several complications, which emanate from the sickling of erythrocytes due to a point mutation in the β -globin chain of hemoglobin. Sickled erythrocytes are unable to move smoothly through small blood capillaries and therefore, cause vaso occlusion and severe pain. Apart from pain, continuous lysis of fragile sickled erythrocytes leads to the release of heme, which is a strong activator of the NLRP3 inflammasome, thus producing chronic inflammation in sickle cell disease. In this study, we identified flurbiprofen among other COX-2 inhibitors to be a potent inhibitor of heme-induced NLRP3 inflammasome. We found that apart from being a nociceptive agent, flurbiprofen exerts a strong anti-inflammatory effect by suppressing NF-κB signaling, which was evidenced by reduced levels of TNF- α and IL-6 in wild-type and sickle cell disease Berkeley mice models. Our data further demonstrated the protective effect of flurbiprofen on liver, lungs, and spleen in Berkeley mice. The current sickle cell disease pain management regime relies mainly on opiate drugs, which is accompanied by several side effects without modifying the sickle cell disease-related pathology. Considering the potent role of flurbiprofen in inhibiting NLRP3 inflammasome and other inflammatory cytokines in sickle cell disease, our data suggests that it can be explored further for better sickle cell disease pain management along with the possibility of disease modification.
