    Abstract: Background: In this study, we assessed the status of antibiotic resistance of H. pylori isolates to antimicrobial agents at Hospital of Hue University of Medicine and Pharmacy in Central Vietnam and identified the underlying molecular mechanisms related to tetracycline and amoxicillin resistance. In addition, the cagA, cagE, cagT, vacA, iceA genotypes of H. pylori strains were investigated to predict clinical outcomes. Materials and methods: H. pylori was successfully cultured in 52 patients with different gastrointestinal disorders at the Hospital of Hue University of Medicine and Pharmacy in Central Vietnam. The minimum inhibitory concentrations (MICs) of five antimicrobials; clarithromycin (CLR), metronidazole (MTZ), levofloxacin (LE), amoxicillin (AMX) and tetracycline (TE) were determined by the E-test method. Genetic determinants of AMX and TE resistance were identified with the polymerase chain reaction (PCR), followed by sequencing and gene analysis. Allelic variants of cagA, cagE, cagT, vacA, iceA were identified by the PCR. Results: The resistance to CLR, MTZ, LE, AMX, and TE were 90.4%, 86.5%, 65.4%, 40.4% and 0%, respectively. Multidrug resistance was observed in 88.5% of the isolates investigated. Several known AMX resistance mutations were identified in PBP1A (A369T, V374L, S543R, T556S, N562Y), whereas a known mutation in 16S rRNA (A926G) was detected in strains with higher MIC level (TE MICs of 0.25 and 0.5 mg/L). The cagA, cagE and cagT genotypes were found together in 46 isolates (88.5%), vacAs- region genotype in 51 (98.1%, predominantly vacAs1), vacAmregion genotype in all strains studied (vacAm1 - 51.9%, vacAm2 - 40.4%, vacAm1 and vacAm2 - 7.7%), iceA1 in 22 (42.3%) and iceA2 in 20 (38.5%) of strains. The allelic variant vacAs1m1 was prominent (57.4%), and vacAs1m2 (42.6%). Conclusion: Overall, resistance rates to CLR, MTZ, LE and AMX were high in H. pylori from Central Vietnam, except for TE, which serves as a foundation for developing local guidelines for more effective therapeutic strategies. Neither the single genes nor the combination of genes was significantly helpful in predicting the clinical outcome of H. pylori infection in patients in our study.     Abstract: Objective: To study the change of central venous pressure (CVP) placed through the basilic vein and to find a correlation between the value of central venous pressure with clinical features in Dengue hemorrhagic fever shock. Patients and methods: included 43 patients ≤ 15 years old and hospitalized with diagnosis of Dengue Hemorrhagic fever with shock (level 3, level 4) at intensive care unit of Can tho Children's Hospital from 03/2009 - 08/2010. Each patients had their CVP measures through catheter placed in basilic vein. Results: The average value of CVP measured for the first time was 7.3 ± 3.0 CmH2O, after 30 minutes was 9.5 ± 2.6 CmH2O, after 60 minutes was 10.2 ± 2.3 CmH2O, after 2 hours was 10.4 ± 2.3 CmH2O, after 3 hours was 10.5 ± 2.4 CmH2O, after 6 hours was 10.5 ± 2.5 CmH2O, after 12 hours was 10.8 ± 2.5 CmH2O, after 24 hours was 12 ± 2.8 CmH2O, and after 36 hours was 12.4 ± 4.1 CmH2O. There are an inverse correlations between CVP value measured for the first time with capillary refill time (r = - 0.32, p 〈 0.05) and with increased heart rate (r = -0.43, p 〈 0.05). There is a moderate correlation between CVP value with systolic blood pressure (r = 0.31, p 〈 0.05). Conclusion: The value of CVP is the most obvious change in the first hour of treatment. CVP values slightly changed in the 2, 3, 6 and 12 hours interval. There are an inverse correlations between CVP value measured for the first time with capillary refill time and with increased heart rate. There is a moderate correlation between CVP value with systolic blood pressure.

    Abstract: Background: Adverse drug event (ADE) is one of the most common causes of injury to patients, accounting for 19% of adverse events (AEs), particularly among the elderly due to their sensitivity to ADE. Thus, taking ADE detection is the first vital step in improving medication safety. Objective: This study aimed to develop an appropriate modified triggers tool based on Institute for Healthcare Improvement (IHI) Global Trigger tool for Adverse Events detecting potential ADEs in patients at Hue University Hospital. Materials and methods: Using a retrospective study on the medical records of inpatients treated in Hue University of Medicine and Pharmacy Hospital in 2020. Results: The modified ADE trigger tool encompassed 13 triggers. In the screening stage, the ADEs were found in 18 medical records (1.9% of total screening records), and 22 ADEs were identified, in which 19 ADEs were determined as adverse drug reactions (ADRs). The study also found that 17/19 ADRs had not been reported via a spontaneous reporting system (89.5%). The modified ADE Trigger tool’s positive predictive value (PPV) was 0.06. Conclusion: A modified ADE Trigger tool for detecting ADE has been constructed and applied at Hue University of Medicine and Pharmacy Hospital. This study would be the first motivation for further studies that attempt to apply this tool into daily clinical pharmacy activities and emphasize the role of Clinical Pharmacists at the hospital.     Abstract: Objective: Phacoemulsification cataract surgery is the method applyied commonly and the first choice of Ophthalmologist. Materials and Method: We study comprised 49 eyes of 42 cataract patients for Phacoemulsification at Hue Hospital University from 5/2013 to 4/2014 with within 3 months after surgery. Result: 3 months after surgery, the visual acuity ≤ 4/10 accounted for 75,5%, corrected vision ≥ 4/10 accounted for 95,9%. The mean postoperative astigmatism was 1,02D ± 0,58, stable and does not change much compared to before surgery. The mean intraocular pressure (IOP) increased to 10,51mmHg ± 1,77 at 3 months postoperatively,the IOP was 3,9mmHg lower at 3 months than preoperative. Anterior chamber depth (ACD) increased by 1,0mm deeper than before surgery. Corneal refractive power increased 0,43D. Corneal radius of curvature decreased 0,11mm by 3 month postoperatively.     Abstract: Purpose: To evaluate IOP reduction effect of phaco surgery in senile cataract patients, and to assess the correlation between the changes of IOP and the some affects. Methods: Prospective correlation study. Phaco surgery was performed on 107 senile cataracts of 102 patients in Ophthalmology Department of Hue Central Hospital from January 2014 to July 2014. IOP was measured preoperatively, 4 – 8 hours, 1 day, 10 days, 1 month, and 3 months postoperatively. Some related factors were also analyzed. Results: Mean preoperative IOP declined from 17.27 ± 2.42 mmHg to 14.71 ± 2.20 mmHg at 1 month after the surgery and remained stable until the last follow-up (3 months). The IOP decreased correlated proportionally to the morphology of cataract, the firmness and thickness of lens, anterior chamber depth, anterior chamber angle. Conclusion: IOP was significantly reduced after phacoemulsification. This effect sustained through out the study time. Key words: Cataract, post-operative IOP reduction, phacoemulsification.

    Abstract: Objectives: To investigate the clinical characteristics of cataract patients with small pupils and to evaluate the result of cataract surgery on the eye have small pupils by phacotechnique. Subjects and methods: Descriptive study, prospective, uncontrolled interventions. Sample selection. The sample size of 70 patients with 70 eyes of cataracts with small pupils was treated by phaco technique and intraocular lens implant within posterior chamber. Follow up to 3 months. Results: 70 eyes, the percentage of men and women was not different from 54.2% (38 male) compared to 45.8% (32 female). The mean age was 80 ± 8.74, from 58 to 99 years. The disease was mainly found in the age group over 70 years old with over 80% (51.5%). Visual acuity before surgery was very poor under 3m CF (count finger) for 68.6% (48/70). Visual acuity over 1/10 was only a small amount with 2.8% (2 eyes). Pseudoexfoliation was the most common reason complications of mydriasis with 32/70 eyes (45.7%) and 22/70 eyes (31.4%) for age. The preoperative pupilarysizewas mostly small with 63/70 eyes (90.0%), non-dilated pupils (7/70 eyes) (10.0%). Average pupil size was 3.34 mm (2 - 4mm). Iris condition: iris atrophy 20/10 eyes (28.6%), iris synechiae 11/10 (15.7%), irregular iris muscle with 51, 4% and good iris muscle accounted for 48.6%. Grade of cataract: Grade III: 31/70 eyes (44.3%), Grade IV: 32/70 eyes (45.7%), Grade II: 5/70 eyes (7.1%) and V:2/70 eyes (2.9%). Pupil expander technique: OVD injection with 42/70 eyes (60%), using iris hook with 23/70 eyes (32.9%). Pupillary size before and after intervention has changed from 3.7mm to 4.48mm. Conclusions: Iris expander techniques have been shown to have good dilated pupils: 60.0% OVD injection, iris hook was 32.9%, other methods 7.1% One-week visibility of good visual acuity was higher than that of postoperative one day (12.2%) and increased at 1 month and 3 months (20.0%). Very good visual acuity was not available and low vision group was 1.4% after 3 months. Key words: cataract surgery; phacoemusification, small pupil

    Abstract: Aims: To evaluate surgical results of primary close angle glaucoma. Subjects and methods: Prospective correlation study. Trabeculectomy was performed on 48 eyes of PCAG in Ophthalmology Department of Hue Central Hospital from March 2013 to April 2014. Results: After treated, 48 eyes of PCAG have result of IOP: control IOP (89.6%), half control IOP (8.3%) and uncontrol IOP (2.1%). Visual acuity after treated: increasing visual acuity (64.6%), unalterable visual acuity (33.3%) and decreasing visual acuity (2.1%). Blebs: the rate of diffuse was 10,4%, cystic (77.1%), encapsulated (12.5%) and not have flat blebs. Complications: uncomplicated (83.3%), rate of uveitis (12,5%) and cataract (6.3%). Conclusions: Trabeculectomy was effective and safety method to decrease IOP. Key words: Primary close angle glaucoma, trabeculectomy.

    Abstract: Introduction: Glaucoma is the leading cause of irreversible blindness and the second leading cause of blindness in general. Early detection and timely treatment help reduce the progression of the disease, indirectly limit the economic burden and ensure the quality of life for patients. Currently, access to glaucoma prevention medical services is difficult in developing countries due to limited access to eye care facilities. Objective: To assess the effectiveness of interventions to increase the access to glaucoma services of people over 40 years old in Hue city. Methods: Applied the study design for community intervention with the comparison between intervened and control groups. Results: Our intervention approach included 3 groups of activities: active communication to change behaviors, medical staff training to improve knowledge and practice, treatment and management for glaucoma patients, suspected glaucoma cases or those at risk factors for glaucoma. Results showed that, in the intervention group, the percentage of people with good knowledge increased from 2.5% to 49.1%, good attitude increased from 3.4% to 51.6%, good practice increased from 2.3% to 46.3%. Regarding the improvement of access to glaucoma services, the proportion of people having annual eye exams increased from 30.7% to 49.9%, the figure for having glaucoma screening increased from 26.9% to 59.0%, proportion of those who were glaucoma patients, suspected and at risk with glaucoma that regularly monitored is 90.8%, percentage of those completely followed medication used instruction is 60%. In terms of the effective of the intervention program, the change in glaucoma good practice is 43.2%, the change in having annual eye examination is 46.7%, and the change in having glaucoma screening is 57.7%. Ability to access glaucoma screening services increased by 8 (95%CI: 5.31-12.04, p 〈 0.05) times, good knowledge increased by 3.67 (95%CI: 1.47-9.1, p 〈 0.05) times, good attitude increased by 2.13 (95%CI: 1.02-4.43, p 〈 0.05) times and practice increased by 2.39 (95%CI: 1.42-1.56, p 〈 0.05) times compared to the control groups. Conclusions: There is need to enhance the communication and education on glaucoma for the people, improve the knowledge and ability to detect glaucoma for local health care workers within the existing capacity of the primary health care facility, closely coordinate between medical facilities at different levels in providing glaucoma medical services for all people.     Abstract: Introduction: Glaucoma is a common eye disease that is quite dangerous to public health as one of the major causes of permanent blindness. Although glaucoma can be prevented, most glaucoma patients are underdiagnosed. It’s require a screening system to help patients detect glaucoma and treat it promptly. Objective: 1) To describe the glaucoma situation of person over 40 years old in Hue city. 2) To find out some factors related to glaucoma at study sites. Methods: A desciptive cross-sectional method was conducted with 2025 people over 40 years old in 27 wards of Hue city. Results: The percentage of glaucoma is 4.7%, with closed glaucoma accounting for 55.2%, with 9.1% of people suspected of glaucoma. The percentage of glaucoma patients suffering from blindness was 21.7%. There was an association between glaucoma and age, gender, hypertension, diabetes, cardiovascular disease, family history of glaucoma, previous history of ocular surgery. Conclusions: The percentage of glaucoma is 4.7%, blindness is 21.7%, glaucoma is related to age, gender, hypertension, diabetes, cardiovascular disease, family history of glaucoma, preoperative eye surgery.     Abstract: Purpose: To determine the clinical characteristics of the chronic dacryocystitis and the success rate of external dacryocystorhinostomy. Methods: The retrospective, interventional study without comparing on 27 patients (32 eyes) of chronic dacryocystitis who underwent dacryocystorhinostomy (DCR) from 2010 to 2011. Results were evaluated with standards: epiphora, purelence, lacrimal duct irrigation. Results: Over period of 6 months, 27 patients including 25 (92.59%) female, 2 (7.41%) male. The mean age was 49.8 years (range, 22-79 years). All patients demonstrated epiphera (range, 3 months – 11 years). Dacryocystorhinostomy was performed unilaterally in 81.48%, bilateral 18.52%. Successrate was 90.06% overall. Conclusions: Although techniques in dacryocystorhinostomy of DUPUY-DUTEMPS is old, its result is mainstay of treatment for chronic dacryocystitis in Vietnam.
