Abstract: 〈 i 〉 Purpose: 〈 /i 〉 Primary thyroid marginal zone B-cell lymphoma of the mucosa-associated lymphoid tissue type (pTY-MZL) is an extremely uncommon form of lymphoma. Due to its rarity, the natural history and optimal treatment modality for this disease have yet to be clearly established. 〈 i 〉 Methods: 〈 /i 〉 A total of 27 patients with histologically confirmed pTY-MZL were retrospectively analyzed. 〈 i 〉 Results: 〈 /i 〉 The median age of our subjects was 53 years (range 25–82). This study involved 17 females (63.0%) and 10 males (37.0%). Twenty-four out of 27 patients (88.9%) initially presented with localized disease, defined by Ann Arbor stage I/II. Bone marrow involvement was detected in 8.3% of the patients (2 patients), and 91.7% of the patients (25 of 27) were categorized into the low or low-intermediate risk group, according to the International Prognostic Index criteria. Accompanying Hashimoto’s thyroiditis was detected in 72% of the patients, whereas thyroglobulin antibody levels were elevated in 70% of the patients. Twenty-six patients were treated with surgery, radiotherapy or chemotherapy, and 25 patients achieved complete remission. During the follow-up period, only 2 patients evidenced progression, and no deaths occurred over the course of the study. 〈 i 〉 Conclusion: 〈 /i 〉 pTY-MZL tends to be an indolent disease. However, unlike other mucosa-associated lymphoid tissue site MZLs, pTY-MZL was well controlled via several treatment modalities, and the patients’ responses were sustained for a prolonged period.

Abstract: Although statins are established therapy for the secondary prevention of ischemic stroke, factors associated with adherence to statin treatment following ischemic stroke are not well known. To address this, we assessed the 6-month statin adherence using 8-item Morisky Medication Adherence Scale-8 in patients with acute ischemic stroke. Of 991 patients, 65.6% were adherent to statin at 6-month after discharge. Multiple logistic regression analysis showed that patients’ awareness of hyperlipidemia (OR 1.62; 95% CI 1.07–2.43), large artery stroke subtype (versus non-large artery stroke, OR 1.79; 95% CI 1.19–2.68), and alcohol drinking habits (OR 1.64; 95% CI 1.06–2.53) were positively associated, while high statin dose (versus low dose, OR 0.6; 95% CI 0.40–0.90) and higher daily number of medication pills (OR 0.93; 95% CI 0.88–0.97) were found to have a negative association with self-reported good adherence to statin medication after acute ischemic stroke. However, stroke severity and diagnosis of hyperlipidemia were not associated with adherence. These results suggest that educational and motivational interventions may enhance statin adherence because modifiable factors were associated with statin adherence.

Abstract: 〈 i 〉 Background/Aims: 〈 /i 〉 The development of acute renal failure (ARF) is a very rare complication in patients with acute hepatitis A (AHA). 〈 i 〉 Methods: 〈 /i 〉 We retrospectively investigated the overall incidence, risk factors, and clinical outcomes of ARF associated with AHA. Diagnosis of AHA was made according to the typical hepatitis symptoms and positivity of immunoglobulin M anti-hepatitis A virus in 208 patients with AHA. 〈 i 〉 Results: 〈 /i 〉 ARF was noted in 12 (5.7%) patients, and dialysis was required in 8 (66%) patients. The median duration of hospitalization for patients with ARF was 18 days (range, 6–50 days). The development of ARF was observed in older patients (p = 0.004) and in patients with diabetes (p = 0.001), excessive alcohol consumption (p = 0.01), prolonged international normalized ratio (p = 0.019), and elevated aspartate aminotransferase concentration (p = 0.034). Multivariate analysis revealed that old age (odds ratio, OR, 1.2), elevated aspartate aminotransferase concentration (OR, 1.05), and diabetes (OR, 18.5) were independent risk factors for ARF (each p 〈 0.001). The prognosis of patients with ARF was good, and renal function recovered completely. 〈 i 〉 Conclusion: 〈 /i 〉 ARF associated with AHA is not rare, and the possibility of AHA should be considered in patients with ARF with hepatic dysfunction.

Abstract: 〈 b 〉 〈 i 〉 Background: 〈 /i 〉 〈 /b 〉 Eyelashes of Asians differ from those of Caucasians in morphology and growth characteristics. Ethnic differences also exist for the tolerability profile of prostaglandin analogues. 〈 b 〉 〈 i 〉 Objective: 〈 /i 〉 〈 /b 〉 To evaluate the long-term utility and durability of bimatoprost 0.03% in eyelash augmentation in Asian females. 〈 b 〉 〈 i 〉 Methods: 〈 /i 〉 〈 /b 〉 One cohort received bimatoprost 0.03% for 36 weeks and another for 20 weeks, with the latter cohort followed for 16 weeks after treatment cessation. The primary endpoint was the percent change in eyelash length at week 20. Secondary measures included percent change in eyelash thickness and darkness, physician's Global Eyelash Assessment and patient satisfaction. 〈 b 〉 〈 i 〉 Results: 〈 /i 〉 〈 /b 〉 At week 20, eyelash length was enhanced in a time-dependent manner, with maximum improvement achieved (19.3%; p 〈 0.0001). Significant improvements in thickness and darkness were also achieved (22.9%, 6.0%; p 〈 0.0001). 77.8% of subjects improved by ≥1 grade on Global Eyelash Assessment, with 83.1% satisfied/very satisfied. Improvements were maintained with ongoing treatment to 36 weeks, while these effects were progressively lost with discontinuation. 〈 b 〉 〈 i 〉 Conclusion: 〈 /i 〉 〈 /b 〉 Bimatoprost 0.03% safely enhanced eyelashes in Asian females, maintained with ongoing treatment. Cessation of treatment was associated with progressive loss of effects.

    Abstract: 〈 b 〉 〈 i 〉 Background: 〈 /i 〉 〈 /b 〉 Kidney dysfunction and albuminuria may be associated with BMD. However, little evidence has been reported on relationships between BMD and eGFR and albuminuria. 〈 b 〉 〈 i 〉 Methods: 〈 /i 〉 〈 /b 〉 A total of 8,992 subjects aged 50 years or older participated in a survey conducted. Participants had their lumbar spine and femoral neck BMD measured by a Lunar Prodigy bone densitometer (GE, Madison, WI). Kidney function was assessed using MDRD eGFR and diagnosis of albuminuria was based on albumin-creatinine ratio. 〈 b 〉 〈 i 〉 Results: 〈 /i 〉 〈 /b 〉 ACR was negatively associated with lumbar spine and femur neck BMD in females (lumbar spine: 1.001, 0.988, 0.974 and 0.979 g/cm 〈 sup 〉 2 〈 /sup 〉 , p 〈 0.001; femur neck: 0.796, 0.790, 0.783 and 0.782 g/cm 〈 sup 〉 2 〈 /sup 〉 , p = 0.002), but not in males, after adjusting for covariates. Additionally, eGFR was shown to be negatively associated with lumbar spine BMD after adjusting for covariates (male: 1.181, 1.166, 1.152 and 1.149 g/cm 〈 sup 〉 2 〈 /sup 〉 , p = 0.001; female: 0.997, 0.980, 0.979 and 0.982 g/cm 〈 sup 〉 2 〈 /sup 〉 , p = 0.005), but demonstrated no association with femur BMD. 〈 b 〉 〈 i 〉 Conclusions: 〈 /i 〉 〈 /b 〉 ACR in females was negatively associated with lumbar spine and femur neck BMD, but not in males. eGFR was negatively associated with lumbar spine BMD in both males and females.

Abstract: 〈 b 〉 〈 i 〉 Background: 〈 /i 〉 〈 /b 〉 With the increased incidence of diabetes mellitus, the importance of early intervention in prediabetes has been emphasized. We previously reported that fermented kimchi, a traditional Korean food, reduced body weight and improved metabolic factors in overweight participants. We hypothesized that kimchi and its fermented form would have beneficial effects on glucose metabolism in patients with prediabetes. 〈 b 〉 〈 i 〉 Methods: 〈 /i 〉 〈 /b 〉 A total of 21 participants with prediabetes were enrolled. During the first 8 weeks, they consumed either fresh (1-day-old) or fermented (10-day-old) kimchi. After a 4-week washout period, they switched to the other type of kimchi for the next 8 weeks. 〈 b 〉 〈 i 〉 Results: 〈 /i 〉 〈 /b 〉 Consumption of both types of kimchi significantly decreased body weight, body mass index, and waist circumference. Fermented kimchi decreased insulin resistance, and increased insulin sensitivity, QUICKI and disposition index values (p = 0.004 and 0.028, respectively). Systolic and diastolic blood pressure (BP) decreased significantly in the fermented kimchi group. The percentages of participants who showed improved glucose tolerance were 9.5 and 33.3% in the fresh and fermented kimchi groups, respectively. 〈 b 〉 〈 i 〉 Conclusions: 〈 /i 〉 〈 /b 〉 Consumption of kimchi had beneficial effects on glucose metabolism-related and anthropometric factors in participants with prediabetes. Fermented kimchi had additional effects on BP and insulin resistance/sensitivity. The percentage of participants who showed improvement in glucose tolerance was high in the fermented kimchi group.

Abstract: 〈 i 〉 Objective: 〈 /i 〉 To determine the feasibility of stereotactic body radiotherapy (SBRT) using 3 fractions for isolated colorectal lung metastases. 〈 i 〉 Methods: 〈 /i 〉 From June 2003 to December 2006, 13 cases of isolated pulmonary metastasis from colorectal cancer were treated by SBRT due to an inoperable state (7 patients), or the patient’s refusal to undergo surgical excision (6 patients). All patients underwent chemotherapy for salvage treatment. SBRT doses ranged from 39 to 51 Gy in 3 fractions. Nine patients had a solitary lesion, 3 patients had 2 lesions, and 1 patient had 3 lesions. Median tumor volume for the 18 lesions was 5.9 ml (range 1.6–45 ml). 〈 i 〉 Results: 〈 /i 〉 Follow-up duration was 15–57 months. Three-year overall survival, local control and progression-free survival rates were 64.7, 52.7 and 11.5%, respectively.Univariate analysis showed that total internal target volume was a significant prognostic factor for local control. During the follow-up, 11 of the 13 patients experienced local recurrence, distant metastasis or both. The most frequent site of failure was in a nontargeted lung region. No severe complication was attributed to SBRT. 〈 i 〉 Conclusion: 〈 /i 〉 Our study suggests the potential feasibility of SBRT for selected patients with 1–3 small metastatic nodules. A further larger-scale study is required to define the indications for SBRT in cases with isolated pulmonary metastasis from colorectal cancer.

Abstract: Influenza A virus (IAV) poses a constant worldwide threat to human health. Although conventional vaccines are available, their protective efficacy is type or strain specific, and their production is time-consuming. For the control of an influenza pandemic in particular, agents that are immediately effective against a wide range of virus variants should be developed. Although pretreatment of various Toll-like receptor (TLR) ligands have already been reported to be effective in the defense against subsequent IAV infection, the efficacy was limited to specific subtypes, and safety concerns were also raised. In this study, we investigated the protective effect of an attenuated bacterial outer membrane vesicle harboring modified lipid A moiety of lipopolysaccharide (fmOMV) against IAV infection and the underlying mechanisms. Administration of fmOMV conferred significant protection against a lethal dose of pandemic H1N1, PR8, H5N2, and highly pathogenic H5N1 viruses; this broad antiviral activity was dependent on macrophages but independent of neutrophils. fmOMV induced recruitment and activation of macrophages and elicited type I IFNs. Intriguingly, fmOMV showed a more significant protective effect than other TLR ligands tested in previous reports, without exhibiting any adverse effect. These results show the potential of fmOMV as a prophylactic agent for the defense against influenza virus infection.

Abstract: 〈 i 〉 Background/Aims: 〈 /i 〉 The antibody monitoring system (AMS) is a recently developed enzyme-linked immunosorbent assay (ELISA) crossmatch assay to detect donor-specific anti-HLA immunoglobulin G antibodies (DS-HLA Abs). This study was conducted to compare the AMS with the flow cytometric crossmatch (FCXM) test in renal transplant recipients with high panel-reactive antibody (PRA). 〈 i 〉 Methods: 〈 /i 〉 Thirty-two sera were obtained from 10 patients with panel reactivity above 50%. When anti-HLA Ab was detected by ELISA PRA and the matched donor had the corresponding HLA antigen, it was considered to indicate DS-HLA Ab. The results of the AMS assay and FCXM were compared with the DS-HLA Abs. 〈 i 〉 Results: 〈 /i 〉 Twenty-three (71.9%) sera were positive for DS-HLA Abs by ELISA PRA. The AMS assay showed that the number of compatible sera with DS-HLA Abs was 27 (84.4%), and it was significantly concordant (κ = 0.649, p 〈 0.0001). For FCXM, the number of compatible sera with DS-HLA Abs was 26 (81.3%), and it was also significantly concordant (κ = 0.614, p 〈 0.0001). There was a significant degree of concordance between the AMS assay and FCXM in detection of DS-HLA Abs (κ = 0.452, p = 0.010). 〈 i 〉 Conclusion: 〈 /i 〉 The AMS assay is comparable to FCXM in detecting DS-HLA Abs in high PRA recipients.

    Abstract: 〈 b 〉 〈 i 〉 Background/Aims: 〈 /i 〉 〈 /b 〉 The presence and clinical importance of tissue-resident memory T (T 〈 sub 〉 RM 〈 /sub 〉 ) cells have been recently described in association with various cancer types. However, the frequency and the traditional naïve–effector–memory phenotypic characteristics of T 〈 sub 〉 RM 〈 /sub 〉 cells are largely unknown. 〈 b 〉 〈 i 〉 Methods: 〈 /i 〉 〈 /b 〉 We analyzed single-cell populations of colorectal cancer (CC, 〈 i 〉 n 〈 /i 〉 = 18), stomach cancer (SC, 〈 i 〉 n 〈 /i 〉 = 13), renal cell carcinoma (RCC, 〈 i 〉 n 〈 /i 〉 = 19), and breast cancer (BC, 〈 i 〉 n 〈 /i 〉 = 16) by dissociation of tumor tissue with collagenase/hyaluronidase. We investigated populations of naïve, effector, and memory T and T 〈 sub 〉 RM 〈 /sub 〉 cells by flow cytometry. 〈 b 〉 〈 i 〉 Results: 〈 /i 〉 〈 /b 〉 Among CD8 〈 sup 〉 − 〈 /sup 〉 cells, CC was associated with a significantly higher proportion of CD103 〈 sup 〉 + 〈 /sup 〉 T cells than other tumor types ( 〈 i 〉 p 〈 /i 〉 & #x3c; 0.001). Among CD8 〈 sup 〉 + 〈 /sup 〉 cells, CC and SC were associated with higher CD103 〈 sup 〉 + 〈 /sup 〉 T-cell proportions than RCC and BC ( 〈 i 〉 p 〈 /i 〉 & #x3c; 0.001). Significantly more CD8 〈 sup 〉 + 〈 /sup 〉 than CD8 〈 sup 〉 − 〈 /sup 〉 cells expressed CD103 ( 〈 i 〉 p 〈 /i 〉 & #x3c; 0.001). In association with SC, RCC, and BC, CD8 〈 sup 〉 + 〈 /sup 〉 T cells had a similar T-cell phenotype composition pattern: fewer effector T cells and more memory-type T cells among CD103 〈 sup 〉 + 〈 /sup 〉 cells compared with CD103 〈 sup 〉 − 〈 /sup 〉 cells ( 〈 i 〉 p 〈 /i 〉 & #x3c; 0.05). Tumors with higher proportion of CD103 〈 sup 〉 + 〈 /sup 〉 cells had no specific clinicopathologic characteristics than those with lower proportion of CD103 〈 sup 〉 + 〈 /sup 〉 cells. 〈 b 〉 〈 i 〉 Conclusion: 〈 /i 〉 〈 /b 〉 T 〈 sub 〉 RM 〈 /sub 〉 cell abundance and phenotypes varied among CC, SC, RCC, and BC. Further studies regarding the functional differences of T 〈 sub 〉 RM 〈 /sub 〉 associated with various tumors are warranted.
