    Abstract: In the treatment of oral cavity squamous cell carcinoma (OCSCC), surgical quality measures which are expected to affect outcomes, including the achievement of a clear margin, are surgeon-dependent but might not be invariably associated with hospital volume. Our objective was to explore surgical margin variations and survival differences of OCSCC between two highest-volume hospitals in Taiwan. Materials and methods A total of 2009 and 1019 patients with OCSCC who were treated at the two highest-volume Taiwanese hospitals (termed Hospital 1 and Hospital 2, respectively) were included. We examined how a pathological margin & lt;5 mm impacted patient outcomes before and after propensity score (PS) matching. Results The prevalence of margins & lt;5 mm was markedly lower in Hospital 1 than in Hospital 2 (34.5%/65.2%, p & lt;0.0001). Compared with Hospital 2, tumor severity was higher in Hospital 1. On univariable analysis, being treated in Hospital 2 (versus Hospital 1; hazard ratio [HR] for 5-year disease-specific survival [DSS] = 1.34, p =0.0002; HR for 5-year overall survival [OS] = 1.17, p =0.0271) and margins & lt;5 mm (versus ≥5 mm; HR for 5-year DSS = 1.63, p & lt;0.0001; HR for 5-year OS = 1.48, p & lt;0.0001) were identified as adverse factors. The associations of treatment in Hospital 2 and margins & lt;5 mm with less favorable outcomes remained significant after adjustment for potential confounders in multivariable analyses, as well as in the PS-matched cohort. The 5-year survival differences between patients operated in Hospital 1 and Hospital 2 were even more pronounced in the PS-matched cohort (before PS matching: DSS, 79%/74%, p =0.0002; OS, 71%/68%, p =0.0269; after PS matching: DSS, 84%/72%, p & lt;0.0001; OS, 75%/66%, p & lt;0.0001). In the entire cohort, the rate of adjuvant therapy was found to be lower in patients with margins ≥5 mm than in those with margins & lt;5 mm (42.7% / 57.0%, p & lt;0.0001). Conclusions Within the two highest-volume hospitals in Taiwan, patients with OCSCC with a clear margin status (≥5 mm) achieved more favorable outcomes. These results have clinical implications and show how initiatives aimed at improving the margin quality can translate in better outcomes. A clear margin status can reduce the need for adjuvant therapy, ultimately improving quality of life.

    Abstract: To assess the prognostic significance of different nodal parameters [i.e., number of pathologically positive nodes, log odds of positive lymph nodes, lymph node ratio (LNR), and extra-nodal extension (ENE)] in Taiwanese patients with oral cavity squamous cell carcinoma (OCSCC), and to devise an optimized pN classification system for predicting survival in OCSCC. Methods A total of 4287 Taiwanese patients with first primary OCSCC and nodal metastases were enrolled. Cox proportional hazards regression analysis with the spline method was applied to identify the optimal cut-off values for LNR, log odds of positive lymph nodes, and number of pathologically positive nodes. Results On multivariable analysis, we identified a LNR ≥0.078/0.079, the presence of at least three pathologically positive nodes, and ENE as independent prognosticators for 5-year disease-specific survival (DSS) and overall survival (OS) rates. We therefore devised a four-point prognostic scoring system according to the presence or absence of each variable. The 5-year DSS and OS rates of patients with scores of 0−3 were 70%/62%/50%/36% ( p & lt; 0.0001) and 61%/52%/40%25%, respectively ( p & lt; 0.0001). On analyzing the AJCC 2017 pN classification, patients with pN3a displayed better survival rates than those with pN2 disease. The 5-year DSS and OS rates of patients with pN1/pN2/pN3a/pN3b disease were 72%/60%/67%/43% ( p & lt; 0.0001) and 63%/51%/67%/33%, respectively ( p & lt; 0.0001). Conclusions Three nodal parameters (i.e., a LNR ≥0.078/0.079, the presence of at least three pathologically positive nodes, and ENE) assessed in combination provided a better prognostic stratification than the traditional AJCC pN classification.

    Abstract: Diffraction enhanced imaging is a new X-ray radiographic imaging method using monochromatic X-rays from a synchrotron source which produces high contrast and spatial resolution images of thick absorbing objects. Use of this method in medicine, material science and industrial applications in non-destructive testing is highlighted at present. The purpose of this study is to discuss diffraction enhanced imaging computer tomography which combines computer tomography (CT) and diffraction enhanced imaging (DEI) method. Comparison between DEI-CT images with conventional X-ray CT images shows that the DEI-CT imaging technique gives not only surface information but also the internal structure of organs or soft tissues with high contrast. The obtained images show the micro-structures details of DEI-CT.

    Abstract: In this paper, the novel gradient concrete is innovatively applied to the bridge towers of Chizhou Yangtze River Bridge to solve the cracking and insufficient durability problems of concrete towers. Fiber-reinforced concrete is used in the outer functional area of the bridge tower, to significantly improve its crack resistance during construction and service. Moreover, the integrated design of anti-cracking and mechanical properties of tower materials is achieved. To study the performance of the novel functional gradient concrete (FGC) tower, the mechanical properties of the FGC tower material are tested, and the overall finite element stress is analyzed. Based on the material properties, the mechanical behavior of the cable-stayed bridge tower is studied. The temperature and stress of the FGC tower during the generation of the hydration heat are compared with that of the ordinary concrete tower. The crack resistance of the FGC tower is analyzed by the finite element method. The results show that the FGC tower has good mechanical properties and durability for the cable-stayed bridge towers.

    Abstract: An enclosure experiment was conducted to evaluate the processes involved in algal bloom formation at different trophic levels in Chaohu Lake, which is a large hyper-eutrophic shallow lake in China. In situ investigations were conducted concurrently to quantitatively describe the effects of wind on the horizontal transference and vertical hybridization of algal blooms. The results revealed that certain nutrient trophic levels played a crucial role in the formation of algal blooms. Specifically, sediments were identified as an important source of nutrients and algal seeds that are necessary to maintain the trophic level and growth of algal blooms. In addition, the disturbance caused by wind accelerated the multiplication of algae, as well as their sedimentation and suspension at the water-sediment interface. Furthermore, when the wind speed was less than 3m•s-1, algal clusters drifted on the water surface at a speed that was exponentially related to the wind velocity with a coefficient of 0.9052. When the wind speed was greater than 3m•s-1, the algae mixed together vertically and then sank. The algal bloom distribution in the lake was dominated by leading wind directions. Moreover, the algal biomass of the surface lake water in leeward areas was 8.8 times greater than the biomass in the windward areas during the study period.

    Abstract: The phenotypic change of macrophages (Mφs) plays a crucial role in the musculoskeletal homeostasis and repair process. Although mesenchymal stem cells (MSCs) have been shown as a novel approach in tissue regeneration, the therapeutic potential of MSCs mediated by the interaction between MSC-derived paracrine mediators and Mφs remains elusive. This review focused on the elucidation of paracrine crosstalk between MSCs and Mφs during musculoskeletal diseases and injury. The search method was based on the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) and Cochrane Guidelines. The search strategies included MeSH terms and other related terms of MSC-derived mediators and Mφs. Ten studies formed the basis of this review. The current finding suggested that MSC administration promoted proliferation and activation of CD163 + or CD206 + M2 Mφs in parallel with reduction of proinflammatory cytokines and increase in anti-inflammatory cytokines. During such period, Mφs also induced MSCs into a motile and active phenotype via the influence of proinflammatory cytokines. Such crosstalk between Mφs and MSCs further strengthens the effect of paracrine mediators from MSCs to regulate Mφs phenotypic alteration. In conclusion, MSCs in musculoskeletal system, mediated by the interaction between MSC paracrine and Mφs, have therapeutic potential in musculoskeletal diseases.

    Abstract: Anterior cruciate ligament injury prevention should focus primarily on reduction of the knee abduction moment (KAM) in landing tasks. Gluteus medius and hamstring forces are considered to decrease KAM during landing. The effects of different muscle stimulations on KAM reduction were compared using 2 electrode sizes (standard 38 cm 2 and half size 19 cm 2 ) during a landing task. Twelve young healthy female adults (22.3 [3.6] y, 1.62 [0.02]  m, 50.2 [4.7] kg) were recruited. KAM was calculated under 3 conditions of muscle stimulation (gluteus medius, biceps femoris, and both gluteus medius, and biceps femoris) using 2 electrode sizes, respectively versus no stimulation during a landing task. A repeated-measures analysis of variance determined that KAM differed significantly among stimulation conditions and post hoc analysis revealed that KAM was significantly decreased in conditions of stimulating either the gluteus medius ( P   〈  .001) or the biceps femoris ( P   〈  .001) with the standard electrode size, and condition of stimulating both gluteus medius and biceps femoris with half-size electrode ( P  = .012) when compared with the control condition. Therefore, stimulation on the gluteus medius, the biceps femoris, or both muscles could be implemented for the examination of anterior cruciate ligament injury potential.

    Abstract: Background and Purpose: The availability of oncology biosimilars is deemed as a fundamental strategy to achieve sustainable health care. However, there is scarce systematic evidence on economic effectiveness of cancer biosimilars. We aimed to synthesize evidence from pharmacoeconomic evaluation of oncology biosimilars globally, provide essential data and methodological reference for involved stakeholders. Materials and Methods: This systematic review was conducted in PubMed, embase, the Cochrane library, CRD, ISPOR and NICE utill December 31, 2019. Information on basic characteristics, evaluation methodology and results were extracted. Quality of included studies was assessed using the Consolidated Health Economic Evaluation Reporting Standards Checklist. Results: For 17 studies identified (13 from Europe and four from United States), the overall quality was generally acceptable. A total of seven biological molecules involved with filgrastim, EPOETIN α , and trastuzumab leading the three. The mostly common evaluation perspective was payer, but the time horizon varied greatly. There were ten studies which adopted cost minimization analysis to evaluate efficiency while seven studies adopted budget impact analysis to address affordability, with cost ratio and cost saving being its corresponding primary endpoint. Although the comparability of included studies was limited and specific results were largely affected by uptake and price discount rates of the oncology biosimilar, the comprehensive results consistently favored its promotion. Conclusion: Globally, the economic evaluation of cancer biosimilars is in its initial phase. However, limited evidence from developed countries consistently supported both cost-effectiveness of efficiency and affordability of oncology biosimilars, while they were largely affected by uptake and price discount rate.

    Abstract: The interactions between X-ray and the sample and crystals were investigated, and the influence of small angle scattering on images was emphatically analyzed in diffraction enhanced imaging in this paper. Not only were the additional noise term caused by small angle scattering introduced, and more general DEI equation established, but also expressions of absorption contrast, extinction contrast and refraction contrast were given for the peak image and the middle slope image. With the new DEI equation, the contrasts of two kinds of resultant images (apparent absorption image and refraction image) were discussed.
