    Abstract: Mild autonomous cortisol secretion (MACS) presents with a marked female preponderance, but whether the sex difference in its distribution has any relevance to the presentation and outcome of the disease is unknown. The aim of this study was therefore to compare biochemical indices of hypercortisolism and impaired glucose metabolism between male and female patients with MACS. Method We enrolled a total of 98 patients with autonomous/possible autonomous cortisol secretion in our study, and indices of hypercortisolism and glucose metabolism were collected and compared between the male and female patients. Logistic regression models were used to evaluate the association between sex and cortisol-secretory ability, as well as between the latter and glucose metabolism. In addition, we conducted further stratified analyses according to the degree of autonomous cortisol secretion and menopausal status. Results Cortisol levels at 00:00 and 08:00 h after a 1-mg dexamethasone suppression test (DST) and low-dose DST were significantly higher in female than in male MACS patients, and the inhibition rate of 1-mg DST was lower in the women than in the men. This significant difference still remained after adjusting for age, BMI, and the course of the disease. Logistic regression analysis revealed a significant association between autonomous cortisol secretion and fasting C-peptide, as well as with the C-peptide-to-glucose ratio in females relative to male patients. In addition, stratified analyses indicated that this association was observed only among women with autonomous cortisol secretion and who were premenopausal. Conclusion The level of autonomic cortisol secretion in female patients with MACS was higher than in male patients, and the association between autonomous cortisol secretory ability and glucose homeostasis was only noted in patients with autonomous cortisol secretion and in premenopausal women. This phenomenon will, however, require closer follow-up.

    Abstract: This study aims to investigate the potential prognostic significance of programmed death ligand-1 (PD-L1) protein expression in tumor cells of breast cancer patients received neoadjuvant chemotherapy (NACT). Methods Using semiquantitative immunohistochemistry, the PD-L1 protein expression in breast cancer tissues was analyzed. The correlations between PD-L1 protein expression and clinicopathologic characteristics were analyzed using Chi-square test or Fisher’s exact test. The survival curve was stemmed from Kaplan-Meier assay, and the log-rank test was used to compare survival distributions against individual index levels. Univariate and multivariate Cox proportional hazards regression models were accessed to analyze the associations between PD-L1 protein expression and survival outcomes. A predictive nomogram model was constructed in accordance with the results of multivariate Cox model. Calibration analyses and decision curve analyses (DCA) were performed for the calibration of the nomogram model, and subsequently adopted to assess the accuracy and benefits of the nomogram model. Results A total of 104 breast cancer patients received NACT were enrolled into this study. According to semiquantitative scoring for IHC, patients were divided into: low PD-L1 group (61 cases) and high PD-L1 group (43 cases). Patients with high PD-L1 protein expression were associated with longer disease free survival (DFS) (mean: 48.21 months vs. 31.16 months; P=0.011) and overall survival (OS) (mean: 83.18 months vs. 63.31 months; P=0.019) than those with low PD-L1 protein expression. Univariate and multivariate analyses indicated that PD-L1, duration of neoadjuvant therapy, E-Cadherin, targeted therapy were the independent prognostic factors for patients’ DFS and OS. Nomogram based on these independent prognostic factors was used to evaluate the DFS and OS time. The calibration plots shown PD-L1 based nomogram predictions were basically consistent with actual observations for assessments of 1-, 3-, and 5-year DFS and OS time. The DCA curves indicated the PD-L1 based nomogram had better predictive clinical applications regarding prognostic assessments of 3- and 5-year DFS and OS, respectively. Conclusion High PD-L1 protein expression was associated with significantly better prognoses and longer DFS and OS in breast cancer patients. Furthermore, PD-L1 protein expression was found to be a significant prognostic factor for patients who received NACT.

    Abstract: Gliomas are the most common primary tumors of the central nervous system and portend a poor prognosis. The efficacy of emerging and promising immunotherapies varies significantly among individuals. Distinction and transformation of cold and hot tumors may improve the antitumor efficacy of immunotherapy. Methods and Results In this study, we constructed a necroptosis-related lncRNA module based on public databases. The association of this module with survival was assessed using the Cox regression, Kaplan-Meier survival analysis, and nomogram, external validation was also conducted in another public database. Furthermore, we performed gene set enrichment analysis (GSEA), immune checkpoint and tumor microenvironment analysis, and in vitro qRT-PCR validation. Finally, we clustered all samples into 2 clusters based on the expression of model lncRNAs and identified cluster 1 as cold tumors with fewer infiltrating T cells. Conclusions Identifying cold and hot tumors by necroptosis-related lncRNAs can help available immunotherapeutic strategies to achieve efficacy in the precise treatment of individuals. Prior treatment failure can be overcome by targeting necroptosis-related lncRNAs.

    Abstract: Corruption in healthcare is on the rise. When corruption infiltrates global health, causes embezzlement of public health funds, malfunctioning medical equipment, fraudulent or ineffective health services such as expired medicines and fake vaccines that could have life-or-death consequences. A corrupt healthcare system, amid global health crises like the COVID-19 pandemic, when resources are in constraint and trust is in high demand, can lead to devastating, though avoidable, health and economic consequences. It is imperative for policymakers, health experts, patients, caregivers, and global health funders to promptly acknowledge and address corruption in healthcare. The current pandemic generates an emergency and disorder state on health care systems across the globe, especially in low- and middle-income countries, where a weakening of control measures is evident, creating the perfect storm for corruption. This paper builds on existing research to examine processes that support essential stakeholder engagement in anti-corruption efforts. In this context, an extensive review of literature has been conducted by using various databases such as PubMed, Science direct, SCOPUS, Research Gate, and Google Scholar and a total of 45 articles and documents on corruption and COVID-19 were screened and selected by authors independently. To fill the knowledge gaps about the need for actions to be taken during a pandemic like COVID-19, we propose an anti-corruption grassroots movement that focuses on changing the social norms surrounding corruption in healthcare. By pushing forward a practice that normalizes conversations about corruption in everyday health practices and involving more stakeholders in the protection of public health resources, we argue that not only local health systems can become more resilient and resistant to corruption, but also global health initiatives can become more effective and efficient to improve individual and global health.

    Abstract: This paper proposes an improved alternating current (AC) arc model based on the Cassie-Mayr hybrid model and magnetohydrodynamics (MHD) theory. The undetermined parameters of the traditional Cassie-Mayr arc model are mainly obtained by subjective methods such as empirical value, approximate expression, and indirect estimation which are complex, inaccurate, and dependent on experimental data. To overcome these disadvantages, an axisymmetric two-dimensional arc model is established based on the MHD theory, and the undetermined parameter values of the Cassie-Mayr hybrid arc model are obtained based on the results of MHD numerical simulation. The accuracy of the proposed modified Cassie-Mayr model is verified by comparing with experimental data, and the results of the modified Cassie-Mayr model, traditional Mayr model, and Cassie model are discussed. The arc characteristics of serval loads are studied and analyzed based on the modified model.

    Abstract: This study aims to investigate the prognostic significance of p-JNK in breast cancer patients receiving neoadjuvant chemotherapy (NACT) and analyze the relationship between anisomycin, p-JNK. A total of 104 breast cancer patients had NACT were enrolled in this study. The western blot and immunohistochemistry assays were used to determine the protein expressions of p-JNK in human breast cancer cell lines and patients’ cancer tissues. The chi-square test and Fisher’s exact test were adopted to gauge the associations between breast cancer and clinicopathological variables by p-JNK expression, whereas the univariate and multivariate Cox proportional hazards regression models were used to analyze the prognostic value of p-JNK expression. The Kaplan-Meier plots and the log-rank test were adopted to determine patients’ disease-free survival (DFS) and overall survival (OS). Findings indicated that the p-JNK expression had prognostic significance in univariate and multivariate Cox regression survival analyses. Results of log-rank methods showed that: (1) the mean DFS and OS times in patients with high p-JNK expression were significantly longer than those in patients with low p-JNK expression (χ 2 = 5.908, P = 0.015 and χ 2 = 6.593, P = 0.010, respectively). p-JNK expression is a significant prognostic factor that can effectively predict the survival in breast cancer patients receiving NACT. Treatment with the JNK agonist anisomycin can induce apoptosis, lead to increased p-JNK expression and decreased p-STAT3 expression. Moreover, the p-JNK expression was inversely correlated with p-STAT3 expression.

    Abstract: A nomogram was developed in this work to predict the probability of delayed cerebral infarction (DCI) after ruptured intracranial aneurysms (RIA) clipping. Methods Clinical data of patients with intracranial aneurysm were obtained from the neurosurgery department of the First Affiliated Hospital of Chongqing Medical University from January 2016 to December 2020. A total of 419 patients receiving surgery of ruptured intracranial aneurysm clipping were included and a total of 37 patients with DCI were set as the observation group. The control group consisted of 382 patients without DCI. Risk factors of DCI were screened by univariate and multivariate logistic regression analysis and included in the nomogram. Results Univariate analysis showed that female ( P  = 0.009), small aneurysm ( P  = 0.031), intraoperative aneurysm rupture ( P  = 0.007) and cerebral vasospasm ( P   & lt; 0.001) were risk factors for postoperative DCI while smoking history ( P  = 0.044) were protective factors for postoperative DCI. Multivariate Logistic regression analysis showed that small aneurysm ( P  = 0.002, OR = 3.332, 95%–7.104), intraoperative aneurysm rupture ( P  = 0.004, OR = 0.122, 95%-CI, 0.029–0.504)and cerebral vasospasm ( P   & lt; 0.001, OR = 0.153, 95%-CI, 0.070–0.333) were independent risk factors of postoperative DCI. The calibration curve of the probability of occurrence showed that the nomogram was in good correspondence with the observed results with a C-index of 0.766 (95% CI, 0.684–0.848). Meanwhile, the Decision curve analysis (DCA) showed that the established predictive model had a good clinical net benefit. Conclusion The well-established nomogram is expected to be an effective tool to predict the occurrence of DCI after intracranial ruptured aneurysm and can be used to assist clinicians to develop more effective treatment strategies and improve the prognosis of patients.
