    Analysis of Complications Development Predictors after Radical Prostatectomy

    Abstract: Introduction: The standard treatment for prostate cancer is radical prostatectomy (RP). This surgical technique results in typical complications such as intraoperative blood loss, urethral strictures (vesicourethral anastomosis), urinary incontinence, erectile dysfunction, lymphocele, and the ureter or rectum injury. The study aims to analyze the development of complications after radical prostatectomy. Materials and methods: The article presents a retrospective analysis of clinical cases with an established diagnosis of prostate cancer from 2012 to 2018 in Irkutsk, Russian Federation. During this period, 52 patients met the study criteria. A multivariate analysis established Non-Nerve-sparing RRP (OR-0.9; 95% CI-1.9; -0.07; p=0.035) as a significant incontinence predictor after 2 years. Results: Multivariate analysis also established previous transurethral operations as a significant predictor of the vesicourethral anastomosis stricture (OR 6.09; 95% CI 0.71; 11.4; p=0.026), which indicates a six times risk of developing a vesicourethral anastomosis stricture if the patient already had one or more transurethral surgery. Obesity (OR 0.12; 95% CI 0.03; 0.21; p = 0.008), diabetes (OR 2.3; 95% CI 0.45; 4.2; p = 0.015) and coagulopathy (OR 3.1; 95% CI 0.5; 5.7; p = 0.019) became independent predictors of lymphocele development. Conclusions: The study revealed some new information on the possible predictors of such complications as urinary incontinence in the late period, the lymphocele, and stricture of the vesicourethral anastomosis. Some of the results require further study and confirmation.

    Abstract: Objective: Renal vascular thrombosis is a thrombotic blockage of the major renal arteries resulting in acute damage or chronic kidney disease. The study aims to analyze rare clinical cases of renal artery thrombosis with following renal infarction. Materials and methods: The article present a retrospective study of examination and treatment results of patients diagnosed with the renal arteries thrombosis in the urological hospital of the Irkutsk Clinical Hospital No. 1 in the period 2012-2020. Results and Discussion: During the entire observation period, there were two clinical cases of acute renal artery thrombosis. The 86-year-old woman developed thrombosis because of atrial fibrillation, which was probably the result of inadequate correction (insufficient dose of anticoagulant). Kidney infarction had complications like rapid suppuration because of chronic urinary infection and calculus of the renal pelvis. It resulted in nephrectomy with subsequent recovery. The second case was the 45-year-old man examined according to existing standards, and the diagnosed-on time in the first hours of the disease. However, complete thrombosis of the right renal artery neutralized the effectiveness of conservative therapy with warfarin. In this case, nephrosclerosis and renal failure were natural. Conclusion: Renal artery thrombosis is a rare pathology that requires special attention from the clinician due to the high risk of renal function loss. Timely diagnosis and correct treatment tactics are especially important. Bangladesh Journal of Medical Science Vol.20(3) 2021 p.550-555

    Abstract: Themodern stage of modelling of behavior of reinforced concrete structures is associated with the widespread use of numerical methods. Thedistinctive paper is devoted todevelopment and numerical implementation of methods of structural analysis including progressive collapse analysis of spatial plate-shell reinforced concrete structures with allowance for physical nonlinearity, crack formation and inducedanisotropy. The relevance of the research topic is substantiated, the current status of research on this topic in Russia and abroad (including various aspects dealing with types of diagrams for modelling of reinforced concrete structures,construction of general deformation models of reinforced concrete, strength criteria for reinforced concrete structures and methods of structural analysis) is analyzed, the goals, objectives and boundaries of the study are determined, the provisions constituting scientific novelty, theoretical significance and practical significance are formulated, publications on the topic are under consideration. It should be noted that generallyfurther improvement and modifications of reinforced concrete models and their integration incontemporarysoftware systems for structural analysis remain very important. It is assumed that developing methods of analysis of reinforced concrete structures will replace multi-iterative approaches to the solution of physically nonlinear problems and move from the practically possible high-precision analysis of individual structures to the analysis of complex structural systems with allowance for various factors of physical nonlinearity and anisotropy. As a result,reliability of design solutions will increase significantly. The strength criteria used in this way, in turn, will also eliminate a number of errors in existing methods for strength analysis.

    Abstract: Рассмотрены теоретические аспекты применения асинхронных электродвигателей серии ДАТ (ДАО) в промышленных приводах с частотным преобразователем: принципиальная схема, Т-образная схема замещения и механические характеристики тягового электропривода в диапазоне 40…120 Гц. Проведены экспериментальные исследования значений виброскорости (частотный диапазон 10…1000 Гц), виброускорения (частотный диапазон 10…10000 Гц) в контрольных точках корпуса статора при изменении частоты питания в диапазоне 60…120 Гц и соответственно частоты вращения 1800…3600 об/мин. При обработке данных использованы аппарат спектрального состава и анализ временных реализаций вибрационного сигнала. Выявлена неравномерность вибрационных и тепловых полей. Полученные диагностические признаки нарушения равномерности воздушного зазора и магнитных полей могут служить основанием для проверки электрической части двигателя на испытательном стенде.

    Abstract: Improving the fuel, economic and environmental indicators of the process of generating electrical energy is an urgent problem of small-scale power generation, due to a significant share of traditional solutions based on diesel generator sets, which are not highly efficient in the overall structure of primary fuel use (including the stages of production, transportation and final transformation). At the same time, the modern development of technologies in the field of promising energy sources and power electronics contributes to the sustainable diversification of energy resources, strengthening the position of environmentally friendly renewable energy. However, the use of alternative types of fuels for variable speed engine generators, photovoltaic and wind power plants is associated with the need to use additional devices for converting electrical energy – stabilizing voltage converters. This article is devoted to issues related to the design of autonomous power systems based on these voltage converters using mathematical modeling tools. The paper describes a complex mathematical model of an autonomous power plant with a variable speed motor-generator and an electric energy storage device, presents a mathematical description of the main components of such a system, and also provides examples of the model implementation in a specialized software environment. A distinctive feature of the proposed model is an approach to modeling the operating modes of an engine-generator set, which is a synthesis of multi-parameter characteristics of an internal combustion engine, analytical expressions for energy storage and conversion systems. The presented complex model makes it possible to determine the electrical, energy and environmental indicators of autonomous power plants when the power of the electrical load changes. The article provides an example of the nature of the results obtained in computer modeling.

    Abstract: The subject of the paper is theoretical justification of legal nature of positive constitutionalresponsibility legal institute. The evolution of views on the institution of positive constitutionalresponsibility from the first works on it (S.A. Avak`yan, Yu.P. Eremenko, F.M. Rudinsky, N.A. Bobrova) to the present time is analyzed.The purpose is to clarify its role in establishment and maintaining the regime of constitutionallegality.The results, scope of application. Doubts about the legal nature of positive constitutionalresponsibility up to its complete denial are identical with doubts about the legal nature ofmany constitutional norms, the denial of their direct action. These disputes will last forever.Direct service of constitutional and legal responsibility to the quality of governance is a featureof this type of legal responsibility along with its pronounced political character, as wellas the specific guilt of the subject of constitutional tort (liability not only for their acts butfor the acts of their subordinates).The emphasis on positive moral aspect to the detriment of "sanction" (retrospective) aspectof the constitutional responsibility does not meet the challenges of the new time.Proponents of affirmative responsibility had good purpose to build its high creative andeducational role from the positive side of the legal liability. However, this good purpose inpractice has not led to optimistic results.The authors come to the conclusion the legal regulation of mechanisms of responsibilityenforcement in Russia is necessary.
