    Abstract: Nijmegen breakage syndrome (NBS) is a DNA repair disorder characterized by combined immunodeficiency and a high predisposition to lymphoid malignancies. The majority of NBS patients are identified with a homozygous five base pair deletion in the Nibrin ( NBN) gene (c.657_661del5, p.K219fsX19) with a founder effect observed in Caucasian European populations, especially of Slavic origin. We present here an analysis of a cohort of 136 NBS patients of Eastern Slav origin across Belarus, Ukraine, Russia, and Latvia with a focus on understanding the geographic distribution, incidence of malignancy, and treatment outcomes of this cohort. Our analysis shows that Belarus had the highest prevalence of NBS (2.3 per 1,000,000), followed by Ukraine (1.3 per 1,000,000), and Russia (0.7 per 1,000,000). Of note, the highest concentration of NBS cases was observed in the western regions of Belarus and Ukraine, where NBS prevalence exceeds 20 cases per 1,000,000 people, suggesting the presence of an “Eastern Slavic NBS hot spot.” The median age at diagnosis of this cohort ranged from 4 to 5 years, and delay in diagnosis was more pervasive in smaller cities and rural regions. A total of 62 (45%) patients developed malignancies, more commonly in males than females (55.2 vs. 34.2%; p =0.017). In 27 patients, NBS was diagnosed following the onset of malignancies (mean age: 8 years). Malignancies were mostly of lymphoid origin and predominantly non-Hodgkin lymphoma (NHL) ( n =42, 68%); 38% of patients had diffuse large B-cell lymphoma. The 20-year overall survival rate of patients with malignancy was 24%. However, females with cancer experienced poorer event-free survival rates than males (16.6% vs. 46.8%, p =0.036). Of 136 NBS patients, 13 underwent hematopoietic stem cell transplantation (HSCT) with an overall survival of 3.5 years following treatment (range: 1 to 14 years). Indications for HSCT included malignancy ( n =7) and immunodeficiency ( n =6). Overall, 9% of patients in this cohort reached adulthood. Adult survivors reported diminished quality of life with significant physical and cognitive impairments. Our study highlights the need to improve timely diagnosis and clinical management of NBS among Eastern Slavs. Genetic counseling and screening should be offered to individuals with a family history of NBS, especially in hot spot regions.

    Abstract: 14 samples were prepared in the SrO-Al2O2-SiO2 system by solid-phase synthesis. The degree of interaction of the initial reagents during the synthesis was controlled by X-ray phase analysis on a DRON-3M diffractometer using CuKα radiation. The identification of crystalline phases was carried out using the PDF-2 powder diffractometry database. The following crystalline phases were identified in the samples: SrAl2Si2O8; SrAl12O19; Sr2Al2SiO7; SrAl2O4; Sr2SiO4; Sr3Al2O6; Sr4Al2O7; Sr3SiO5; Sr6Al18Si2O37; Al2O3. The results of a study of the physicochemical and mechanical properties of samples synthesized in SrO-Al2O2-SiO2 system are presented. The temperature coefficients of linear expansion, the values of densities, melting temperatures, and Young's modul were determined. The values of the characteristics obtained for the samples under investigation make them promising for use as materials for a wide range of technical applications.

    Abstract: The presence of larvae of the European river lamprey Lampetra fluviatilis was reported for the first time from in macrobenthic communities of the Neva Bay of the Gulf of Finland (St. Petersburg, Russia). It has been shown that ammocoetes inhabit several areas, at depths to 12 meters. Apart from ammocoetes, the communities with lamprey larvae contain abundant oligochaetes, bivalves, and less commonly chironomid larvae. At some sites, the number of larvae was up to 80 specimens/m2, while their biomass was over 90% of the total of the community. The abundance of macrozoobenthos in these sites is generally low (average abundance 190 specimens/m2, biomass – 70 g/m2), which is usual in autumn. Intestines of the ammocoetes constantly contained large quantities of diatoms representing common phytoplankton of the Gulf of Finland.

    Abstract: he authors present the latest data on the hypophosphatasia (HPP) management in children. Hypophosphatasia is a rare genetic disease caused by deficiency of tissue-specific alkaline phosphatase due to mutation in the ALPL gene. The article covers all the features of epidemiology, etiology and pathogenesis, detailed stages of differential diagnostics. Treatment guidelines for pediatric patients are provided, they are based on the principles of evidence-based medicine. Special attention was given to the only effective method of hypophosphatasia management —enzyme replacement therapy (ERT). This material is the clinical guideline draft for the management of patients with hypophosphatasia prepared by the Union of Pediatricians of Russia and the Association of Medical Geneticists.

    Abstract: This article presents modern data on epidemiology, etiology, and clinical manifestations of mucopolysaccharidosis (MPS) type I in children. MPS develops due to deficiency of particular lysosomal enzyme which determines the disease type. The article considers in details disease's pathogenesis and classification. Evidence-based approaches to diagnosis (differential diagnosis included) are covered, moreover, special attention is paid to pathogenetic, symptomatic, and surgical treatment of MPS.

    Abstract: Introduction. Prevention and treatment of glaucoma, which threatens the development of blindness, is one of the most important medical and social problems facing modern society in the era of value-based health care. However, against the background of a wide selection of prostaglandin-F2α analogs, which have an approximately similar level of decrease in ophthalmotonus, a clinical and economic argumentation is required for the approach when forming a budget for drug treatment of glaucoma. The aim of the study. To assess the burden on the health budget of the city of Moscow when using the fluorinated prostaglandin-F2α analog as compared to other prostaglandin-F2α analogs for the treatment of adult patients with primary open-angle glaucoma. Material and methods. There was studied data on the population of patients with a diagnosis of «glaucoma» receiving medication from the budgetary funds of the city of Moscow. The direct medical costs of healthcare in Moscow were calculated for preparations of prostaglandin-F2α analogs for the treatment of adult patients with primary open-angle glaucoma in various clinical options for 5 years. Results. The use of the fluorinated prostaglandin-F2α analog drug Tafluprost in patients with primary open-angle glaucoma is economically justified, while budget savings in the 5-year horizon amounted to 196,777,224.60 rubles, 12,295,378.28 rubles in the baseline scenario (default appointment). and 116 084 865.02 rubles, and in alternative scenarios (appointment for adverse events on Latanoprost) - 125 113 646.70 rubles, 43 942 565.04 rubles. and 89,609,415.58 rubles, respectively. Conclusion. The inclusion of the drug Tafluprost in the drug supply programs of the Moscow healthcare system will increase the efficiency of the use of financial resources and increase the availability of drug supply.

    Abstract: Introduction. In recent years the researchers have noted an increase in the number of occupational diseases detected for the first time in one employee of mining and metallurgical enterprises in the Arctic. The study aims to obtain new data on occupational polymorbidity among employees of enterprises in the Arctic zone of the Russian Federation. Materials and methods. We have studied the results of social and hygienic monitoring under the section "Working conditions and occupational morbidity" of the population of the Russian Federation in 2007-2020. Results. Scientists first have diagnosed occupational diseases in 7685 workers in 2007-2020. The authors have identified one nosological form of occupational pathology in 6164 employees (80.2%), two forms - in 1128 employees (14.7%), three forms - in 243 employees (3.2%), four forms - in 92 employees (1.2%), five forms - in 41 employees (0.5%), six forms - 14 employees (0.2%). In addition, one employee had seven diseases, another had eight diseases and the third had nine diseases. The researchers have noted the minimum level of polymorbidity in 2007 (1,013±0.005 cases), and the maximum - in 2012 (1,502±0.040 cases). The highest polymorbidity rates were in charge workers (3,333±0.849 cases), drillers (2,215±0.025 cases) and cleaners of finished products (2,136±0.208 cases), and the lowest - in health workers (1,087±0.054 cases) and civil aviation flight personnel (1,011±0.011 cases). There was no professional polymorbidity among the education workers. Of the nine subjects of the Russian Arctic, the experts have observed the maximum level of polymorbidity in the Murmansk region (1,552±0.021 cases), exceeding the indicators of all other regions. Limitations. There is a possibility of different methodological approaches among occupational pathologists to the diagnosis of occupational pathology in nine remote Arctic regions belonging to four federal districts of Russia. Conclusion. The phenomenon of polymorbidity is due to the combined effect of harmful production factors and a complex of reasons leading to late diagnosis of occupational pathology and, as a result, its progression with the continuation of labor activity. To reduce the level of occupational polymorbidity, it is necessary to improve working conditions, upgrade the skills of occupational doctors and use additional research methods that allow to objectify the early clinical manifestations of diseases.

    Abstract: Introduction. To prevent health disorders under the influence of cooling meteorological factors of open production sites, the assessment of the individual characteristics of the employee’s thermoregulation is of particular importance. The purpose of the study was to describe the individual characteristics of thermoregulation of employees of an oil producing enterprise when working in open production sites during the cold season. Materials and methods. We studied the employees of the oil-producing enterprise of Western Siberia, performing labour operations in open production sites during the cold season (operators, process plant machinists, repairmen). An assessment of general and local violations of thermoregulation of the body of workers was carried out using objective assessment methods (assessment of the thermal state of the body in seventy six people, skin thermometry with cold load - 54 people) according to approved methods. The duration of work in the open area of the workers included in the study ranged from 12 to 31 hours during a 40-hour work week. Results. According the assessment of the thermal state of workers, in 18.8% of workers, the personal indicators for assessing the thermal state were found to fail to correspond to the recommended values in the guidelines. Evaluation of the results of skin thermometry with a cold load in workers performing labour operations in open production sites under cooling conditions of the cold season of the year made it possible to establish the average values of temperature indicators in the study group to indicate the presence of initial signs of violations of the thermal control of the body and neurocirculatory disorders in the hands and fingers. An assessment of individual characteristics showed general and local violations of thermoregulation to be associated with the duration of work at open production sites, the objective indicators of local violations of thermoregulation were additionally affected by the employee’s smoking habit and work experience. Limitations. Significant time costs for assessing the general and local thermoregulation of the employees of the enterprise and distracting them from performing labor operations during the shift can be attributed to the limitations of the methodology. Conclusion. For the first time, the revealed features of thermoregulation in oil production workers in Western Siberia made it possible to develop targeted medical and preventive measures.
