    Abstract: Limited data exist on risk factors for the long-term outcome of pulmonary arterial hypertension (PAH) associated with congenital heart disease (CHD-PAH). We focused on the index of pulmonary vascular disease (IPVD), an assessment system for pulmonary artery pathology specimens. The IPVD classifies pulmonary vascular lesions into four categories based on severity: (1) no intimal thickening, (2) cellular thickening of the intima, (3) fibrous thickening of the intima, and (4) destruction of the tunica media, with the overall grade expressed as an additive mean of these scores. This study aimed to investigate the relationship between IPVD and the long-term outcome of CHD-PAH. Methods This retrospective study examined lung pathology images of 764 patients with CHD-PAH aged & lt;20 years whose lung specimens were submitted to the Japanese Research Institute of Pulmonary Vasculature for pulmonary pathological review between 2001 and 2020. Clinical information was collected retrospectively by each attending physician. The primary endpoint was cardiovascular death. Results The 5-year, 10-year, 15-year, and 20-year cardiovascular death-free survival rates for all patients were 92.0%, 90.4%, 87.3%, and 86.1%, respectively. The group with an IPVD of ≥2.0 had significantly poorer survival than the group with an IPVD & lt;2.0 ( P  = .037). The Cox proportional hazards model adjusted for the presence of congenital anomaly syndromes associated with pulmonary hypertension, and age at lung biopsy showed similar results (hazard ratio 4.46; 95% confidence interval: 1.45–13.73; P  = .009). Conclusions The IPVD scoring system is useful for predicting the long-term outcome of CHD-PAH. For patients with an IPVD of ≥2.0, treatment strategies, including choosing palliative procedures such as pulmonary artery banding to restrict pulmonary blood flow and postponement of intracardiac repair, should be more carefully considered.

    Abstract: We synthesized vertically aligned carbon nanotubes (CNTs) using multilayered catalyst thin films (Fe/Al 2 O 3 and Al 2 O 3 /Fe/Al 2 O 3 ) by RF (13.56 MHz) CH 4 /H 2 /Ar plasma-enhanced chemical vapor deposition. Pretreatment of the catalyst is crucial for CNT growth. In this paper, we analyzed the effect of catalyst reduction on CNT growth. Catalyst thin films on substrates were reduced by H 2 plasma pretreatment at 550 °C to form nanometer-sized catalyst particles. The multilayered thin films were analyzed; the chemical composition and oxidation state by X-ray photoelectron spectroscopy (XPS) and the surface morphology by scanning electron microscopy (SEM). The Fe 2p peak of the XPS spectra showed that Fe x O y in the as-deposited catalyst was effectively reduced to Fe by a pretreatment of duration 4 min. Using this catalyst, we obtained CNTs with an average diameter of 10.7 nm and an average length of 5.3 µm. However, pretreatment longer than 4 min resulted in shorter CNTs and the Fe peak was shifted from Fe to Fe 3 O 4 . These transitions (Fe 2 O 3 →Fe 3 O 4 →Fe→Fe 3 O 4 ) can be explained by the enthalpy of the oxides. This result indicates the presence of an optimum ratio between Fe and Fe x O y to maximize the CNT lengths.

    Abstract: Arterial stimulation and venous sampling (ASVS) is an important technique for localizing insulinoma. The principle behind ASVS is that insulin secretion is promoted from insulinoma cells by the injection of calcium into the insulinoma-feeding artery. However, the mechanism for ASVS-induced insulin secretion remains unclear. Both insulinoma and familial hypocalciuric hypercalcemia (FHH) are rare diseases. This study reports on a case in which both of these diseases occur concurrently. Design and patient The patient with FHH also suffered from insulinoma. We reasoned that insulin secretion for ASVS is dependent on the calcium-sensing receptor (CaSR). ASVS was performed on this patient. The expression of the CaSR protein and corresponding mRNA were confirmed. Results No significant changes in the plasma levels of insulin and C-peptide were observed during ASVS. The patient was clinically diagnosed as having FHH. We confirmed that a mutation in the CaSR gene was present in the genomic DNA of this patient and that there were no mutations in the multiple endocrine neoplasia type 1 gene. In addition, expression of both CaSR mRNA and CaSR protein was confirmed in the insulinoma samples. Conclusion These results suggest that the CaSR gene is involved in ASVS-induced insulin secretion.

    Abstract: Chronic inducible urticaria (CIndU) is a subgroup of chronic urticaria induced by a specific stimulus. We evaluated basophil characteristics in patients with CIndU and compared with those in patients with chronic spontaneous urticaria (CSU) and healthy controls (HCs). Methods Blood was collected from patients, and a basophil activation test (BAT) was performed. Basophil responsiveness and surface marker expression in patients with CIndU were compared with those in patients with CSU and HCs. For some patients with CIndU, blood was collected before and after wheals were induced. In these cases, we compared the responsiveness of basophils before and after the appearance of wheals. Result HCs (n=23) and patients with CIndU (n=24) or CSU (n=38) were enrolled in the study. The degree of basophil activation at steady state in patients with CIndU was higher than in HCs. Basophil responsiveness via high-affinity IgE receptor (FcϵRI) stimulation with anti-IgE or anti-FcϵRI antibody in patients with CIndU was equivalent to that in HCs, and higher than that in patients with CSU. No abnormalities in IgE and FcϵRI expressions on the surface of basophils in patients with CIndU were observed. When we induced wheals in some patients with CIndU and performed a BAT before and after the appearance of wheals, no significant changes were found. Conclusion Peripheral blood basophils in CIndU were slightly activated at steady state, but no abnormalities in basophil responsiveness. In future, a higher number of cases should be enrolled to confirm the role of basophils and refine therapeutic targets for CIndU.

    Abstract: Xanthine oxidase is involved in the production of uric acid and the generation of superoxide anion. We evaluated the long-term effect of febuxostat, a non-purine selective xanthine oxidase inhibitor, on endothelial function in patients with asymptomatic hyperuricemia. Methods In the PRIZE study, patients with hyperuricemia were randomly assigned to either add-on febuxostat treatment (febuxostat group) or non-pharmacologic hyperuricemia treatment (control group). Among the 514 participants, endothelial function was assessed in 41 patients in the febuxostat group and 38 patients in the control group by flow-mediated vasodilation (FMD) of the brachial artery at the beginning of the study and after 12 and/or 24 months of treatment (63 men; median age, 68.0 years). Results The least squares mean concentration of serum uric acid was significantly lower in the febuxostat group than in the control group at 6 months (mean between-group difference [febuxostat group - control group], −2.09 mg/dL [95% confidence interval (CI), −2.520 to −1.659] ; P & lt; 0.001), 12 months (mean between-group difference, −2.28 mg/dL [95% CI, −2.709 to −1.842]; P & lt; 0.001), and 24 months (mean between-group difference, −2.61 mg/dL [95% CI, −3.059 to −2.169]; P & lt; 0.001). No significant differences were found between groups in the least squares mean estimated percentage change in FMD at 12 months (mean between-group difference, −0.56% [95% CI, −1.670 to 0.548]; P = 0.319) and at 24 months (mean between-group difference, −0.60% [95% CI, −1.886 to 0.685]; P = 0.357). Conclusion Febuxostat treatment did not alter endothelial function assessed by FMD during a 2-year study period in patients with asymptomatic hyperuricemia.

    Abstract: Strained Ge nanowire MOSFETs with a compressive strain as high as -3.9 % were fabricated by the 2-step Ge-condensation technique without intentional doping for the S/D. Record high hole mobility ( μ eff = 1992 cm 2 /Vs @ N s = 1.7x10 12 cm -2 ) was achieved among Ge nanowire pMOSFET ever reported. Inserting a plasma-oxide (GeO x ) inter-layer between the high-k dielectric and the strained Ge nanowire channel greatly improved not only the mobility but the cut-off characteristics. A low off-current (2.7 nA/μm at V d = -0.5 V) was achieved by the device with the gate length of 45 nm, thanks to the reduced interface state density.

    Abstract: Source/drain formation by ion doping using self-activation technique without thermal anneal on polysilicon thin film transistors (TFT's) have been investigated. Phosphorus ions and protons were simultaneously implanted into polysilicon films. At total ion doses over 8×10 15 ions/cm 2 , resistivities of self-activated films have been found equal to those of the ones annealed after the implantation. This self-activation technique enabled us to fabricate self-aligned TFT's having low-resistance Al gates and, at the same time, to hydrogenate active layers. With the case of self-activation technique, field-effect mobilities as high as 58 and 49 cm 2 /(V·s) were achieved in n-channel and p-channel TFT's, respectively, fabricated on glass substrates.

    Abstract: Ambulatory care sensitive conditions (ACSCs) are defined as clinical conditions for which the risk of emergency hospital admission can be reduced by timely and effective ambulatory care. However, the actual status of patients with ACSCs who are transported by ambulance and their outcomes have not been fully elucidated. The purpose of this study was to reveal characteristics and outcomes of patients with ACSCs who were transported by ambulance using population-based registry data in Osaka, Japan. Methods This descriptive epidemiological study was conducted in the 5-year period from January 2016 to December 2020, and included patients who were transported by ambulance due to sudden illness. In this study, ACSC was further classified into acute ACSCs, chronic ACSCs, and preventable ACSCs based on the ICD-10. The number of patients transported by ambulance for ACSCs per 100,000 population in each age group was calculated for each year. In addition, Poisson regression models were used to assess the trend in the number of ACSCs patients transported by ambulance. Results A total of 1,572,152 patients were included in this study (acute ACSCs, n = 69,621; chronic ACSCs, n = 12,250; preventable ACSCs, n = 96,036; and non-ACSCs, n = 1,394,245). The overall median age was 71 years (95% confidence interval [CI], 46-92). Patients with acute ACSCs (median age: 76 years [IQR: 60-84] ) and chronic ACSCs (median age: 80 years [IQR: 72-87]) were older, while patients with preventable ACSCs were younger (median age: 66 years [95%CI: 3-81] ) and included a high proportion of children (33.3%, 32,002/96,036). Regarding the outcome after treatment at the emergency department, 42.6% (670,392/1,572.152) of patients were hospitalized, while 82.3% (10,079/12,250) of patients with chronic ACSCs were hospitalized. No change was observed in adults ( P = 0.001) or elderly (age ≥65 years) individuals ( P = 0.376) with preventable ACSCs, however, among children, the number increased until 2019 (732.5) and then decreased in 2020 (371.8) ( P = 0.392). Conclusion In this study, patients with chronic ACSCs were predominantly elderly, while patients with preventable ACSCs were polarized between children and the elderly. Among patients with preventable ACSCs, there was no change over time in adults and children, but there was a marked decrease among the elderly after 2020.
