    Abstract: With the rollout of the world's largest vaccine drive for SARS-CoV-2 by the Government of India on January 16 2021, India had targeted to vaccinate its entire population by the end of 2021. Struggling with vaccine procurement and production earlier, India overcome these hurdles, but the Indian population still did not seem to be mobilizing swiftly toward vaccination centers. The severe second wave has slowed the vaccination pace and was also one of the major contributing factors to vaccine hesitancy. To understand the nature of vaccine hesitancy and its underlying factors, we conducted extensive online and offline surveys in Varanasi and adjoining regions using structured questions. Most respondents were students (0.633). However, respondents from other occupations, such as government officials (0.10), have also participated in the study. Interestingly, most people (0.75) relied on fake news and did not take COVID-19 seriously. Most importantly, we noticed that a substantial proportion of respondents (relative frequency 0.151; mean age 24.8 years) reported that they were still not interested in vaccination. We observed a significant association between vaccine hesitancy and socioeconomic status (χ 2 = 307.6, p & lt; 0.001). However, we failed to detect any association between vaccine hesitancy and gender (χ 2 = 0.007, p & gt; 0.5). People who have neither been vaccinated nor have ever been infected may become the medium for spreading the virus and creating new variants, which may lead to the vaccine-resistant variant. We expect this extensive survey to help the Government upgrade their vaccination policies for COVID-19 in North India.

    Abstract: The entire globe faces a very precarious situation during pandemic Covid-19. During national lockdown in India, it was hazardous for the migrant labourers and more unsafe for women migrant workers. They came across many livelihood challenges like: employment, nutrition, government support, societal security, health facility, etc. which were repeatedly underlined in numerous mass medianews. Therefore, to know the actual problems encountered by the women migrant returnees, this particular study was undertaken in Jharkhand state where most of women workers go on migration for earning. For the purpose, 41 women migrant returnees were randomly selected as respondents from 18 villages covering five blocks of the state. By the use of a survey schedule, data was collected in the arena of socio-economic outline, place of migration, reason of migration, sector of engagement, income, difficulties confronted and support need from government for livelihood enhancement which are pronounced in this study.

    Abstract: ntroduction: Peritoneal folds are double layered peritoneal membranes produced during reflexion of peritoneum from viscera to the body wall or between adjacent viscera. They are named as ligaments, mesenteries, omentum etc. Sometimes during the course of development some abnormal peritoneal folds develop, which are of surgical importance because these folds or bands may constrict a part of intestine leading to obstruction and gangrene. These sometimes produce peritoneal recesses, which are the potential sites for internal herniation. Aim: To study abnormal peritoneal folds in cadavers. Materials and Methods: This was a descriptive study, conducted on 40 cadavers during August 2010-July 2020. Abdominal cavity was dissected and carefully observed for organisation of viscera and peritoneal reflexions in the abdominal cavity. Results: Out of 40 cadavers, congenital abnormal peritoneal bands were found in four cases and two cases had multiple peritoneal bands. Three cases had associated congenital abnormalities of the gut. One case of nonrotation of gut was observed, where small intestine was situated on the right side and colon on the left side. In this case, the ascending colon was fixed by a peritoneal band to right upper abdominal wall. In the second case, there was incomplete fusion of descending mesocolon. The mobile segment of descending colon was fixed by an abnormal peritoneal band. In the third case, there was absorption of pelvic mesocolon. Conclusion: Though the cause of development of abnormal peritoneal band is not very clear, this study shows that they may be present in cases of abnormal rotation or abnormal peritoneal fixation of gut.

    Abstract: Background: Spinal anesthesia with hyperbaric bupivacaine provides a dense neural block in cesarean delivery but associated with side effects like hypotension, bradycardia nausea and vomiting. Addition of low dose fentanyl with low dose of hyperbaric bupivacaine may decrease the in­cidence of these complications. The aims of study was to compare the hemodynamic parameters (blood pressure and heart rate), nausea and vomiting with low dose intrathecal hyperbaric bupivacaine with fentanyl vs a conventional dose of intrathecal hyperbaric bupivacaine in patient undergoing elective cesarean section. Methods: Seventy-four pregnant women aged 20-35 years old which un­derwent elective cesarean section at Chitwan Medical College were ran­domized into two groups. One group received spinal anesthesia with 8mg of0.5% hyperbaric bupivacaine and 25μg fentanyl and another group re­ceived 12mg 0.5% bupivacaine. Results: The mean age, baseline heart rate, systolic and diastolic blood pressure were comparable in both groups. Significant difference in hypo­tension (24.31% vs. 62.16%, p 〈 0.05) and Nausea and vomiting (16.20% vs. 27%, p 〈 0.05) were found in bupivacaine-fentanyl group versus a conven­tional dose of spinal bupivacaine group. Conclusions: Low dose of bupivacaine with Fentanyl provides good spinal anesthesia for cesarean section with less hypotension, nausea and vomit­ing in comparison to bupivacaine alone.

    Abstract: Introduction: The superior gluteal nerve (SGN) is a branch of sacral plexus with root value of L4, L5 and S1. It leaves pelvic cavity through greater sciatic foramen along with superior gluteal artery above piriformis. This neurovascular bundle lies in close proximity to superior acetabular rim. Iatrogenic damage to SGN is common during hip arthroplasties and may be primarily attributed to inappropriate placement of retractors. Alarmingly high percentage of affected individuals are stuck with persistent irreversible damage to SGN. Vascular injuries are not as common but pose a challenging scenario to surgeons. Hence in both situations prevention is of supreme importance. Precise knowledge of course and relation of superior gluteal neurovascular bundle (SGNVB) to clinically useful landmarks such as the superior rim of acetabulum is desired. With an aim to provide baseline data for the Indian population we conducted this study. Material and methods: 200 dry adult Indian hip bones {Left side -109(male:66, female:43); Right side-91(male:66, female:43)} were photographed in anatomical position. Two lines- line A and line B were drawn. Line A corresponded to a horizontal passing through the anterior inferior iliac spine (AIIS) and roof of GSN while line B passed tangentially through the highest point on the acetabular rim parallel to line A. The vertical distance (white line) between the 2 lines was measured (Fig.2) was measured using Image J software. Results: The mean distance calculated was 0.62 ± 0.16 cm (0.68 ± 0.38 cm in right hip bones and 0.60 ± 0.30 cm in left side hip bones). The difference between the two sides and the two genders were compared and found to be statistically non- significant. Conclusion: A safe zone of 0.5 to 0.7 cm beyond the superior acetabular rim should be considered during surgeries around hip joint. The safe zone can be easily measured by the surgeons intraoperatively and be used as a guide to careful positioning of the retractors while performing surgeries around the hip joint. Better localization of SGNVB using the anatomic landmark defined in this study may be used to decrease surgical morbidity.
