    Abstract: Objective: To investigate the association between sarcopenia and functional improvement in patients older and younger than 65 years upon completion of an inpatient rehabilitation program. Design: Prospective cohort study. Participants: Adult consecutive patients who completed the inpatient rehabilitation program at a metropolitan tertiary referral hospital general inpatient rehabilitation unit. Methods: Sarcopenia status was determined using the European Working Group on Sarcopenia in Older People 2 algorithm, using muscle mass measured by BioImpedance Analysis and grip strength. Progress in rehabilitation was measured using change in the Functional Independence Measure and Goal Attainment Scaling score. To investigate the age group by sarcopenia status interaction we used quantile regression models with bootstrapped standard error estimation for functional improvement and linear regression model with robust standard error estimation for GAS score. Results: 257 participants [128 (50%) male, median age 63 years (IQR: 52–72)], 33(13%) with sarcopenia, completed inpatient rehabilitation [median length of stay 16 days (IQR: 11–27.5)] . Participants' median Functional Independence Measure change was 24 (IQR 15–33.5) and mean total Goal Attainment Scaling score was 57.6 (SD 10.2). Adjusting for admission Functional Independence Measure score, the median difference in Functional Independence Measure change between participants with and without sarcopenia was: −4.3 (95% CI: −10.6, 1.9); p = 0.17 in participants 65 years and younger, and 4.6 (95% CI: 1.0, 8.2); p = 0.01 in participants older than 65; age-by-sarcopenia interaction p = 0.02. Conclusions: Unlike younger people, older people with sarcopenia have greater functional improvement in inpatient rehabilitation than those without sarcopenia.

    Abstract: The paper deals with redistribution of energetic and plastic resources in the organism during stress, acute phase response and diabetes mellitus. Assuming metabolic regulation is based upon balanced contradictory influences of diverse chemical signals (substrates, ions, autacoids, hormones, neurotransmitters, physiological autoantibodies) the peculiarities of insulin- and counter-insulin effects during acute and chronic adaptation under stress, acute phase response and diabetes mellitus have been discussed. The role of chronic stress and accompanying metabolic disorders has been accentuated as a basis for stress-associated diseases’ development. The role of outstanding Argentinean pathophysiologist Bernardo Alberto Houssay (1887-1971) is discussed. He had devoted himself to studies of pituitary bioregulators’ influence on insulin effects immediately after insulin discovery and had established in 1924 that pituitary removal is able to increase sensitivity to insulin in dogs, and developed “secondary diabetes mellitus” concept as a result of counter-insulin hormones’ excess (especially hormones of adenohypophysis and the adrenal glands (“steroid diabetes”). Bernardo Alberto Houssay was really the first to experimentally prove pituitary and adrenal bioregulators to oppose the insulin effects. Bernardo Alberto Houssay has been 46 times nominated for the Nobel prize for 17 years (1931 through 1947), and finally was awarded the Nobel prize in 1947 for achievements in Physiology and Medicine for disclosing the role of pituitary in carbohydrate metabolism (he was the first ever Nobel prize winner from Latin America). The paper discusses publications by B. A. Houssay, current development of his ideas, as well as historical and biographical information on his research and his scientific school of endocrinologists-pathophysiologists in the context of his epoch, on the background of history of Argentina [3 figs, bibliography: 34 refs].

    Abstract: Hashimoto thyroiditis is the most common thyroid disease. This form of pathology has a diverse clinical picture, including neuropsychiatric disorders. There are frequent cases of comorbidity of autoimmune thyroiditis and psychiatric forms of pathology, along with such a nosological entity as Hashimotos encephalopathy (aka: Steroid-responsive encephalopathy of autoimmune thyroiditis), characterized by an increased level of antithyroid autoantibodies and various mental disorders, with still unclear pathogenesis. The question arises, how to regard patients with psychiatric disorders and Hashimoto thyroiditis either as patients having autoimmune thyroiditis, comorbid with psychiatric forms of pathology, or as patients with Hashimotos encephalopathy? We studied groups of patients with autoimmune thyroiditis free from any psychiatric disorders, autoimmune thyroiditis comorbid with psychiatric forms of pathology, and a group of healthy donors similar as regards to their age and sex. We also studied medical history, clinical manifestations of the disease, instrumental data and the serum levels of thyrotropin, thyroid hormones, various antithyroid autoantibodies, and prolactin. We analyzed the correlation of laboratory and instrumental parameters and clinical data in all groups of patients. Therewas a significant relationship (p 0,05) between various psychiatric symptoms and a decreased level of free thyroxine, an increased level of thyroid stimulating hormone (TSH), an increased level of prolactin and an increased volume of a thyroid gland. Asignificant relationship (p 0,05) was also found between various symptoms of hypothyroidism and a decreased level of free triiodothyronine (FT3), an increased level of antibodies to thyroglobulin (anti-TG Ab), and an increased level of antibodies to thyroid peroxidase (anti-TPO Ab).

    Abstract: The article presents second ever published description of rheumatoid arthritis case co-morbid with Rendu — Osler — Weber disease (hereditary hemorrhagic teleangiectasia) in a 63-yearsold female patient. The aim is describing the first case report of a patient who after a confirmed hereditary hemorrhagic teleangiectasia diagnosis developed rheumatoid arthritis one year later. Historical data and brief pathophysiological characteristic of both diseases and their possible intermingle are included: (i) mutant genes in hereditary hemorrhagic teleangiectasia all encode proteins involved in the TGF-beta signaling pathway, and increased plasma levels of TGF-beta-1 and vascular endothelial growth factor have been seen in such patients; (ii) TGF-beta plays a role in the development of synovial cell proliferation, inflammation, and angiogenesis in rheumatoid arthritis; (iii) TGF-β regulates thymic T-cell selection, inhibits cytotoxic T lymphocyte (CTL), Th1-, and Th2-cell differentiation while promoting peripheral * Contribution of L.     Abstract: The article written by the co-chairmen of the First St. Petersburg Congress on Autoimmunity "Bridge between East and West", held in St. Petersburg State University June 30-July 2, 2017 describes this event and gives an overview of the main lectures and discussions during the Congress. The latest data on the role of diet, adjuvants, vaccines, viruses and microbiome, on genetic factors, stress, smoking, lifestyle, drugs - in etiology, as well as the role of lymphocytes, macrophages, various autoantibodies, complement and cytokines - in the pathogenesis and diagnosis of autoimmune diseases were discussed. The congress discussed their comorbidity, ways of treatment and prevention, markers, methodology and contribution of serological tests in the diagnosis and classification of autoimmune diseases. Current data on physiological autoimmunity, as well as the early predictive role of autoantibodies were described. The participants performed philosophical discussions about immunological concepts, and organizational discussions of the standardization of diagnostics and the principles of working with large databases, as applied to this field. The programme included Clinical Pathophysiology все of various autoimmune rheumatologic, psycho-neurological, endocrine, cardiovascular, obstetric-gynecological and many other diseases. The authors conclude that the rapid formation of Autoimmunology as a new integral medical branch happens nowadays (9 figs, bibliography: 69 refs).

    Abstract: Background: Stroke survivors are at high risk of dementia, associated with increasing age and vascular burden and with pre-existing cognitive impairment, older age. Brain atrophy patterns are recognised as signatures of neurodegenerative conditions, but the natural history of brain atrophy after stroke remains poorly described. We sought to determine whether stroke survivors who were cognitively normal at time of stroke had greater total brain (TBV) and hippocampal volume (HV) loss over 3 years than controls. We examined whether stroke survivors who were cognitively impaired (CI) at 3 months following their stroke had greater brain volume loss than cognitively normal (CN) stroke participants over the next 3 years. Methods: Cognition And Neocortical Volume After Stroke (CANVAS) study is a multi-centre cohort study of first-ever or recurrent adult ischaemic stroke participants compared to age- and sex-matched community controls. Participants were followed with MRI and cognitive assessments over 3 years and were free of a history of cognitive impairment or decline at inclusion. Our primary outcome measure was TBV change between 3 months and 3 years; secondary outcomes were TBV and HV change comparing CI and CN participants. We investigated associations between group status and brain volume change using a baseline-volume adjusted linear regression model with robust standard error. Results: Ninety-three stroke (26 women, 66.7 ± 12 years) and 39 control participants (15 women, 68.7 ± 7 years) were available at 3 years. TBV loss in stroke patients was greater than controls: stroke mean ( M ) = 20.3 cm 3 ± SD 14.8 cm 3 ; controls M = 14.2 cm 3 ± SD 13.2 cm 3 ; [adjusted mean difference 7.88 95%CI (2.84, 12.91) p -value = 0.002]. TBV decline was greater in those stroke participants who were cognitively impaired ( M = 30.7 cm 3 ; SD = 14.2 cm 3 ) at 3 months ( M = 19.6 cm 3 ; SD = 13.8 cm 3 ); [adjusted mean difference 10.42; 95%CI (3.04, 17.80), p -value = 0.006]. No statistically significant differences in HV change were observed. Conclusions: Ischaemic stroke survivors exhibit greater neurodegeneration compared to stroke-free controls. Brain atrophy is greater in stroke participants who were cognitively impaired early after their stroke. Early cognitive impairment was associated greater subsequent atrophy, reflecting the combined impacts of stroke and vascular brain burden. Atrophy rates could serve as a useful biomarker for trials testing interventions to reduce post-stroke secondary neurodegeneration.
