    Abstract: Background: The COVID-19 pandemic has seen an unprecedented lockdown with restrictions on human movement and interaction, imposed throughout the world to contain the spread of the disease. This gave us the unique opportunity to study the pattern of patients presenting to the dermatology out-patient department during this period.Methods: The study was a retrospective observational study involving the assessment of patient records from 25th March 2020 to 7th June 2020. This period corresponds to the duration of strictly imposed nationwide lockdown which was relaxed with phased resumption from 8th June 2020, termed as unlock 1.Results: A decrease in the absolute as well as proportional number of patients was seen in majority of the cases with a statistically significant proportional decline being seen in cases of allergic contact dermatitis (p-value =0.007), acne (p value 〈 0.001), male pattern hair loss (p value 〈 0.001) and female pattern hair loss (p value 〈 0.001), verrucae (p value=0.01), seborrheic dermatitis (p value 〈 0.001), ephelids (p value 〈 0.001), melasma (p value 〈 0.001), post-inflammatory hyperpigmentation (p value=0.006). However, there were certain disorders whose proportionate representation increased significantly during the lockdown period contrary to prevalent belief. These disorders included urticaria (p value 〈 0.001), herpes zoster (p value 〈 0.001), scabies (p value =0.01), generalized pruritus (p value 〈 0.001) and prurigo (p value 〈 0.001).Conclusions: Disorders like acne, male pattern hair loss, female pattern hair loss, verrucae, seborrheic dermatitis, ephelids, melasma and post-inflammatory hyperpigmentation which do not cause significant morbidity showed a significant decrease in proportional representation. The proportionate representation of disorders like urticaria, herpes zoster, scabies, generalized pruritus and prurigo increased significantly during the lockdown period.

    Abstract: Introduction: Distal humerus lateral condyle fractures are common injuries in childhood and need effective treatment to prevent complications. Open reduction and internal fixation with Kirschner (K) wires is a proven, cost-effective and convenient technique for treating pediatric lateral condyle distal humeral fractures. K-wires may be buried beneath the skin or inserted through the skin with the wire ends exposed. There is no clear consensus as to whether K-wires should be buried or left exposed outside the skin. The aim of this study to compare the results and complications of divergent buried and non-buried K-wires in the treatment of lateral condyle fractures of the distal humerus. Methods: A study was conducted between March 2019 to August 2020 in 32 pediatric patients with lateral condyle fractures of distal humerus who were treated with open reduction with divergent k-wire fixation in our hospital. All patients under the age of 14 years presenting with lateral condyle humerus fractures with more than 2 mm displacement were included in the study. The exclusion criteria were open fractures, undisplaced fractures treated conservatively, and pathological fractures. Results: 32 patients under the age of 14 years were included in this study and were divided into two groups (Buried K-wire group and Exposed K-wire group) based on the treatment they received. Out of the 16 cases with exposed wires, 2 cases had superficial pin tract infection. And out of 16 cases with buried wires, 3 cases developed an infection, and 5 cases had skin erosion. However, no case of severe infection was seen. No cases of non-union were seen and all patients returned to preinjury function.

    Abstract: Objective: To early diagnose the patients of AMI by using cardiac markers like CK, CK-MB, to find out the significance of these enzymes in comparison with high sensitive Troponin T hs and to find out the presence of a micro-infarction by using newly developed cTnT hs method. Patients and Methods: This was a cross sectional study, conducted at Punjab Institute of Cardiology and included 545 patients. Blood sample of every patient “Suspected of AMI” was sent to the laboratory for estimation of CK, CK-MB by conventional kit method and a high sensitive Troponin T reagent kit was used for prompt diagnosis of AMI. Patients of varying age were included and demographic details like hypertension, diabetes and smoking, family history of IHD, were collected to find out association with AMI. Results: Out of 545 study participants 62.65% (n=341) were male and 37.43% (n=204) were females, the mean age was 54.8 ± 13.42 years, the most frequent risk factor seen was hypertension (HTN in 72.0 % (n=90), followed by Smoking 45.5% (n=90), Diabetes mellitus (DM) 43.3% (n=90), and frequency of family history was seen 18.8% (n=90) in patients positive for acute myocardial infarction (AMI). 78.8% (n=90) males and 21.2%(n=90) females, indicated more prevalence of AMI in male population with frequency of AMI in patients from 41-60 years of age. There was significant association between hypertension, smoking and diabetes with AMI. Family history was not significantly associated with the incidence of AMI. CPK, CK-MB were performed on patient’s serum sample according to the procedures determined by reagent kits of Roche Diagnostic company on Chemistry Analyzer Hitachi 912. Optimized UV test method was used according to DGKC (German Society of Clinical Chemistry and Laboratory Medicine) and IFCC. Special Chemistry analyzer Cobas e 411, automated instrument was used for the quantitative measurement of Troponin T by using Chemiluminescence method (Troponin T hs reagent kits of Roche Diagnostic Company). Conclusion: It was concluded that a direct relationship exists between Troponin T and CPK, and CK-MB was more specific with even more closer and linear relationship with Troponin T hs. It was further observed that high sensitive Troponin T reagent method alone was sufficient to make a final diagnosis of AMI. Troponin T hs alone not only reduces the investigation time but reduces the overall cost as well. Keywords: Acute Myocardial Infarction.

    Abstract: BACKGROUND: Cardiac syndrome X is a subject with yet unsettled etiology and management. Conventional investigations have not been able to establish that chest pain is due to myocardial ischemia. Magnetic resonance imaging has higher resolution and is more accurate for detecting ischemia. AIMS AND OBJECTIVES: To establish subendocardial ischemia as the cause of chest pain in cardiac syndrome X by virtue of stress perfusion cardiac MRI. METHODS: Contrast enhanced cardiac MRI was performed in 15 cases and 7 matched controls both at rest and during a six minute infusion of adenosine. Both visual and quantitive analysis were performed. In quantitative analysis both myocardial perfusion index and myocardial perfusion reserve index was measured. RESULTS: There was a significant increase in myocardial perfusion in both subendocardium as well as in subepicardium in both cases as well as in controls upon stress with adenosine. In controls the subendocardial perfusion index rose from 0.13 0.3 to 0.18 .03 and in the subepicardium from 0.12 .02 to 0.18 .03. In patients with cardiac syndrome X subendocardial perfusion index rose from 0.14 .03 to 0.19 .03 and subepicardial perfusion index rose from 0.13+.03 to 0.19 .03. Visual analysis showed short lasting subendocardial dark rim artificats in both cases and controls which lasted for only 3 to 5 beats. CONCLUSION: Our cardiovascular MR study of patients with cardiac syndrome X demonstrated significant and almost similar magnitude adenosine induced increase in both subendocardial and subepicardial myocardial perfusion indices in both study as well as control group. We found no evidence of subendocardial ischemia in patients with cardiac syndrome X. JMS 2011;14(2):46-51

    Abstract: Background: Arrhythmogenic right ventricular cardiomyopathy /dysplasia (ARVC/D) is a genetic form of cardiomyopathy and is one among the most common causes of sudden cardiac death (SCD). The aim of our study was to analyze the clinical profile of (ARVC/D) patients presenting with sustained Ventricular Tachycardia (VT).Methods: This single center cohort study evaluated 107 patients who presented with sustained ventricular tachycardia (VT) in our hospital. After aetiological evaluation of all these patients, 15 patients were found to have ARVC/D as the cause of sustained ventricular tachycardia (VT) as per the Modified Task Force Criteria. The clinical profile of these patients was observed in detail to enhance our knowledge about this entity in our part of the world.Results: Mean age at presentation was 30 years and 12 patients were males. Nine patients were haemodynamically stable at the time of sustained VT and the rest of patients were haemodynamically unstable. Left Bundle Branch Block (LBBB) was the most common ECG morphology present in 11 patients. Antiarrhythmic drugs terminated VT in 7 patients. All the 6 patients presenting in a state of haemodynamic instability received DC cardioversion. Mortality occurred in 2 patients during the hospital stay.Conclusions: ARVC/D presenting with sustained VT is an important manifestation of the disease. Males are more commonly affected than females. Haemodynamic instability at the time of presentation carries a poor prognosis.

    Abstract: Background: Chronic urticaria (CU) is a common skin condition, affecting 0.5-1% of the population. Multiple studies have demonstrated that plasma D-dimer levels could be employed as a biomarker for disease activity and treatment response in patients with CU. Aim: The aim was to evaluate the correlation between D-dimer levels and the disease activity of CU. Materials and Methods: This study was a case-control study conducted on 120 patients suffering from CU and 50 healthy subjects. Plasma D-dimer levels were measured using ELISA. Results: Mean plasma D-dimer levels in the patients with CU (720.5 ng/mL) were significantly higher than those in the controls (405.6 ng/mL) (p 〈 0.001). A significant correlation was observed between plasma D-dimer levels and the UAS (urticaria activity score). Conclusion: The patients with CU had higher plasma D-dimer levels than the control group. A positive statistical correlation was observed between plasma D-dimer levels and the severity of CU.
