    Abstract: A problem of the novel coronavirus infection pandemic is the absence of specific biomarkers, the determination of which would make it possible to assess the likelihood of a severe disease course, development of complications, immediate and long-term consequences, and effective etiotropic (antiviral) therapy. The severity of the novel coronavirus infection depends on various factors such as the initial state of health, immune status, age, smoking status, concomitant cardiovascular diseases, and diabetes mellitus. However, a severe disease course is also observed in patients without the aforementioned risk factors. The development of the disease and its complications depends on sex and geographical identity. Angiotensin-converting enzyme 2 (ACE2), associated by gene-gene interaction with ACE, plays a main role in the pathogenesis of the penetration of severe acute respiratory syndrome-2 coronavirus into the cell. The main body of information on this problem is represented by systematic meta-analyses and results of single-center cohort studies, which offer insufficient information to unequivocally assert the associations of ACE and ACE2 gene polymorphisms with pathological changes in the circulatory system during and after a new coronavirus infection. Differences in the incidence of ACE and ACE2 alleles may explain the differences between susceptible populations and/or response to the severe coronavirus infection. The above studies were carried out on the effect of the coronavirus in the initial period of the pandemic. For a more complete molecular genetic picture of the influence of polymorphism, persons with different strains of the coronavirus must be considered. In addition, no data are available regarding the expressions of ACE and ACE2 genes in response to a coronavirus infection. Moreover, the identification of the polymorphic variants of the genes of the reninangiotensinaldosterone system and ACE2 associated with a high risk of developing and worsening cardiovascular diseases may be one of the promising areas for the early diagnosis and prevention of post-COVID-19 changes. Therefore, all scientific interest research is aimed at studying genetic factors, such as single nucleotide polymorphisms that affect susceptibility to infection, severity of the disease course, and development of circulatory system consequences. In general, polymorphic variants of ACE and ACE2 and their interaction will help us understand this problem and systematize knowledge for further research in this area.

    Abstract: BACKGROUND: The death of bone tissue cells with impaired blood supply, followed by the development of secondary arthrosis/arthritis of the adjacent joint, is most characteristic of osteonecrosis of the femoral head. This condition is common in young patients mainly in men of working age. Hip arthroplasty at a young age has a high risk of loosening of the endoprosthesis components, which can lead to many repeated surgical interventions. In recent years, hypotheses about the possible role of infectious agents in articular pathology have been discussed, as evidenced by the detection of specific antibodies and antigens of pathogens in the joint fluid and periarticular tissues. Mycoplasmas can trigger the development of osteonecrosis because the bone marrow is the main depot and source of endogenous reinfection (when immunity is weakened). АIM: This study aimed to identify signs of the presence of mycoplasmas in the soft tissues of the hip joint and bone marrow of the femur during hip arthroplasty in patients with idiopathic avascular necrosis of the femoral head. MATERIALS AND METHODS: Clinical and laboratory data of 36 patients with idiopathic aseptic necrosis of the femoral head who underwent total hip arthroplasty were analyzed, and 11.1% of the patients had a history of sexually transmitted diseases. The tissues of the synovium and bone marrow were examined by polymerase chain reaction for the presence of DNA to sexually transmitted infections. RESULTS: The average age of the patients was 42.7 years, and the average period from disease onset to the time of surgery was 46.2 months. Bilateral joint damage was found in 78% of cases, and 91.7% of the patients had stage 4 femoral head disease (ARCO). The preoperative Harris score was 33.7 points on average. In history, 4 (11.1%) patients had sexually transmitted diseases. During the study, when analyzing tissue samples of the synovial membrane and bone marrow, DNA fragments of Chlamydia trachomatis, Mycoplasma genitalium, Ureaplasma spp., and M. hominis were not isolated in any case, including in patients with previous mycoplasma and chlamydial infection. CONCLUSION: The absence of mycoplasma suggests that a history of sexually transmitted diseases does not significantly affect the outcomes of hip arthroplasty in aseptic necrosis of the femoral head.

    Abstract: The early bronchodilatory effects of the first dose of long-acting anticholinergics (glycopyrronium and tiotropium) were compared with those of a fixed double combination of long-acting bronchodilators of various classes (indacaterol/glycopyrronium) in patients with stable chronic obstructive pulmonary disease. The possibility of using the results of an early bronchodilatory response to predict their effectiveness in the basic therapy of chronic obstructive pulmonary disease is evaluated. A total of 176 patients with chronic obstructive pulmonary disease were examined. The patients were randomized into three groups. The first group (n = 66) took glycopyrronium, the second group (n = 60) received a combination of indacaterol/glycopyrronium, and the third group (n = 50, control) took tiotropium. Broncholytic tests with the listed drugs were evaluated. The early bronchodilatory effect of the first dose of 110/50 mcg indacaterol/glycopyrronium was manifested by significant bronchodilation (p 0.001) from 30 min, reached its maximum value 60 min after drug intake, and persisted after a 28-day course of treatment. The combination of indacaterol/glycopyrronium provided rapid and prolonged bronchodilation in patients with stable chronic obstructive pulmonary disease, demonstrating advantages over the isolated use of glycopyrronium and tiotropium. Maximization of bronchodilation by the sequential use of glycopyrronium and salbutamol leads to an increase in the volume of forced exhalation in the first second on the 90th min, comparable with the results of indacaterol/glycopyrronium on the 60th min after drug intake, which indicates the clinical feasibility of maximizing bronchodilation with drugs both separately and in combination. Based on a direct positive correlation between the initial value of the forced expiratory volume in the first second and the value on day 28 of indacaterol/glycopyrronium therapy, an equation for predicting the individual effectiveness of the drug during treatment is derived.

    Abstract: It is believed that hypoxia-induced factor (HIF1) is the key mediator of oxygen metabolism. It was first identified as a transcription factor activated in cells and tissues by lowering the partial pressure of oxygen (O2). The HIF1 activator spectrum includes both external factors hypoxia, psycho-emotional stress and in ternal factors and varies from hormones to iron chelators. This review is dedicated to the molecular mechanisms of HIF1 activation, some of its natural activators HIF1, the potential for which is due to the low level of toxicity and the reduced likelihood of undesirable side effects. In turn, this opens up new options to treat diseases associated with local and general ischemia and hypoxia, the possibilities of their prophylactic use for researchers and clinicians in order to reduce the degree of damage in the event of an unforeseen condition of acute injurious to organs and tissues by hypoxia and reperfusion after it.

    Abstract: Nowadays there is no uniform classification and methods of diagnosis of the dysplasia presence in the examined human connective tissue, however it is known that all diseases are depending especially on the severity of the connective tissue disorders. By itself, the connective tissue dysplasia, affecting all systems and organs in the process of embryogenesis, is manifested mainly in the bodies of one of the systems. So far, the cardiovascular system has been studied the most fully. It is considered as an obligatory element of adaptation to the environment, testing the most expressed load at any activity and pathology. The aim of the study was to determine peculiarities of morpho-functional parameters of a healthy person activity with a minimum of the severity of the symptoms of cardiovascular system connective tissue dysplasia, and also to determine quantitative criteria for the evaluation of DST (6 tables, bibliography: 17 refs).
