    Notes: This study examined the ECG traces of 397 deaf children (age 12.5 ± 2.9 years, range 6–19 years), after exclusion of cases with Jervell and Lange-Nielsen syndrome (JLNS), and compared them to those of 361 normal hearing counterparts (age 12.5 ± 2.7 years; range 7–18 years). An observer, who was unaware of the hearing status of the subjects, measured QT and QTc intervals and calculated dispersions of QT and QTc from standard 12-lead ECGs recorded at a speed of 25 mm/s at rest. Although the mean QT was found to be longer in deaf children than that observed in the control group (P 〈 0.0001), the mean QTc was significantly shorter (P 〈 0.0001). The mean heart rate was significantly lower in deaf children. When QT and QTc data were recompared after the children were grouped according to the heart rate, the observed difference became less significant or disappeared. In conclusion, there are no major abnormalities for repolarization parameters in children with congenital sensorineural deafness, when compared to hearing counterparts, if heart rates are similar. Based on these results, routine ECG screening of deaf children for repolarization abnormalities may be unnecessary unless they have a history of syncope or positive family history of syncope and/or early sudden death.

    Notes: Paroxysmal atrial fibrillation (PAF) can be initiated by ectopic activation from the crista terminalis. The crista terminalis conduction gap is also a critical isthmus in atrial reentrant arrhythmias like upper and lower loop reentry. The aim of this study was to investigate the mechanism and results of catheter ablation for complex atrial arrhythmias originating from the crista terminalis using the noncontact mapping system (NCM). The study population consisted of six patients (5 men, 1 woman; 70 ± 9 years) with drug refractory PAF and typical/atypical atrial flutter. NCM identified the earliest ectopic activation originating from the crista terminalis in these six patients. The reentry circuit of atypical atrial flutter propagated around the upper crista terminalis in five patients, and lower crista terminalis in one patient. The reentry circuit of atypical atrial flutter and the initial reentry circuit of AF conducted through the crista terminalis gap in all patients. Radiofrequency applications were delivered on the sites of ectopy, which initiated AF. Substrate modification was also performed over the crista terminalis gap (six patients) and cavotricuspid isthmus (three patients) responsible for the reentry. During a mean follow-up of 9 ± 5 months (range 5–18 months), five patients were free of AF without antiarrhythmic drugs, and one patient did not have AF or atrial flutter using propafenone. NCM demonstrated the mechanism of crista terminalis ectopy-initiating AF and associated typical/atypical atrial flutter. Catheter ablation of crista terminalis ectopy and substrate for the reentry guided by NCM successfully eliminated these atrial arrhythmias.

    Notes: The upper pulmonary veins (PVs) are responsible for the majority of atrial fibrillation (AF) triggering foci, whereas the inferior PVs are more difficult to ablate and prone to postablation ostial stenosis. Most procedure failures can be attributed to incomplete isolation or recurrent PV left atrial reconnection rather than to identification of another focus. Furthermore, in certain patients AF triggers can be detected outside the PVs, and local denervation of the ganglionic plexus around the superior PV left atrial junctions following the ablation procedure may also play a role in eliminating AF. Based on these data, the authors propose that in AF patients the superior PVs should be ablated first, and in case of recurrence a second procedure should be performed for identification of PV left atrial reconnection or extrapulmonary foci and additional ablation of the inferior PVs. Such a staged approach might offer slightly lower success rates but with a significantly lower radiation exposure and procedural time and at a smaller risk of ablation induced PV stenosis.     Notes: A worldwide cardiac pacing and ICD survey was undertaken for calendar year 2001. Fifty countries, 22 from Europe, 16 from the Asia Pacific region, 3 from the Middle East and Africa, and 9 from the Americas contributed to the survey. The United States had by far the largest number of cardiac pacemaker implants, although Germany had the highest new implants per million population. Virtually all countries that participated in the 1997 survey showed significant increases in implant numbers over the 4 years. High degree atrioventricular block and sick sinus syndrome remain the major indications for implantation of a cardiac pacemaker with 〈 2% biventricular pacing in those countries that implanted such systems in 2001. There remains a high percentage of VVIR pacing in the developing countries with only a few countries using substantial numbers of single lead VDD and AAIR systems. There has been an increase in the use of DDDR systems in most countries since the 1997 survey. Pacing leads were predominantly transvenous, bipolar, and passive fixation. There was an increased use of active-fixation leads in the atrium. There was a significant rise in the use of ICDs with the largest usage occurring in the United States. A group of enthusiastic survey coordinators has now been established. Recruitment of new countries will hopefully continue to obtain a fully global experience of cardiac pacing and ICD usage.     Notes: The presence of a persistent left superior vena cava was encountered in a 64-year-old man undergoing implantation of a biventricular pacing system. Leads with active fixation were positioned in the right atrium and right ventricle, through the persistent left superior vena cava and the proximal segment of the coronary sinus. For left ventricular pacing, a standard bipolar lead was positioned directly in the posterior branch of the coronary sinus without the use of special guiding catheters. (PACE 2004; 27:1014–1016)

    Notes: Cardiac stimulation from right ventricular apical or free-wall lead positions alters inter- and intraventricular impulse conduction and distorts biventricular contractility. This may contribute to eventual cellular remodeling and the development of histopathological changes which, over time, adversely affect left ventricular systolic and diastolic functions. This concept has especially important implications when pacemaker therapy is initiation in young patients. Recent studies demonstrating physiological benefits of right ventricular septal, outflow, or bundle of His pacing, in deference to the apical implant site, have gained interest to potentially prevent dysfunction and improve paced myocardial contractility. Pacing initiated in children can be expected to have more far-reaching consequences than pacing initiated in the elderly. Unfortunately, there have been limited clinical pediatric studies that evaluate precise site-specific lead locations. This current report presents a review of pacemaker applications in children, both with and without structural congenital heart defects, including the earliest applications in which patient survival was the prime concern, to more recent studies attempting to optimize physiological and histological parameters associated with pacemaker induced contractility. The past decade has seen direct evidence that right ventricular apical pacing in children contributes to adverse histological remodeling and eventual contractile dysfunction. More recent studies demonstrate that selective site pacing can be effectively applied to all children with and without structural congenital defects and shows promise in the prevention of previously documented adverse remodeling and deterioration of systemic ventricular contractility.     Notes: Lead fracture due to twiddler's syndrome, was detected in a 68-year-old patient 1 month after implantation of an ICD by means of the incorporated home monitoring system. The patient was admitted and the lead replaced. This case illustrates the clinical benefit of the home monitoring system.
