Zusammenfassung
Die Entdeckung des Penicillins war einer der größten Fortschritte in der Medizingeschichte. Diese Errungenschaft wird durch die zunehmende Resistenzentwicklung bei bakteriellen Erregern bedroht. Während die Zunahme multiresistenter grampositiver Erreger wie MRSA und VRE gebremst werden konnte, gibt es eine deutliche Zunahme der Multiresistenz bei den gramnegativen Erregern. Da gramnegative Erreger bei Infektionen des Magen-Darm-Trakts einen großen Teil des Erregerspektrums ausmachen, wird die Wahl des Antibiotikums bei der empirischen Therapie schwieriger. Die mikrobiologische Diagnostik wird trotz der häufig schwierigen Zugangswege zur Gewinnung des Probenmaterials immer wichtiger. Risikofaktoren für eine Infektion mit multiresistenten Erregern sind nosokomiale Infektionen oder in Zusammenhang mit der Gesundheitsversorgung entstandene Infektionen sowie eine Antibiotikavortherapie. Diese Faktoren müssen bei der Wahl der empirischen antibiotischen Therapie in Betracht gezogen werden.
Abstract
The discovery of penicillin was one of the most important milestones in medicine. This achievement is jeopardized due to the increase of multiresistant bacteria. Whereas the increase of multiresistant gram-positive pathogens, such as methicillin-resistant Staphylococcus aureus (MRSA) and vancomycin-resistant enterococci (VRE) has been slowed down, the rate of multiresistant gram-negative bacteria has significantly increased. Because gram-negative pathogens play an important role in gastrointestinal infections, the selection of the correct empirical antibiotic therapy is becoming increasingly more difficult. Despite the fact that access to test sample material is often difficult and complicated, microbiological analysis is becoming even more important. Risk factors for infections with multiresistant bacteria are healthcare-associated or nosocomial infections and the prior use of antibiotics. These factors have to be taken into account when considering empirical antibiotic treatment.
