    Description / Table of Contents: Zusammenfassung 1 Std nach einem generalisierten tonisch-klonischen Krampfanfall infolge hyperbarer Sauerstoffexposition wurden bei der Ratte Nervenzellveränderungen in Gehirn und Rückenmark festgestellt. Zu den für die Sauerstoffintoxikation typischen Prädilektionsstellen gehören die Formatio reticularis, der Colliculus inferior, der Nucleus cochlearis und das Corpus mamillare. Bei diesen Schädigungen handelt es sich weder um Auswirkungen der gleichzeitig beobachteten, z. T. Schweren Atemstörungen noch um Krampffolgen, sondern mit großer Wahrscheinlichkeit um morphologische Äquivalente der Zellstoffwechselstörung, die durch die experimentelle Hyperoxie hervorgerufen wurde. Die besondere Lokalisation dieser Befunde läßt einen gewissen RÜckschluß auf den Entstehungsmechanismus der zentralnervösen und pulmonalen Intoxikationserscheinungen zu, soweit sie zentral ausgelöst sind. In Korrelation zu neurophysiologischen Untersuchungen kommt daher der Formatio reticularis und insbesondere der unteren Vierhügelgruppe bei der Entstehung des Sauerstoffkrampfes (Paul Bert-Effekt) große Bedeutung zu.
    Notes: Summary Histological alterations in the nerve cells of the rat's brain and spinal cord were observed one hour after a generalized tonoclonic convulsion brought on by hyperbaric oxygen exposure. To the typical predilection sites for changes induced by oxygen at high pressure belong the formatio reticularis, the colliculus inferior, the nucleus cochlearis and the corpus mamillare. These lesions are not the effects of the in part severe breathing disturbances simultaneously observed, nor are they the result of convulsion, but are with great probability morphologically visible signs of the disturbance of cellular metabolism brought about by the experimental hyperoxia. The particular localization of these findings allows a certain a priori conclusion to be drawn as to the developmental mechanism of the central nervous and pulmonary symptoms of intoxication—so far as these are induced centrally. In regarding the developmental process of the oxygen convulsion (Paul Bert-effect) great importance must be attached to the formatio reticularis and in particular to the lower quadrigeminal group in correlation with neurophysiological examinations.

    Description / Table of Contents: Zusammenfassung Bei speziellen Indikationen und Notfallsituationen der Dickdarm-Chirurgie ist eine Diagnostik mit bariumsulfathaltigen Kontrastmitteln nicht möglich. Hier können mit gutem Erfolg ersatzweise trijodierte, wasserlösliche Kontrastmittel Verwendung finden. Sie werden auch bei Perforationen und Austritt aus dem Darm in die freie Bauchhöhle resorbiert und ermöglichen Diagnosen mit mäßig eingeschränkter Qualität gegenüber bariumsulfathaltigen Kontrastmitteln. Bei Doppelkontrastuntersuchungen in Anlehnung an die Malmö-Technik ist die Oberflächendetailerkennbarkeit als ausreichend zu bezeichnen. Vor- und Nachteile sowie Indikationen sind in Tabellenform aufgelistet. Beispieldokumentation vervollständigen das Ergebniskapitel.
    Notes: Summary When surgery of the colon must be carried out for special indications and emergencies, it is not possible to make diagnosis with Ba SO4 contrast mediums. In these cases, reasonable success can be expected with substitution of three-iodized water-soluble contrast enemas. Through perforations and discharge from the colon they are absorbed into the free peritoneal cavity and enable diagnoses which are nearly comparable in quality to those done with Ba SO4 contrast mediums. In double contrast radiography according to the Malmö technique recognition of surface detail can be considered sufficient. Advantages and disadvantages as well as indications are listed in table form. Examples are documented to complete the chapter on results.

    Description / Table of Contents: Zusammenfassung Bei 91 mit der EEA-Nahtpistole erstellten und prospektiv analysierten Colon- und Rectumanastomosen (darunter 71 Carcinomträger, $$\bar x = 71$$ Jahre alt, 16 Diverticulitiden, $$\bar x = 55$$ Jahre alt) ergaben sich folgende Resultate: 4% intraoperative Anastomosendehiscenzen wurden sofort übernäht. Eine Blutung konnte sofort operativ durch eine erneute EEA-Anastomose behoben werden. Wundheilungsstörungen (Abscesse, Serome) ergaben sich in 11%. Röntgenologisch nachweisbare Nahtdehiscenzen und Fisteln in 18 % hatten nur in 7% ein klinisches Korrelat. Patienten mit röntgenologisch nachweisbaren relativen Stenosen (11%) wiesen nur in 1% temporär klinische Symptome auf. Durch operativ technisch bedingte Komplikationen verloren wir keinen Patienten. Besonders geeignet ist die Nahtpistole für Resektionen mit einer Anastomosenhöhe von 3–5cm.
    Notes: Summary The following results were obtained in 91 colon and rectum anastomoses (71 cases of carcinoma, $$\bar x = 71$$ years, 16 cases of diverticulitis, $$\bar x = 55$$ years). During the operation 4% anastomotic dehiscences occurred, which were immediately oversewed. One case of anastomotic bleeding could be remedied immediately by renewed anastomosis. Healing was impaired (abscesses, seromas) in 11%. Suture dehsicences were demonstrable by X-ray on the 8th–12th day in 18%, but only in 7% was there a clinical correlation. Patients with relative stenoses that were demonstrable by X-ray (11%) showed temporary clinical symptoms in only 1%. We lost no patients as a result of operative complications. From the standpoint of manageability, the EEA stapler is particularly suitable for low resections with an anastomosis in the lowest part of the rectum.
