Résumé Les différences considérables des contributions entre les caissesmaladie sont un obstacle au bon fonctionnement de la politique sociale. Des interprétations trop rapides ont fréquemment été déduites de ces différences. Ces interprétations sont le plus souvent infirmées par l'examen approfondi des informations provenant des rapports de gestion des caisses. En réalité, il y a de nombreux déterminants au niveau des contributions qui se situent aussi bien au sein même des caisses-maladie qu'en dehors de celles-ci. Afin d'estimer l'impact de chacun de ces déterminants, il convient de décrire les charges en fonction de l'âge et du sexe de chaque assuré (affilié direct ou membre dépendant de sa famille). L'identification ultérieure des composantes du coût, du volume, et des structures permet de mieux connaître les causes de l'augmentation des dépenses pour la santé. Il est ainsi possible d'influencer de manière plus spécifique la politique des caisses-maladie à l'égard des coûts. En plus, l'amélioration de l'information obtenue pour les caisses-maladie devrait constituer une contribution importante pour le fonctionnement du système de santé en général.
Abstract:
Summary Contribution rates to the German statutory health insurance differ considerably between different sickness funds. While insurance benefits are largely standardized, these differences are widely regarded as a problem for the state's social policy. The health insurance bodies often offer hasty interpretations to justify the differences of contribution rates. These interpretations, however, cannot be checked on the basis of data from the annual reports of the health insurance. In reality, there is a number of determinants of the contribution rate which lie as well inside as outside the sphere of influence of the health insurance bodies. In order to estimate the respective impact of single determinants it is important to differentiate the health expenditures by age and sex of all insured persons (both the contribution paying members and their co-insured dependants). A further disentanglement of health expenditures into prices, quantities and structural components allows to identify the reasons of increases in health costs. This is also a necessary precondition for creating and implementing suitable measures that are directed to control health costs. At the same time, the improved information system of the statutory health insurance would contribute to a better understanding of the performance of the health system in general.

Zusammenfassung Die teilweise beträchtlichen Beitragssatzdifferenzen zwischen den Krankenkassen gelten bei einheitlichem Leistungsrecht als sozialpolitisch problematisch. Zur Begründung unterschiedlicher Beitragssätze werden von den betroffenen Kassen oft vorschnelle Interpretationen angeboten, die sich zudem Mithilfe der Informationen aus den Geschäftsberichten der Kassen häufig nicht nachprüfen lassen. In Wirklichkeit gibt es eine Vielzahl von Beitragssatzdeterminanten, die sowohl innerhalb als auch ausserhalb des unmittelbaren Einflussbereichs der Kassen liegen. Für eine Abschätzung der jeweiligen Bedeutung dieser Determinanten ist es erforderlich, insbesondere die Ausgaben in einzelnen Leistungsbereichen künftig differenziert nach Alter und Geschlecht der Versicherten (Mitglieder und Familienangehörige) auszuweisen. Die weitere Isolierung von Preis-,Mengen- und Strukturkomponenten ermöglicht es, die Ursachen von Ausgabensteigerungen zu identifizieren und gegebenenfalls durch gezielte Kassenpolitiken zu beeinflussen. Eine derart verbesserte Berichterstattung der Kassen würde zugleich einen wichtigen Beitrag zur allgemeinen Gesundheitsberichterstattung leisten.
