    Notes: Evidence-based diagnosis and treatment have been proposed recently in various medical fields. Evidence-based diagnostic and therapeutic guidelines for benign prostatic hypertrophy (BPH), one of the most common urological diseases, have been proposed in foreign countries. This paper examines common therapeutic drugs for BPH in Japan from the viewpoint of evidence-based medicine (EBM).The term ‘BPH’ and drugs indicated for BPH were used as key words to search related articles in the PubMed website. A total of 813 articles extracted as of October 2001 were examined. The articles were ranked in levels ranging from I–V, where Level I indicated a large randomized controlled trial and Level V indicated a non-controlled case accumulation study.Among the 813 articles extracted, 132 clinical articles were suitable for evaluation. There were many reliable articles on the effectiveness of α-blockers. However, it seemed necessary to examine further how to choose the optimum α-blocker for each clinical case and the combination of antiandrogen drugs with α-blockers. It was also considered necessary to evaluate the effectiveness of drugs, such as eviprostat and hachimi-jio-gan, that are available only in Japan.The present study evaluated the effectiveness of commonly available therapeutic drugs for BPH in Japan from a viewpoint of EBM.

    Notes: Abstract  Left radical nephrectomy was performed on a 39-year-old-man because of renal cell carcinoma (grade 1, clear cell and granular cell carcinoma: pT3b pN0 pM0), 6 years after the beginning of hemodialysis. The second surgical intervention for local recurrence was performed 6 years after the first operation (grade 2 〉 3, clear cell and granular cell carcinoma).This is the second published case report of the resection of a local recurrence of renal cell carcinoma after nephrectomy in a hemodialysis patient. Even after the curative operation, follow up for local recurrence should be continued in hemodialysis patients, especially for high stage renal cell carcinoma.

    Notes: Abstract In Workshop 8 at the International Conference on Interstitial Cystitis (Kyoto March 2003, future research needs for the definition/diagnosis of interstitial cystitis (IC) were discussed by the participants. To structure discussions a study proposal was used. This workshop decided that research should focus on identifying and validating classification criteria for the diagnosis of IC. This might well be done in a (multi-centre) study with a control group of urge-frequency patients without pain (OAB?). However, before doing so standards should be defined as on how to perform and interpret vaginal examination, urodynamic investigation, cystoscopy under anesthesia, potassium sensitivity test, and histopathology in these patients.

    Notes: Abstract Aim: In the present study, we evaluated the relationships between prostate-specific antigen (PSA) level and bone metastasis, between Gleason sum and bone metastasis, and between clinical T stage and bone metastasis in Japanese patients. Methods: Between November 1998 and June 2004, we performed ultrasound-guided biopsies on 709 patients (mean age: 70.5 years, range: 39–90). Prostate cancer was detected in 339 patients (47.8%), 297 (87.6%) of whom underwent a radionuclide bone scan. In close collaboration with orthopedists, bone computed tomography scans, bone magnetic resonance imaging and/or plain rentogenograms were performed for cases that were difficult to diagnose as bone metastasis through radionuclide bone scans only. Results: We detected 61 (20.6%) bone metastasis cases in 296 patients. A simple linear regression analysis between log[PSA] and bone metastasis (n = 296) produced a significant relationship (P 〈 0.05). When we set the cut-off PSA value for the indication for a bone scan at 15 ng/mL, the possibility of bone metastasis was 10%. However, from our experience, there was no bone metastasis in the patients whose Gleason sums were less than five, and in the patients whose Gleason sum were five or more, and the PSA levels were less than 15, there was no bone metastasis. The rate of bone metastasis increased with the increase of PSA level. In the clinical T1–T2 stage cases, there were significant higher PSA levels in the cases with bone metastasis. In the T1–T2 patients whose PSA levels were less than 16, there was no bone metastasis. Conclusions: From the analysis of PSA, Gleason sum and clinical T stage, we suggest that bone scan is unnecessary for patients whose PSA level is less than 15 ng/mL or Gleason sum is less than five.

    Notes: SUMMARY:  A 57-year-old man was admitted to hospital because of motor aphasia on 16 September 2000. He had an approximate 2-year history of haemodialysis treatment, since April 1997, and had received a cadaveric renal transplantation in July 1999. Computed tomography and magnetic resonance imaging (MRI) revealed a mass with ringed enhancement in the left temporal-parietal lobe. Primary central nervous system (CNS) lymphoma (diffuse large B cell type) was diagnosed based on an open brain tumour biopsy. Epstein–Barr virus early RNA (EBER) in situ hybridization showed positive signals in the nuclei of the CNS lymphoma cells. A dose reduction of immunosuppressant and three series of high-dose methotrexate (MTX) therapy with leucovorine rescue followed by irradiation (whole brain irradiation, 30 Gy; partial brain tumour irradiation, 20 Gy) were carried out for his primary CNS lymphoma. The patient is currently in good condition (September 2004) with no enlargement of the lymphoma, as examined by MRI every 3 months, and preserved renal function.

    Notes: The Izu–Ogasawara arc contains, from east to west, a volcanic front, a back-arc extensional zone (back-arc knolls zone), and a series of across-arc seamount chains that cross the extensional zone in an east-northeast and west-southwest direction and extend into the Shikoku Basin. K–Ar ages of dredged volcanic rocks from these across-arc seamount chains and extension-related edifices in the back-arc region of the Izu–Ogasawara arc were measured to constrain the volcanic and tectonic history of the arc since the termination of spreading in the Shikoku Basin. K–Ar ages range between 12.5 and 1 Ma. Andesitic to dacitic rocks of 12.5–2.9 Ma occur mainly on the western part of the chains. The western part of the chains are the locus of volcanism behind the front which erupted mainly calc-alkaline andesitic lavas. The youngest rocks (〈 2.8 Ma), characterized by cpx-ol basalt, occur along the western margin of the back-arc knolls zone. Basaltic rocks of 12.5–2.9 Ma have relatively high concentrations of Na2O (〉 2.0 wt%), Zr (〉 50 p.p.m.) and Y (〉 20 p.p.m.) and low CaO (〈 12 wt%). On the other hand, basalts of 2.8–1 Ma have lower Na2O (〈 1.8 wt%), Zr (〈 50 p.p.m.) and Y (〈 20 p.p.m.), but significantly higher CaO (〉 12 wt%). The age inferred for the initiation of back-arc rifting (∼ 2.35–2.9 Ma: Taylor 1992) behind the current volcanic arc coincides with the time that basalt chemistry changed drastically (eruption of the low-Na2O and high-CaO basalt). This implies that post-2.8 Ma volcanism in the back-arc knolls zone is associated with rifting. Similarly, the change in chemical composition might be explained by a different type of source mantle following rift initiation. Volcanism in the western seamounts ceased after the onset of rifting at ∼ 2.8 Ma.

    Notes: Abstract To study possible differences in α1-adrenoceptor involvement in the spinal mechanisms mediating bladder activity induced by volume (bladder filling), central (L-dopa), and peripheral (capsaicin) stimulation, we investigated if these types of bladder activity were modified by intrathecal (i.t.) or intra-arterial (i.a.) administration of the α1-adrenoceptor antagonist, indoramin. Indoramin is selective for the α1A-adrenoceptor subtype, whereas most clinically used α1-adrenoceptor antagonists, including doxazosin, have no subtype selectivity. The drug effects were studied by continuous cystometry in normal, conscious rats and rats with bladder activity evoked by intraperitoneal L-dopa (50 mg/kg after carbidopa pretreatment), or by intravesical capsaicin (30 µM). I.t. indoramin (50 nmol) significantly decreased micturition pressure, and increased bladder capacity and micturition volume. Dribbling incontinence due to urinary retention was observed in one of ten rats. L-dopa-stimulated bladder overactivity was significantly attenuated by i.t. or i.a. indoramin (50 nmol). Similar effects of i.t. and i.a. doxazosin (50 nmol) have been reported previously. Intravesical capsaicin (30 µM) caused bladder activity, which was attenuated by i.t. indoramin (50 nmol), but not by i.t. doxazosin (50 nmol). I.a. indoramin did not reduce capsaicin-induced bladder activity; doxazosin was moderately effective. The results suggest that the bulbospinal micturition reflex evoked by bladder filling and L-dopa involves a descending pathway where transmission is partly mediated by spinal α1-adrenoceptors. Bladder overactivity evoked by intravesical capsaicin, which elicits a vesico-spinal-vesical reflex, was not affected by i.t. doxazosin in a dose that attenuates activity mediated through the bulbo-spinal pathway. This suggests less involvement of spinal α1-adrenoceptors in the vesico-spinal-vesical than in the bulbo-spinal voiding reflex.

    Notes: Diallyl quaternary ammonium chlorides, bromides and N-alkyldiallylamine hydrochlorides were polymerized with ammonium persulfate (APS) in dimethyl sulfoxide (DMSO). The dependences of yield and molecular weight of polymers on polymerization conditions were examined and quaternary ammonium chlorides were found to have better polymerizability than bromides. The poly(diallyl quaternary ammonium chlorides) obtained with APS - DMSO system are expected to have quite high molecular weights, as determined from the measurement of limiting viscosity numbers of the polymers in NaCl aqueous solution.
