    Abstract: In Germany, approximately three million children under the age of eighteen have a mentally ill parent. These children are at an increased risk of developing a mental illness themselves (1) as well as a physical illness (2). While research has identified numerous evidence-based family-oriented interventions, little is known about how to implement such interventions effectively and efficiently in clinical practice in Germany. This implementation study (ci-chimps) evaluates three clinical implementation projects with three different implementation interventions for the optimal implementation of the tailored family-oriented preventive and therapeutic interventions in the CHIMPS-NET (children of mentally ill parents—research network) with an implementation model for children of mentally ill parents. Methods A two-group randomized controlled multicenter trial will examine changes in family-oriented practice and aspects of implementation at baseline as well as at 12- and 24-months follow-up. The CHIMPS-Network consists of 20 clinical centers. The centers in the intervention group receive the support of all of the three implementation interventions: (1) optimal pathways to care, (2) education and a training program for professionals, and (3) systematic screening for children. The centers in the control group do not receive this specific implementation support. Discussion While we know that children of mentally ill parents are an important target group to be addressed by preventive and therapeutic interventions, there is often a lack of structured implementation of family-oriented interventions in clinical practice in Germany. Using a randomized controlled multicenter trial design with a large and wide-ranging sample (clinics for adult psychiatry and clinics for child and adolescent psychiatry, university clinics and clinics at the real health care) will provide a robust understanding of implementing family-oriented changes in German clinical practice. Trial Registration The CHIMPS-NET-study was registered with the German Clinical Trials Register on 2019-12-19 (DRKS00020380) and with Clinical Trials on 2020-4-30 (NCT04369625), the ci-chimps-study was registered with the German Clinical Trials Register (DRKS00026217) on 2021-08-27, the Clinical Trials registration is in review process.

    Abstract: Previous research has supported the utility of the Double ABCX model of family adaptation for parents in various diseases. Nonetheless, it remains unclear how raising a child with rare congenital surgical diseases impacts the mental health of both mothers and fathers. Methods The potential predictors of maternal and paternal mental health in a German sample of 210 parents of children with rare congenital surgical diseases were investigated. Parents were investigated cross-sectionally utilizing standardized psychometric questionnaires that assessed factors attributed to parental adaptation within the Double ABCX model. Results Stressor pile-up, family functioning, perceived stress, and mental health were positively associated with mothers and fathers. However, further analyses revealed that family functioning, social support, and perceived stress fully mediated the positive association between stressor pile-up and mental health in mothers, but not fathers. Conclusion Our findings suggest that parental adaptation to a rare congenital surgical disease in their children may be improved by increased intra- and extrafamilial resources and decreased perceived family-related stress in mothers, but not fathers. Our results may help to identify gender-specific factors that may guide clinicians and future interventions.

    Abstract: Hintergrund: Eine gelungene, angemessene Krankheitsbewältigung psychisch erkrankter Eltern wird als Resilienzfaktor für eine gesunde Entwicklung der Kinder betrachtet. Methode: In der vorliegenden Studie werden in einer Stichprobe von N = 53 Familien die Veränderungen in der Krankheitsbewältigung von psychisch kranken Eltern vor und nach ihrer Teilnahme an der manualisierten CHIMPs-Intervention im Vergleich zu einer Wartelistenkontrollgruppe, die Zusammenhänge zwischen ihrer Krankheitsbewältigung und der Gesundheit ihrer Kinder sowie die Zusammenhänge zwischen ihrer veränderten Krankheitsbewältigung und einer veränderten Gesundheit ihrer Kinder untersucht. Die Krankheitsbewältigung der Eltern wird mit dem Freiburger Fragebogen für Krankheitsverarbeitung (FKV-LIS, Muthny, 1989 ), die psychischen Auffälligkeiten der Kinder mit der Child Behavior Checklist (CBCL, Achenbach, 1991 ) gemessen. Ergebnisse. Im Ergebnis werden zwei elterliche Krankheitsbewältigungsstrategien (Ablenkung und Selbstaufbau, Bagatellisieren und Wunschdenken) positiv im Verlauf der Intervention beeinflusst. Zwei elterliche Krankheitsbewältigungsstrategien (aktives, problemorientiertes Coping und Religiosität und Sinnsuche) weisen Zusammenhänge zur Gesundheit der Kinder auf. Die Ergebnisse können als erste Hinweise für die Wirksamkeit der CHIMPs Intervention im Konstrukt der Krankheitsbewältigung gelten.

    Abstract: Parental mental illness can be linked to reduced family functioning, which is associated with more conflicts, less adaptability and cohesion as well as a disorganized pattern of everyday planning. Concurrently, family functioning is an important moderator for the influence of parental mental disorders on the development of the children. Consequently, the current study addresses the correlation of family functioning in families with mentally ill parents and the psychological health of the children. The sample consists of 67 mentally ill parents. Both parents and therapists completed questionnaires related to family functioning and the psychological health of the children. Family functioning was rated as dysfunctional in 38% of the families. The psychological health of the children was classified as clinical or subclinical in 43% of the cases. 52% of the children were rated to have no psychological problems. In families with good family functioning, children were assessed to have less psychological problems than in families with poor functioning. Children outside the clinical range lived in families with good family functioning and vice versa. Significant positive correlations were found between the FB-A scales, the CBCL/4-18 syndrome scales and the CBCL/4–18 total score. Results indicate that family functioning and psychological health of children in families with mentally ill parents correlate closely and represent potential targets for future family interventions.

    Abstract: Parents caring for children with rare diseases fear the long-term progression of the child’s disease. The current study aims to systematically investigate the quality of life (QoL) in parents of children with different rare diseases. We performed a systematic literature search including quantitative studies on QoL of parents caring for children and adolescents with rare diseases in five databases (APA PsycArticles, APA PsycInfo, MEDLINE, PSYNDEXplus, and PubMed) published between 2000–2020. Of the 3985 titles identified, 31 studies met the inclusion criteria and were selected for narrative review. Studies were included if they investigated predictors of parental QoL or reported QoL compared to normative samples, parents of healthy children, or children with other chronic diseases. We used the Newcastle–Ottawa Scale to assess methodological quality. The systematic review revealed that parents of children with rare diseases experience reduced QoL compared to parents with healthy children and norm values. Psychosocial factors, beyond disease-specific predictors, were shown to influence parental QoL substantially and may thus present an essential aspect within interventions for this highly burdened group. Health care professionals should consider and address the impairment of parental QoL due to the child’s rare disease. We discuss insights into existing research gaps and improvements for subsequent work.

    Abstract: Advances in genetic and pharmaceutical technology and pediatric care have enabled treatment options for an increasing number of rare diseases in affected children. However, as current treatment options are primarily of palliative nature, the Health-Related Quality of Life (HRQoL) and mental health of this impaired population and their siblings are of increasing importance. Among children and adolescents with rare diseases, those who are technology-dependent carry a high disease burden and are selected as the target population in our study. In a cross-sectional observational design, the children’s HRQoL was assessed with the DISABKIDS (DCGM-37) as well as KIDSCREEN-27, while mental health was assessed with the Strengths and Difficulties Questionnaire (SDQ) by both the affected children, their parents, and siblings. Results Results of the study sample were compared to normative data. Affected children scored significantly lower than the norm on almost all HRQoL subscales as reported by parent and child. From the parental perspective, more mental health subscales were significantly impaired compared to the child’s perspective. Siblings showed no impairment in HRQoL as well as significantly fewer behavioral problems and higher prosocial behavior regarding their mental health compared to the norm. Conclusion Children and adolescents with rare diseases seem particularly impaired in social and emotional aspects of HRQoL and mental health. Interventions may focus primarily on promoting social skills, fostering prosocial behavior and peer relationships.

    Abstract: Eine psychische Erkrankung der Eltern kann Beeinträchtigungen im Wohlergehen ihrer Kinder zur Folge haben, die mit einer schlechteren gesundheitsbezogenen Lebensqualität einhergehen. Aktuelle Studien belegen eine schlechtere gesundheitsbezogene Lebensqualität dieser Kinder als die der Normalbevölkerung. Etwa ein Drittel der Familien mit psychisch kranken Eltern erleben auch eine mangelhafte soziale Unterstützung. Im Projekt CHIMPs (Children of mentally ill parents) wurde ein Interventionsprogramm entwickelt und manualisiert, das unter anderem auf die Verbesserung der gesundheitsbezogenen Lebensqualität der Kinder und eine verbesserte soziale Unterstützung abzielt. Die Befunde dieser Studie (n=53 Interventionsgruppe; n=14 Wartelistenkontrollgruppe) belegen Verbesserungen der gesundheitsbezogenen Lebensqualität und der sozialen Unterstützung nach der Intervention im Vergleich zu einer Wartelistenkontrollgruppe, die über ein Jahr stabil bleiben. Mit diesen Befunden liegen im deutschsprachigen Raum erstmals Evaluationsergebnisse vor, die die Angemessenheit gezielter Interventionen für diese Kinder belegen.

    Abstract: Das Interventionsprogramm CHIMPs wird im zentralen Zielkriterium der psychischen Gesundheit der Kinder evaluiert. Die psychischen Auffälligkeiten der Kinder aus Elternsicht werden mit der Child Behavior Checklist (CBCL/4 – 18) von Achenbach (1991) , aus Sicht der Kinder mit dem Youth Self Report (YSR) und ihre psychische Beeinträchtigung aus Therapeutensicht mit dem Beeinträchtigungs-Schwere-Score für Kinder (BSS-K) erfasst. Die psychischen Auffälligkeiten der Kinder in allen drei übergeordneten Skalen der CBCL verringern sich aus der Patientenperspektive (N=43) und bleiben stabil im Katamnesezeitraum. In Gesamtauffälligkeit und internalisierenden Auffälligkeiten berichten auch die Lebenspartner (N=37) eine Verbesserung der Kinder. Die Kinder (N=17) selbst nehmen weniger internalisierende Auffälligkeiten wahr. Die psychische Gesamtbeeinträchtigung verbessert sich auch aus Sicht der Therapeuten. Die Befunde können als Beleg für eine Verbesserung der psychischen Gesundheit der Kinder psychisch kranker Eltern nach der CHIMPs-Intervention gelten.

    Abstract: Background: Children of mentally ill parents have a three to seven times higher risk of developing mental disorders compared to the general population. For this high-risk group, specialized prevention and intervention programs have already been developed. However, there has been insufficient sytematic evaluation to date. Moreover, effectiveness and the cost-effectiveness data of the respective programs until today is very scarce and at the same time constitutes the pre-condition for the program's implementation into regular health care. Methods: The study consists of a two-group randomized controlled multicenter trial conducted at seven study sites throughout Germany and Switzerland. Participants are families with mentally ill parents and their children aged from 3 to 19 years. The intervention comprises 6 to 8 semi-structured sessions over a period of about 6 months. Topics discussed in the intervention include parental mental illness, coping, family relations and social support. Families in the control condition will receive treatment as usual. The children's mental health, assessed using the K-SADS-PL by blinded external raters will constitute the primary efficacy outcome. Further outcomes will be assessed from the parents' as well as from the children's perspectives. Participants are investigated at baseline, 6, 12, and 18 months after baseline assessment. In addition to the assessment of various psychosocial outcomes, a comprehensive health-economic evaluation will be performed. Discussion: This paper describes the evaluation of a family-based intervention program for children of mentally ill parents (CHIMPs) in the regular health care system in Germany and Switzerland. A methodically sophisticated study design has been developed to reflect the complexity of the actual health care situation. This trial will contribute to the regular health care for the high-risk group of children of mentally ill parents.     Abstract: Offspring of mentally ill parents is at heightened risk for psychological symptoms. The identification of environmental factors that predict their mental health is crucial for the development of preventive and therapeutic measures. In the current study, we addressed the combined role of family functioning and social support by taking mentally ill patients’, their partners’, and children’s perspectives into account. The cross-sectional sample included n =195 families (195 patients, 127 partners, and 295 children). Family members completed questionnaires related to family functioning, social support as well as parental and child psychopathology. We conducted multilevel analyses to investigate the associations with internalizing and externalizing problems in children. Family functioning and social support were significantly associated with child internalizing and externalizing problems. However, results varied depending on the rating perspective. We found significant interaction effects of family functioning and social support on child psychopathology. The findings point to the importance of family functioning and social support as potential targets for interventions. Findings should be replicated in future longitudinal studies.
