    Abstract: Background: With the increasing meta-analysis studies of noninvasive brain stimulation (NIBS) a major review has become a logical step to provide evidence to support decision-makers. Objective: Umbrella reviews of the Working-Group of NAPeN Network (a Brazilian scientific network for NIBS development) will summarize the results of existing evidence in meta-analysis with focus on NIBS techniques applied for clinical settings, exercise and sports science. Methods and materials:  Firstly, a screening was performed to identify meta-analysis in which NIBS were applied to neurological and psychiatric disorders and healthy subjects. A second literature search was conducted in the Pubmed using a PICO-question for each population and NIBS techniques found in the first search. Methodological quality and certainty of evidence will be evaluated using the AMSTAR 2 and GRADE framework, respectively. Partial results: After the first search, we found meta-analyses including repetitive transcranial magnetic stimulation and transcranial direct current stimulation, applied in populations with neurological (cerebral palsy, chronic pain, dementia, epilepsy, essential tremor, multiple sclerosis, Parkinson’s disease, tinnitus, tourette syndrome, and stroke) and psychiatric disorders (anxiety, attention-deficit hyperactivity, autism spectrum, obsessive-compulsive and post-traumatic stress disorders, schizophrenia, craving/addiction and depression), and in healthy subjects. A total of 118 meta-analyses will be included in the qualitative review. The results of evidence were identified in the outcomes of six umbrella reviews. Conclusion:  Evidence of therapeutic and nontherapeutic use of NIBS techniques will support experts to produce consensus statements and assist professionals in making decisions of incorporating or not NIBS into clinical practice.

    Abstract: We herein present twenty myotonic dystrophy of Steinert patients with the main objective to evaluate and classify the oropharyngeal swallowing by the phonoaudiological clinical and nasofibrolaryngoscopical analysis. The age of the patients varied from 12 to 53 years, being 13 male and 7 female. The mean data: (1) statistically significant relation between the phonoaudiological clinical evaluation and nasofibrolaryngoscopical one; (2) stomatognatical system disorders present in 100%; (3) swallowing disorders present in 95%, when clinically evaluated, and in 70% when evaluated by the nasofibrolaryngoscopy; (4) higher difficulty to swallow consistent feed; (5) stomatognatical muscles very altered, pharyngeal phase disorders, cough after swallowing, antecedents of pneumonia and complaints of chewing/swallowing presented statistically significant correlation with severity of the sickness. The analysis were able to evaluate statical and functionally the involved structures in the swallowing, having got to take part of the rotine of attendance to the patients with myotonic dystrophy of Steinert.

    Abstract: OBJETIVO: Verificar alteração da deglutição após acidente cerebrovascular isquêmico (AVCI). MÉTODO: Estudo retrospectivo de 596 prontuários. Critérios de inclusão: pacientes hospitalizados com diagnóstico de AVCI. Critérios de exclusão: pacientes com problemas cardíacos associados e com mais de 14 dias de internação. RESULTADOS: 50,5% homens e 49,5% mulheres, idade média 65,3 anos (DP=±11,7) (p 〈 0,001). Entre os fatores de risco, 79,4% hipertensão arterial, 36,7% diabetes (p 〈 0,001), 42,7% tabagismo. Óbito ocorreu em 13,3% dos pacientes. Alteração de deglutição ocorreu em 19,6%, sendo 91,5% com dificuldade leve e 8,5% com dificuldade grave; 87,1% tiveram recuperação espontânea com tempo médio de 2,4 meses. A lesão em região de tronco encefálico ocorreu em 6,8% (p 〈 0,001). CONCLUSÃO: A alteração da deglutição ocorreu em quase 20% da população e a dificuldade de deglutição mais encontrada foi a leve. Os fatores preditores para alteração de deglutição foram aumento da idade, diabetes mellitus e lesão em região de tronco encefálico.

    Abstract: Dysphagia, or difficulty swallowing, is a common problem following myasthenia gravis (MG) and may lead to aspiration of saliva, food or liquids. We herein present 22 MG patients, with complaint of dysphagia, evaluated by phonoaudiological evaluation, nasofibrolaryngoscopical analysis and manometry of upper esophageal sphincter. The main objective was to evaluate the phases of the swallowing process and anatomical and functional aspects of oropharyngeal musculature. The age of patients varied from 19 to 74 years; being 19 female and 3 male. The main data were: (1) statistically significant relation between the phonoaudiological clinical evaluation and nasofibrolaryngoscopical one; (2) stomatognatical system disorders present in 100%; (3) swallowing and chewing disorders present in 100%, when clinically evaluated, and in 81,8% when evaluated by the nasofibrolaryngoscopy; (4) statistically significant relation between penetration/aspiration and antecedents of pneumonia; (5) stomatognatical muscles very altered, difficulty controlling the bolus, collection of food in the pharynx or larynx and presence of penetration and/or aspiration showed significant correlation with weakness of a pharyngeal muscles. These findings justify the necessity to evaluate clinically the swallowing phases in MG patients, with the main purpose to prevent both aspiration and dehidration from inadequate oral intake.

    Abstract: Parkinson’s disease shows a higher incidence in the elderly population generating progressive motor impairment, which affects several functions, among which stands out swallowing. The purpose of this study is to correlate the dysphagia in Parkinson’s disease with immediate and / or late effect of the drug treatment, which directly or indirectly, will interfere with speech therapy management. We carried out a literature review in electronic databases Lilacs, Scielo, Medline and Pubmed from 2001 to 2011, using the free terms “Parkinson Disease”; (Parkinson Disease), “swallowing”; (deglutition), “dysphagia”; (dysphagia), “pharmaceutical preparations”; (pharmaceutical preparations), “levodopa”;, “videofluoroscopy”; (videofluoroscopy), a government document (OPAS, 2002), relevant articles and copies of American and Brazilian literature about the theme. The literature appointments levodopa as the main pharmacological treatment of the Parkinson’s disease. However, the resolution of the motor symptoms must be balanced in function of important collateral effects, being immediate or later. Actually, there aren’t consistent answers in favour of the resolution of the dysphagia in consequence of pharmacological treatment, wich effects may interfere, direct or indirectly, on the dysphagic manifestations and of several ways. In this way, turns fundamental the record of the medications like part of the anamnese, considering that, such data may help orientation/reorientation of the phonoaudiologic management, specially interdisciplinary context. In despite of possibility of Parkinson’s patient to answer inconsistently to pharmacological therapy, is noteworthy that professional must pay attention to presence of collateral effects like modifiers factors of the dysphagia oropharyngea profile in the idiopatic Parkinson’s disease.

    Abstract: The purpose of this study is to analyze tests and assessments tests of child language in order to discuss the applicability of the population with physical, sensory, mental and multiple disabilities. In the second half of 2011, was researched about the deficiencies and direct evaluations of children's oral, gestures and writing language, national or translated. Articles and theses were consulted in online databases, also books and evaluations published. Were selected 28 measurements, which were grouped by goals application, described as stimulus and expected achievement, and analyzed by following criteria: modality of evaluation, required skills and code conversion. There were found 23 evaluation forms, which analysis suggests that individual who are able to use vision, upper limb and mind, and who can understand and use image and oral or written Portuguese codes, will probably have greater range of evaluation that meets. The dimensions semantic and pragmatic appeared to be the most accessible, supporting the application found in the literature. On the possibility of complete evaluation, was observed that only a couple of skills "vision/upper limb" would allow this. A survey of information about the examinee's communication profile compared to the profile required to assess communicative aids in the decision on the compatibility of these and subsequent applicability. In general, considering the particularities of cases and assessment instruments pre-selected can so be applicable to individuals with disabilities. However, it may happen that, for some patients, cannot perform a complete evaluation instruments using only the direct type.

    Abstract: Objetivo. avaliar a fadiga, a Capacidade Vital Forçada (CVF) e as pressões respiratórias estáticas máximas dos pacientes com Esclerose Múltipla (EM) e verificar a existência de uma relação entre esses pa­râmetros e a queixa de fadiga. Método. foram avaliados 20 pacientes da Associação Brasileira de Esclerose Múltipla de ambos os sexos, com EM na forma remitente recorrente, Expanded Disability Status Scale (EDSS) até 6,0, não fumantes, sem diagnóstico de doença respiratória e sem história de surto nos últimos 30 dias. Foi realizada avaliação res­piratória e aplicada a escala de gravidade de fadiga (EGF) Resultados. não houve correlação da ESF com os parâmetros respiratórios, EDSS, idade e tempo de diagnóstico. Os parâmetros respiratórios estavam abaixo do previsto. As pressões estáticas máximas se correlacionaram com o EDSS e com a idade. Conclusão. a fadiga foi observada na maioria dos casos, predominando na forma severa. As alterações dos parâmetros respiratórios não apresentaram influência sobre a queixa de fadiga. O aumento da idade e do EDSS influenciam de forma ne­gativa as pressões estáticas máximas.

    Abstract: Background: The influence of changes in body composition on swallowing in patients with Amyotrophic Lateral Sclerosis (ALS) is unknown. Understanding the interrelation between body compartments and dysphagia may establish specific treatments related to both nutritional aspects as to myofunctional ones designed to delay swallowing loss. Aim: The aim of the study was to evaluate the relationship between body composition and dysphagia during the course of the disease. Methods: The protocol of this study included assessments carried out quarterly for one year and included: analysis of body composition by multi-frequency segmental bioimpedance, nutritional diagnosis, maximum strength test of the tongue and lips, analysis of swallowing using the Functional Oral Intake Score (FOIS) and Swallowing Rating Scale of the American Speech-Language-Hearing Association (ASHA), speech intelligibility and analysis of disease severity. To measure the degree of relationship between quantitative variables, Spearman’s correlation was used. Results: Thirty-four patients were evaluated, 28 Spinal Group and 6 Bulbar Group. The results did not show any significant differences in the analysis of body composition between the groups. Positive associations were found between body compartments and swallowing analysis. The phase angle showed a strong correlation between the FOIS scales (r = 0.74, p 〈 0.01), ASHA (r = 0.77, p 〈 0.01) and tongue (r = 0.66, p 〈 0.01). Conclusions: Changes in body compartments were related to swallowing functionality and speech intelligibility in ALS patients, emphasizing the importance of analyzing body compartments for decision making by the interdisciplinary team. Although these preliminary data were collected in a small sample size, they serve to motivate future studies in this area.

    Abstract: Amyotrophic Lateral Sclerosis (ALS) is a degenerative disease that occurs with the deterioration of motor neurons. The beginning of clinical impairment may be bulbar with the average life time, after the first symptoms, between 2 to 5 years, presenting serious swallowing, speech and breathing disorders. Deglutition disorders can lead to malnutrition, dehydration, aspiration, displeasure, and more serious complications such as aspiration pneumonia and death. With the evolution of the disease, the patient needs procedures that generate doubts in the professionals and family, as the correct moment of indication of the use of alternative long-term feeding routes called gastrostomies (GTT). The objective of this article is to analyze the impact of dysphagias and the most favorable moment for the placement of gastrostomies. Early GTT placement may help prevent the patient from clinically debilitating more quickly, respond better to multidisciplinary team therapeutics, and feel more comfortable.Key-words: dysphagia, enteral nutrition, gastrostomy, deglutition disorders, motor neuron disease, amyotrophic lateral sclerosis.
