Zusammenfassung 11 Patienten, bei denen die Milz wegen einer Verletzung entnommen und in das große Netz replantiert worden war, konnten wenigstens 1 Jahr nachbeobachtet werden. — Immunoglobuline und intraerythrocytäre Einschlußkörper normalisieren sich in allen Fällen. Allerdings trat in 3 Fällen später ein passagerer Anstieg der „pitted” Erythrocyten auf. Bei 5 Patienten persistierte eine relative Lymphocytose. In jedem Fall wurde die Proteolyse- und Phagocytosefähigkeit des Transplantates nachgewiesen. — Autotransplantate der Milz nehmen wichtige Organfunktionen wahr. Da jedoch Teilbereiche der Milzaufgaben nicht in allen Fällen vollständig erfüllt werden, sollte die Autotransplantation nur dann zur Anwendung kommen, wenn eine orthotope Organerhaltung nicht möglich ist.

Summary Ruptured spleens have been removed in 11 patients and reimplanted into the greater omentum. All cases were followed up for at least 1 year. Serum immunoglobulin levels increased to normal in all cases. HowellJolly bodies and pitted RBCs disappeared. However, pitted RBCs reappeared temporarily in three cases. A relative lymphocytosis did not normalize in five patients. Phagocytic and proteolytic activity was demonstrated in every case. Splenic replants preserve important functions of the organ. Since not all parameters return to normal, autotransplantation cannot substitute splenography and should only be used if splenectomy is mandatory because of hemostasis.
