    Notes: Summary The platelet glycoprotein (GP) IIb/IIIa receptor can bind fibrinogen, von Willebrand factor, and other adhesive ligands; this binding is the final common pathway mediating platelet aggregation. The purpose of this study was to evaluate the safety and platelet inhibitory characteristics of the Fab fragment of the murine monoclonal anti-GPIIb/IIIa 7E3 antibody (m7E3 Fab) when administered intravenously as a single bolus dose, as a single and repeat bolus dose, and as a single bolus dose followed by continuous infusions of varying duration. Various dosage regimens of m7E3 Fab were studied in 74 patients with stable angina. Dosage regimens included single doses of m7E3 Fab from 0.1 to 0.3 mg/kg, a single dose of 0.20–0.30 mg/kg, and a repeat dose of 0.05 mg/kg, or a loading dose followed by a continuous infusion of m7E3 Fab for up to 36 hours. To assess the effect of m7E3 Fab on platelet function, quantitative blockade of GPIIb/IIIa receptors, inhibition of ex vivo platelet aggregation, and template bleeding time were measured in all patients. Dose-dependent inhibition of platelet function was evident in response to escalating bolus doses of m7E3 Fab, with maximum inhibition observed at 0.25–0.30 mg/kg body weight; at the 0.30 mg/kg dose, mean (±SE) GPIIb/IIIa receptor blockade was 81±3%, ex vivo platelet aggregation in response to 20 µM ADP was 14±6% of baseline, and the median bleeding time was 〉20 minutes. Although platelet function gradually recovered following a single bolus injection, platelet inhibition could be sustained by continuous, low-dose infusion of the antibody. Platelet inhibition occurred within minutes, but m7E3 Fab that did not bind to platelets cleared rapidly from circulation. Sixteen percent of the m7E3 Fab-injected subjects exhibited low titer, human anti-murine antibody responses. No significant bleeding or allergic reactions were observed in any patients. One of the 74 patients developed transient thrombocytopenia soon after receiving m7E3 Fab. These studies establish that m7E3 Fab can be administered safely at doses that cause profound inhibition of platelet function.

    Notes: Abstract Objectives. To determine whether addition of basic fibroblast growth factor (bFGF), an angiogenic growth factor, enhances the angiogenic effects of transmyocardial laser revascularization (TMR). Background. TMR is an investigational therapy for treating patients with medically refractory angina not amenable to traditional therapies. Histologic and blood flow studies in animals have suggested that TMR enhances angiogenesis above that normally seen in ischemic myocardium. We tested the hypothesis that bFGF administered into TMR channels further enhance the angiogenic effects of TMR. Methods. Chronic ischemia was created in 3 groups of dogs using an ameroid constrictor on the proximal LAD. In the bFGF group (n = 5) non-transmyocardial channels were created in the LAD territory and bFGF, (100 ng/ml) dissolved in pluronic gel was injected into the each channel. In the TMR group (n = 7), transmyocardial channels were created without bFGF. A control group (n = 7) had ischemia without TMR of bFGF. 5-bromo-2'-deoxyuridine (BrdU) was administered to mark proliferating cells. After 8 weeks survival, colored microspheres were injected to assess the regional myocardial blood flow. Results. TMR and TMR+bFGF increased total vascular density by ∼40% over that observed in the control group. However, the number of large vessels (internal diameter ≥ 50 μm) was doubled by the addition of bFGF, and this correlated with a 50% increase in the density of proliferating vascular cells and a tripling of the total estimated vascular cross sectional area. Blood flow to the LAD territory was increased by TMR compared to controls, with no further benefit observed in the bFGF group. Conclusions. On a histologic basis, basic fibroblast growth factor further enhances angiogenesis following TMR in ischemic myocardium mainly by increased the size but not the total number of vessels.

    Notes: Abstract In 1975, the Johns Hopkins Medical house staff was reorganized into four Firms. Each Firm provides inpatient and outpatient care to a group of patients. Two of the goals of the reorganization were to improve accessibility and continuity of care. This study, based on a before and after research design utilizing routinely collected data, sought to determine whether these goals had been attained. Accessibility was assessed by observing changes in waiting time for appointments, broken appointments, and number of patients seen before their scheduled appointments. The results showed that mean waiting time for a Medical Clinic appointment fell from 15 days to 1 day (p〈0.01); broken appointments for new patients fell from 54% to 34% (p〈0.01); no significant changes occurred in broken appointments for old clinic patients (34% vs 32%); and patients seen before their scheduled appointments increased from 30% to 38% (p〈0.001). Continuity was assessed by observing changes in use of emergency and walk-in clinic services, the proportion of Medical Clinic patients lost to follow-up care, the proportion of hospital readmissions returning to the same nursing unit, and the proportion of patients discharged from the hospital who returned to the Medical Clinic. The results showed that use of emergency and walk-in clinic services fell slightly, from 24% of all visits before to 22% of all visits after the Firm System (p〈0.001); no change occurred in the proportion of patients lost to Medical Clinic follow-up (21% before and after the Firm System); the proportion of hospital readmissions returning to the same nursing unit increased from 35% to 73% (p〈0.005); and the proportion of patients discharged from the hospital who returned to the Medical Clinic increased from 21% to 35% (p〈0.001). These data suggest that implementing the Firm System led to improvement in several selected aspects of accessibility and continuity of care but that further improvements could be made.

    Description: Organic falls can form nutrient-rich, ephemeral hotspots of productivity and biodiversity at the deep-sea floor, especially in food-poor abyssal plains. We report here the first wood falls and second carcass fall recorded from the Clarion-Clipperton Zone in the tropical eastern Pacific Ocean, an area that could be mined for polymetallic nodules in the future. A small cetacean fall in the mobile-scavenger stage likely recently arrived on the seafloor was observed, whereas most of the wood falls were highly degraded. There were multiple species in attendance at the wood falls including organic-fall specialists such as Xylophagaidae molluscs. Many of the taxa attending the carcass fall were known mobile scavengers that regularly attend bait parcels in the Pacific Ocean. These results further confirm that wood falls can occur at large distances (〉1450 km) from major land masses, providing an adequate supply of wood to the abyssal seafloor for colonization by wood-boring molluscs and associated fauna. Organic falls may be regionally abundant and are likely to influence species and habitat diversity in the abyssal areas of the Clarion-Clipperton Zone.

    Description: The deep seafloor of the Northeastern Pacific Ocean between the Clarion and Clipperton Fracture Zones (CCZ) hosts large deposits of polymetallic nodules that are of great commercial interest as they are rich in valuable metals such as manganese, nickel, copper and cobalt. However, mining of these nodules has the potential to severely affect the benthic fauna, whose distribution and diversity are still poorly understood. The CCZ is characterized by strong gradients in sea surface productivity and hence changes in the amount of organic carbon reaching the seafloor, decreasing from mesotrophic conditions in the southeast to oligotrophic conditions in the northwest. Uncovering and understanding changes in community composition and structure along this productivity gradient are challenging but important, especially in the context of future mining impacts. Here, we summarize published data on benthic annelids (polychaetes), a major component of macrobenthic communities in the CCZ. Unlike previous studies, we attempt to explore all available data based on both morphology and genetics collected by box corer and epibenthic sledge. In this regard, we specifically aimed to (a) summarize and compare morphological and molecular data in relation to surface water nutrient conditions and (b) provide recommendations to advance the studies of polychaete biodiversity. Although initial studies on polychaetes in the CCZ were performed as far back as the 1970s, there are still large data gaps further explored in our review. For example, most of the current data are from the eastern CCZ, limiting understanding of species ranges across the region. An association between polychaete communities and the available food supply was generally observed in this study. Indeed, mesotrophic conditions supported higher abundance and species richness in polychaetes as a whole, but for certain groups of species, the patterns appear to be opposite — illustrating that relationships are likely more complex at lower taxonomic levels. A better understanding of biogeographical, ecological and evolutionary processes requires a concerted effort involving increased sampling and sharing of data and material to close existing knowledge gaps.
