    Abstract: Ferries are usually used for transporting passengers and vehicles among docks, and any deviation of the course can lead to serious consequences. Therefore, transportation ferries must be watched closely by local maritime administrators, which involves much manpower. With the use of historical data, this article proposes an intelligent method of integrating artificial potential field with Bayesian Network to trigger deviation warnings for a ferry based on its trajectory, speed and course. More specifically, a repulsive potential field-based model is first established to capture a customary waterway of ferries. Subsequently, a method based on non-linear optimisation is introduced to train the coefficients of the proposed repulsive potential field. The deviation of a ferry from the customary route can then be quantified by the potential field. Bayesian Network is further introduced to trigger deviation warnings in accordance with the distribution of deviation values, speeds and courses. Finally, the proposed approach is validated by the historical data of a chosen ferry on a specific route. The testing results show that the approach is capable of providing deviation warnings for ferries accurately and can offer a practical solution for maritime supervision.

    Abstract: Introduction: Colorectal cancer is the third most common malignancy and the second most deadly cancer worldwide. In this present study, the effects of eupafolin on DMH-induced colon cancer in rats were assessed. Methods: The acute and sub-acute oral toxicity study in the balb/c mice was performed to evaluate the LD50 dose and the chemotherapeutic doses of eupafolin. The colon cancer was induced in the animals through a single intraperitoneal injection (i.p) of 30 mg/kg of dimethylhydrazine followed by 2% DSS for 7 days in the drinking water in male Wistar rats. The rats were treated with eupafolin (50, 100, and 200 mg/kg) through oral route for 18 weeks. The animals were sacrificed and colon tissues were subsequently investigated for aberrant crypt foci (ACF), in vivo antioxidant studies, histology and immunohistochemical analysis, and apoptosis by TUNNEL technique after 18 weeks of eupafolin therapy. Results: The acute oral toxicity data represented the LD50 dose of eupafolin which was found to be 500 mg/kg body weight. Along with that, the sub-acute toxicity study suggested the chemotherapeutic doses of eupafolin, that is, 50, 100, and 200 mg/kg body weight. Eupafolin therapy inhibits ACF development in rat colon mucosa efficiently. Additionally, eupafolin has improved the colonic lesions and the structural integrity of the colonic mucosa. Eupafolin therapy causes anti-oxidant enzymes such as superoxide dismutase, catalase, and glutathione to increase as well. Increased levels of P53, BAX, and PCNA and a simultaneous decrease in Bcl2 and IL-6 expressions show eupafolin therapy successfully regulated these biological markers in colorectal cancer. Eupafolin also induced apoptosis efficiently in the rat colon mucous membrane. Conclusion: These results show that eupafolin can improve colon cancer by modulating the p53, Bcl2, BAX, and IL-6 pathways in rats.

    Abstract: Most patients return successfully to shoulder involving sports or activity after rotator cuff repairs. It has not been decided yet whether postoperative participation in shoulder activity adds to the risk of retear. Purpose/Hypothesis: The purpose was to verify whether patients who participate in shoulder activities after rotator cuff repair have a higher risk of structural failure than nonactive patients and to investigate the relationship between postoperative shoulder function and tendon integrity in active and nonactive patients. The hypotheses were that (1) active patients have a higher retear rate than nonactive patients and (2) structural failure is associated with worse functional outcomes in active patients. Study Design: Cohort study; Level of evidence, 3. Methods: A cohort study was performed using 145 patients who underwent arthroscopic rotator cuff repair from 2015 to 2017. Functional assessments and magnetic resonance imaging were performed at least 24 months postoperatively. Shoulder activities since 6 months after surgery were rated in 4 categories (sports, job, activities of daily life, and weight of general lifting) as sedentary, light, moderate, or strenuous. The activity level of each patient was defined by the highest rated category. Patients who were involved in light, moderate, and strenuous activity were identified as active for the present study, and the rest were defined as sedentary. The proportion of retears between groups and the functional conditions between retorn and intact tendons were compared. Results: A total of 48 patients were enrolled in the active group, and 97 were enrolled in the sedentary group. The active group demonstrated a significantly higher retear rate than the sedentary group (27.1% vs 11.3%, respectively; P = .016; risk ratio, 2.39 [95% CI, 1.16-4.93]). In the active group, patients with retears showed higher visual analog scale scores for pain, decreased abduction strength, and lower shoulder functional scores (American Shoulder and Elbow Surgeons score, Fudan University Shoulder Score, and Constant-Murley score) than those with healed tendons, whereas in the sed entary group, functional outcomes were generally similar across patients with and without retears. Conclusion: Shoulder activity after the early postoperative period was associated with a high risk of retears in patients who underwent rotator cuff repair. A correlation between inhibited function and structural failure was detected but only in active patients, while sedentary patients with retears retained functional improvements similar to those with intact tendons.

    Abstract: Our aim was to compare the efficacy and safety of bivalirudin (Biv) versus heparin (Hep) with or without similar usage rate of glycoprotein IIb/IIIa inhibitors (GPIs) during percutaneous coronary intervention (PCI). The PubMed and EMbase were searched. Randomized trials comparing Biv versus Hep were eligible for inclusion. With imbalanced GPI use, Biv had significantly lower major bleeding (pooled risk ratio [RR], 0.67; 95% confidence interval [CI] , 0.54-0.83) without difference in mortality (pooled RR, 0.95; 95% CI, 0.80-1.14). With comparable GPI use, no significant difference was observed in major bleeding (pooled RR, 0.95; 95% CI, 0.82-1.10) and mortality (pooled RR, 1.13; 95% CI, 0.85-1.50). With no GPI use, Biv was associated with numerically higher mortality (pooled RR, 1.17; 95% CI, 0.83-1.65) without significant difference in major bleeding (pooled RR, 0.81; 95% CI, 0.64-1.02). In conclusion, when comparing different anticoagulants during PCI, the effect of GPIs should not be underestimated. Heparin as such was found noninferior to Biv.

    Abstract: The surrogacy of progression-free survival (PFS) for overall survival (OS) in esophageal squamous cell carcinoma (ESCC) remains unelucidated. This study aimed to determine the validity of PFS as a surrogate endpoint for OS in ESCC patients treated with definitive radiotherapy or definitive chemoradiotherapy (dRT/dCRT), as well as characterize the prognostic factors and survival of such patients. Methods: A total of 3662 patients from 10 cancer centers were enrolled. One-, 2-, and 3-year PFS (PFS12, PFS24, and PSF36, respectively) were used as time points for analysis. At each time point, ESCC-specific mortality and OS were characterized using competing risk and conditional survival models, while correlation between PFS and OS was evaluated by linear regression. Results: At PFS12, PFS24, and PFS36, a progressive decrease in 5-year ESCC-specific mortality (35.2%–13.4%) and increase in 5-year OS (46.6%–62.9%) were observed. Regardless, the OS of patients remained markedly lower than those of the age- and sex-matched Chinese general population. TNM stage remained a significant prognostic factor at PFS36. Strong correlation was found between 3-year PFS and 5-year OS, which was further externally validated. Conclusions: Three-year PFS may act as a potential surrogate endpoint for 5-year OS. TNM stage was considered a significant prognostic factor for OS, and may represent the optimal prognostic tool to guide clinical decision-making and post-treatment follow-up.

    Abstract: Rotator cuff tears with shoulder stiffness remain a difficult issue. Despite the reported satisfactory results of 1-stage surgery, little information is available regarding the factors that affect clinical outcomes. Purpose/Hypothesis: To evaluate the 1-stage arthroscopic treatment of rotator cuff tears with shoulder stiffness and to present the influence of duration of symptoms (DOS) on postoperative functional outcomes. The hypothesis was that a long preoperative DOS is related to worse functional outcomes. Hypothesis: A long preoperative DOS is related to worse functional outcomes. Study Design: Cohort study; Level of evidence, 2. Methods: A cohort study was performed with consecutive patients who underwent 1-stage surgery between January 2012 and July 2014. Forty-four patients were enrolled in the long DOS group (DOS ≥6 months or LDOS), and 38 were enrolled in the short DOS group (DOS 〈 6 months or SDOS). There were no significant differences in the other variables between the groups. The patients were followed for a mean of 33.8 months, and the functional and radiographic outcomes were compared. Results: Both groups achieved apparent functional postoperative improvements in terms of range of motion, pain, strength, and functional scores ( P 〈 .001 for all). Despite the overall improvements, the patients in the SDOS group had significantly better outcomes according to all functional instruments. The mean postoperative abduction and external rotation at the side in the SDOS group were higher than in the LDOS group (abduction: 162.2° vs 152.8°, respectively [ P = .002]; external rotation: 64.7° vs 56.9°, respectively [ P = .004] ). The mean postoperative functional scores in the SDOS group were all higher than in the LDOS group (American Shoulder and Elbow Surgeons [ASES] score: 91.1 vs 81.9, respectively; Constant-Murley score: 76.9 vs 71.8, respectively; Fudan University Shoulder Score [FUSS] , 90.6 vs 81.1, respectively), and the mean postoperative visual analog scale (VAS) score for pain in the SDOS group was lower (0.7 vs 1.8, respectively) ( P 〈 .001 for all). The difference in the retear rates was not significant, with 7 retears in the SDOS group and 4 in the LDOS group ( P = .216). Conclusion: One-stage surgery effectively achieved overall improvements. A preoperative DOS of ≥6 months led to poorer functional outcomes, which suggests that surgeons should propose a surgical treatment for this condition before symptoms persist for 6 months.

    Abstract: The rapid rise of glacioeustatic change is the most extreme paleoenvironment alteration in the aftermath of Snowball Earth. Although geologists conducted a lot of multi-subdiscipline research on this issue previously, there still exists the potential room for further discussions of the process in detail. For decades, the practice proved that the Fischer plot is a simple and robust tool to illustrate the fluctuations of accommodation patterns v.s. cycle sets or strata depth which could be interpreted as relative sea-level changes. This research simulates the Fischer plot to unravel the sea-level change in the aftermath of Marinoan glaciation by measuring the lower Ediacaran Doushantuo Formation in Shennongjia, South China. The result shows that 131 fifth-order cycles and nine third-order cycles help us propose a completed second-order cycle variation of ice melting-forced sea-level change, i.e., (1) early high-frequency and slow to rapid stepwise rising, and (2) followed by a stable decrease in the latter. In addition, the vertical sedimentary facies of the lower Doushantuo Formation display, in ascending order, (1) intertidal carbonate rock, (2) subtidal clastics with turbidite, and (3) intertidal lagoon fine clastics, indicating the process of the relative waxing and waning of sea-level. Such interpretation of the Fischer plot and the sedimentary facies’ vertical evolution is beneficial for studying the high-frequency sea-level change and paleogeographic reconstruction of post-Marinoan deglaciation.

    Abstract: It has remained undefined whether the combination of being elderly (ie, 〉 75 years old) and female is a risk factor for worse outcomes after percutaneous coronary intervention (PCI). A total of 29 211 consecutive patients who underwent PCI were analyzed. Kaplan-Meier estimated 3-year rate of cardiac death was significantly higher in elderly females in comparison with all other groups ( P 〈 .05). Using Cox proportional hazard models, being an elderly female was a significant risk factor for cardiac death and cardiac death/MI in comparison with being a young female, OR (95% CI): 2.53 (1.15-5.59), 2.26 (1.27-4.03), or young male, OR (95% CI): 2.22 (1.26-3.91), 2.25 (1.44-3.51); however, it was not a significant risk factor in comparison with being elderly male, OR (95% CI): 1.30 (0.97-1.71), 1.21(0.94-1.55). Elderly females had worse outcomes after PCI therapy than other gender and age groups, but being an elderly female was not an independent risk factor for worse PCI outcomes.

    Abstract: Impact force identification has always been of significance for structure health monitoring especially on the applications involving composite materials. As a typical inverse problem, impact force reconstruction and localization is undoubtedly a challenging task. The well-known ℓ 1 sparse regularization has a tendency to underestimate the amplitude of impact forces. To alleviate this limitation, we propose an accelerated generalized minimax-concave (AGMC) for sparse regularization that employs a non-convex generalized minimax-concave (GMC) penalty as the regularizer and incorporates an acceleration technique to expedite the attainment of the global minimum. Compared with the classic ℓ 1 -norm penalty, the GMC penalty can not only induce sparsity in the estimation, but also maintain the convexity of the cost function, so that the global optimal solution can be obtained through convex optimization algorithms. This method is applied to solve the impact force identification problem with unknown force locations to simultaneously reconstruct and localize impact forces in the under-determined case utilizing a limited number of sensors. Meanwhile, K-sparsity criterion is used to adaptively select regularization parameters by taking advantage of the sparse prior knowledge on impact forces. Simulations and experiments are conducted on a composite plate to verify the computational efficiency and robustness of the AGMC method in terms of impact force reconstruction and localization, particularly in the presence of noise. Results demonstrate that the proposed AGMC method achieves faster convergence and provides more accurate and sparse reconstruction and localization of impact forces compared to other state-of-the-art sparse regularization methods.
