    Abstract: 〈 b 〉 〈 i 〉 Background: 〈 /i 〉 〈 /b 〉 Fatigue is a frequent symptom in cancer patients. In Europe and Northern America fatigue questionnaires were developed and tested, but their generalizability to other cultural contexts is largely unknown. The aim of this study is to provide normative values for the Multidimensional Fatigue Inventory (MFI-20) based on a representative sample of the general population in Colombia and to test psychometric properties. 〈 b 〉 〈 i 〉 Methods: 〈 /i 〉 〈 /b 〉 1,500 individuals completed a questionnaire that contained the MFI-20, as well as other questionnaires, and questions on sociodemographic variables and chronic diseases. 〈 b 〉 〈 i 〉 Results: 〈 /i 〉 〈 /b 〉 The mean values of the scales were marginally higher than those for 2 European samples. The mean value of the total score was 44.3 ± 14.1. Women were affected by fatigue more than men, and there was an almost linear age trend, with higher mean scores for older subjects. People with chronic diseases were affected by fatigue more than people without chronic conditions. The best psychometric properties were obtained for the total scale (sum score) of the MFI-20. 〈 b 〉 〈 i 〉 Conclusion: 〈 /i 〉 〈 /b 〉 The normative values presented here can help us to assess the individual burden of fatigue in a Latin American context. Psychometric properties of the MFI-20 in Colombia are similar to those obtained in Europe.

    Abstract: 〈 b 〉 〈 i 〉 Background: 〈 /i 〉 〈 /b 〉 Within the theoretical framework of the ICD diagnostic criteria of cancer-related fatigue, we aim to investigate how those criteria can be used for screening purposes. 〈 b 〉 〈 i 〉 Methods: 〈 /i 〉 〈 /b 〉 Fatigue was assessed in a mixed sample of 1,225 cancer patients during their stay in a rehabilitation clinic using four different fatigue measures and the diagnostic criteria (Criterion A; DC-A). Psychometric evaluations (e.g., acceptance, reliability, validity, item analyses) were conducted for the sum score and for all items of the diagnostic criteria. 〈 b 〉 〈 i 〉 Results: 〈 /i 〉 〈 /b 〉 A total of 678 (57.1%) patients tested positive according to the DC-A. The sum score of the DC-A had good reliability (Cronbach’s alpha = 0.87) and validity, corresponded closely with other fatigue scales, and, with less than 1% missing values, showed a high degree of patients’ acceptance. The highest accuracy (approx. 0.80) and Youden indices ( & #x3e;0.55) were found for the items “Difficulties completing daily tasks” (A9), “Struggle to overcome inactivity” (A7), “Decreased motivation or interests” (A4), and “Unrefreshing sleep” (A6). 〈 b 〉 〈 i 〉 Conclusion: 〈 /i 〉 〈 /b 〉 The DC-A proved to be a well-accepted, easy to use, and reliable tool for measuring the severity of fatigue. In this paper, we make suggestions for the use of single items of the DC-A as a short and economical screening tool.

    Abstract: Background: Fatigue is common among patients with sarcoidosis. The etiology of this problem is unknown and multifactorial. Fatigue can be confounded with excessive daytime sleepiness (EDS). Fatigue and sleepiness have rarely been studied simultaneously in sarcoidosis patients. Objectives: The aim of this study was the confounder-adjusted estimation of risks for severe fatigue and EDS in a large population of sarcoidosis patients and the development of multivariate predictors from this population. Methods: 1,197 German sarcoidosis patients were examined using the Epworth Sleepiness Scale (ESS), the Fatigue Assessment Scale (FAS), the Hospital Anxiety and Depression Scale (HADS), and the Medical Research Council (MRC) dyspnea scale. Results: 16.5% (123 patients) had EDS (ESS ≥16), 16.4% had severe fatigue (FAS ≥35), and 6.3% had both extreme findings. In a multivariate logistic regression model, predictors of the risk of EDS were a history of sleep apnea (odds ratio [OR] 2.46, 95% confidence interval [CI] 1.5-3.9), dyspnea MRC grade ≥2 (OR 2.29, 95% CI 1.5-3.5), and organ involvement of 4-7 organs (OR 1.60, 95% CI 1.1-2.4). Significantly associated with higher risk of severe fatigue were the following: conspicuous depression (OR 5.95, 95% CI 4.1-8.7), conspicuous anxiety (OR 2.38, 95% CI 1.6-3.4), and muscle pain (OR 1.92, 95% CI 1.32-2.75). The logit models for severe fatigue with and without simultaneous EDS differed only slightly. Conclusion: An extreme form of fatigue and/or sleepiness was found in 27% of all sarcoidosis patients questioned. Because there is a certain overlap, both should be examined simultaneously to allow for a combined assessment.

    Abstract: Langerhans cell histiocytosis (LCH) is a rare group of idiopathic disorders (previously termed “histiocytosis X”) which is characterized by the presence of cells with characteristics similar to bone marrow-derived Langerhans cells which infiltrate various tissues and organs. Like Langerhans cells located in the skin, they express histiocytic markers such as S100, CD1a, and CD68 and contain Birbeck granules, which are rod-shaped intracytoplasmic organelles best demonstrated in electron microscopy. LCH primarily affects the skeleton, but lung, skin, liver, and lymph node involvement may occur alike. Hepatic involvement is well recognized in children, with sclerosing cholangitis occurring in 10–15% of those with multisystemic involvement, whereas LCH confined to the liver appears to be very unusual in adults. Up to date, only one case of a solitary LCH affliction of the extrahepatic bile duct lacking liver involvement in adulthood has been reported on in the literature. We here report on a 60-year-old male patient with classical indolent progressive obstructive jaundice. The diagnostic workup revealed a tumorous lesion in the middle third of the common hepatic bile duct, initially being highly suspicious of an extrahepatic cholangiocarcinoma. CT scans further suspected an infiltration of the right portal vein, implicating a potentially extensive tumor growth, but a preoperative histological confirmation was not feasible. The serum tumor marker CA19-9 was 48 U/mL. The patient then underwent an explorative laparotomy with a pylorus preserving pancreaticoduodenectomy, as all frozen section tissue specimens revealed no tumor infiltration. The final result of the histopathological examination revealed an isolated LCH in the extrahepatic bile duct with a consecutive secondary sclerosing cholangitis. To complete the tumor staging, a thorax CT scan was performed and a generalized histiocytosis was ruled out, hence confirming the localized character of the disease. To the best of our knowledge and after a comprehensive literature review, we report on the second case globally, which describes a primary LCH limited to the extrahepatic bile duct in adulthood. A generalized sclerosing cholangitis in the liver was ruled out by radiological imaging. Preoperative histological affirmation of such findings is very confined due to the complexity and hence can only be diagnosed in the postoperative specimen. However, patients with nondisseminated sole findings, usually report a good prognostic outcome after surgical resection despite the paucity of corresponding data.

    Abstract: 〈 b 〉 〈 i 〉 Introduction: 〈 /i 〉 〈 /b 〉 Individuals suffering from overweight or obesity frequently experience weight-based stigmatization. The widespread belief that weight is a matter of personal will and self-control results in various weight-based stereotypes (e.g., laziness, lack of self-discipline, or neglect). 〈 b 〉 〈 i 〉 Objective: 〈 /i 〉 〈 /b 〉 Based on the modified version of the Weight Bias Internalization Scale (WBIS-M), a short form for the economic assessment of weight bias internalization in the general population was compiled and validated. 〈 b 〉 〈 i 〉 Methods: 〈 /i 〉 〈 /b 〉 A three-item short form (WBIS-3) was derived based on data from a representative sample of the German population ( 〈 i 〉 n 〈 /i 〉 = 1,092). This new short form was validated in a second representative population sample ( 〈 i 〉 n 〈 /i 〉 = 2,513). Item characteristics and internal consistency were obtained. Measurement invariance was tested. Construct validity was established via the correlation with theoretically related constructs (depression, anxiety, eating behavior, discrimination, weight status). To establish scale validity, all analyses were performed for the whole sample as well as for the subsample of individuals with overweight. Age- and gender-specific population norms were provided. 〈 b 〉 〈 i 〉 Results: 〈 /i 〉 〈 /b 〉 The WBIS-3 exhibited excellent psychometric properties. Internal consistency was α = 0.92. Strong measurement invariance was confirmed regarding age, gender, discrimination, and weight status in both the whole sample as well as the overweight subsample. 〈 b 〉 〈 i 〉 Conclusions: 〈 /i 〉 〈 /b 〉 The WBIS-3 constitutes a valid and economical tool for the assessment of weight bias internalization in epidemiological contexts. Measurement invariance allows for an unbiased comparison of means, correlation coefficients, and path coefficients within structural equation modeling across groups.

    Abstract: 〈 i 〉 Objective: 〈 /i 〉 The objective of this study was to examine the age-specific health-related quality of life (HRQOL) of prostate cancer patients (PCPs). 〈 i 〉 Patients and Methods: 〈 /i 〉 387 PCPs were asked to self-assess their HRQOL with the EORTC QLQ-C30 questionnaire. Patients’ data were compared with those of the general German population. 〈 i 〉 Results: 〈 /i 〉 The reported global health/QOL scores of the study group and the general German population are nearly equal. However, most of the subdomains of HRQOL are negatively affected in PCPs, especially in younger patients (≤59 years). 〈 i 〉 Conclusion: 〈 /i 〉 Medical professionals should pay more attention to the emotional burden of PCPs, because their stress and strains during the time before treatment have a rather more emotional than physical character.

    Abstract: 〈 i 〉 Background: 〈 /i 〉 Anxiety and depression are often found in cardiac patients, but also in the general population. Therefore, evaluation of these symptoms in patients requires a comparison with norm values. The purpose of this study was to explore differences between cardiac patients and the general population in age dependency of anxiety and depression, and to discuss possible reasons for these differences. 〈 i 〉 Methods: 〈 /i 〉 A sample of German cardiac patients (n = 2,696) and a sample of the German general population (n = 2,037) were tested using the Hospital Anxiety and Depression Scale (HADS). 〈 i 〉 Results: 〈 /i 〉 While we confirmed a linear age trend of anxiety and depression in the general population, we observed an inverted U-shaped age dependency in the patient sample. Young patients are especially affected by anxiety and depression. Five items of the HADS that mainly contributed to the age differences were identified. Formal characteristics of these 5 items could not explain the age differences. Concerning the meaning of the items, however, most of the items refer to worrying about the future. 〈 i 〉 Conclusions: 〈 /i 〉 The relatively low rates of anxiety and depression in older patients (compared with the general population) indicate that adaptation processes took place, which should be taken into account in studies concerning the psychological status of patients. Young patients need special attention when dealing with mental distress.

    Abstract: 〈 b 〉 〈 i 〉 Objectives: 〈 /i 〉 〈 /b 〉 Current evidence of sequence-targeted therapy (TT) for patients with metastatic renal cell carcinoma (mRCC) beyond fourth-line is sparse. The aim of this study was to describe the efficacy and toxicity of fifth-line TT in patients with mRCC. 〈 b 〉 〈 i 〉 Methods: 〈 /i 〉 〈 /b 〉 Out of 406 patients treated in first-line, 25 patients (6.16%) with more than 4 lines of TT were retrospectively reviewed at a German academic high-volume cancer center. Response was assessed by the use of standard Response Evaluation Criteria in Solid Tumors version 1.0, and toxicity was graded according to the Common Toxicity Criteria for Adverse Events version 3.0. Progression-free survival (PFS) and overall survival (OS) were estimated using the Kaplan-Meier method. Cox proportional hazard models were applied to explore predictors of PFS and OS in univariable and multivariable analyses. 〈 b 〉 〈 i 〉 Results: 〈 /i 〉 〈 /b 〉 Disease control rate for fifth-line treatment was 20%. Median OS from the beginning of first-line therapy was 50.2 months (IQR (interquartile range) 38.9-76.7). Median OS from the time of initiation of fifth-line therapy was 6.2 months (IQR 3.1-23.8). Median PFS for fifth-line TT was 4.1 months (IQR 1.81-9.07) and did not correlate to treatment response in first-line TT. 〈 b 〉 〈 i 〉 Conclusions: 〈 /i 〉 〈 /b 〉 Highly selected patients might benefit from fifth-line treatment independently from treatment response in first-line TT.

    Abstract: 〈 b 〉 〈 i 〉 Background: 〈 /i 〉 〈 /b 〉 Breast cancer survivors often rate their general health as good even though they experience detriments in functioning and multiple symptoms. The hypothesis of this study is that breast cancer survivors changed their frames of reference for evaluating their own health. 〈 b 〉 〈 i 〉 Methods: 〈 /i 〉 〈 /b 〉 A sample of 308 breast cancer survivors were asked to assess their state of health on a 0-100 scale. In addition, 2 vignettes (fictional persons suffering from various health complaints) were designed. The cancer survivors were asked to assess the health of the characters in these vignettes as well. A sample of the general population (n = 639) served as controls. 〈 b 〉 〈 i 〉 Results: 〈 /i 〉 〈 /b 〉 There were only marginal differences between the cancer survivors (M = 65.8) and the controls (M = 65.5) in how they assessed their own state of health. However, the cancer survivors rated the vignette character with physical problems as being significantly healthier compared to the general population (effect size: d = 0.55). The group differences in the assessment of the vignette character with mental problems was markedly lower (d = 0.12). 〈 b 〉 〈 i 〉 Conclusions: 〈 /i 〉 〈 /b 〉 It is not sufficient to assess the general self-assessed health state of cancer survivors. Anchoring vignettes are a promising tool for use in correcting for response shift effects.
