    Intestinal Transit Time During Infection with Eimeria nieschulzi in Rats (1978)

    Notes: SYNOPSIS. Experiments were designed to test whether or not intestinal transit time increases significantly during severe coccidiosis in the rat. Intraduodenal catheters were surgically implanted into 25 rats. Six to 12 days after surgery 11 rats were inoculated orally with 104 sporulated oocysts of Eimeria nieschulzi Dieben, and 11 were inoculated with 106 oocysts; 3 rats were retained as uninfected controls. At 2, 4, 8, 9, and 16 days postinoculation (PI) Na251CrO4 was injected through the catheter into the duodenum of fasted rats and allowed to progress through the small bowel for 15 min, at which time the rats were killed. The distribution of 51Cr in the gut was plotted as a function of gut length. The leading edge of radioactivity traversed 70% of the gut length in controls, and ∼ 50–60% in parasitized rats on days 2, 4, 8, and 9 PI. Also, a reflux of gut contents, as evidenced by radioactivity in the stomach, occurred early (PI days 2 & 4) in rats infected with 104 oocysts and throughout patency in rats infected with 106 oocysts.A 2nd study was undertaken to determine if chemically induced suppression of gut transit time during early infection would enhance infectivity as measured by increased parasite fecundity. Nine rats were injected subcutaneously with an antidiarrheal agent, Loperamide®, known to slow small bowel motility significantly. Another group of 9 control rats was injected with the ethanol-propylene glycol solvent. Ten min after injection, all rats were inoculated per os with 104E. nieschulzi oocysts. The daily number of oocysts discharged/rat was followed from PI days 5–11. Patency began for all rats on PI day 7. The total number of oocysts discharged by the drugged rats as compared with controls was not significantly different.

    Comparison of Complete and Incomplete Revascularization by Coronary Angioplasty for Unstable Angina (1988)

    Notes: When a “culprit lesion” can be identified in a patient with unstable angina, it may be possible to achieve clinical improvement with incomplete revascularization. We analyzed actuarial survival free of an event (severe angina, myocardial infarction, coronary artery bypass graft, or death) at 6, 12, 18, and 24 months in 83 patients with multi-vessel disease and unstable angina who had undergone successful percutaneous transluminal coronary angioplasty (PTCA); revascularization was complete in 31 patients and incomplete in 52. Event-free survival in 85 patients with single-vessel disease and unstable angina who had undergone successful PTCA also was analyzed. Event-free survival at 24 months was worse in the multivessel disease patients than in the single-vessel disease patients (62% vs 85%; P = 0.001). Multivessel disease patients with complete revascularization had the same event-free survival as those with incomplete revascularization (63% vs 61%; P NS). Diagnostic angiograms revealed thrombus or an irregular ulcerated lesion in 42 of the multivessel disease patients. The event-free survival of these 42 patients was not different from that of the multivessel disease patients as a whole (64% vs 60%; P NS). We conclude that in patients with multivessel disease and unstable angina the event-free survival after PTCA is poorer than in patients with single-vessel disease and unstable angina. In the former patients, event-free survival does not necessarily depend on the completeness of revascularization. The outcome of patients who have intra-coronary thrombus or an irregular ulcerated lesion resembles the outcome of patients who lack these findings. (J Interven Cardiol: 1988:1:1)

    Notes: Four cases are presented in which probable bullous pemphigoid and probable psoriasis co-existed. One case represents a simple coincidence ofthe two diseases. In the other three cases, the bullous lesions were transient and seemed to be induced by elements ofthe Gocckerman regimen. The hypothesis is presented that these three cases represent a subclinical form of bullous pempbigoid induced by the irritant effects of coal tar, ultraviolet light, the reduced barrier function ofa pre-existing derma-tosis, or a combination of these factors.

    Notes: We have reviewed the results of the direct immunofluorescence test performed on oral mucosal biopsy specimens since June 1973. During this period, immunopathological examinations were performed extensively to confirm clinical impressions and to provide insight into the pathogenesis of the bullous, erosive and ulcerative diseases affecting the oral mucosa. Early diagnosis of these diseases was afforded with the adjunctive use of direct immunofluorescence examinations. Diagnostic accuracy was excellent. The direct immunofluorescence test was positive in 15 15 specimens of pemphigus vulgaris, 33 of bullous pemphigoid, 22 of lupus erythemaicsus and 19,22 of cicatricial pemphigoid. Patients with desquamative gingivitis could be classified as‘cicatricial pemphigoid suspects' by this examination, and 5/6 of these patients have evolved into cicatricial pemphigoid. No diagnostic pattern was noted for lichen planus. False positive examinations were not apparent among the 124 specimens examined.

    Notes: Of 144 patients with buUous or cicatricial pemphigoid, nine with localized pemphigoid were seen at the Mayo Clinic between 1968 and 1975. In two patients the disease had become generalized before presentation, and in one it had evolved into bullous pemphigoid, the only case with positive indirect immunofluorescence. One additional patient showed mucosal lesions. Direct immunofluorescence was performed in flve patients and showed linear deposition of C3 and fibrin but not immunoglobulins in three; in two patients immunofluorescence was negative. Localized pemphigoid can be divided into two types: that with scarring plaque-like lesions usually occurring on the head and neck predominantly in males, and that with localized bullous lesions usually occurring on the lower part of the legs of females.

    Notes: The role of T-cell-derived cytokines in the regulation ofLeishmania donovani replication was studied in a murine model. It was observed that in H-2d mice at the early and later stages of the disease IFN-γ-secreting T cells predominate, whereas in between the above stages IL-4–secreting T cells predominate. Possibly, IL-4 abrogates the protective ability of IFN-γ and thereby exponential parasite growth is ensured at the active stage of the disease. By contrast, H-2b mice were possibly incapable of inducing IL-4-secreting T cells and therefore parasite replication remains under control at any point post infection.
