Zusammenfassung Da bei einem Teil der Patienten mit schwerem Thoraxtrauma nach Beherrschung der primären respiratorischen Veränderungen es im weiteren Verlauf zu erneuten lebensbedrohlichen Störungen kommt, wurden bei insgesamt 12 Patienten Untersuchungen der Hämodynamik, des Gasaustausches, des Säure-Basenhaushaltes und des Stoffwechsels der Kohlenhydrate und der Fette durchgeführt. Die Ergebnisse von Patienten, die verstarben, wurden solchen gegenübergestellt, die das Trauma überlebten. Bei den Verstorbenen wurden außerdem histologische Untersuchungen des Lungengewebes durchgeführt2. Dabei ergibt sich, daß es nach Beherrschung der Kreislaufsituation und der Atemmechanik bei schweren Thoraxtraumen — ähnlich wie auch bei anderen schweren posttraumatischen und postoperativen Verläufen — zum Bild der disseminierten intravasalen Gerinnung oder Verbrauchskoagulopathie kommt. Sie manifestiert sich am stärksten im Niederdruck-System der Lungenstrombahn. Am Anfang steht der erniedrigte arterielle Sauerstoffdruck, der hohe Lactatspiegel und eine Erhöhung des Druckes in der Arteria pulmonalis als einziges hierbei verwertbares hämodynamisches Kriterium. Zusätzlich sehen wir eine kombinierte respiratorisch-metabolische Acidose mit. gleichzeitigem Anstieg der Neutralfette und der freien Fettsäuren im zentralvenösen Blut. Diese Ergebnisse finden ihre Erklärung in den morphologischen Lungenveränderungen: Ödem von Gefäßwand, Perivasculärraum und Interstitium, Verbreiterung der Alveolarsepten und Mikrothrombosierung der Lungencapillaren.

Summary As some patients with severe chest injuries may, after control of the initial respiratory problems has been achieved, develop in the course of their illness further disorders which again threaten their lives, we carried out studies of haemodynamic function, gaseous interchange, acid-base metabolism and carbohydrate and fat metabolism in 12 of our patients. The results obtained in patients who subsequently died were compared with those in patients who survived. In addition, histological studies of lung tissue of the patients who died were carried out'. It was shown that after controlling circulation and respiratory function in severe chest injuries the picture of a disseminated intravasal coagulation or consumption-coagulopathy develops, similar to that seen in other severe posttraumatic and post-operative conditions. This manifests itself most markedly in the low pressure of the pulmonary circulation. At the beginning the only haemodynamic criteria which can be utilized are reduced arterial O2 pressure, high lactate levels and the increased pressure in the pulmonary artery. In addition, we observed a combined respiratory—metabolic acidosis with simultaneous increase of the level of neutral lipids and free fatty acids in the central venous blood. These findings are explained by the morphological changes in the lung tissue: oedema of the wall of the blood vessels, of the perivascular and the interstitial spaces, broadening of the alveolar septa and micro-thrombosis of the pulmonary capillaries.
