Zusammenfassung Im Zeitraum von 1962 bis 1973 wurden an der Kieler Universitätsklinik 15 Patienten mit einem Gallensteinileus operativ behandelt. Anhand dieses Krankengutes wird zu der Frage des geeigneten Operationsverfahrens Stellung genommen. Die Enterolithotomie ist als vitale Maßnahme selbstverständlich. Die Indikation zu einer Beseitigung der ursächlichen Cholecystolithiasis und der biliodigestiven Fistel stellen Entzündungen oder weitere Konkremente in den Gallenwegen sowie die Gefahr der malignen Entartung der Gallenblase dar. Die Kombination der Enterolithotomie mit Beseitigung des entzündlichen Tumors in gleicher Sitzung wurde bei 2 eigenen Patienten erfolgreich angewandt. Die folgenden Kriterien rechtfertigen die primäre Ausweitung der Operation: 1. Kurze Ileusdauer ohne Peritonitis. 2. Guter Allgemeinzustand, keine ernsthaften Begleiterkrankungen. 3. Ausgeglichener Elektrolyt- und Flüssigkeitshaushalt.

Summary Between 1962 and 1973 15 patients with gallstone-ileus were operated in the Surgical Clinic of Kiel. By means of these cases we give our opinion on suitable methods of operation. The removal of the obturating gallstone is a vital measure. The indication for the operation of the underlying gallstone-disease in the gallbladder and the biliodigestive fistula depends on the risk of further concrements in the biliary tree, inflammation and the development of cancer in the gallbladder. The combination of enterolithotomy with the removal of the inflamed gallbladder in one stage was done in 2 of our patients. The following criteria justify the primary prolongation of the operation: 1. A short duration of ileus without peritonitis. 2. A good general condition without another severe affection. 3. A good balance of water and electrolytes.
