    Abstract: Using a large consortium of undergraduate students in an organized program at the University of California, Los Angeles (UCLA), we have undertaken a functional genomic screen in the Drosophila eye. In addition to the educational value of discovery-based learning, this article presents the first comprehensive genomewide analysis of essential genes involved in eye development. The data reveal the surprising result that the X chromosome has almost twice the frequency of essential genes involved in eye development as that found on the autosomes.

    Abstract: Background: PICC is a well known central venous access device that plays an important role in successful administration of chemotherapeutic agents and other supportive medications. This system is very popular not only because of its low incidence of complications and cost compared with other central venous catheters, but also it can be easily inserted by personnel trained in the procedure. Except for very small children, anti-cubital fossa of upper arm is the most common point of insertion and its distal end terminates into the SVC. Long term I.V. therapy, hyper-osmolar solution infusion, repeated chemotherapy administration, difficulty in accessing peripheral venous system in obese individuals are common indications for the procedure. Due to low incidence of procedure related infection, it is now becoming the method of choice in dealing patients with hemato-oncological conditions. Considering all the merits mentioned above and the demerits like risk of infection, thrombosis, venous damage, stenosis, veno-spasm, non-thrombosis occlusion, catheter mal position, removal difficulty with “stuck-PICC”, we did a prospective study to compare beneficial effects versus complications in our pool of patients with hematological malignancies in this centre. Materials & Methods: For this study we recruited a total of 76 patients with hematological malignancies admitted in our hospital from July 2005 to June 2008. The age range was between 2 and 65 years, median was 20 years, irrespective of gender status. To eliminate bias in the study we selected the PICC from Bard Access System, USA, with traditional peel-away cannula insertion technique and placed only in the vein in the ante-cubital fossa under strict aseptic condition and performed by doctors & registered nurses in our study team. Result : We evaluated the results of first 50 cases of insertion outcome. Out of them, 37 patients came out without complication with regard to PICC insertion, while the PICC was in-situ, and also during removal phase after successful chemotherapy administration. 5 patients developed severe inflammation along with elevated ANC. Coagulase negative staphylococcus was the most common pathogen responsible for the catheter induced sepsis. We initiated vancomycin therapy for those patients who developed these type of infection and the success rate was 100%. While 4 of them developed blood oozing from insertion site, there were neither documented incidence of thrombus formation nor catheter malposition. One of the patients developed unexplained pain in the upper extremity where the PICC was inserted without any sign of inflammation or elevated ANC. This could be attributable to short lasting veno-spasm. Unfortunately, there were three incidence of catheter damage possibly due to defective products or improper care. Conclusion: Our study revealed that 74% of the patients did not face any complications, 10% developed catheter induced sepsis. Six (6%) percent PICC required removal. We can conclude that PICC is the method of choice for long term accessing of central venous system for patients with hematological malignancies which has the merits like minimum discomfort, easy to implant, well tolerated, fewer complications rate and without any need for repeated venipuncture.

    Abstract: Background: Bone marrow transplant is often the preferred method of treatment for a variety of blood disorders including leukaemias, aplastic anaemia, inborn errors of metabolism and certain haemoglobinopathies (thalassaemia, sickle cell disease). Recently, several investigations have shown that cord blood is a rich source of pluripotent stem cells and it can be used effectively in place of BMT. Cord blood transplantation as a therapeutic option is a recent phenomenon in India. To date umbilical cord blood has been used in sibling and other related donors in a few centers in India. Cord blood transplant can also be used in case of a large number of patients for whom no acceptable family donor is available. The aim of our study is to see the effectiveness of umbilical cord blood as a source of stem cell transplant. MATERIAL AND METHODS: We recruited 5 patients for the sibling CBT, from July 2002 – June 2008, at our institute, which is a tertiary level cancer research institute in eastern India. The patients were properly screened for Lucarelli staging and HLA matching prior to the procedure. Out of the 5 cases 3 were fully HLA matched and 2 were mismatched. Three (3) children were in Lucarelli stage 1 and 2 were in stage 1. The sibling CBT was done at a median age of 4.5 yrs (range 3 – 12 yrs). In all the cases, cord blood was obtained shortly after the birth of the donor child, employing the closed system of collection. About 100 – 120 ml of blood was collected in each case. The blood obtained was tested for several infectious diseases including HIV and hepatitis. The blood was then sent to the appropriate laboratory for storage where it was cryopreserved at −80°C and then stored in liquid nitrogen (− 192°C) for future use. The mean no of nucleated cells infused were 2.8 × 107/kg (Range:1.2 – 7.2 × 107/kg)and the mean number of CD4+ cells infused were 3.2 × 106/kg (Range: 2.2 – 7.8 × 106/kg). The infused CFU-GM for our cases were 6 ×104/kg (4.1 – 9.2 ×104/kg). RESULTS: The median number of days to achieve engraftment was 13 days (range 9 – 13 days) for neutrophil, 33 days(range 24 – 48 days) for platelets and 80 days (33 – 148 days) for the red blood cells. All 3 patients who received HLA identical cord blood transplant engrafted, but rejection of the graft was seen in 2 patients later on at day + 180 and day + 250. Two patients who received 4/6 and 5/6 mismatched cord blood only had a partial engraftment. Out of the 3 pts who received HLA identical transplants 1 developed Gr 1 AcuteGVHD and no GVHD was seen the 2 patients who received HLA non identical CBT. Complete engraftment was seen in 2 HLA matched patients with a second transplant with the bone marrow of the same sibling. With a median follow up of 3.6 years (3 months – 6 years) all the patients are surviving – 3 being disease free(transfusion independent),2 with infrequent transfusion. CONCLUSION: In conclusion, it appears that umbilical cord blood transplant appears to be a viable option in terms of providing a cure for thalassaemia major and other haemoglobinopathies, provided it is combined with bone marrow of a matched sibling donor. In the future, we are intending to perform a mixed transplant (stored umbilical cord blood and bone marrow) if a HLA matched sibling donor is available. If no matched donor is available we will consider mismatched related or unrelated cord blood transplant.

    Abstract: Background: In developing countries 40 % children suffer from mal-nutrition. It has been shown that a good nutritional status can reduce complications of treatment, strengthen the immune system and contribute to the patient’s general well being throughout treatment. A good nutritional status is therefore essential for optimal treatment of a leukemic child. The aim of our study was to see the nutritional status of leukemic children on diagnosis and effect of nutrition on outcome of therapy. Material & Methods: During period from January 2004 to December 2007 we prospectively analyzed the nutritional status of 400 paediatric patients in Netaji Subhash Chandra Bose Cancer Research Institute, a tertiary cancer center of Eastern India. The age of the patients were 1 to 18 years (median age 12.5 years). The parameter analyzed were weight for age, height for age, total protein, serum albumin and skin fold thickness. The weight for age, height for age and skin fold thickness were taken as normal if they were between 3rd and 97th percentile curve of the growth chart recommended by the Indian Council of Medical Research. The albumin level and the total protein was considered normal if the value is equal to or more than 3gm% and 5.8gm% respectively. Result: It was seen that total 56 patients (14% children) were low weight for age, 40 patients (10% children) were low height and 67 patients (16.75% children) had low mid arm circumference. Total 44 patients (11% children) had low serum albumin while 80 patients (20% children) had low serum protein. It was found that low weight for age, low serum albumin and low mid arm circumference were significance factors in remission induction, disease free survival and toxicity of chemotherapy (p & lt; 0.005). Conclusion: We conclude that mal-nutrition is a major finding in cancer patients with leukemia in developing country like ours. The patient with mal-nutrition had less remission in induction, disease free survival and more toxicities during therapy as compared to well-nourished children.
